        가상세계에서의 건강증진교육의 개념분석

        Purpose: Virtual worlds are emerging in health care as an innovative way of helping clients take responsibility for and promote their own care. Yet, the concept of health promotion education in this environment has not been elucidated. Therefore, we undertook a concept analysis to discover the underlying attributes and contextual basis to generate a conceptual understanding of virtual worlds in the context of health promotion education. Methods: Using Rodgers’ evolutionary view of concept development, we analyzed literature on virtual worlds that describe their use to discover critical attributes, antecedents, and consequences of this phenomenon. Eleven studies matching inclusion criteria were reviewed. Results: Analysis of the literature highlighted three areas of important domains: user, simulated environment, and education. This concept analysis extends our understanding of a virtual world for health promotion education. Conclusion: This study may help anticipate future trends in the use of virtual worlds in the context of health promotion education, serving as a stepping-stone for further research on this aspect of the study. The next step should focus on the development of an evaluation tool that can measure the factors identified in this analysis to determine their effects on outcomes and environmental interactions.

        Validity and Reliability of the Korean Version of the Women’s Toileting Behavior Scale

        Purpose: The purpose of this study was to investigate the validity and reliability of the Korean version of the Toileting Behavior Scale (KTBS) to assess women’s toileting behavior related to urinary elimination. Methods: The original English version, the Toileting Behavior: Women’s Elimination Behaviors scale, was translated into Korean with forward and backward translation. Examinations of internal consistency reliability, construct validity using exploratory factor analysis and confirmatory factor analysis, item convergent validity, and discriminant validity were conducted with SPSS/WIN 23.0 and AMOS/WIN 23.0 software. Concurrent validity was examined with the International Consultation on Incontinence Questionnaire-Short Form. Results: Cronbach α for the overall scale was 0.78, and the 5 subscales ranged from 0.79 to 0.94. The exploratory factor analysis revealed 5 factors for the 17-item scale. Confirmatory factor analysis supported good convergent and discriminant values (λ=0.49–0.96, critical ratio=4.51–15.68>1.97, P<0.05, construct reliability=0.72–0.97). The concurrent validity was supported by correlation with the International Consultation on Incontinence Questionnaire-Short Form (r=0.146, P=0.011). Conclusions: The KTBS (17 items) is an appropriate tool to measure older Korean women’s toileting behavior with good validity and reliability.

        The Effect of a Workshop on a Urinary Incontinence Self-Management Teaching Program for Community Health Nurses

        Purpose: This study aimed to examine the effectiveness of the workshop on the nurses' knowledge about urinary incontinence (UI) self-management, attitudes toward UI, and self-efficacy to plan and implement a UI self-help group program for their clients. Methods: A one-group pretest and posttest design was used to examine changes in knowledge, attitudes, and self-efficacy following a one-day training workshop. Twenty-seven community health nurses completed a questionnaire before and after the workshop. Before participating in the workshop, the participants were required to take a UI online continuing education program developed by the researchers. During the workshop, the participants took four sessions which consisted of an introduction of a self-help group program, demonstration of a 5-week UI self-management program contents, pelvic floor muscle training and biofeedback practice, and group discussions to plan the implementation in their workplaces. Results: A significant improvement in knowledge of and attitudes toward UI were found (t=3.53, p=.002; t=2.83, p=.009, respectively) after the workshop. Participants also demonstrated improvement in their self-efficacy to plan and operate a UI self-help group program (Z=-2.64, p=.008). Conclusion: The one-day workshop for community health nurses is a feasible strategy to increase their abilities and confidence in operating a UI self-help group program.

        회복마취 간호사의 셀프리더십, 간호 전문직관, 비판적 사고성향이 임상수행능력에 미치는 영향

        The purpose of this study was to identify factors associated with clinical competency of PACU (post-anesthesia care unit) nurses and to examine the relationship among them. A total of 193 nurses participated in this cross-sectional, descriptive study using a structured questionnaire. Self-leadership, nursing professionalism, and critical thinking disposition were positively correlated with clinical competency (r=.585, p<.001; r=.394, p<.001; r=.662, p<.001, respectively). The factors related to clinical competency were critical thinking disposition (β=0.432, p<.001), total clinical career (β=0.267, p<.001), self-leadership (β=0.247, p<.001), and nursing professionalism (β=0.140, p<.001), which accounted for 57.7% of the variance in clinical competency. Critical thinking disposition was the most significant predictor of clinical competency. Thus, in order to enhance the clinical competency of nurses in the PACU, it is necessary to develop and apply an education program that can improve their critical thinking disposition. 본 연구는 회복마취 간호사의 셀프리더십, 간호 전문직관, 비판적 사고성향을 파악하고 임상수행능력에 영향을 미치는 요인을 확인하고자 실시하였다. 서울·경기 지역 회복마취 간호부서에서 근무하는 간호사 193명을 대상으로 자료를 수집하였으며, 임상수행능력은 결혼상태(t=4.627, p<.001), 연령(F=11.092, p<.001), 직위(F=9.302, p<.001), 전체 근무경력(F=10.912, p<.001), 회복마취 간호부서 근무경력(F=9.630, p<.001), 마취전문간호사 자격증 소지여부(F=3.087, p=.002)에 따라 통계적으로 유의한 차이가 있었다. 회복마취간호사의 임상수행능력은 셀프리더십(r=.585, p<.001), 간호 전문직관(r=.394, p<.001), 비판적 사고성향(r=.662, p<.001)과 양의 상관관계로 나타났다. 회복마취간호사의 임상수행능력에 영향을 미치는 요인은 비판적 사고성향(β=.432, p<.001), 전체 근무경력(β=.267, p<.001), 셀프리더십(β=.247, p<.001), 그리고 간호 전문직관(β=.140, p<.001) 순으로 나타났으며, 임상수행능력에 대한 이들 변수의 전체 설명력은 57.7%였다. 본 연구의 결과를 토대로 회복마취간호사의 임상수행능력을 높이기 위해는 셀프리더십과 간호 전문직관을 높일 수 있는 방안이 고려되어야 할 것이라 사료되며, 비판적 사고성향을 갖추기 위한 지속적인 교육과 다양한 전략의 개발 및 운영을 제언한다.

        미국내 아시아 이민자들의 의료서비스 이용에 영향을 미치는 요인

        Purpose: This study was considered in order to identify the factors affecting healthcare utilization by Asian immigrants in the United States. Methods: From February to April 2011, a descriptive survey study was conducted in a convenience sample of 250 Korean and Asian Indian immigrants aged between 40 and 64 in the Triangle area of North Carolina. An author-developed instrument was used to assess predisposing, enabling, and need factors according to Anderson’s Behavioral Model of Health Services. Utilization Data analysis was performed by X²-test, t-test, and binary logistic regression. Results: Participants’ healthcare services experiences were significantly different when they had a longer stay in the U.S., had been employed, had higher income, were Asian Indians, had better English-speaking skills, better health status, more knowledge of health system and health insurance, had higher satisfaction with the healthcare system, and when they were taking prescribed medications and having health insurance. The strongest association with experience of healthcare services was having health insurance with an adjusted odds ratio (OR) of 15.37 (95% CI 4.95-47.71, p<.001) and self-reported English proficiency (OR=1.99, 95% CI 1.00-3.96, p=.05). Conclusion: Intervention strategies to increase accessibility to healthcare services should focus on these significant predictors.

        간호정보학 이해를 위한 영화 ‘The Island’ 활용

        영화는 간호교육에서 효과적으로 활용되는 학습매체이다. 본 연구는 간호사의 핵심역량의 하나인 간호정보학의 개념을 이해하기 위하여 영화 ‘The Island’를 활용하고 그 효과를 분석한 것이다. 2012년 2학기 부산광역시 소재 I대학 간호학과에 개설된 “간호와 정보‘ 과목을 수강한 2학년 학생 중 본 연구의 목적과 의의를 알고 동의한 학생 68명이 연구에 참여하였다. 영화를 활용한 수업은 15주 중 첫 5주에 걸쳐 이루어졌다. 참여 학생들은 매 차시마다 자가 보고식 질문지에 응답하였고, 응답지는 내용분석을 실시하였다. 자료분석 결과, 첨단의료기술을 사용한 질병치료와 건강증진으로서의 ‘간호정보학의 개념’, 정보학의 실제적인 간호실무로의 통합으로서의 ‘컴퓨터정보기술의 활용’, 그리고 높은 보건의료기술뿐만 아니라 ‘환자안전과 보안’의 주제에 대한 학습목표를 달성하였다. 종합적으로 볼 때 영화 ‘The Island’는 간호정보학의 개념을 잘 이해할 수 있는 유용한 학습자료임을 알 수 있었다. 간호사를 포함한 보건의료전문가들의 정보학 역량 향상을 위해서도 본 영화가 효과적으로 활용될 수 있을 것으로 기대한다. The purpose of this study was to examine the effect of the film ‘The Island’ on 2nd year nursing students’ nursing informatics (NI) competency. The participants were sixty-eight students in a baccalaureate nursing program of a private university, taking the course, ‘Nursing & Informatics’ in the 2012 fall semester. Over the 15 weeks, the film was used for five weeks as introduction to NI, peer-to-group discussions, and self-reflection on lessons learned regarding NI. A qualitative content analysis was used to explore the students’ experiences and perceptions on their NI competency. As a result, students signified the NI concept as the assessment of biometrics data, promotion of optimal health with the support of various technologies, and integration of patient-centered care into routine practice. They also highlighted the importance of security and safety measures as well as high quality health technology including the ubiquitous health monitoring system. Overall, the lesson outcomes of the course were met. As a supportive, instructional strategy, the use of the movie, ‘The Island’, was effective for nursing students in achieving NI competencies. Further study is warranted to determine if movies can be used as a means of continuing education to improve informatics competences in healthcare professionals.
