        Identification of Genes Modulated by High Extracellular Calcium in Coculture of Mouse Osteoblasts and Bone Marrow Cells by Oligo Chip Assay

        Calcium concentration in the bone resorption lacunae is high and is in the mM concentration range. Both osteoblast and osteoclast have calcium sensing receptor in the cell surface, suggesting the regulatory role of high extracellular calcium in bone merabolism. In vitro, high extracellular calcium stimulated osteoclastogenesis in coculture of mouse osteoblasts and bone marrow cells. Therefore we examined the genes that were commonly regulated by both high extracellular calcium and 1,25(OH)_(2)vitaminD_(3)(VD3) by using mouse oligo 11 K gene chip. In the presence of 10 mM[Ca^(2+)]e or 10 nM VD3, mouse calvarial osteoblasts and bone marrow cells were co-cultured for 4 days when tartrate resistant acid phosphatase-positive multinucleated cells start to appear. Of 11,000 genes examined, the genes commonly regulated both by high extracellular calcium and by VD3 were as follows; 1) the expressions of genes which were osteoclast differentiation markers or were associated with osteoclastogenesis were up-regulated both by high extracellular calcium and by VD3; trap, mmp9, car2, ctsk, ckb, atp6b2, tm7sf4, rab7, 2) several chemokine and chemokine receptor genes such as sdf1, scya2, scyb5, scya6, scya8, scya9, and ccr1 were up-regulated both by high ectracellular calcium and by VD3, 3) the genes such as mmp1b, mmp3 and c3 which possibly stimulate bone resorption by osteoclast, were commonly up-regulated, 4) the gene such as c1q and msr2 which were related with macrophage function, were commonly down-regulated, 5) the genes which possibly stimulate osteoblast differentiation and/or mineralization of extracellular matrix, were commonly down-regulated;slc8a1, admr, plod2, lox, fosb, 6) the genes which possibly suppress osteoblast differentiation and/or mineralization of extracellular matrix, were commonly up-regulated;s100a4, npr3, mme, 7) the genes such as calponin 1 and tgfbi which possibly suppress osteoblast differentiation and/or mineralization of extracellular matrix, were up-regulated by high extracelluar calcium but were down-regulated by VD3. These results suggest that in coculture condition, both high extracellular calcium and VD3 commonly induce osteoclastogenesis but suppress osteoblast differentiation/mineralization by regulating the expression of related genes.

        정신과를 방문하는 환자의 통증호소의 빈도 및 특성

        연구목적: 정신과 방문 환자에서 통증 호소는 적지 않을 것이라고 알려져 있으나 국내에서 정신과 외래 환자의 통증 관련 연구는 부족한 실정이다. 이에 정신과 외래에 오는 환자들에서 통증의 빈도와 특성을 1,2,3차 의료기관에서 알아보고자 하였다. 대상과 방법: 1차 의료기관(개인정신과의원) 외래환자 298명, 2차 의료기관(종합병원) 정신과 외래환자 99명, 3차 의료기관(대학병원) 정신과 외래환자 446명으로 총 843명의 성인 남녀를 대상으로 이들이 정신과에 방문했을 당시의 통증 여부와 조사 당시의 통증 유무 및 현재 통증의 정도를 평가하였다. 그리고 기질적 원인의 여부, 만성 통증의 여부, 통증의 부위, 통증 악화 요인, 보상문제의 유무, 약물 투여 내용 등을 조사하였다. 결 과: 1) 전체 대상자 843명 중 통증이 동반되었던 환자의 비율은 41.3%였으며, 기관별로 보면 개인의원 44.9%, 종합병원 44.5%, 대학병원 38.1%였다. 2) 조사 당시에 통증을 갖고 있는 경우는 3개 기관에서 평균 24.1%였다. 통증의 정도는 VAS상 평균 46.7㎜였으며 개인의원에서 대학병원에 비해 의미있게 그 정도가 심했다. 또한 `강하다` 이상의 통증 정도를 호소한 경우는 34.0%, `괴롭다` 이상의 불쾌정도를 표현한 경우는 26.2%였다. 3) 통증이 있었던 환자 중 기질성이 뚜렷한 경우는 적었으며(1.5∼5.9%), 통증의 부위는 머리가 50.9%로 가장 많았다. 통증의 지속기간은 6개월 이상인 만성통증의 경우가 68.7%를 차지했다. 4) 주요 약물로는 항불안제가 41.4%로 가장 많이 사용되고 있었으며, 통증과 관련하여 진통제를 처방받는 환자는 3.2%에 불과하였다. 결 론: 정신과 외래를 방문하는 환자들의 통증의 특성은 의료기관에 따라 인구통계학적 차이 뿐만 아니라 진단분포도 다르므로 3기관을 직접 비교하기에는 무리가 있으나, 본 연구 조사 결과 정신과 외래를 방문하는 환자들 중에서 40% 이상이 통증을 경험하고 있으며 2/3이상은 만성 통증이라 할 수 있었다. 이들은 중등도 이상의 통증을 갖고 있으며 당장 통증을 해결해 주어야 할 숫자도 적지 않았다. 따라서 정신과 환자의 통증에 대한 평가와 관리가 앞으로 더욱 강조되어야 할 필요가 있다고 사료된다. Objectives: The purposes of this study were to investigate the frequency and clinical characteristics of pain among the psychiatric out-patients in three clinics. Methods: A Total of 843 psychiatric adult out-patients(298 subjects from a private clinic, 99 subjects from a general hospital, 446 subjects from a university hospital) were assessed for the presence of pain during OPD follow-up and at the time assessed. And, the presence of organic causes, duration and site of pain, aggravating factors, compensation problems, and prescribed drugs were also investigated. Results: 1) The frequency of pain among psychiatric out-patients was overall 41.3%(44.9% in private clinic, 44.5% in general hospital, and 38.1% in university hospital, respectively). 2) The frequency of pain at the time assessed was 24.1%. In severity of pain, the average of VAS was 46.7±18.0㎜. Patients of private clinic showed more severe pain than that of general hospital. Moreover, patients who experienced over `marked` occupied 34.0% and pain as over `distressed` occupied 26.2%. 3) Only a few subjects had the definite organic causes(1.5-5.9%). The most common site of pain was on head. The patients having a pain over 6 months(i.e. chronic pain) occupied 68.7%. 4) The most common drugs prescribed were anxiolytics; 41.4% of patients were prescribed. The analgesics were prescribed only in 3.2% of patients. Conclusion: This result revealed that more than 40% of patients experienced pain in psychiatric OPD and two third of patients were suffered from chronic pain. Therefore, the evaluation and management of pain should be more emphasized.

        초음파 펄스에코 신호의 3차원 처리

        비파괴 시험을 위한 3차원 구조의 초음파 영상에는 다양한 결함을 명백하게 보여줄 수 있을 만큼 상세하고 쉽게 알아볼 수 있는 정보가 제공되어야 한다. 수년 동안 원자력 발전소에서 사용된 금속관에 발견되는 소규모의 균열은 전형적인 결함들인데, 이러한 밀리미터 이하의 균열이나 결함은 최종 3차원 영상에서 묘사되어야만 의미있는 검사가 될 것이다. 향상된 선명도와 그에 따른 결함의 발견 과정의 한 단계로써, 펄스에코(pulse-echo) 초음파를 사용한 3차원 영상제작 기술을 제안한다. 이 기술은 필요한 스캐닝과 펄스에코 데이터의 처리과정을 통한 검사로 3차원 물체의 3차원 영상을 생성하는데, 2차원 위너필터(Wiener filter)에 의해 초음파 빔을 선명하게 하는 기술을 포함한다. 제안하는 위너필터는 빔의 전달에서 펄스에코 데이터를 초음파 빔 방향의 수직방향에 따라 필터링한다. 이 3차윈 처리과정은 결함의 선명성을 증진시키고 사용자에게 3차윈 구조물의 좌우 회전 및 축 회전과 같은 조작 능력을 제공한다. 이러한 조작 능력은 3차원에서 다양한 결함들의 크기와 위치의 분명한 묘사를 가능하게 한다. Infrasonic imaging of 3-D structures for nondestructive evaluation must provide readily recognizable images with enough details to clearly show various flaws that may or may not be present. Typical flaws that need to be detected are miniature cracks, for instance, in metal pipes having aged over years of operation in nuclear power plants; and these sub-millimeter cracks or flaws must be depicted in the final 3-D image for a meaningful evaluation. As a step towards improving conspicuity and thus detection of flaws, we propose a pulse-echo ultrasonic imaging technique to generate various 3-D views of the 3-D object under evaluation through strategic scanning and processing of the pulse-echo data. We employ a 2-D Wiener filter that filters the pulse-echo data along the plane orthogonal to the beam propagation so that ultrasonic beams can be sharpened. This three-dimensional processing and display coupled with 3-D manipulation capabilities by which users are able to pan and rotate the 3-D structure improve conspicuity of flaws. Providing such manipulation operations allow a clear depiction of the size and the location of various flaws in 3-D.

      • 장관에서의 액체 이동에 미치는 히스타민의 작용에 관한 연구

        In vitro perfusion experiment was done to observe the effects of histamine on the transport of fluid from capillary into the intestinal lumen of rabbits weighing 2-2.8㎏. After performing the control perfusion with an artificial perfusate, histamine was added to the perfusing solution at a concentration of 1 ㎍/50ml perfusate and perfusion experiment was repeated. Mean capillary pressure (Pc), precapillary resistance(Ra), postcapillary resistance(Rv) and total tissue resistance (Rt) were calculated from the measured data of isogravimetric capillary pressure (Pci), arterial pressure (Pa), venous pressure (Pv), venous flow rate (Qv), luminal flow rate (Q_L) and weight of the intestinal loop. Decreased Pci from 19.5㎜ Hg of the control group to 15.2㎜ Hg of the histamine group had resulted from increased capillary permeability. And increased Pc was observed in the histamine group compared to the control due to the decrease in pre-to-postcapillary resistance ratio in the histamine perfused loop. Decreased Rt in the histamine group was also responsible in facilitating the fluid movement from capillary to intestinal lumen in addition to the effect of increased Pc. When Pc and Q_L were plotted linear relationships were observed and the slope of the histamine group was steeper than that of the control indicating increased volume movement "coefficient" in the histamine group.

        Lupus erythematous panniculitis is a chronic inflammatory disease of subcutaneous fat that occurs in 2-3% of patients with systemic lupus erythematosus. Lupus erythematous panniculitis typically presents with firm, indurated subcutaneous nodules or plaques commonly developed in proximal extremities, trunks, head and neck. Face, breast and upper extremities involvement with lupus erythematous panniculitis has been infrequently reported. We report a case of lupus erythematous panniculitis developed in a 27-year-old female confirmed by histologic features and laboratory findings.

        마라톤후 발생한 열사병 1례

        Heat stroke, a medical emergency, occurs when the body's thermal regulation is upset and unable to dissipate adequate amounts of heat with a rise in body temperature. It is characterized by hyperpyrexia, with a core temperature of 40˚C or more. hot dry skin, and central nervous system disturbance and usually results in rhabdomyolysis and multiorgan failure. Our case, a 43-year-old healthy male, was caused by a marathon, half course, on a sunny day in late summer. He suddenly fell down on the road and was delivered to a nearby hospital, where a seizure developed. He was transfered to our hospital and then displayed Central Nervous System disturbance, hot dry skin, acute liver failure, rhabdomyolysis, anurlc acute renal failure, and disseminated intravasculular coagulopathy. He was treated with general supportive care and hemofiltration. Despite the aggressive management, he died of shock on the fifth day after admission.

      • 초음파 펄스에코 데이터 기반 3차원 시각화 방법론

        The nondestructive testing using ultrasonic pulse-echo data is an effective test methodology esperially for metal structure. Typically, the ultrasonic pulse-echo data is processed and the results are shown in A-scan, B-scan or C-scan formats. Upon viewing B-scan and C-scan data the viewer is able to identify the location of faults as well as their rough dimension. In this paper, we evaluate the applicability of various 3-D visualization methods frequently used in medical imaging, namely, surface rendering (SR), volume rendering (VR) and maximum intensity projection (MIP), for nondestructive testing purposes. Such 3-D visualization of ultrasonic pulse-echo data enables easier identification of the location and dimension of faults more accurately.
