      • 실험적 부자 시신경병증에서 Methylprednisolone의 효능

        동물실험에서 aconite 또는 aconitine은 신경계통에 독성을 나타낸다고 이미 알려져 있다. 그러한 독성은 시유발전위와 병리조직에서 탈수초증으로 나타난다. 저자들은 실험적으로 aconite을 주입하여 탈수초신경증을 일으킨 후 고용량이 methylprednisolone(MP)투여가 효능이 있는지를 알아보고자 하였다. Aconite를 복강내 주입하고 곧 이어 가토의 이연정맥으로 MP를 주입한 군과 aconite만을 주입한 군으로 나누었고 이들을 서로 비교하였다. 그리고 이 두 군을 정상군과도 비교하였다. 시유발전위에서 aconite를 주입후 MP를 투여한 군에서 aconite만을 주입한 군에 비하여 onset latency와 peak latency 그리고 진폭 모두에서 2주째에 통계학적으로 의미있게 회복되었으나 (p<0.05) 정상군과 비교하여 의미있는 차이를 나타내지 아니하였다. 그러나 정상군과 비교하여 1개월과 2개월에서 onset latency은 의미있게 지연되었고 (p<0.05) peak latency와 진폭은 다소 회복되었으나 정상군의 평균값에는 미치지 못하였다. 병리조직에서 MP으로 치료한 군과 aconite만을 주입한 군 모두에서 수초의 탈수초변화가 관찰되었으나 MP으로 치료한 군은 상대적으로 탈수초변화가 현저하지 아니하였다. 본 연구에서 실험적으로 가토에 aconite를 전신에 투여하여 시신경병증을 일으킨 후 고용량의 MP를 즉시 투여한 경우에 시신경의 기능을 정상상태까지 회복시키는데는 한계가 있으나 부분적으로 시신경의 기능을 회복시키는데 도움이 될 것으로 생각된다(한안지 37:1730~1740, 1996). Studies in animals indicate that aconitine or aconite has toxic effects in the visual system of a rabbit model. Such a toxic effects causes myelo-optic neuropathy in the visually evoked cortical potential(VECP) and histopathologic studies. Whether the administration of intravenous high-does of methyprednisolone(MP) in the myelo-optic neuropathy caused by aconitine(1.5㎖/㎏, equivalent to 0.7㎎/㎏ of aconitine) is experimentally effective remains uncertain. The group treated with MP(daily 30㎎/㎏×2 for 3 days followed by 15㎎/㎏ for 3 days) in addition to aconite was compared with the aconite alone group and the normal control group. In the MP treated group, increased recovery of onset latency, peak latency, and amplitude in VECP in comparision with the aconite alone group was recorded at 2 weeks(p<0.05). The MP treated group in comparision with the normal control group showed a significant delay in onset latency at 1 month and 2 months(p<0.05). But, the amplitude in both the MP treated group and the aconite alone group increased at 2 months and did not show a significant difference in comparision with the normal group(p<0.05). In histopathologic study, pathologic findings of myelin sheath in the MP treated group were less severe than in the aconite alone group generally. The true benefit of high-does MP was at least clear within 2 weeks. The authors conclude that treatment with intravenous high-does MP immediate after aconite injection may have some beneficial effects in recovery of neurologic function although such a treatment do no have the definite recovery in aconite induced myelo-optic neuropathy(J Korean Ophthalmol Soc 37:1730~1740, 1996).

        측두하악관절염에서 발생한 희귀한 골화현상

        Osteoarthritis is a noninflammatory degenerative disease affecting the articular surfaces and is accompanied by remodeling of the underlying bone. The sympotms of osteoarthritis of the temporomandibular joint are pain in the joint and muscles of mastication, limitation. Osteoarthritis is generally accepted that several factors can contribute to the trauma, aging process, internal derangement and MPDS. Radiographic features of the osteoarthritis are seen flattening of joint, sclerosis on flattened area and osteophyte or anterior lipping. In the past, osteoarthritis was considered to self-limiting disease. Currently, synovial chondro-membrane is part of the process of osteoarthritis secondary to trauma. Synovial chondromatosis is an uncommon disease of cartilaginous transformation of synovial membrane with formation of loose bodies within the joint space. The pathogenesis in more an active metaplastic than a neoplastic process. The cause of synovial chondromatosis is unknown. Although trauma and inflammation have all been cited as possible factors in tis pathogenesis. The clinical sign and symptoms are unilateral swelling of the joint region, pain in the joint area and crepitus seem to be the most reliable signs and symptoms. Radiographic evidence of loose bodies may or may not be present. This is a case report of 66 year old female with synovial chondromatosis, that is advanced disease of the osteoarthrits. We treated patient with surgical excision of lose bodies, diskectomy and synovectomy. The defected articular fossa area was reconstructed with temporalis fascia flap. The result was satisfactory.

        RF DIN Connector의 Passive IMD 개선에 관한 연구

        최근 이동통신 서비스가 확대됨에 따라 인접 기지국간의 간섭문제가 증가하게 되었으며, 그에 따라 CDMA시스템의 active IMD 뿐만 아니라 PIMD (Passive Inter Modulation Distortion)에 대한 문제도 시스템 설계 시 고려되어야 한다. 본 논문에서는 RF connector 중 대표적 connector인 7/16“ DIN connector를 통하여 동촉선에서의 PIMD발생 정도를 분석하였고, PIME 개선 방안을 제안하였다. 첫째, 대부분의 전류가 통과하는 경로에는 표면조도가 좋은 금속이 전도율이 좋아 유리하며, 둘째 RF에너지가 충분ㅎ히 한 매질에서 통과할 수 있도록 최소 5m 이상으로 도금을 해야 함을 알수 있었다. 셋째, 도금 재료의 노화현상 방지를 위해 비열이 높고 전도율이 높은 금속 선택이 필수적이며, 비용절감 면에서는 삼원합금 등 대체 도금 재질 개발이 요구됨을 알 수 있었다. 현재 국내 PIMD 규격인 KTF-150dBc를 만족시키기 위해서는 7/16“ DIN connector 의 경우 도금재질에 관계없이 표면조도 6s, 도금두께 5m일때 KTF의 PIMD 규정을 만족시킬 수 있음을 알 수 있었다. Recently, as the range of mobile communication services is extended, the interference between adjacent base-stations is increased. Nowdays, on of the important factors causing interference is IMD(Inter-Modulation Distortion) problems. Not only active IMD but also passive IMD effects must be considered to design a CDMA system. In this study, we design and implement 7/16'' DIN connectors which have a various intensity of surface illumination, thickness and quality of plating material to analyze the effects of PIMD. And propose the methods for improving the PIMD characteristics: First, it is more profitable to use the metal which has good intensity of surface illumination where most of all electric currents passes through it. Secondly, we should plate metals more than 5 μm for RF energy enough to propagate in a medium. Thirdly, it is necessary to select a metal having high conductivity and specific heat to protect the aging phenomenon of plate material. And it is required to develop a new plating material to replace the current materials, such as the alloy of three components for cost reduction. We have to know that the plate which has intensity of surface illumination 6 S and the thickness of plating material 5 μm satisfy domestic PIMD specification (KTF) - 150 dBc, regardless of the plate material in case of 7/16'' DIN connector.

      • The Protection of the Retina from Ischemic Injury by the Free Radical Scavenger EGb 761 and Zinc in the Cat Retina

        We investigated the effects of a free radical scavenger (EGb 761) and zinc in experimentally induced ischemic injury in the cat retina. Total retinal ischemia for 90 min was produced in the left eyes of 40 cats by raising intraocular pressure. In group 1, 10 cats were used as control. The free radical scavenger (EGb 761, 100 mg/kg) in group 2 (10 cats)and zinc chloride (250㎍/kg) in group 3 (10 cats) were administered intravenously at the end of ischemia. In group 4, both EGb 761 (100 mg/kg) and zinc chloride (250㎍/kg) were infected into 10 cats. ERG and a histologic study were performed 1 h, 1 day, 3 days, 1 week and 2 weeks after ischemia. The amplitude of the ERG b-wave was 62.73±0.32, 84.31±6.10, 83.65 ±12.23 and 102.4±14.27%, and the summed amplitude of oscillatory potentials was 66.16±16.42, 99.44±14.92, 95.45±6.42 and 99.62±12.32% in each group 2 weeks after ischemia. There was no significant effect in animals that received zinc alone (group 3) by the end of 1 week but some additive effect in combining EGb 761 and zinc chloride (group4) 1 h after ischemia. These findings suggest that the free radical scavenger EGb 761 may efficiently protect the retina from ischemic injury and zinc may have an additive effect when combined with a radical scavenger.

        Predictive Capability of Anorectal Physiologic Tests for Unfavorable Outcomes Following Biofeedback Therapy in Dyssynergic Defecation

        <P>The purpose of this study is to evaluate the predictive capability of anorectal physiologic tests for unfavorable outcomes prior to the initiation of biofeedback therapy in patients with dyssynergic defecation. We analyzed a total of 80 consecutive patients who received biofeedback therapy for chronic idiopathic functional constipation with dyssynergic defecation. After classifying the patients into two groups (responders and non-responders), univariate and multivariate analyses were performed to determine the predictors associated with the responsiveness to biofeedback therapy. Of the 80 patients, 63 (78.7%) responded to biofeedback therapy and 17 (21.3%) did not. On univariate analysis, the inability to evacuate an intrarectal balloon (<I>P</I>=0.028), higher rectal volume for first, urgent, and maximal sensation (<I>P</I>=0.023, <I>P</I>=0.008, <I>P</I>=0.007, respectively), and increased anorectal angle during squeeze (<I>P</I>=0.020) were associated with poor outcomes. On multivariate analysis, the inability to evacuate an intrarectal balloon (<I>P</I>=0.018) and increased anorectal angle during squeeze (<I>P</I>=0.029) were both found to be independently associated with a lack of response to biofeedback therapy. Our data show that the two anorectal physiologic test factors are associated with poor response to biofeedback therapy for patients with dyssynergic defecation. These findings may assist physicians in predicting the responsiveness to therapy for this patient population.</P>

      • Long‐term outcomes of obscure gastrointestinal bleeding after CT enterography: Does negative CT enterography predict lower long‐term rebleeding rate?

        <P><B>Abstract</B></P><P><B>Background and Aim: </B> Computed tomography enterography (CTE) is a promising modality for small bowel imaging. However, the role of CTE in the evaluation of obscure gastrointestinal bleeding (OGIB) has not been established. We investigated the efficacy of CTE in diagnosing OGIB and the long‐term outcomes based on CTE findings, with special reference to negative CTE.</P><P><B>Methods: </B> A total of 63 consecutive patients who had undergone CTE for OGIB were enrolled, and their pre‐ and post‐CTE clinical data were collected. “Specific treatments” were defined as treatments directly aimed at resolving presumed bleeding causes, including hemostasis and operation, while “non‐specific treatments” were defined as symptomatic treatments for anemia.</P><P><B>Results: </B> Among 60 patients for whom long‐term follow‐up data were available, positive lesions were found in 16 patients (26.7%). The overall rebleeding rate was 21.7% during a mean follow up of 17.6 ± 4.7 months. There was no significant difference in the cumulative rebleeding rates between patients with positive and negative CTE results (<I>P</I> = 0.241). All patients who received specific treatments after CTE did not rebleed (0/8). In positive CTE patients, specific treatments significantly reduced the rebleeding rate (<I>P</I> = 0.023).</P><P><B>Conclusions: </B> CTE has a high rate of detecting overt OGIB. However, negative CTE results do not predict lower long‐term rebleeding, and such patients with OGIB should be closely observed. In patients with positive CTE, more vigorous management significantly reduces the incidence of rebleeding.</P>

        켄터키 블루그래스(Poa pratensis L.) 종자의 보증 기준에 따른 품질 분류와 적용

        종자보증의 목적은 종자의 유전적 순수성과 품종의 혈통을 보장하는 것이다. 본 연구는 골프장에 사용되는 켄터키 블루그래스 종자의 품질 보증 규정과 절차에 대한 정보를 제공하기 위하여 행하여졌다. 연구 방법은 미국3개주(와싱턴, 아이다호, 오리건주)의 종자 보증과정과 종자 품질 보증 규정을 비교 분석하였다. 보증과정은 포장과 실험실 내에서의 최소 종자품질기준을 만족하여야 한다. 종자 수확포장은 정해진 수준의 종자를 파종하여야 하고, 다른작물로부터 일정거리를 격리하여야 한다. 더구나 수확포장은 청결하며 금지잡초로부터 안전하여야 한다. 종자보증 시험은 발아율, 순수종자 백분율, 이종종자, 잡초 및 이물질 함량 검사를 포함한다. 품질보증종자와 뗏장용 종자의 품질기준은 3개 주에서 비슷하였다. 뗏장용 종자의 품질기준은 최대 0.02%, 품질보증종자의 경우에는 0.3% 이내의 잡초종자 함량을 허용 기준치로 제한하고 있다. 이러한 종자품질기준은 종자의 품질을 보장하고 골프코스의 잔디조성 품질을 보장하고 있다. The purpose of seed certification is to preserve the genetic purity and identity of seed varieties. This study is to provide information concerning seed certification procedures and certification standards of Kentucky bluegrass especially used in golf courses. We analyzed data from the seed certification standards of three states (Washington, Idaho and Oregon) in U.S.A. The certification processes both field inspection and laboratory requirement satisfying the minimum seed quality standards. The seed harvesting field must be propagated with the specified class of seeds and requires an adequate isolated distance from other crops. Moreover, the field should be clean and free from the objectionable weeds. The seed analysis tests include a germination rate, a percentage of pure seed, contents of other crop seed, weed seed, and inert matter. The certification standards of the certified seed and the sod quality seed showed general similarity in all three states. The certification standards of the sod quality seed should have less than 0.02% of maximum weed seed. The certified seed should have less than 0.3% of maximum weed seeds. Those certification standards of seed quality should guaranty the quality of turfgrass establishment of golf course.

        Adaptive Transition of Aquaporin 5 Expression and Localization during Preimplantation Embryo Development by In Vitro Culture

        Adaptive development of early stage embryo is well established and recently it is explored that the mammalian embryos also have adaptive ability to the stressful environment. However, the mechanisms are largely unknown. In this study, to evaluate the possible role of aquaporin in early embryo developmental adaptation, the expression of aquaporin (AQP) 5 gene which is detected during early development were examined by the environmental condition. To compare expression patterns between in vivo and in vitro, we conducted quantitative RT-PCR and analyzed localization of the AQP5 by whole mount immunofluorescence. At in vivo condition, Aqp5 expressed in oocyte and in all the stages of preimplantation embryo. It showed peak at 2-cell stage and decreased continuously until morula stage. At in vitro condition, Aqp5 expression pattern was similar with in vivo embryos. It expressed both at embryonic genome activation phase and second mid-preimplantation gene activation phase, but the fold changes were modified between in vivo embryos and in vitro embryos. During in vivo development, AQP5 was mainly localized in apical membrane of blastomeres of 4-cell and 8-cell stage embryos, and then it was localized in cytoplasm. However, the main localization area of AQP5 was dramatically shifted after 8-cell stage from cytoplasm to nucleus by in vitro development. Those results explore the modification of Aqp5 expression levels and location of its final products by in vitro culture. It suggests that expression of Aqp5 and the roles of AQP5 in homeostasis can be modulated by in vitro culture, and that early stage embryos can develop successfully by themselves adapting to their condition through modulation of the specific gene expression and localization.
