      • 주요 선진국의 의약품 분류 및 재분류 사례 비교

        연구목적: 올바른 의약분업 시행을 위해 중요한 요소인 국내 의약품분류의 개선에 참고자료로 활용하 기 위해 주요 외국의 의약품분류 및 재분류 사례를 조사하여 우리나라 현황과 비교하고자 하였다. 대상 및 방법: 주로 비처방용 의약품들에 대한 국제적인 의약품분류 및 재분류 현황을 DB로 제공하고 있는 WSMI(world self-medication industry)에서 우리나라를 포함해 주요 9개국의 의약품분류 현 황 데이터베이스를 제공받아 비교 분석에 활용하였다. 결과: 우리나라는 비교 대상이 된 미국, 일본 등 주요 선진국에 비해 더 많은 종류의 비처방 의약품들 을 사용하고 있다. 처방약과 비처방약의 상대적 비율은 0.49로 비교 대상국 중 영국을 제외하고 가장 낮아 처방약에 비해 비처방약이 상대적으로 많은 상태에 있다. 각국의 재분류 사례를 비교한 결과, 우 리나라는 비교 대상국 중 분류전환(switch) 실적이 가장 낮은 것으로 확인되었다. 결론: 우리나라는 주요 선진국에서 사용하지 않는 불필요한 의약품들을 다수 사용하고 있으며 상대적 으로 더 많은 의약품들이 비처방약으로 분류되어 있는 것으로 평가된다. 의약품 재분류 제도의 운영도 비교 대상국들에 비해 매우 미흡한 것으로 확인되었다. 현행 의약품분류의 개선과 향후 의약품 재분류 제도의 활성화가 요구된다.

        This paper deals with stress analysis and deflections for probe stage systems connected to semi-conductor apparatus. Most of the real probe stage systems are composed of three mechanical parts such as slider, X-base and Y-base. In order to calculate the maximum stress and deflection for probe stage systems, three-dimensional finite element model is applied. Numerical results are obtained by changing various design parameters related to the above three parts. Through the numerical simulation, optimal design shape satisfying von Mises yield criterion is presented. The analysis was performed by using commercial COSMOS/M FEA code.

        편측 하악 과두골절의 관혈적 치료에 있어서 예후에 영향을 줄 수 있는 인자들에 관한 임상 통계학적 연구

        The purpose of this study is to reveal the factors leading to the problem of unilateral condylar fractures and suggest a treatment guideline of treatment for good prognosis in surgical treatment, The factors can be age, sex, fracture site, degree of displacement, posterior occlusion loss, post-operative alteration of condylar head position, post-operatlve condylar head resorption, and maxillomandibular fixation period. One hundred and eleven patients with unilateral condylar fractures, who were treated by surgical method from 1990 Feb. to 2000 Feb., were studied. Minimum follow-up period was 6 months. The results were as follows ; 1. In the age group of 41 ∼60, females had significantly higher complication rate than males, therefore we must be careful about treatment of female in this age group 2 In level I fractures of the mandibular condyle, because there were abundant complications when the patients were treated with fragment removal, conservative treatment is recommended over the surgical approach. 3. There were no differences in the complication rate, in the level Ⅱ, Ⅲ fractures. but were severe complications in the cases of patients treated by Dr.Nam's method or fragment removal. Therefore, open reduction and internal fixation is recommended over Dr.Nam's method or fragment removal. 4. In level Ⅳ fractures, open reduction and internal fixation is recommended 5. Although there was a higher complication rate depending on the degree of deviation, there was no correlation between the degree of deviation and development of complications in each level of fracture 6. Because the complication rate was higher in cases of condylar resorption, vertical dimension loss, and alteration of condylar head position, we must make an effort to prevent such complications during treatment

      • 킬레이트 형성기로서 Ethylenediamine을 도입한 다공성 중공사막에 의한 금속이온의 흡착특성

        폐수나 초순수 중의 금속이온을 효율적으로 회수하기 위해 킬레이트 형성기로서 에틸렌디아민(EDA)을 방사선그래프트중합법에 의해 생성된 그래프트 체인에 도입하였다. 그래프트율에 따른 에틸렌디아민기의 밀도를 측정하였으며, 그래프트율 177%의 경우 4.25mol/kg으로 나타났다. 에틸렌디아민기가 도입된 중공사막의 금속이온에 대한 흡착용량은 투과법에 의해 측정되었다. 투과압력의 변화에 무관한 흡착용량을 나타내었으며, 흡착된 금속이온은 용리액에 의해 100% 용리가 되었다. 구리(Cu)와 카드뮴(Cd)의 2성분 흡착에서는 구리이온이 카드뮴이온을 치환흡착 하는 것을 알 수 있었다. An ethylenediamine(EDA)-group as a chelate forming group was appended onto a hollow fiber membrane for effective recovery of metal ion from waste water or ultra pure water by radiation-induced graft polymerization of an epoxy-group-containing monomer, glycidylmethacrylate(GMA), and subsequent chemical modification. The density of EDA-group was investigated as a function of the degree of GMA grafting(dg), and it was 4.25mol/kg in case of the dg 177%. The adsorption of metal-ion onto toe EDA-hollow fiber membrane was measured by the permeation method. The metal adsorption capacity during permeation of a copper solution through the EDA-hollow fiber at various flow rate demonstrated that a higher flow rate is not affected to the collection efficiency. In the binary adsorption of Cu^2+/Cd^2+, we could see that Cu^2+ ion was substituted for Cd^2+ ion and adsorbed finally.

      • 방사선 조사를 받은 흰쥐 신경뇌하수체 관문의 미세구조 : 관문의 새로운 개념설정 Establishment of a New Concept on the Barrier

        신경뇌하수체는 뇌의 일부지만 뇌실둘레기관(circumventricular organ)으로서 전형적인 혈액-뇌관문이 없으므로 혈관 안팎의 물질이동이 자유로운 신경내분비기관이다. 이 실험에서는 심한 방사선 조사시에 혈액-신경뇌하수체관문의 형태적 대응을-연구함으로써, 관문의 특성을 분석했으며, 관문의 구성을 구체적으로 설정하여 이를 개념화 할 것을 제안하였다. 체중 200-250g의 숫흰쥐를 sodium thiopenthal 로 마취시킨 후 Mitsubishi선형가속기로 방사선 조사를 시켰다. 조사조건은 거리 80cm, 조사구역 30 X 30cm, 조사깊이 1.2cm, 조사속도 분당 200 rads로 하여 실험군에 따라 3,000 rads 또는 6,000 rads가 되도록 했다. 조사후 6시간, 2일, 6일후에 각군의 동물을 도살 하여 신경뇌하수체를 떼어냈다. 떼어낸 조직은 1% glutaradehyde- 1% paraformaldehyde액에 1차 고정하고, 1% osmium tetroxide액에 2차 고정하였고, araldine혼합액에 포매된 조직은 절편을 만들어 uranyl acetate와 lead citrate액으로 염색해서 전자현미경으로 관찰하였다. 주로 모세혈관주위공간에서 관문구조를 중심으로 관찰한 결과 3,000 rads 조사군과 6,000 rads조사군 모두가 비슷하게 심한 변화를 보였고, 변화의 정도가 2일군에서 가장 심했으며 6일군에서는 약간 안정되는 모습이었으나 변화의 양상은 모든 실험군을 통해서 비슷하였다. 가장 두드러진 변화는 모세혈관주위공간에서 나타났는데 공간의 확장, 큰포식세포의 증가와 활성화, 신경뇌하수체세포속에 들어 있던 신경종말들의 이탈, 먼지 같은 물질의 증가 등을 들 수 있었다. 특히 큰 포식세포의 변화는 매우 특징적이어서 넓은 세포질관(판상족, lamellipoda)을 형성하여 모세혈관과 신경뇌하수체세포사이를 차단시킬 뿐아니라 왕성한 포식작용으로 공간내의 축삭종말들을 용해시켰다. 큰포식세포의 세포질안에는 과립형질내세망들이 매우 발달하였고, 세포질 영역이 매우 넓어진 것으로 볼 때, 이들이 기동타격대로서 활발히 움직이는 매우 효율적인 관문의 구성성분으로 생각되었다. 특히 신경뇌하수체는 전형적인 혈액-뇌관문이 없으므로 신경내분비기능의 수행에는 유리하나 면역활성물질이나 독성물질등 뇌에 해로운 물질의 왕래를 차단하여 제거하거나, 과다하게 분비된 신경호르몬을 조절하는 일이 중요하다고 볼 때, 큰포식세포가 이같은 역할에 적합한 구성성분이라는 결론을 얻었다. 이 실험에서는 효율성이 높은 관문으로서 혈액-신경뇌하수체관문이 구성성분을 차례로 기재하고 이를 개념화 할 것을 제안한다. 모세혈관과 축삭종말사이에 형성되어 있는 혈액-신경뇌하수체관문이 구성을 차례로 표기하면 다음과 같다 (그림 9,10). 1. 유창모세혈관 내피 2. 모세혈관 내피의 기저판 3. 혈관주위세포와 기저판("이차방어선", 한정된 범위내에서 움직일 수 있다). 4. 큰포식세포, 필요에 따라 판상족(lamellipoda)을 넓게 뻗는다.("삼차방어선", 모세혈관 주위공간 속을 이동해 다니는 "기동타격대") 5. 신경뇌하수체세포의 기저판 6. 신경뇌하수체세포의 세포질돌기(호르몬분비 조절) To study the morphological characteristics of the blood-neurohypophysis barrier system in a severely altered situation, the heads of rats were exposed to heavy X-irradiation. Rats weighing 200-250 g each were anesthetized with sodium thiopenthal, and placed on the table of Mitsubishi linear accelerator ML-4MV. Only heads of rats were placed within the exposure area of 30 cm X 30 cm. Irradiation was processed at the distance of 80 cm, with the speed of 200 rads/min in the radiation depth of 1.2cm. Total doses were 3,000 rads or 6,000 rads according to the animals of the different experimental groups. Rats were sacrificed on 6 hours, 2 days or 6 days following radiations. Tissue blocks of neurohypophyses were fixed in the 1% glutaraldehyde-1% paraformaldehyde solution, and they were refixed in the 1% osmium tetroxide solution. Ultrathin sections were contrasted with uranyl acetate and lead citrate solutions. Electron micrographs exhibited dramatic changes within the perivascular space of neurohypopysis. The perivascular space was greatly enlarged, and it contained many macrophages, floating axonal endings and plentiful flocculent materials. The enlargement was largest in the 2 day-group animals. Many macrophages are activated, and it showed tremendous cytoplasmic lamellipoda. Wide plate of macrophagic cytoplasm usually engulfed the floating axon terminals, and are located between the cappillary and the pituicytes. The situation gave the impression that macrophages prevent, filter and/or retrieve the excessive materials transported between the capillary and the axonal endings. Since the perivascular microenvironment of neurohypophysis is more vulnerable as compared with those brain areas equipped with blood-brain barrier, the functional barrier system by macrophages in the neurohypophysis should have important role. The concept of blood-neurohypophyseal barrier with the following components is proposed. The components of the barrier system from the blood pool to the hormonal pool are 1. Endothelium of fenestrated capillary 2. BAsal lamina of endothelium 3. Pericyte("second line of defense". mobile in a limited area) 4. Macrophage, extending its wide cytoplasmic plate(the lamelipoda) in need("third line of defense". freely movable in the perivascular space, "active surveillance system") 5. Basal lamina of pituicyte 6. Cytoplasmic processes of pituicyte

        The twin sisters case of folie simultanee is reported here in this paper. A case of folie simultanee is described as two or more people becoming psychotic simultaneously and sharing the same delusion. The results of psychodynamic approach for this case are as follows. 1. There are a variety of conflicting hypotheses on the orgin of the disorder, ranging from the etiological explanation of organic factor to various psychodynamic approach. In this case, there is no evidence of hereditary factor on the laboratory findings and family history. Therefore the authors stressed that the psychodynamic formulation of the disorder is on the importance .of the etiological rol. It seem significant to describe that the etiology is multidimensional and includes factors such as character structure formation in processing of object relations, the emotional relationship between the partners, socio-environmental conditions and hereditary predisposition. 2. There are two issues in folie simultanee. The one is related to diagnosis. It is consider to distinguish the inductor from the recipient. The other is subject to treatment. It is important to determine whether separate the inductor and recipient by admitting the inductor for a psychiatric center or not. Furthermore, the inductor and recipient's weak ego state should be recognized in planning various psychiatric treatments. 3. It could be found in the role reversal phenomenon between the inductor and recipient during long-term course.

        朝鮮後期 燕行錄에 나타난 遼東

        This paper is aimed at reviewing various aspects of discourse on Liantong of the period through Liaotong-related text in yeonghaengnok in the latter period of Chosun Dynasty. The discourse on Looting was spread especially after Pyeongja Hoarn- Manchu's invasion in 1636-, and it was formed and developed through the yeonhaengnok. We can see three aspects in the discourse of Liaotong that was captured in Yeonhaengnok. First, It is the memorial discourse of the ancient territory. The recognition the fact that Liantong was our ancient territory was rapidly spread while Yeonhaengsa traveled through the Liaotong area. This was successfully applied to forget the disgrace of the Manatee's invasion. Secondly, It's the discourse on space of Liaotong. This focused on examination of the space meaning for the Liaotong with experience of prosperous street and vast open field. Through this effort, they had confirmed the Liaotong as the space of communication, which Chosen could be connected to the world. Lastly, It's the discourse on the history of Ming-Ching war. The discourse on finding out the origin of Liaotong's political situation was found when passed on the Ming-Ching war area. this discourse tried to find out the way of Chorine's right recognition of Liaotong political situation analyzing the reason for the shift of Ming-Ching Dynasty unlike the effort to overcome the disgrace of Manta's invasion through the memory of the ancient territory. As explained above, we confirm that the discourse of Liaotong of the latter period of Chosen Dynasty does not merely rely on the memory of the ancient territory like the one of Koguryo.

        The Sae-Mauel Undong is recognized that it is to develope community Consciousness [We-feeling], sprits of deligious, cooerative sprits and sprits of self-help in order to grow the wealth of this nation. The social welfare institutions in the Sae-Mauel Undong can be accepted in aspects of social welfare approach because they are to promote the well-being of all people. The social welfare institutions contain a public assembly hall, a public workshop, a public palyground for children, a public bath in the village. These institutions are very important because they elevate the cooperative sprits. Then, in the present time these institutions are excluded out or disregarded in the Sae-Mauel Undong. It is necessary in the Sae-Mauel Undong to obtain the construction of these institution. In future, according as the Sae-Mauel undong should be continued, the rural society shall be modernized. Therefore, there should be prepared of theprivisions for enrichment of community consciousness among villager, these institutions shall contribute to develope the community consciousness.
