        스티렌 폭로 근로자들의 기중 및 혈중 스티렌과 요중 만델산의 관계 분석

        The concentration of styrene in air in blood and mandelic acid in urine were checked for the 60 workers with normal liver function, exposed to styrene. Styrene in air were sampled with personal air sampler at least 4 hours and analyzed by gas chromatography. Blood and spot urine were collected at the end of shift with a vacuum tube and a polythylene bottle and analyzed by gas chromatography and high performance liquid chromatography. Means of air and blood styrene and urine mandelic acid were 8.16 ppm(geometric mean), 0.199 mg/L, and 0.519 g/g creatinine, respectively. The concentration of styrene in air and mandelic acid in urine were high in the FRP factories and low in polymerization factory. Styrene in blood showed large difference by the working process. Styrene in air showed a good correlation with mandelic acid in urine(r=0.6369) and styrene in blood(r=0.6371). The mandelic acid in urine and styrene in blood corresponded to exposure of 50 ppm styrene were 0.890 g/g creatinine and 0.434 mg/L. However, hippuric acid in urine did not show any correlation with styrene in air. Urine mandelic acid excretion expected ratio showed a tendency to decrease according to obesity index and to increase with alcohol consumption.

        Fibrinolytic activity of fermented kimchi and ingredients used for the preparation were assayed by measuring the lysis area on plasminogen-rich fibrin plate. Fermented kimchi and pickled fish sauces from changlan, prwan, and anchovy showed the activity, and the activity of pickled fish sauces were high in the order of pickled changlan, pickled anchovy(pickled mulchi), and pickled prawn. However, the activity of kimchi may not be attributed to pickled fish sauce because kimchi containing fish sauce did not possess activities before fermentation. To investigate fibrinolytic agents in the kimchi if protein, the activities were determined from the samples heated for 30min at 100℃. There was no changes in the activities before and after heat treatment indicating the agents may be components other than protein. Since major changes occur during kimchi fermentation were increased sour taste due to production of organic acids such as lactic, citric, oxalic, and succinic acids, the authentic organic acids present were examined for fibrinolytic activities. The results indicates that the major component posses the activity is lactic acid.

        Ultrasensitive and selective C<sub>2</sub>H<sub>5</sub>OH sensors using Rh-loaded In<sub>2</sub>O<sub>3</sub> hollow spheres

        <P>Rh-loaded In<SUB>2</SUB>O<SUB>3</SUB> hollow spheres with diameters of ∼2 μm were prepared by a one-pot hydrothermal reaction of aqueous solution containing indium nitrate, rhodium chloride, and glucose and subsequent heat treatment at 500 °C for 2 h. The response to 100 ppm C<SUB>2</SUB>H<SUB>5</SUB>OH (<I>R</I><SUB>a</SUB><I>/R</I><SUB>g</SUB>, <I>R</I><SUB>a</SUB>: resistance in air, <I>R</I><SUB>g</SUB>: resistance in gas) of 1.67 at% Rh-loaded In<SUB>2</SUB>O<SUB>3</SUB> hollow spheres was 4748, which was ∼180 times higher than that of pure In<SUB>2</SUB>O<SUB>3</SUB> hollow spheres. Rh loading decreased the temperature for maximum gas response from 475 °C to 371 °C, which also enhanced the selectivity to C<SUB>2</SUB>H<SUB>5</SUB>OH 15.1–24.7 times and recovery speed. The ultrahigh sensitivity and selectivity to C<SUB>2</SUB>H<SUB>5</SUB>OH, the lower sensing temperature, and the reduced recovery time were attributed to electronic interactions between Rh and In<SUB>2</SUB>O<SUB>3</SUB> and the promotion of catalytic dissociation of C<SUB>2</SUB>H<SUB>5</SUB>OH into reactive gases.</P> <P>Graphic Abstract</P><P>The Rh-loaded In<SUB>2</SUB>O<SUB>3</SUB> hollow spheres with ultrahigh response and selectivity to C<SUB>2</SUB>H<SUB>5</SUB>OH were prepared by glucose-mediated, one-pot hydrothermal reaction and subsequent removal of core carbon by heat treatment. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c1jm14252f'> </P>

      • 닭에서 Ethanol의 中樞作用이 血漿內 葡萄糖 濃度에 미치는 影響

        The present experiments were undertaken in an attempt to elucidate the central effect of ethanol on the plasma glucose level in the hen. After intravenous administration of 30mg/kg ethanol to control group, plasma glucose concentration was not affected but over 100mg/kg, it induced hypoglycemia. Hypoglycemic effect of intravenous ethanol was potentiated by adrenergic blocking agents. Intraventricular administration of 30mg/kg ethanol resulted in a significant increase in the plasma glucose level, and this hyperglycemic effect of intraventricular ethanol was inhibited by regitine, propranolol, or reserpine. It suggests that metabolic effect of ethanol on the plasma glucose concentration in the hen is mediated partially by the activation of sympathetic center.

        To evaluate the in-vivo effects of mitomycin-C-polyglycolic acid composite(MMC-PGA) the disk: shaped composite were implanted into the peritoneal cavity of the Sprague-Dawley rats. The pathologic changes were examined at various time points up to 12 weeks. Initially the soft tissue around the inserted disks showed capillary congestion at 3 days. Inflammatory infiltrates with foreign body giant cells appeared from the 2nd week and reached peak response at 6-8 weeks. These reaction diminished prominently at 12 weeks. No specific pathologic change was found in the liver, the kidneys, and the heart. The above results suggest that the MMC-PGA composite can serve as a new device for intraperitoneal chemotherapy of various types of cancers.

        National survey of Korean hepatobiliary-pancreatic surgeons on attitudes about the enhanced recovery after surgery protocol

        Backgrounds/Aims: The purpose of this study was to investigate attitudes regarding the Enhanced Recovery After Surgery (ERAS) protocol of hepato-biliary-pancreatic (HBP) surgeons in Korea and the extent to which they use the protocol for perioperative management. Methods: An online survey was conducted among members of the Korean Association of Hepato-Biliary-Pancreatic Surgery (KAHBPS) for eight weeks beginning on August 2019. The questionnaire, which was written in Korean, was based on the latest ERAS guidelines. Total responses were collected from 127 surgeons. Results: Of the 127 total respondents, the largest proportion (44.9%) were working in Seoul. In terms of established in-hospital clinical pathways (CP), 19.7% of the participating surgeons had and followed a CP in pancreaticoduodenectomy (PD) and 21.3% in hepatectomy. Regarding the ERAS protocol for each surgery, four items (18.2%) regarding PD and seven items (35.0%) related to hepatectomy were followed by more than 50% of respondents. Conclusions: ERAS guidelines are one of the consensuses for better recovery in perioperative management of patients undergoing major surgeries and encompass the overall process of patient recovery including patient education, pain control, physiologic balance, and perioperative nutrition. A novel project is needed to successfully implement an evidence-based enhanced recovery strategy.

        새로운 물질이 개발되어 신약으로 탄생하기까지 많은 시간과 노력, 예산이 필요하다. 최근, 우리 나라에서도, 신약캐발이 어릴고 힘든 분야이지만 새로운 신약개발을 총해 얻어지는 분가가치가 막대함을 인식, 연구에 박차를 가하 있는 실정이다. 본 연구는, 의약품 개발 선진국인 미국 FDA에서 발행하는 참고자료 중 항균제 임상편가에 대한 가이드라인 및 주요 임상적응증별 항균제 기발(지역사획회득성 폐렴, 원내감염성 계렴, 급성기관지염의 2차세균감염, 만성기지염의 급성세균성 악화, 연쇄구균에 의한 인두영 및 편도염 , 단순성 요로감염증, 복합성 요로감염증 및 신우신영, 급성 세균성 부비동옆, 항 바이러스제 개발시 전임상단계에서 고려사항, 항 바이러스제 허가와 된련 잉상적 고려사항, 카레타 괸련 혈류감염)에 있어서의 파이드라인 등의 자료를 소개함으로져, 향후 제약사의 항균제 신약개발 및 허가 신청된 의악품의 전임상차료 및 잉상시험자료의 검토 평가업무에 참고자료로 활용될 수 있을 것이다. Changing or unclear interpretations of clinical trial data needed to demonstrate the effectiveness and safety of antimicrobial drug products have a times led to confusion and frustration on the part of both applicants and division of these drug products reviewers. The FDA published guidance on desing clinical protocols, implementing, and analyzing data from clinical trials for antimicrobial drug products has been presented and additional companion guidances introduced specific issues to individual infections. This document provides applicants and reviewers with minimal information appropriate for the clinical development of routine antimicrobial drug products and identifies issues common to many antimicrobial drug applications. The agency can use the kind of information to determine whether the antimicrobial drug product under study is safe and effective in the treatment of the specific infection studied.

        Isolation and Characterization of Micrococcaceae in Dental Clinic

        It has been reported that staphylococci present in clinic and personnel are transmissible to patients. The purpose of this study is to ascertain the distribution. the drug resistance and plasmid pattern in Micrococcaceae isolated in dental clinic. A dental clinic was monitored in the morning in the afternoon and in the evening. S. hemolyticus was isolated from the cover of instruments and S. hominis was isolated from the margin of tray in the morning. S. epidermidis was isolated from light switch and head rest. S. aureus from cuspidor and head rest. and coagulase-negative staphylococci from suction tip in the afternoon. S. aureus was isolated from head rest and coagulase-negative staphylococci was isolated from suction rubber tip in the evening. Micrococci were isolated from the forceps tip and the switch in the evening. Staphylococci isolated in the afternoon showed susceptibility to ceftriaxone. and those isolated in the evening to ampicillin. carbenicillin. tetracyclin. gentamycin. amikacin. cephalothin. ceftriaxone and cefoxitin. Micrococcus isolated in the evening showed susceptibility to 15 antibiotics. In the analysis of plasmid. there were two clear bands about 4.3 kb, in S. aureus isolated from the head rest in the afternoon. These results show that Micrococcaceae can become ambient in dental clinic. and staphylococci isolated then show antibiotic resistance to some drugs.

        간장 ( 肝臟 ) 및 담도 ( 膽道 ) : 간세포암에 대한 화학색전요법의 항종양 효과

        N/A Eighty-seven TACEs were performed in 38 patients with hepatocellular carcinoma: one in 16 patients; twice in seven patients; three times in seven patients; four times in four patients; and five times in four patients. In all patients, TACE was performed with adriamycin, mitomycin-C and lipiodol. In 21 patients, embolization with gelfoam powders were added following TACE. Six patients had the hepatocellular carcinoma of 4.0~4.9 cm in diameter: 14 patients, 5.0~9.9 cm; and 18 patients, larger than 10 cm. Response rate was evaluated in 33 patients, among whom fifteen patients (45$) achieved response (complete response, 6%; partial response, 33%). The cumulative survival rates at one year and two years for 38 patients were 55.3$ and 23.2%, respectively, with the median survival time of 12.0 months. Sex, ascites, HBsAg, esophageal varix, liver cirrhosis, gelfoam embolization, serum albumin level, size of the tumor, and a-fetoprotein were analyzed to investigate the effect of TACE on survival time, demonstrating that no significant differences in the survival times were observed between these factors except HBsAg. Theses results suggest that TACE is not curable for hepatocellular carcinoma, however prolong the life span in patients with hepatocellular carcinoma.

        흉부방사선치료후 발생된 심낭염 및 심근섬유화 1 예

        N/A The heart was considered to be relatively resistant to ionizing radiation in the range of doses used in radiation therapy before systemic floolw up and review of a large number of patients who had undergone radiation therapy to the thorax and survived for at least several years. Various types of radiation-induced heart disease were reported and a clear dose response was evident for the degree and type of damage. In some settings, the heart has become recognized as the dose-limiting organ for radiation therapy directed to thoracic neoplasms and the tolerable dose may be further reduced by concomitant or sequential use of anticancer chemotherapeutic agents, especially doxorubicin (Adriamycin). In this article, we report e case of radiation-induced pericar-ditis and myocardial fibrosis in a patient with breast cancer who received postoperative adjuvant radiotherapy. A 43-year-old woman was admitted to the hospital because of progressive dyspnea and pitting edema. She underwent a simple mastectomy and axillary dissection due to left breast cancer (T2NIMO) in May 1979. After adjuvant radiotherapy for 1 1/2 months, she was followed up without further treatment and there was no evidence of local recurrence and systemic metastasis. In Aug. 1986, minimal pericardial effusion was noticed for the first time. Thereafter, exertional dyspnea developed and progressed slowly. In Dec. 1988, she suffered from cardiac tamponade and an emergency pericardiostomy was performed. The pericardial fluid was bloody but there was no evidence of local tumor invasion or granulomatous disease such as tuberculosis. Only extensive fibrosis of the parietal pericardium was observed, and also in that time, there was no other evidence of recurrence of breast cancer in spite of extensive work up. Under the impression of possible malignant effusion, tamoxifen (10 mg P.O. bid) was prescreibed but pericardial sclerosing therapy was not performed. After discharge, exertional dyspnea reappeared soon and abdominal distension with pitting edema was also noticed. Reaccumulation of massive pericardial effusion was confirmed by echocardiography and symptomatic treatment including diuretics was tried with improvement. In follow-up echocardiography done at OPD, pericardial effusion disappeared, but pulsating distension of the jugular vein and hepatomegaly were persistently observed. In Feb. 1989 she underwent confirmatory diagnostic procedures including cardiac catheterization and endomyocardial biopsy under the impression of radiation-induced heart disease. Cardiac catheterization revealed prominent y descent of right atrial pressure and equalization of right ventricular and left ventricular diastolic pressure. Histologic and electron microscopic examination of the endomyocardial biopsy showed moderate myocardial fibrosis and activation of endothelial cells which effaced some capillary lumens. Inflammatory changes were meager and there was no evidence of myocardial necrosis, which is consistent with a radiation effect. She is under OPD follow up with digitalis, captopril and small doses of the diuretics in functional class II of the New York Heart Association. Among various types of radiation-induced heart disease, chronic pericardial effusion in the cancer patient who received thorax radiation therapy is the most important to the attending physician. It should be deter-mined whether the pericardial effusion is due to invasion of malignancy or radiation therapy because the appropriate management is different, In some cases, constrictive pericarditis and myocardial fibrosis can develop after pericarditis, and cardiac catheterization and/or endomyocardial biopsy is required for correct diagnosis.
