<P><B>Background</B></P><P>Exosomes are thought to play an important role in exchanging information between cells. The proteins and lipids in exosomes play roles in mediating inflammatory and autoimmune diseases. The aim of this study was to identify exosomal candidate proteins that are related to other inflammatory parameters in rheumatoid arthritis (RA).</P><P><B>Methods</B></P><P>The study population consisted of 60 patients with RA: 30 in the clinical remission (CR) group with a Disease Activity Score in 28 joints based on erythrocyte sedimentation rate (DAS28-ESR) ≤2.6 and 30 in the non-clinical remission (non-CR) group with a DAS28-ESR >2.6. Preparation of exosomes from patient serum samples was performed with the ExoQuick kit, and protein identification/quantification was performed using tandem mass tag labeling/mass spectrometry and an enzyme-linked immunosorbent assay. Comparisons between groups were made using Student’s <I>t</I> test or the Mann-Whitney <I>U</I> test, as appropriate. Spearman’s correlation coefficients (ρ) were calculated.</P><P><B>Results</B></P><P>We identified six candidate proteins. Exosomal levels of amyloid A (AA) and lymphatic vessel endothelial hyaluronic acid receptor-1 (LYVE-1) differed between the CR and non-CR groups. Both serum and exosomal AA levels were higher in the non-CR group than in the CR group (<I>p</I> = 0.001). Significant positive correlations were found between exosomal AA and C-reactive protein (CRP) as well as between serum AA and CRP (ρ = 0.614, <I>p</I> = 0.001, and ρ = 0.624, <I>p</I> = 0.001, respectively). Although serum levels of LYVE-1 did not differ between the non-CR and CR groups, exosomal levels of LYVE-1 were lower in the non-CR group than in the CR group (<I>p</I> = 0.01). We identified positive correlations between serum/exosomal LYVE-1 and CRP only in the non-CR group (serum ρ = 0.376, <I>p</I> = 0.04; exosome ρ = 0.545, <I>p</I> = 0.002).</P><P><B>Conclusions</B></P><P>Exosomal LYVE-1 shows potential for use as an additional marker of disease activity in patients with RA, and exosomes may carry other useful markers for RA.</P>
Flavonoids with Bradykinin Antagonistic Effects from Scutellariae Radix

Seven flavonoid components were isolated from fr. C of Scutellariae radix which showed antagonistic effects against bradykinin(BK). The results indicated that two oxygen functions (either -OH or -OCH₃) at 2`- and 6`-positions and/or an oxygen function at 6-position of flavone seemed to be favored for the BK inhibitory activities. Skullcapflavone-II(IV) which contains 6-OCH₃, 2`-OH and 6`-OCH₃ in the structure was the most active among the tiavones tested.
Frequent outpatient visits prevent exacerbation of chronic obstructive pulmonary disease

Background: Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory airway disease requiring frequent outpatient visits and lifelong management. We aimed to evaluate the roles of frequent outpatient visits in prognosis of COPD. Methods: We used claims data in the national medical insurance review system provided by the Health Insurance Review and Assessment Service of Korea from May 1, 2014 to April 30, 2015. A definition of COPD was used based on the diagnosis code and medication. Frequent visitors were defined as subjects who visited the outpatient clinic for COPD three or more times per year. Results: Among 159,025 subjects, 117,483 (73.9%) were classified as frequent visitors. Frequent visitors underwent pulmonary function tests and used various inhalers more often than did infrequent visitors. The rates of COPD exacerbation requiring admission to a general ward, emergency room, or intensive care unit were significantly lower in frequent visitors than in infrequent visitors. In multivariable analysis, frequent visits were identified as an independent factor preventing COPD exacerbation that required admission to a ward (odds ratio [OR], 0.387), emergency room, (OR, 0.558), or intensive care unit (OR, 0.39) (all P<0.001). Conclusions: In conclusion, we showed frequent outpatient visits reduce the risk of COPD exacerbation by 45%-60%.

Myasthenia gravis (MG) is an uncommon disease characterized by muscular weakness and fatigability, caused in 85% of the cases by Acetylcholine receptor (AChR) antibodies. And it is reported as a rare side effect of interferon administration which can develop during chronic hepatitis C treatment. And about 15% of MG patients have thymoma while half of the patients with thymoma eventually develop MG. But it is hardly reported that MG was triggered by antiviral therapy in patient with thymoma. We experienced a rare case of myasthenia gravis induced by antiviral therapy for chronic hepatitis C in patient with mediastinal thymoma. A 47-year-old male complained of diplopia after 9 weeks of administration of peginterferon and ribavirin. A brain imaging study showed no abnormality but we diagnosed as MG by positive repetitive nerve stimulation test and positive anti AChR antibody. And we found thymoma after chest CT scan.
Lack of Haptic Feedback Is Replaced by More Developed Visual Sense during Robotic Myomectomy

In the reproductive age, many women have several uterine myomas and present with abnormal uterine bleeding, dysmenorrhea, and occasionally infertility. There are three surgical approaches to perform myomectomy, including robotic-assisted, laparoscopic, and abdominal myomectomy. Compared to laparoscopic procedures, robotic myomectomy allows free approach of myoma bases using fine instruments and endoscopes. Fine uterine wall sutures can be performed layer-by-layer with robots. However, robotic surgery is difficult to perform because there is no sense of touch during the operation. We report two clinical myomectomy cases with replaced lack of haptic feedback during robot surgery. The patients received robotic myomectomy with/without right ovarian cystectomy and adhesiolysis. Sixty-five leiomyomas were removed in case 1. Forty-six leiomyomas were removed in case 2. Lack of haptic feedback is replaced by more developed visual sense during robot myomectomy of multiple tiny intramural myomas, and robotic surgery can be performed much more effectively even in complicated cases.
Effects of particulate matter in a mouse model of oxazolone-induced atopic dermatitis

Background: Recent epidemiological studies have demonstrated that air pollution is associated with the inflammatory response and may aggravate inflammatory skin diseases such as atopic dermatitis (AD). However, it is unclear whether particulate matter (PM) aggravates AD symptoms. Objectives: This study aimed to investigate whether PM exposure affects the skin barrier dysfunction and aggravates AD symptoms using human keratinocytes (HaCaT) cells and a mouse model of oxazolone-induced AD-like skin. Methods: SRM 1649b, which mainly comprises polycyclic aromatic hydrocarbons, was used as the reference PM. HaCaT cells and mouse model of oxazolone-induced AD-like skin were treated with PM. The mRNA or protein expression levels of stratum corneum (SC) and tight junction (TJ) proteins, inflammatory cytokines, as well as clinical and histological changes of the AD-like skin of mouse model were evaluated. Results: PM downregulated the expression levels of several SC and TJ-related proteins in the mouse model with AD-like skin. Clinically, epidermal and dermal thickness was significantly increased and dermal inflammation was prominent in PM treated AD-like skin. Conclusion: The results of our study suggest that PM may trigger the exacerbation of AD symptoms via skin barrier dysfunction-related mechanisms.

Background: There is an increasing interest in the use of hyaluronic acid (HA) fillers in skin rejuvenation beyond its conventional use which is supplementing decreased dermis volume and filling deep wrinkles. Objectives: This study aimed to evaluate the efficacy and safety of a novel device (Dermashine® balanceTM) that injects HA into the dermis using a stamp-type microneedle for maintenance of hydration and elasticity of the skin. Methods: A single-center randomized double-blinded parallel-group clinical study involving 60 participants was conducted. The subjects were randomized to receive HA injections or a placebo 3 times across the face using an automatic intradermal injector. At 4, 8, and 12 weeks after the treatment, skin hydration was measured using a corneometer and skin elasticity was measured using a cutometer. Results: The patients who received HA showed significantly greater skin hydration than those who received the placebo. However, a significant difference was not noted in skin elasticity between the groups. No severe adverse event was reported. Conclusion: The results of this study suggest that intradermal supplementation of HA using mesogun multi-needle injector may be a safe and effective treatment for improving skin hydration.
