        만성 간염에서 Interleukin-6의 간내발현

        N/A Rackground/Aims: Interleukin-6 (lL-6), also known as B cell stimulatory factor 2(BSF-2), induces the final maturation of B cells to antitxxiy-producing cells. IL-6 has many biologic properties including the immune and intlammatory responses. This study wos aimed to evaluate the role of local interleukin 6(IL-6) in the pathogenesis of chronic hepatitis. Methods; We examined the cellular site and grade of IL-6 staining in paraffin sections of the liver from 24 patients with liver disease, using immunohistochemistry with a polyclonal antitwdy. The patient. Were divided into two groups; Group A(n=l3) with high histologic uctivi1y consisted of CAH-type B(n=10) ond active cirrhosis(n=3), whilc Group B(n= l l) with low hi.itologic activity consisted of CPH-type B(n=4), inactive cirrhosis(n=2) and fatty liver(n=S). Results: There was no staining of IL-6 in normal liver tissue. Thv grade.I of IL-6 staining in Group A were three positive in seven cases (53.81o), two positive in five ca.ics(38.3%) and one positive in only one case(7.7%), while those in Group B were one positivc in three cases(27.3%) ancl trace in eight case.(72.7ln). IL-6 stained cells in chronic hepatitis were hepatocytcs, cspecially in the areu ot' piecemeol necrosi.I, bilc duct cel1., infiltrating inflammatory cells and endothelial cell.I. The score of histological activity index(HAJ), piecemeal necrosis and fibrasis and thc gradv. Of 1L-6 staining of Group A were ull significantly higher than those of Group B. The grade of IL-6 staining and HAI werc well correlated(r =0.74, p 0.0l), Conclusion: Locally produced IL-6 in the liver may contribute to the inflammatory process and immunological response in chronic hepatiti..       • 만성 신부전 환자에서 빈혈 개선에 따른 혈장 수용성 interleukin-2 수용체 농도의 변화

        To evaluate the effect of anemia on the immune system in chronic renal failure (CRF) patients on hemodialysis (HD), we measured plasma sIL-2R levels in 10 CRF patients with anemia and 10 CRF patients without anemia on HD. In 10 CRF patients with anemia, plasma sIL-2R levels were measured also after correction by transfusion of packed RBCs or subcutaneous injection of rH-EPO. Plasma sIL-2R level was measured using h-IL-2R ELISA kit (Boehringer Manheim Biochemical, Germany) before and after correction of anemia. The results were as follows : Plasma sIL-2R level was 287.36±70.18 pmol/L in uremic patients with anemia and 211.35±49.85 pmol/L in uremic patients without anemia (p<0.05). After correction of anemia, plasma sIL-2R level was 299.55±98.85 pmol/L in "Low-Hct" group (Hct<26%) and 222.61 ±60.05 pmol/L in "High-Hct" group (Hct≥26%) (p<0.05). In anemic patients, plasma sIL-2R level before correction of anemia was 290.50±80.25 pmol/L and after correction of anemia was 243.74± 64.89 pmol/L (p<0.05). On conclusion, plasma sIL-2R level was higher in CRF patients with anemia than CRF patients without anemia. Plasma sIL-2R level in CRF patients with anemia became lowerd after correction of anemia.       • 탈미네랄화 골분(DBP)이 PLGA 지지체의 염증반응을 완화시킨다

        We developed the demineralized bone particle(DBP) impregnated poly(lactide-co-glycolide)(PLGA) scaffolds(PLGA/DBP) to investigate the effect of adhesion, growth, viability and inflammatory reaction of the cells. PLGA/DBP scaffolds were prepared by solvent casting/salt leaching method and were characterized by porosimeter, and scanning electron microscopy. NIH/3T3 fibroblast cells were cultured in the PLGA/DBP scaffolds as well as film and MTT assay was used to assess the viability of cells. Also, human promyelocytic leukemia cells(HL-60) were cultured in the PLGA/DBP scaffolds. We observed IL-1ß? and TNF-α expression of HL-60 cells seeded in PLGA/DBP scaffold by RT-PCR. NIH/3T3 fibroblast cell seeded on PLGA/DBP film were more adhere and spread with increasing DBP content due to increasing hydrophilicity and bioactivity. The fluorescence intensity of the band of TNF-α and IL-1ß? gene was decreased with increasing the concentration of DBP. It seems that the DBP affected on the improvement of physicochemical properties of PLGA such as biocompatibility, wettability and inflammatory response.

        Effect of Cytokines and bFGF on the Osteoclast Differentiation Induced by 1α,25-(OH)<SUB>2</SUB>D<SUB>3</SUB> in Primary Murine Bone Marrow Cultures

        <P> Bone is a complex tissue in which resorption and formation continue throughout life. The bone tissue contains various types of cells, of which the bone forming osteoblasts and bone resorbing osteoclasts are mainly responsible for bone remodeling. Periodontal disease represents example of abnormal bone remodeling. Osteoclasts are multinucleated cells present only in bone. It is believed that osteoclast progenitors are hematopoietic origin, and they are recruited from hematopoietic tissues such as bone marrow and circulating blood to bone. Cells present in the osteoclast microenvironment include marrow stromal cells, osteoblasts, macrophages, T-lymphocytes, and marrow cells. These cells produce cytokines that can affect osteoclast formation. In vitro model systems using bone marrow cultures have demonstrated that IL-1β, IL-3, TNF-α, bFGF can stimulate the formation of osteoclasts. In contrast, IL-4 inhibits osteoclast formation. Knowledge of cytokines and bFGF that affect osteoclast formation and their capacity to modulate the bone-resorbing process should provide critical insights into normal calcium homeostasis and disorders of bone turnover such as periodontal disease, osteoporosis and Paget s disease.

        Our country has been advanced in the organization of society with rapid industralization for more over 30 years. Gravitation of the population and traffic increase made change in life style and brought about a lot of noise pollution which is not able to be confirmed in sight. The damage of traffic noise will be a serious problem, so the effective and economical counter-measuring of noise will be proceeded when we build up a large building or housing complex and it also needs enlightment activity in order to reduce sound by improvement of how to drive. So, It is possible to be suggested as follows 1) Enforcing the every-ten-day limit on auto usage continuously. 2) Expended execution of noise restriction area. 3) Improvement of procedure in parceling-out. 4) Establishment of walfare facilities on the road side. 5) Application of matrial excluded the outside sounds in the construction on road side. If we consider environmental city condition at the construction and city planning, it is possible to minimize the noise pollution, to make a pleasant environment and to contribute the national health improvement.

        일부지역 성인의 생의 의미

        Purpose: The purpose of this study was to measure meaning in life for a normal adult and to offer basic data for developing nursing strategies for improving the meaning in life of adults. Method: There were 351 subjects, from 20 to 60 years old, staying in Busan and KyungNam Province. The data was collected from August to October 2000. The instrument was designed by researchers. The data were analyzed with frequency, mean(±SD), t test & ANOVA. Result: 1. The mean MIL for adult was 187.7(±18.1). 2. The mean MIL of basic premise phase for discovery of meaning was 30.5, method phase for discovery of meaning was 111.1, and result phase for discovery of meaning was 46.4. 3. The levels of MIL, 56.4% of the subjects were in pursuit of meaning in life, 23.3% felt a loss of meaning in life. 4. There were significant differences in MIL according to the level of education(p=0.00), marital status(p=0.01) and monthly income (p=0.00). Conclusion: On the viewpoint of the above results, 23.3% of the subjects were in a level of existential vaccum which required keens Logotherapy. Therefore, it shows that developing nursing-logotherapy to improve the meaning in life for adults is demanded.

        영·유아 언어, 인지, 사회·정서 발달 평가도구 문항 개발을 위한 예비연구

        본 연구는 만 2~5세 영·유아의 발달 상태를 객관적으로 평가함과 동시에 발달이 지연된 영·유아를 선별할 수 있는 발달평가도구 문항을 개발하기 위한 예비연구이다. 이를 위해 문헌고찰 및 발달관련 평가도구들을 비교 분석하여 발달지표를 선정하고, 만 2세~5세까지의 영·유아의 언어, 인지, 사회·정서발달영역을 교사관찰에 기초하여 평가할 수 있는 문항을 개발하였다. 연구대상은 대도시, 중소도시, 읍면 소재지 별로 표집 된 만 2세부터 5세까지 총 13개 유아교육기관에 다니는 435명의 영·유아들이며, 본 연구에서 최종 개발된 평가도구의 문항은 총 141문항이다. 평가도구 문항은 언어, 인지, 사회·정서발달영역별로 영·유아의 행동과 언어를 중심으로 교사가 관찰하고, 관찰하기 어려운 문항에 대해서는 단서와 kit를 제시하여 평가할 수 있도록 구성하였다. 개발된 평가도구 문항의 적절성을 알아보기 위하여 내용 타당도와 내적 신뢰도를 살펴본 결과 비교적 양호한 것으로 나타났다. This was a preliminary study to construct a developmental rating scale for understanding children's objective developmental levels and screening developmental disabilities. For this purpose, a developmental indicator was selected by a literature review and comparative analysis of related developmental assessment tests. An assessment scale based on teacher's observations to measure language, recognition, and social-emotional developmental areas was developed. The subjects were 435 young children aged 2 to 5 who lived in both urban and rural areas. A trial test consisted of 141 assessment items in the language, recognition, and social-emotional developmental domains. The developmental scale was evaluated by teachers who observed children's behavior and language, and used clues and kits. Tests on validity and reliability for confirming the appropriateness of the developed assessment scale showed that content was relatively valid and internally consistent.

        병리검사 처방오류 개선 시스템 구축

        문제: 조직병리검사의 처방이 복잡하고 전문화됨에 따라 오류처방이 빈번하게 발생되고 이를 해결하기위한 프로세스가 복잡하여 부서간의 갈등과 업무의 가중, 또한 병원 수입 감소가 초래되었다. 목적: 발생된 오류처방의 유형과 발생율, 오류처방을 해결하기 위한 프로세스를 파악하여 이에 대한 문제점을 개선하여 업무의 효율성과 관련부서간의 부가업무 감소와 갈등해소, 더불어 병원수익을 향상시키는 것에 목표를 두고 활동하였다. 의료기관: 서울시에 소재한 종합병원 질 향상 활동: 조직병리검사 처방의 오류처방 발생 원인과 유형, 발생율을 조사하여 원인을 파악하고 개선을 위한 프로세스를 구축하여 질 향상을 도모하였다. 개선효과: 수술실에서 의뢰한 외과 입원환자의 일반조직병리검사 오류율은 개선 전 7.8%에서 개선 후 3.5%, 동결절편조직검사 오류율은 개선 전 28.6%에서 개선 후 0%로 이전보다 모두 개선되었다. 또한 수술실 간호사의 업무 만족도는 일반조직병리검사의 처방시 95%, 동결절편조직검사 처방시 90%로 높은 만족도를 보였다. 단 병리과의 경우 업무가 증가됨에 따라 초과근무시간이 증가하였다.
