        In-mold injection molding system is generally an advanced version of the injection molding system. The injection, printing and coating processes of In-mold injection molding take place simultaneously. The technology is mostly used in home appliances, electronic goods and cell phones. For this research we applied the In-mold technique for injection molding and process of 3D nurb-surfaces. We performed the computer simulation of injection molding to optimize the In-mold processes and identified the credibility of the products through performance evaluation.

        체장 β세포에 지방이 축적되는 lopotoricity 현상은 인슬린 비의존성 당뇨 (Non InsuBin Dependent Diabetes Mellitus :NIDDM)의 발생과 상당히 밀접한 관계가 있으며, 췌장 β세포의 lipoapoptosis는 세포막형성에 관여하는 sphingomyelin의 전군체안 ceramide의 과생산에 의해 유도된다는 보고가 있다. 그러나, 지방산 종류에 따른 췌장 β세포에서의 세포사멸 (apoptosis)유발 기작에 대해서는 야직까지 보고된 바가 없파. 또한, 지방세포에서 생성되며, 지질대사, 음식섭취 및 체중조절에 관여하는 렙틴 (leptin)과 렙틴 수용체와의 관계 역시 체장 세포에서 명착하게 밝혀져 있지 않다. 따라서 본 연구에서는 6주령 53와 Zucker rat의 췌장으로부터 췌도를 분리하는 기술을 확립하고, 배양된 췌장 췌도 세포와 MS-1세포주에 불포차즌방산 (n-3 linolenic acid; n-6 :linoleic acid), 포화지방산 (stearic acid), conjugated linoleic acid (CLA)를 포함하는 4 가지의 지방산을 처리한 후 이들이 췌장 』세포의 사멸에 미치는 영향을 다양한 apoptosis 관련 유전자의 발현을 분석함으로써 확인하고자 하였다. 그 결과 SD rat으로부터 마리 당 평균 약 500개의 췌장 췌도를 분리하는데 성공하였으며, 4 종류의 지방산 중 포화지방산인 stearic acid를 처리하여 ONA fragmentation 증가를 관찰하였다. 또한 RT-PCR 방법을 이용하여 분리된 췌도 세포에사의 Bcl-2, Bax, iNOS가 지방산의 종류에 따라 서료 다른 발현양상을 나타낸다는 」라실을 관귿하였다. 이러한 떤구 결과는 지방산의 종류에 따라 췌장 췌도 세포에서의 세포사멸 유발 효과를 겋증하였으며, 이러한 연구결과는 복잡한 당뇨발병 원인 규명에 중요한 정보를 제공할 것으로 사료된다. Fat contents within the pancreatic islets are closely related to lipotoxicity which induces apoptosis, resulting in non insulin dependent mellituse(NIDDM). High level of fat may contribute to both insulin resistance and obesity in rodent and humans but the evidence is limited. To determine whether dietary fatty acids affect pancreatic cell function, the INS-1 cells and the pancreatic islets isolated from SD and Zucker obese(fa/fa) rats(6 wk, prediabetic stage) were cultured with stearic acid and CLA. As a result, DNA fragmentation laddering was substantially decreased in the INS-1 cells and the isolated pancreatic islets cultured with 2mM CLA compared with those with stearic aced. To investigate the mechanism by which CLA alleviates cell apoptosis as indicated by DNA fragmentation assay, we examined mRNA expressions of apoptosis-related proteins including Bax and Bcl-2 associated with cell death agonist and antagonist, respectively in the both INS-1 cells and islets cultured with 2 mM fatty acids. Bax mRNA expression was not markedly altered by either stearic acid or CLA, whereas Bcl-2 mRNA expression was noticeably enhanced by CLA compared with stearic acid. However, there were no changes in cell apoptosis and cell apoptotic-regulating gene products in the both INS-1 cells and isolated islets treated without or with 2 mM CLA. It is concluded that CLA may have a beneficial effect for maintenance of cell viability via increased Bcl-2 protein expression compared with steraic aced, which lead to delay or prevent, at least somewhat, the onset of NIDDM in the physiological status.

        Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

        Background : In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. Objective & Methods : For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results : In 1997. 99 strains of N. gonorrhoeae were isolated, among which 45(45.5%) were PPNG. Conclusion : The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 45.5% in 1997.

        Enhanced activation of NAD(P)H: quinone oxidoreductase 1 attenuates spontaneous hypertension by improvement of endothelial nitric oxide synthase coupling via tumor suppressor kinase liver kinase B1/adenosine 5&vprime;-monophosphate-activated protein kina

        AIMS:: Guanosine 5&vprime;-triphosphate cyclohydrolase-1 (GTPCH-1) is a rate-limiting enzyme in de-novo synthesis of tetrahydrobiopterin (BH4), an essential cofactor for endothelial nitric oxide synthase (eNOS) coupling. Adenosine 5&vprime;-monophosphate-activated protein kinase (AMPK) is crucial for GTPCH-1 preservation, and tumor suppressor kinase liver kinase B1 (LKB1), an upstream kinase of AMPK, is activated by NAD-dependent class III histone deacetylase sirtuin 1 (SIRT1)-mediated deacetylation. &bgr;-Lapachone has been shown to increase cellular NAD/NADH ratio via NAD(P)H:quinone oxidoreductase 1 (NQO1) activation. In this study, we have evaluated whether &bgr;-lapachone-induced NQO1 activation modulates blood pressure (BP) through preservation of GTPCH-1 in a hypertensive animal model. METHODS AND RESULTS:: Spontaneously hypertensive rats (SHRs), primary aortic endothelial cells, and endothelial cell line were used to investigate the hypotensive effect of &bgr;-lapachone and its action mechanism. &bgr;-Lapachone treatment dramatically lowered BP and vascular tension in SHRs and induced eNOS activation in endothelial cells. Consistent with these effects, &bgr;-lapachone treatment also elevated levels of both aortic cGMP and plasma nitric oxide in SHRs. Meanwhile, &bgr;-lapachone-treated SHRs showed significantly increased levels of aortic NAD, LKB1 deacetylation, and AMPK Thr phosphorylation followed by increased GTPCH-1 and tetrahydrobiopterin/dihydrobiopterin ratio. In-vitro study revealed that AMPK inhibition by overexpression of dominant-negative AMPK nearly abolished GTPCH-1 protein conservation. Enhanced LKB1 deacetylation and AMPK activation were also elicited by &bgr;-lapachone in endothelial cells. However, inhibition of LKB1 deacetylation by blocking of NQO1 or SIRT1 blunted AMPK activation by &bgr;-lapachone. CONCLUSION:: This is the first study demonstrating that eNOS coupling can be regulated by NQO1 activation via LKB1/AMPK/GTPCH-1 modulation, which is possibly correlated with relieving hypertension. These findings provide strong evidence to suggest that NQO1 might be a new therapeutic target for hypertension.

        Is renal replacement therapy necessary in deceased donor liver transplantation candidates with hepatorenal syndrome?: a 2-year experience at a high-volume center

        Purpose: Hepatorenal syndrome (HRS) is a fatal complication in patients with end-stage liver disease awaiting liver transplantation (LT). HRS often develops in patients with high model for end-stage liver disease (MELD) score. This study investigated the outcomes of peritransplant management of HRS in a high-volume LT center in Korea for 2 years. Methods: A total of 157 recipients that deceased donor liver transplantation (DDLT) from January 2017 to December 2018 were included. In-hospital mortality (IHM) was analyzed in relation to pre- and posttransplant application of renal replacement therapy (RRT). Results: Primary diagnoses for DDLT were alcoholic liver disease (n = 61), HBV-associated liver cirrhosis (n = 48), retransplantation for chronic graft failure (n = 24), and others (n = 24). Mean MELD score was 34.6 ± 6.2 with 72 patients at Korean Network for Organ Sharing MELD status 2 (45.9%), 43 at status 3 (27.4%), 36 at status 4 (22.9%), and 6 at status 5 (3.8%). Pretransplant RRT was performed in 16 patients (10.2%) that did not show IHM. Posttransplant RRT was performed in 69 patients (44.0%), for whom IHM incidence was 15.9%. In 53 patients that had undergone de novo posttransplant RRT, IHM incidence increased to 20.8%. IHM in the 88 patients not requiring RRT was 2.3%. Conclusion: The majority of adult DDLT recipients in Korean MELD score-based allocation system have very high MELD scores, which is often associated with HRS. Pretransplant RRT appears to improve posttransplant survival outcomes. We thereby recommend that, if indicated, pretransplant RRT be performed while awaiting DDLT.

        법조인 선발제도별 비용분석과 진입유인 분석

        본 연구의 목적은 기존 사법시험이 갖는 소위 ‘고시낭인’이 되어 역기능으로서 사회가 부담하고 있는 사회적비용을 감소시키고 법조인의 다양성과 전문적 지식을 법에 도입하기 위한 제도로서 법학전문대학원(law school; 로스쿨: 이하 ‘법전원’)이 갖는 장점에도불구하고 법전원의 고비용구조가 우리 국민 모두가 진입할 수 있는 법조영역에 대한 진입장벽이 되어 특정 소득계층이 진입하지 못하는 문제가 과연 존재할 수 있는지의 여부를 분석하고자 하는데 있다. 법전원이 출범했음에도 불구하고 지금까지 구체적인 분석이 이루어지지 않고 막연한 추측과 여론에 의하여 고비용구조이기 때문에 법조인이 되는데 진입장벽이 되고, 오히려 기존의 사법시험을 존치하여 문제를 해결하여야 한다는 주장이 많이 제기되고 있는 실정이다. 현재 국회에는 김용남 의원 등이 발의한 사시 존치 관련 법안 4건이 계류되어있다. 여기에 오신환 의원이 대표발의한 사시 존치 법안이 지난 6월 제출되어 법안소위 회부를 기다리고있는 상태다. 그러나 기존의 사법시험과 비교하여 얼마나 많은 비용차이가 발생하는지 구체적인 분석은 이루어진 적이 없다. 본 분석의 목적은 기존 사법시험제도와 법전원체제에 대한 가치판단의 문제에 대한 접근은 제외하고, 순수한 비용분석에 의하여 국민소득계층에서 법조계에 진입하지 못하는 소득계층이 존재하는지 여부를 비교분석하는데 있으며, 만일 존재한다면 어떠한 계층이 진입할가능성이 적은지를 분석하였다. 이러한 분석은 국민의 교육과 직업기회에 대한 형평성과 법조분야의 발전에 매우 중요한 의미를 갖는다고 할 수 있다. 따라서 본 비용분석은 법전원 출신의 변호사시험과 기존 사법시험과의 분석에서 측정 불가능한 요인은 배제하고, 측정 가능한 범위 내에서 비용의 총액과 차액분석을 통하여 비교하고, 법조인이 되기 위한 기회비용을 이용하여 비용의 차이를 비교 분석하여 소득계층의 진입가능여부의 판단기준을 제시한다는 것은 원가(비용)분석의 사례로서 큰 의미가 있다고 생각한다. The goal of this cost analysis is to explore and analyze, despite the advantages the new ``law school`` system, which reduces the social costs of the ‘entrance exam vagabond’ problem by solving this social issue the previous judicial bar entrance exam had, and allows the for the diversification of the pool of applicants and encourages professional knowledge in the law industry, if in fact there is a possibility that the cost structure of law school serving as an entrance barrier for certain socio-economic groups to apply to be a Judicial Officer is a problem that exists. Even though the law school system was established, there has not yet been a detailed analysis, and the reality is that through vague guesswork and media spin, the system is blamed as being of high cost and therefore the cause for being a high barrier of entry to certain socioeconomic classes to become a Judicial Officer, and that it also retains the problems of the previous judicial bar examination, so there are many opinions that these problems of the new law school system need to be solved. However, there has yet to have been a detailed cost comparison analysis to explore how much the cost difference is with the old judicial bar examination system. The goal of this analysis is in the case where the previous judicial bar examination has been abolished and the law school system goes into full effect, excluding the qualitative comparison of which selection process is more practical in turning out Judicial Officers, to evaluate from a strictly cost analysis point of view whether the law school system serves as a barrier of entry to any socioeconomic group. And if in fact this barrier exists, to analyze which socioeconomic group has the least chance of being able to enter. Such analysis can be seen as having significant meaning in supporting the fairness of Korea’s education system and job opportunity system and also the advancement of the Judicial Officer selection and law industry. Therefore, this study excludes any elements in the current law school entrance exam system and the previous judicial bar examination system that cannot be accurately quantified, and compares the two systems within the bounds of quantifiable ranges, such as the analysis of the total and differential costs, and also the opportunity cost of becoming a Judicial Officer. All these factors are used for comparative analysis, and to present an official set of criterion that can be used to predict the chance of a certain socioeconomic group being successfully selected as a Judicial Officer has significant meaning as a case study of cost analysis. If the law school system continues, a phenomenon in which the entry of individuals into legal circles is determined according to their economic income quantiles will set in. Therefore, various reviews on the lawyers training system should be performed. In relation to the costs of training lawyers, critics about the national judicial examination point out the seriousness of the problem of social opportunity costs due to numerous “unemployed individuals produced after failing in the examination” because the acceptance ratio remains at 3%. The costs borne by examinees who prepare for the existing national judicial examination are explicit costs that are borne by the examinees themselves. As they are willing to pay the costs, these costs can be viewed as avoidable costs. However, the costs borne by examinees for law schools are imputed costs under a system, and therefore, are more of unavoidable costs. Operating costs borne by universities to operate law schools, and scholarships and other costs borne by universities to meet the conditions required to operate law schools are indirect costs, and thus, these may be categorized as indirect opportunity costs borne by society. In addition, we should be aware that if the bar examination following the law school system continues to be operated for a long time, opportunity costs incurred by declines in the acceptance ratio should be borne. This study conducted an analysis that is limited to the analysis of costs that will actually be incurred or are projected to be incurred while excluding overall axiological judgements on the lawyers training system or discussions on the appropriateness of the system. Therefore, it should be noted that there can be a number of variations according to circumstantial changes. However, there was a limitation in the cost analysis. For example, the operating costs of law schools borne by universities and implicit social indirect costs were excluded from the analysis because it was actually impossible to obtain data on these costs or measure them.

      • 공간정보에 기반한 도로 데이터 자동생성 방법

        효율적인 도로정보의 구축은 GIS에서 가장 중요한 주제이다. 본 논문에서는 도로정보를 자동으로 생성, 구축하기 위하여 모바일 매핑 기술과 영상인식 기술을 결합한 방법을 제안하였다. 모바일 매핑 시스템은 CCD 카메라, GPS, INS를 장착한 차량의 형태를 가지며, 취득한 영상에 나타난 공간객체의 좌표를 사진측량기법을 이용하여 계산한다. 모바일 매핑 시스템에 의한 공간객체 좌표추출과 데이터 구축은 자동화되지 않아 시간이 많이 드는 단점이 있다. 도로의 자동 인식은 영상인식 분야에서도 자동주행차량에 대한 연구의 형태로 진행되어 왔다. 그러나 영상인식에 기반한 방법들은 도로 차선에 적용할 경우 차선의 끊김 차량에 의한 가려짐 좋지 않은 날씨와 조명 등 실제의 도로나 도로변의 다양한 예외상황 때문에 원하는 결과를 얻기 힘든 경우가 많다. 이러한 단점을 보완하기 위하여 본 논문에서는 모바일 매핑 시스템으로부터 획득된 GPS/INS 데이터 및 영상인식 기술을 모두 이용한 자동 도로데이터 생성방법을 제안하였다. 영상에 나타난 도로 차선의 3차원좌표로부터 영상에서 객체가 나타날 위치를 추정하기 위한 방법을 고안하였으며, 이러한 방법은 도로 차선을 찾기 위한 복잡한 영상처리 과정을 대폭 줄일 수 있다. 예외상황 때문에 도로차선을 추출하지 못한 경우에는 스플라인 인터폴레이션에 의하여 값을 얻는다. 인터폴레이션은 교차로나 급격한 변화 지점에 따라 구분된 도로 구간 단위로 이루어진다. 본 논문에서는 제안된 객체좌표 추정방법과 인터폴레이션 기법에 대한 실험 및 결과를 제시하였다. VEfficient generation of road data is one of the most important issues in GIS (Geographic Information System). In this paper, we propose a hybrid approach for automatic generation of road data by combining mobile mapping and image processing techniques. Mobile mapping systems have a form of vehicle equipped with CCD camera, GPS, and INS. They can calculate absolute position of objects that appear in acquired image by photogrammetry, but it is labor-intensive and time-consuming. Automatic road detection methods have been studied also by image processing technology. However, the methods are likely to fail because of obstacles and exceptive conditions in the real world. To overcome the problems, we suggest a hybrid method for automatic road generation, by exploiting both GPS/INS data acquired by mobile mapping system and image processing algorithms. We design an estimator to estimate 3-D coordinates of road line and corresponding location in an image. The estimation process reduces complicated image processing operations that find road line. The missing coordinates of road line due to failure of estimation are obtained by cubic spline interpolation. The interpolation is done piecewise, separated by rapid change such as road intersection. We present experimental results of the suggested estimation and interpolation methods with image sequences acquired by mobile mapping system, and show that the methods are effective in generation of road data.

      • Optimal Timing of Percutaneous Coronary Intervention in Patients With Non–ST-Segment Elevation Myocardial Infarction Complicated by Acute Decompensated Heart Failure (from the Korea Acute Myocardial Infarction Registry-National Institutes of Healt

        <P>The optimal timing of percutaneous coronary intervention (PCI) in patients with non–ST-segment elevation myocardial infarction (NSTEMI), complicated by acute decompensated heart failure (ADHF), is unclear. A total of 1,027 patients with NSTEMI complicated by ADHF who underwent successful PCI were analyzed using a Korean multicenter registry. All patients were divided into 4 groups by the timing of PCI: group 1 (PCI < 2 hour after admission, n = 149), group 2 (2 to 24 hours, n = 577), group 3 (24 to 72 hours, n = 189), and group 4 (≥72 hours, n = 112). We analyzed the incidences of 12-month mortality, nonfatal myocardial infarction (MI), target-vessel revascularization, and rehospitalization because of HF. The prevalence of ADHF in patients with NSTEMI was 15.2% at initial presentation, and in-hospital mortality was higher in group 1 than in the other groups. There were no significant differences in mortality, nonfatal MI, target-vessel revascularization, or rehospitalization for HF during the 12-month follow-up between groups, regardless of initial PCI timing, except for a higher 12-month mortality in patients who received PCI within 24 hours (vs ≥24 hours) (hazard ratio 1.52, 95% confidence interval 1.09 to 2.29, p = 0.046). Early PCI did not reduce adverse clinical outcomes in patients with NSTEMI complicated by ADHF. Delayed PCI after stabilization may be reasonable in such high-risk patients.</P>

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Acute hyperglycemia on admission is common in acute myocardial infarction (AMI) patients regardless of diabetic status, and is known as one of prognostic factors. However, the effect of hyperglycemia on non-diabetic patients is still on debate.</P> <P><B>Methods</B></P> <P>A total of 12,625 AMI patients (64.0±12.6years, 26.1% female) who were enrolled in Korea Acute Myocardial Infarction Registry-National Institute of Health between November 2011 and December 2015, were classified into 4367 diabetes (65.4±11.6years, 30.4% female) and 8228 non-diabetes (63.3±13years, 23.9% female). Patients were analyzed for in-hospital clinical outcome according to admission hyperglycemic status.</P> <P><B>Results</B></P> <P>In diabetic patients, independent predictors of in-hospital mortality were old age, high HbA<SUB>1</SUB>C, pre-Thrombolysis In Myocardial Infarction (TIMI) flow 0, left ventricle ejection fraction<40%, cardiogenic shock and ventricular tachycardia. In non-diabetic patients, independent predictors of in-hospital mortality were old age, high admission glucose (≥200mg/dL), pre TIMI flow 0, failed percutaneous coronary intervention, low left ventricle ejection fraction<40%, cardiogenic shock, stent thrombosis and decreased Hb≥5g/dL. In hospital mortality was significantly higher in diabetic patients compared to non-diabetic patients (5.0% vs. 3.4%, <I>p</I> <0.001). However, non-diabetic patients with hyperglycemia have significantly higher mortality compared to diabetic patients (17.4% vs. 7.2%, <I>p</I> <0.001). Comorbidity including cardiogenic shock (<I>p</I> <0.001), cerebral hemorrhage (<I>p</I> =0.012), decreased Hb≥5g/dL (<I>p</I> =0.013), atrioventricular block (<I>p</I> <0.001) and ventricular tachycardia (<I>p</I> =0.007) was higher in non-diabetic with hyperglycemia than in diabetic patients.</P> <P><B>Conclusions</B></P> <P>These findings underscore clinical significance of admission hyperglycemia on in-hospital mortality in non-diabetic AMI patients.</P>
