BackgroundBloodstream infections (BSI) remain a frequent complication during the pre-engraftment period after hematopoietic stem cell transplantation (HSCT), resulting in high mortality rates. This study evaluated risk factors for mortality in hematopoietic stem cell transplant recipients with BSI in the pre-engraftment period.MethodsThis prospective case control study was performed at the Center of Hematology and Bone Marrow Transplantation in Minsk, Republic of Belarus. Data relating to patient age and gender, date and type of transplantation, conditioning chemotherapy regimen, micro-organisms isolated from blood, and antibacterial therapy were prospectively collected from all hematopoietic stem cell recipients with microbiologically proven cases of BSI in the pre-engraftment period. The primary outcome was all-cause 30-day mortality after onset of febrile neutropenia. ResultsA total of 135 adult patients with microbiologically proven BSI after HSCT were studied, with 65.2% of cases caused by gram-negative microorganisms and 21.5% by non-fer-menting bacteria. Inadequate empiric antibacterial therapy and isolation of carbape-nem-resistant non-fermenting gram-negative bacteria (Acinetobacter baumannii and Pseudomonas aeruginosa) were independently associated with increased all-cause 30-day mortality in these patients. ConclusionThe risk factors for mortality in adult patients with BSI in the pre-engraftment period after HSCT were inadequacy of empirical antibacterial therapy and isolation of carbapenem-re-sistant A. baumannii or P. aeruginosa.
The Effect of Methylphenidate on Neurological Soft Signs in ADHD

ObjectiveaaNeurological soft signs are very common in children with the attention deficit hyperactivity disorder (ADHD), and the first line medication of this disorder is methylphenidate. The aim of the study was to assess the effect of methylphenidate on the neurological soft signs in children and adolescents suffering from ADHD depending on the dose of methylphenidate. MethodsaaThirty five patients with ADHD were investigated by the ADHD RS-IV parent version questionnaire and the Revised Neurological Examination for Subtle Signs before treatment adjustment and after four weeks of methylphenidate medication. The changes in hyperactivity symptomatology, neurological soft signs during therapy and the influence of the methylphenidate dose were statistically analyzed. ResultsaaA significant decrease in hyperactivity symptomatology was found after one month of methylphenidate medication (p= 0.0001) and significant decrease in neurological soft signs was demonstrated in 21 from a total of 26 items (p<0.05). Correlation analysis showed no relationship between the dose of methylphenidate and the improvement of neurological soft signs. Similarly, the improvement of ADHD symptomatology had not correlation with the improvement of neurological soft signs. ConclusionaaThe study demonstrated the positive effect of methylphenidate on neurological soft signs in which improvement occurred independently of the dose, indicating that their progress may be due to methylphenidate treatment of any dose. The unrelated effect of methylphenidate on the attention deficit hyperactivity disorder and neurological soft signs suggest that methylphenidate might be useful in the therapy of clumsy child syndrome and in ADHD treatment of non-responders.

Background/Aims: Radiofrequency ablation (RFA) is the first-line therapy for dysplastic Barrett’s esophagus (BE). Therefore, cryotherapy has emerged as an alternative treatment option. This study aimed to compare the efficacies of these two techniques based on the rates of complete eradication of intestinal metaplasia (CE-IM) and dysplasia (CE-D). Adverse events and recurrence have also been reported. Methods: An electronic search was conducted using the Medline (PubMed), Embase, LILACS, and Google Scholar databases until December 2022. Studies were included comparing cryotherapy and RFA for treating dysplastic BE with or without early esophageal neoplasia. This study was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Results: Three retrospective cohort studies involving 627 patients were included. Of these, 399 patients underwent RFA, and 228 were treated with cryotherapy. There was no difference in CE-IM (risk difference [RD], –0.03; 95% confidence interval [CI], –0.25 to 0.19; p=0.78; I2=86%) as well as in CE-D (RD, –0.03; 95% CI, –0.15 to 0.09; p=0.64; I2=70%) between the groups. The absolute number of adverse events was low, and there was no difference in the recurrence rate. Conclusions: Cryotherapy and RFA were equally effective in treating dysplastic BE, with or without early esophageal neoplasia.

The article assesses the level of navigation safety, in theoretical terms, defines the complexity of managing navigational risks in practice. The issues of assessing the navigational safety have been studied due to the importance and relevance of the issue in question, however, due to the great complexity of the problem under consideration, the article considers and indicates the directions for the development of the solution of the given direction, where, first of all, it became necessary to analyze the issue of assessing the levels of navigation risks when navigating vessels of various types in difficult navigation conditions.

This paper focuses on a critical component of the situational awareness, the neural control of autonomous vertical flight for an unmanned aerial vehicle. Autonomous vertical flight is a challenging but important task for tactical unmanned aerial vehicles to achieve high level of autonomy under adverse conditions. The fundamental requirement for vertical flight is the knowledge of the height above the ground, and a properly designed controller to govern the process. With the situational awareness strategy, we proposed a two stage flight control procedure using two adaptive neural networks to address the dynamics variation and performance requirement difference in initial and final stages of flight trajectory for a nontrivial small-scale helicopter model comprising five states, two inputs and two outputs. This control strategy for chosen helicopter model has been verified by simulation of descending and landing manoeuvres using software package Simulink and demonstrated good performance for fast situational awareness in real-time search-and-rescue operations.

The frequency of plant transformation can beimproved by addition of various chemical into transformationmedia. In the past, we showed that exposure oftobacco, wheat and triticale explants to ammonium nitrate,cerium and lantanium chloride and potassium chlorideresulted in an increase in the frequency of transformation. Here, we tested whether a combination of increased concentrationsof the aforementioned salts yielded a highertransformation frequency. We found that exposure to61.8 mM ammonium nitrate caused a 5.0-fold increase intransformation frequency, whereas exposure to 1.0 lMcerium chloride or 47.0 mM potassium chloride resulted in1.2- and 2-fold increases, respectively. Exposure to61.8 mM ammonium nitrate and 1.0 lM cerium chlorideled to a 4.8-fold increase in transformation frequency,whereas exposure to 61.8 mM ammonium nitrate and47.0 mM potassium chloride let to a 5.2-fold increase. Finally, exposure to 61.8 mM ammonium nitrate, 1.0 lMcerium chloride and 47.0 mM potassium chloride produceda 5.1-fold increase. The analysis of the intactness ofT-DNA borders showed that plants exposed to ammoniumnitrate and a combination of ammonium nitrate with othersalts had the more intact right borders and the less intactleft borders. The best results were observed when all threesalts (ammonium nitrate, potassium chloride and ceriumchloride) were used. Thus, we concluded that the additionof cerium chloride and potassium chloride does not substantiallyimprove the transformation rate beyond theimprovement observed upon treatment with 61.8 mMammonium nitrate, but may slightly improve the intactnessof T-DNA borders.
Growth and Tolerance Assessment of a Lutein-fortified Infant Formula

Purpose: To evaluate safety, gastrointestinal tolerance, and growth of a new experimental starter formula (NESF) fortified with lutein, prebiotics, probiotics, nucleotides and beta-carotene, fed to infants within the first months of life. Methods: This was a non-randomized, open, uncontrolled study conducted from December 2010 to May 2011. Twenty-three healthy infants aged 10 days to 2 months old were enrolled. Outcomes included gastrointestinal tolerance, physical growth and safety. Prior to the initiation of the trial, the Scientific Research Institute of Nutrition of the Russian Academy of Medical Sciences confirmed that the NESF met all safety and nutritional parameters. Results: NESF was well tolerated. The majority of infants fed this formula passed semi-liquid, yellow or yellow-brown. The mean stool frequency/day was 2.5±0.4 on study-day 14 and 1.8±0.5 on study-day 28. The mean daily weight gain was 30.9±3.8 grams and the mean length gain during the 28 days of follow up was 3.1±0.8 cm, corresponding to the average physical growth normally seen in the first months of life in Russian infants. Six children left the study: one refused to drink the formula, one left the study as parents changed residence; and one child’s parents have recalled their informed consent due to adverse event unrelated to the product. Three infants presented adverse events possibly related to the product (rash; colic and abdominal pain; constipation). Seventeen infants completed the trial. Conclusion: This study demonstrated that lutein-fortified formula is safe, well-tolerated and supported physical growth of evaluated infants.
