Background and Purpose To assess the long-term outcomes of intracranial dural arteriovenous fistula (DAVF) treated with stereotactic radiosurgery (SRS) alone or embolization and SRS (Emb- SRS) and to develop a grading system for predicting DAVF obliteration. Methods This multi-institutional retrospective study included 200 patients with DAVF treated with SRS or Emb-SRS. We investigated the long-term obliteration rate and obliteration-associated factors. We developed a new grading system to estimate the obliteration rate. Additionally, we compared the outcomes of SRS and Emb-SRS by using propensity score matching. Results The 3- and 4-year obliteration rates were 66.3% and 78.8%, respectively. The post-SRS hemorrhage rate was 2%. In the matched cohort, the SRS and Emb-SRS groups did not differ in the rates of obliteration (P=0.54) or post-SRS hemorrhage (P=0.50). In multivariable analysis, DAVF location and cortical venous reflux (CVR) were independently associated with obliteration. The new grading system assigned 2, 1, and 0 points to DAVFs in the anterior skull base or middle fossa, DAVFs with CVR or DAVFs in the superior sagittal sinus or tentorium, and DAVFs without these factors, respectively. Using the total points, patients were stratified into the highest (0 points), intermediate (1 point), or lowest (≥2 points) obliteration rate groups that exhibited 4-year obliteration rates of 94.4%, 71.3%, and 60.4%, respectively (P<0.01). Conclusions SRS-based therapy achieved DAVF obliteration in more than three-quarters of the patients at 4 years of age. Our grading system can stratify the obliteration rate and may guide physicians in treatment selection.

'Healthy Japan 21' : A New Perspective on Health Promotion Policy for Japan in the 21st Century

'Healthy Japan 21' is a new health policy that has been proposed for the 21st century: it embodies a totally new concept for its viewpoints and methods. To start with, for its goal, the focus is placed on the 'quality of life' or a life that is free of diseases, rather than mere prolongation of life. For its doctrine, the emphasis has shifted dramatically from improving the health of the entire population(the traditional approach for health improvement) to 'achieving an ideal health status for each individual. The ultimate aged society that arrives first in Japan is a society in its ultimate form for human being. Why did Japan become westernized, giving up her traditional culture? Why did she go through industrialization, sacrificing her nature? And why does she try so hard to industrialize the developing countries? These efforts are all preparation for the arrival of a ultimate aged society. During the 20th century, we believed in unlimited possibilities and expanded our social frontier. In the 21st century, on the other hand, a super-aged society(the ultimate society), a glimpse of which we have witnessed from time to time, will descend on us sooner or later. It is expected to arrive first in Japan. 'Healthy Japan 21' is intended to prepare for the arrival of the hitherto unheard of super-aged society by building the physiological basis of people. This policy is social experimentation on an immense social scale, in which questions are posed on the understanding of health, the relationship between individuals and society, the relationship between administration and citizens, the manner by which central and local governments operate, and the new relationship between prevention and therapy, 'Healthy Japan 21' may be summarized as an experiment on a huge scale directed to the ultimate form of human society, in which Japan and each of her citizens play a role and set an example for the rest of the world. Even just by considering various approaches newly suggested for this venture, one may be convinced that it is a policy with features suitable for a country that has already achieved the world's highest longevity.

Study DesignCombination of retrospective and prospective study.PurposeWe aimed to compare the clinical outcomes between local fixation surgery and spinopelvic fixation surgery for the treatment of kyphosis secondary to osteoporotic vertebral fractures with spinopelvic malalignment.Overview of LiteratureThe clinical characteristics of patients with rigid kyphosis due to osteoporotic vertebral fracture differ from that of middle-aged patients with vertebral fractures in terms of bone fragility and presence of spinopelvic malalignment. Little is known about the surgical strategies for these deformities, most especially the extent of fusion of vertebra involved.MethodsWe analyzed 24 patients with vertebral osteotomy at the level of the fracture and spinal fixation without pelvic fixation (local group), and 22 patients with vertebral osteotomy and pelvic fixation (pelvic group). Radiographic parameters, the incidence of proximal junctional kyphosis (PJK), distal junctional kyphosis (DJK), rod fractures, and the Oswestry Disability Index (ODI) were compared between the two groups over a 2-year follow-up period.ResultsIn the pelvic group, postoperative spinopelvic parameters significantly improved, with the improvements maintained. No remarkable changes in spinopelvic parameters were seen in the local group. The mean ODI scores 2 years after surgery were 45.3 and 33.0 in the local and pelvic group, respectively (p-value <0.05). There was no significant difference in the incidence of PJK in the local and pelvic groups, but there was a higher rate of DJK (41.7%) in the local group. In contrast, rod fractures were more common in the pelvic group (45.5%). Patients with DJK had higher ODI scores 2 years after surgery (52.0 in DJK patients vs. 34.8 in non-DJK patients; p-value <0.05).ConclusionsFor patients with rigid kyphosis due to osteoporotic vertebral fractures, better spinopelvic alignment and health-related quality of life can be achieved through extensive corrective surgery with pelvic fixation.

Interfaces play important role in future III-V compound semiconductor ULSIs. The present status of understanding and control of comound semiconductor interfaces involving semiconductor, insulator and metal are reviewed and key issues for future research and development are identified with a particular emphasis laid on the passivation technology. Presence of a strong correlation among interfaces is pointed out and model's for unified understanding interface's re discussed.
Relationships among Behavior, Physiological States and Body Weight Gain in Grazing Holstein Heifers

This study examined the behavior of dairy heifers and the factors affecting the performance of them on pasture. Behavior of 10 Holstein heifers in a herd of 25 animals that rotationally grazed five 8-ha pastures was observed and recorded every 5 minutes during 24 hours; body weights were measured once a month from June to October. Blood and rumen fluid samples were collected from 5 of them bimonthly. Chemical composition was analyzed for the forage samples collected each month. CP content (DM basis) of herbage ranged from 12.2 (June) to 17.2% (October) and ADF from 31.1 (October) to 39.1% (July). Standing (posture) time was different significantly among months (p<0.001) ranging from 48.3 to 61.3% of 24 hours and was longer in July and August (61.3% and 58.3%, respectively) when ADF content of herbage was higher than in the other months. Grazing time which significantly differed among months (p<0.001) ranged from 29.1 to 41.6% of 24 hours and was shorter in June and September (29.1% and 33.0%, respectively) when ADF content was lower than in the other months. Average DG through the experiment period was 0.74 kg/day. August was the lowest in DG (0.41 kg/day) and the longest in rumination time and standing-rumination time among months. Animals of higher DG had a shorter standing time (r=-0.36, p<0.01) and a longer lying-rumination time (r=0.55, p<0.001) throughout the experiment. Total protein concentration in blood ranged from 9.04 to 9.64 g/dl and was negatively correlated with DG (r=-0.65, p<0.05). Phospholipid concentration of blood ranged from 119.66 to 156.40 mg/dl and was negatively correlated with DG (r=-0.57, p<0.05). VFA in rumen fluid, acetic acid proportion (ranging from 69.35 to 74.76%) and butyric acid proportion (ranging from 7.18 to 12.05%) showed significant differences among months (p<0.05, p<0.001, respectively). Butyric acid proportion was significantly related with DG (r=0.60, p<0.05).

<▼1><P>A novel red emitting, Ce<SUP>3+</SUP>-activated Sr3Sc4O9 phosphor were discovered, and its luminescent properties were investigated.</P></▼1><▼2><P>Most conventional white light emitting diodes (white-LEDs) that are widely used as a new lighting system in next generation lights with Y3Al5O12:Ce<SUP>3+</SUP> (YAG:Ce<SUP>3+</SUP>)-based phosphors have a low colour rendering index (CRI) because the YAG:Ce<SUP>3+</SUP> phosphor shows a weak emission intensity in the red spectral region. Therefore, discovering a red-emitting phosphor with a high-efficiency is quite important to enhance the CRI of white-LEDs. In this study, we successfully discovered a novel, red-emitting Ce<SUP>3+</SUP>-activated Sr3Sc4O9 phosphor that can be excited by blue-light irradiation at 425 nm. A crystal structure of the host material was first determined by Rietveld refinement, which indicated that it should be isostructural with Ba3Ln4O9 (Ln = Sc, Y and Dy-Lu). As the Ce<SUP>3+</SUP> content increased, the X-ray diffraction patterns shifted to a lower angle, which suggested that the Ce<SUP>3+</SUP> ion could substitute Sc in the Sr3Sc4O9 host. Under UV and blue-light excitation, the Ce<SUP>3+</SUP>-activated Sr3Sc4O9 phosphor exhibited a broad emission band with a maximum peak at 620 nm, and its full width half maximum (FWHM) was 180 nm (4530 cm<SUP>−1</SUP>). The highest emission intensity was obtained for Sr3(Sc0.997Ce0.003)4O9, and the internal quantum efficiency of this phosphor under excitation at 425 nm was 53%. To the best of our knowledge, the emission band of the Ce<SUP>3+</SUP>-activated Sr3Sc4O9 phosphor is the longest wavelength for a reported Ce<SUP>3+</SUP>-activated oxide phosphor.</P></▼2>
