        We present a case of angiomyofibroblastoma of the vulva in a 32-year-old woman. The asymptomatic lesion measured 4×3cm and appeared as a gray-white myxoid mass on cut sections Histologically, it appeared as a relatively well-circumscribed spindle cell proliferation with alternating hypercellular and hypocellular areas and proliferation of thin-walled blood vessels. Immunohistochemically, the stromal cells expressed vimentin, CD34 and progesterone receptor, but not desmin, α-smooth muscle actin, S-100 protein and estrogen receptor. These features were similar to those of angiomyofibroblastoma as previously reported in the vulva except negativity for desmin A case of pathologically proven very rare tumor of angiomyofibroblastoma of the vulva is presented with a literature review

        Quantification of a Novel PPARγ Partial Agonist (S)-2-Ethoxy-3-(4-{3- methyl-5-[4-(3-methyl-isoxazol-5-yl)-phenyl]thiophen-2-ylmethoxy}- phenyl)-propionic acid (PAM-1616) in Rat Plasma Using Liquid Chromatography-Tandem Mass Spectrometry

        (S)-2-ethoxy-3-(4-{3-methyl-5-[4-(3-methyl-isoxazol-5-yl)-phenyl]thiophen-2-ylmethoxy}-phenyl)-propionic acid (PAM-1616) is a novel peroxisome proliferators-activated receptor γ (PPARγ) partial agonist with excellent antihyperglycemic activity. It is a promising new drug candidate for the treatment of type-2 diabetes with reduced possibility of edema in vitro/in vivo. In order to evaluate the pharmacokinetics of PAM-1616, a reliable, selective and sensitive highperformance liquid chromatography/electrospray ionization tandem mass spectrometry was developed for the quantification of PAM-1616 in rat plasma. The analytes were extracted from rat plasma with ethyl acetate, separated on an Atlantis dC18 column with a mobile phase of 75% acetonitrile in 10 mM ammonium formate (pH 4.5), and detected by tandem mass spectrometry in the selective reaction monitoring mode. The calibration curve was linear (r2 = 0.999) over the concentration range of 0.05-20.0 μg/mL and the lower limit of quantification was 0.05 μg/mL. The coefficient of variation and relative error at four QC levels were 1.8% to 14.3% and −10.0% to 6.5%, respectively. The present method was successfully applied to the pharmacokinetic study of PAM-1616 after intravenous administration of PAM-1616 potassium at a dose of 1 mg/kg in rats.

        저칼슘혈증으로 인한 발작 뇌파소견을 보인 신생아 경련 1례

        저자들은 경련을 주소로 내원한 10일된 신생아에서 응급으로 시행한 뇌파검사상 발작기 뇌파를 관찰하였으며, 혈청학 검사에서 저칼슘혈증이 발견되어 칼슘제재 투여 후 경련이 소실된 증례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Seizures in neonates can occur due to the problems of delivery at birth such as anoxia and cerebral hemorrhage. Other events, such as hypocalcemia, hypoglycemia, hyponatremia, hypernatremia, hypomagnesemia, infection and central nervous system malformations, can cause sezures. Hypocalcemia occurs most commonly in neonates, which is one of the important causes of neonatal seizures over the post few decades. The electroencephalogram (EEG) in neonatal hypocalcemia may show abnormal spikes, specifically focal, rhythmic, high-voltage, and frontocentral discharges that rapidly generalize. We report the ictal EEG findings in a neonate with hypocalcemic seizures.

        소아에서의 일차두통에 대한 자연 경과

        목 적: 두통은 소아에서 비교적 흔하게 경험하는 증상이며, 일차두통의 빈도도 비교적 높은 편이다. 소아에서는 증상이 경미하고 예후가 좋은 편이나 때로 심한 두통을 호소하기도 하고 쉽게 호전되지 않는 경우도 있으며 일부에서는 성인이 되어서까지 두통이 지속된다. 따라서 소아에서의 일차두통 자연 경과를 파악하는 것이 두통의 치료와 상담에 중요할 것으로 생각되나 이에 대한 국내 보고는 적은 편이다. 본 연구에서는 일차두통으로 진단되었으나 예방적 치료를 하지 않은 환아들에서 2년 후 두통 유무에 대해 살펴 보고자 하였다. 방 법: 2003년 9월부터 2005년 10월까지 본원 소아과에 두통을 주소로 내원하여 편두통과 긴장형 두통으로 진단 받았던 환아 중 두통의 예방적 투약을 하지 않은 51명을 대상으로 하였다. 내원 후 2년이 지난 시점에서 부모와의 전화 인터뷰를 통해 두통의 유무와 호전 정도를 파악하였다. 총 51명의 환아 중에서 연락이 불가능하여 상태를 파악할 수 없었던 경우는 9명으로 탈락률은 17.6%였고, 연락이 가능하였던 42명의 환아를 대상으로 초기 방문시 진단에 따른 2년 후 상태를 확인하였다. 결 과 : 일차 두통 환아의 내원 당시 평균 연령은 11.3세였으며, 남아 23명(54.8%), 여아 19명(45.2%)이었다. 초기 진단 시 편두통 25명(59.5%), 긴장형 두통 17명(40.5%)이었다. 각 두통의 남녀 비는 편두통 1:1.5, 긴장형 두통 3.2:1이었다. 두통의 발생부위는 편두통에서 측두부 52.0%, 긴장형 두통에서 미만성 35.3%로 가장 많았다. 두통의 양상에서는 편두통에서 박동성 60.0%, 긴장형 두통에서 둔통 52.9%로 가장 많았다. 전체 두통 환자의 호전률에서는, 두통이 호전된 군 81.0%, 호전되지 않은 군 19.0%로 나타났다. 편두통 군이 80.0%, 긴장형 두통 군이 82.3%의 호전률을 보여 두 군간의 호전률은 비슷했으며 통계적 유의성은 없었다. 두통의 가족력과 두통 호전과의 관계에서는, 가족력이 있는 군에서 85.7%, 가족력이 없는 군에서 78.6%의 호전률로 유의한 차이를 보이지 않아 두통의 가족력은 호전률에 영향을 주지 않는 것으로 나타났다. 결 론 : 편두통이나 긴장형 두통으로 진단되었으나 예방적 치료를 하지 않은 환아들을 2년 뒤 조사한 결과, 상당수에서 두통이 호전 되었다. 하지만 본 내용은 환아들에게서 직접 정보를 얻은 것이 아니라 부모를 통하여 두통의 증상 여부를 확인하였기 때문에 정확성이 다소 떨어질 수 있으며 진단 전후 두통 일지 등을 통해 두통의 특성을 파악하는 것이 중요하리라 사료된다. 따라서 향후 두통 환아를 전향적으로 추적관찰하여 이들의 임상 경과를 확인하는 것이 필요할 것으로 판단된다. Purpose : Headache is a notable problem in clinical practice and a frequent symptom in children, but a systemic evaluation of the clinical course in affected patients is uncommon. The main aim of the present study is to investigate the natural course of primary headache in children after two years of diagnosis. Methods : Based on the international classification of headache disorder(2nd edition), 51 children were diagnosed as migraine or tension-type headache from September, 2003 to October, 2005. They did not received prophylactic treatment. Their symptoms were reassessed in 42 out of 51 children 2 years after the initial diagnosis. We evaluated the condition of the patients with headache by a telephone interview with their parents. Results : The diagnosed group consisted of 25 patients(59.5%) with migraine and 17 patients(40.5%) with tension-type headache. Regarding the location of the headache, the incidence of temporal area was higher in the migraine(52.0%) group than in the tension-type headache(11.8%) group. In the migraine group, 15 cases(60%) had pulsating headache, which was higher than in the tension-type headache group. At the 2-year follow up, headache-free or improvement was observed in 80.0%(20 cases) of the migraine group and 82.3%(14 cases) of the tension-type headache group. Among 14 children with positive headache family history, 12 patients(85.7%) were headache-free or improved. But there was not significant relationship between headache family history and prognosis. Conclusion : Primary headache had a high tendency to improve without long-term management by a 2-year follow-up in this study. Therefore, it is the way of reducing the unnecessary treatment to decide prophylactic treatment after investigating the severity or frequency of the headache using the headache diary or questionnaire.

        실리카에 고정화된 이온성액체를 촉매로 이용한 알릴글리시딜에테르와 이산화탄소의 부가반응

        본 연구에서는 이미다졸륨계의 이온성액체를 실리카에 고정화시킨 촉매를 제조하고, 알릴글리시딜에테르(AGE)와 이산화탄소의 부가반응을 통한 이종 5원환 카보네이트의 합성반응에서 이 촉매의 반응특성을 고찰하였다. 고정화된 이온성액체는 chloropropyl 그룹이 도입된 실리카에 이미다졸이 고정화됨으로써 형성되었다. 제조된 촉매는 XRD, BET, ²?Si MAS-NMR 그리고 SEM 등 다양한 기기분석을 통하여 특성분석을 수행하였다. ²?Si MAS-NMR을 통하여 chloropropyl 그룹이 부과된 실리카 표면에 이온성액체가 잘 형성 되어있었음을 관찰하였다. 고정화된 이온성액체 촉매는 반응온도 80-120℃의 범위에서 AGE의 전환율이 55-67% 이고 생성물의 선택도가 85% 이상으로 우수한 반응성을 나타내었다. 또한 고정화된 이온성액체 촉매는 균일계 촉매인 1-n-butyl-3-methyl imidazolium bromide (BMImBr)보다 더 높은 AGE의 전환율과 생성물의 선택도를 나타내었다. In this study, imidazolium salt ionic liquid on amorphous silica was prepared and its catalytic performance in the cycloaddition of CO₂ with allyl glycidyl ether (AGE) to produce heterocyclic carbonate was investigated. The ionic liquid was generated on chloropropyl functionalized silica through the immobilization of imidazole. The prepared catalyst was characterized using a number of instrumental analysis including XRD, BET, ²?Si MAS-NMR and SEM. ²?Si MAS-NMR showed that the ionic liquid formed adduct with the chloropropyl groups attached to the silica surface. The immobilized ionic liquid showed very good catalytic activity for the cycloaddition of CO₂ with AGE, showing 55-67% of AGE conversion with over 85% of the carbonate selectivity at 80-120℃. Its AGE conversion and selectivity to the carbonate were even higher than the homogeneous analog, 1-n-butyl-3-methyl imidazolium bromide (BMImBr).

        꽁치(Cololabis saira) 과메기의 지질산화 및 biogenic amine 생성에 건조조건이 미치는 영향

        This study reviewed the effect of the drying conditions on the production of biogenic amines and lipid oxidation in semi-dried Pacific saury Cololabis saira or Guamegi. The moisture content of the Guamegi ranged from 32.71±2.37 to 45.9±2.60 g/100 g. The respective ranges of the acid value (AV) and peroxide value (POV) were 1.39±0.40 to 15.79±0.47 mg KOH/g and 76.14±2.19 to 282.84±2.34 meq/kg on drying for 3 days. The AV and POV increased for up to 3 days of drying and the values differed according to the amount of sunlight and temperature. However, lipid oxidation was reduced in Guamegi manufactured using a cold-air drying method. The fatty acid composition and the biogenic amine content in Guamegi during drying did not differ significantly with the drying method or drying date. The main saturated, monoene and polyene fatty acids were palmitic acid, eicosenoic & erucic acids, and eicosapentaenoic & docosahexaenoic acids, respectively. At 16.67 to 71.89 mg/kg, the histamine content was higher than that of other biogenic amines and it increased significantly during drying. In conclusion, this study showed that the packaging and drying conditions of Guamegi products need to be improved to inhibit lipid oxidation and biogenic amine formation.

        <P>We describe a case of primary lung cancer with myocardial metastasis, documented with electrocardiogram (ECG) mimicking myocardial ischemia and manifesting VT. Metastatic tumor to the myocardium proper is uncommon and difficult to diagnose because of nonspecific signs and symptoms. We observed such ECG changes in this case were initially misinterpreted as acute coronary syndrome. The diagnosis of cardiac metastasis was confirmed by contrast-enhanced magnetic resonance imaging and computed tomography.</P>

        Human Embryonic Stem Cells Ameliorate Spatial Recognition After Grafting into the Ischemic Brain of Rats

        Embryonic stem(ES) cells are immature cells that have the ability of self-renewal, and they give rise to neurons, astrocytes and oligodendrocytes. In order to investigate their ability to improve cognitive function in an animal model of stroke, we examined the effect of ES cell transplantation on the ischemia-induced impairments of learning and memory that were observed in the Morris water maze for rats. Human embryonic stem(hES) cells(Mb03, as registered in the NIH) established from frozen-thawed blastocysts were partially differentiated according to the 4-/4+ protocol using retinoic acid. The formed embryoid bodies were dispersed by trypsin and a total of 3x104 cells were grafted into the hippocampus(CA1) 2 weeks after ischemia. Implantation of the hES cells produced significant reversals of the ischemia-induced deficit in learning and memory on the spatial learning task. These results demonstrated that grafted hES cells improved spatial recognition, and they may have valuable therapeutic potential as a treatment for stroke.

      • Estrogen-induced Egr1 is required for P4-dependent counteraction on estrogenic activities for embryo implantation

        Harmonized actions of ovarian estrogen (E2) and progesterone (P4) regulate cell proliferation and differentiation in the uterus with a spatiotemporal manner. Imbalance between the actions and levels of two major regulators often lead to infertility and gynecological diseases, such as endometriosis and endometrial cancer. While numerous works have shown that reduced expression and/or deletion of uterine factors associated with P4 signaling could disturb uterine physiology, local factor(s) to mediate E2 actions has not been extensively studied yet. Here we demonstrate that early growth response 1 (Egr1), a transcription factor which is rapidly induced in the uterus by E2, is required to maintain coordinated actions of E2 and P4 for uterine receptivity for embryo implantation. Given exogenous gonadotrophins to overcome LHβ deficiency in the pituitary of Egr1(-/-) mice, ovulation, fertilization and embryo development normally occurred in these mice. However, they showed complete failure of embryo implantation with reduced uterine responses to artificial decidualization stimuli. While serum levels of E2 and P4 in Egr1(-/-) mice were comparable, genes regulated by E2 and/or P4 in uterine epithelial cells (ECs) were aberrantly expressed on day 4 of pregnancy. Impaired P4 signaling along with absence of PR in ECs caused hypersensitive E2 responses shown as enhanced expression of E2-responsive genes such Muc1 and Ltf as well as reduced levels of P4-dependent genes, such as Ihh and Areg, in ECs of Egr1(-/-) mice. This is consistent with persistent proliferation in ECs and severely impaired proliferation in stromal cells (SCs) in Egr1(-/-) mice treated with E2+P4. Furthermore, primary co-culture of Egr1(-/-) ECs with Egr1(+/+) SCs and vice versa supported a notion that Egr1 itself is required for proper responses to two major regulators, E2 and P4, in both uterine cell compartments. Collectively, our results show that E2-induced Egr1 participates in P4-dependent modulation on E2 activities in the uterus by regulating a spectrum of genes essential for uterine receptivity and embryo implantation.

        Development of a Rapid and Accurate Identification Method for Citrobacter Species Isolated from Pork Products Using a Matrix-Assisted Laser-Desorption Ionization Time-of-Flight Mass Spectrometry (MALDITOF MS)

        Previous detection methods for Citrobacter are considered time consuming and laborious. In this study, we have developed a rapid and accurate detection method for Citrobacter species in pork products, using matrix-assisted laser desorption ionization time-of-flight (MALDI-TOF) mass spectrometry (MS). A total of 35 Citrobacter strains were isolated from 30 pork products and identified by both MALDI-TOF MS and 16S rRNA gene sequencing approaches. All isolates were identified to the species level by the MALDI-TOF MS, while 16S rRNA gene sequencing results could not discriminate them clearly. These results confirmed that MALDITOF MS is a more accurate and rapid detection method for the identification of Citrobacter species.
