        A Practical Grading Scale for Predicting Outcomes of Radiosurgery for Dural Arteriovenous Fistulas: JLGK 1802 Study

        Background and Purpose To assess the long-term outcomes of intracranial dural arteriovenous fistula (DAVF) treated with stereotactic radiosurgery (SRS) alone or embolization and SRS (Emb- SRS) and to develop a grading system for predicting DAVF obliteration. Methods This multi-institutional retrospective study included 200 patients with DAVF treated with SRS or Emb-SRS. We investigated the long-term obliteration rate and obliteration-associated factors. We developed a new grading system to estimate the obliteration rate. Additionally, we compared the outcomes of SRS and Emb-SRS by using propensity score matching. Results The 3- and 4-year obliteration rates were 66.3% and 78.8%, respectively. The post-SRS hemorrhage rate was 2%. In the matched cohort, the SRS and Emb-SRS groups did not differ in the rates of obliteration (P=0.54) or post-SRS hemorrhage (P=0.50). In multivariable analysis, DAVF location and cortical venous reflux (CVR) were independently associated with obliteration. The new grading system assigned 2, 1, and 0 points to DAVFs in the anterior skull base or middle fossa, DAVFs with CVR or DAVFs in the superior sagittal sinus or tentorium, and DAVFs without these factors, respectively. Using the total points, patients were stratified into the highest (0 points), intermediate (1 point), or lowest (≥2 points) obliteration rate groups that exhibited 4-year obliteration rates of 94.4%, 71.3%, and 60.4%, respectively (P<0.01). Conclusions SRS-based therapy achieved DAVF obliteration in more than three-quarters of the patients at 4 years of age. Our grading system can stratify the obliteration rate and may guide physicians in treatment selection.

        'Healthy Japan 21' : A New Perspective on Health Promotion Policy for Japan in the 21st Century

        'Healthy Japan 21' is a new health policy that has been proposed for the 21st century: it embodies a totally new concept for its viewpoints and methods. To start with, for its goal, the focus is placed on the 'quality of life' or a life that is free of diseases, rather than mere prolongation of life. For its doctrine, the emphasis has shifted dramatically from improving the health of the entire population(the traditional approach for health improvement) to 'achieving an ideal health status for each individual. The ultimate aged society that arrives first in Japan is a society in its ultimate form for human being. Why did Japan become westernized, giving up her traditional culture? Why did she go through industrialization, sacrificing her nature? And why does she try so hard to industrialize the developing countries? These efforts are all preparation for the arrival of a ultimate aged society. During the 20th century, we believed in unlimited possibilities and expanded our social frontier. In the 21st century, on the other hand, a super-aged society(the ultimate society), a glimpse of which we have witnessed from time to time, will descend on us sooner or later. It is expected to arrive first in Japan. 'Healthy Japan 21' is intended to prepare for the arrival of the hitherto unheard of super-aged society by building the physiological basis of people. This policy is social experimentation on an immense social scale, in which questions are posed on the understanding of health, the relationship between individuals and society, the relationship between administration and citizens, the manner by which central and local governments operate, and the new relationship between prevention and therapy, 'Healthy Japan 21' may be summarized as an experiment on a huge scale directed to the ultimate form of human society, in which Japan and each of her citizens play a role and set an example for the rest of the world. Even just by considering various approaches newly suggested for this venture, one may be convinced that it is a policy with features suitable for a country that has already achieved the world's highest longevity.

        Dielectric properties of glass-ceramics with Ba<sub>1-x</sub>Y<sub>2x/3</sub>Nb<sub>2</sub>O<sub>6</sub> nanocrystals and laser patterning of highly oriented crystal lines

        <P>Glasses based on the system of Y2O3-BaO-Nb2O5-B2O3, corresponding to the pseudo-binary 64Ba(1 - x)Y(2x/3)Nb(2)O(6)-B2O3 with x = 0.1-0.5, were prepared, and their thermal properties, electronic polarizabilities, optical basicities, and crystallization behavior were clarified. The glasses showed the glass transition temperature of 602-610 degrees C, crystallization peak temperature of 686-676 degrees C, and refractive index of 1.967-2.007. All the glasses showed the bulk crystallization behavior, providing Ba1 - xY2x/3Nb2O6 nanocrystals with a tetragonal tungsten bronze structure. It was found from high resolution transmission electron microscope observations that the morphology of Ba1 - xY2x/3Nb2O6 nanocrystals is ellipsoidal and their average particle size is in the range of 20-40 nm. The glass-ceramics with Ba1 - xY2x/3Nb2O6 nanocrystals showed high dielectric constants (frequency: 1 kHz) of similar to 90 at room temperature and a ferroelectric nature. Highly oriented Ba1 - xY2x/3Nb2O6 crystal lines were patterned by laser (continuous wave Yb:YVO4 fiber laser with a wavelength of 1080 nm) irradiation with the condition of a laser power of 1.9 W and a laser scanning speed of 4 pm/s and with the laser focal position of 20 mu m beneath from the surface in 2NiO-1.1Y(2)O(3)-29.8BaO-33.1Nb(2)O(5)-36B(2)O(3) glass.         Lignosulfonic acid promotes hypertrophy in 3T3-L1 cells without increasing lipid content and increases their 2-deoxyglucose uptake

        Objective: Adipose tissue plays a key role in the development of obesity and diabetes. We previously reported that lignosulfonic acid suppresses the rise in blood glucose levels through the inhibition of ${\alpha}$-glucosidase activity and intestinal glucose absorption. The purpose of this study is to examine further biological activities of lignosulfonic acid. Methods: In this study, we examined the effect of lignosulfonic acid on differentiation of 3T3-L1 cells. Results: While lignosulfonic acid inhibited proliferation (mitotic clonal expansion) after induction of differentiation, lignosulfonic acid significantly increased the size of accumulated lipid droplets in the cells. Semi-quantitative reverse transcription polymerase chain reaction analysis showed that lignosulfonic acid increased the expression of the adipogenic transcription factor, peroxisome proliferator-activated receptor gamma ($PPAR{\gamma}$), leading to increased glucose transporter 4 (Glut-4) expression and 2-deoxyglucose uptake in differentiated 3T3-L1 cells. Additionally, feeding lignosulfonic acid to diabetic KK-Ay mice suppressed increase of blood glucose level. Conclusion: Lignosulfonic acid may be useful as a functional anti-diabetic component of food.

        Flow Field Change before Onset of Flow Separation

        Jets issuing through small holes in a wall into a freestream has proven effective in the control of flow separation. This technique is known as the vortex generator jet (VGJs) method. If a precursor signal of separation is found, the separation control system using VGJs can be operated just before the onset of separation and the flow field with no separation is always attained. In this study, we measured the flow field and the wall static pressure in a two-dimensional diffuser to find a precursor signal of flow separation. The streamwise velocity measurements were carried out in the separated shear layer and spectral analysis was applied to the velocity fluctuations at some angles with respect to the diffuser. The pattern of peaks in the spectral analysis changes as the divergence angle increases over the angle of which the whole separation occurs. This change in the spectral pattern is related to the enhancement of the growth of shear layer vortices and appears just before the onset of separation. Therefore, the growth of shear layer vortices can be regarded as a precursor signal to flow separation.

        Halophilic and salts tolerant protoplast cultures of mangrove plants, Sonneratia alba and Avicennia alba

        The effects of sea salts, NaCl, KCl, $MgCl_2$, $MgSO_4$, and $CaCl_2$, on the growth of protoplast cultures of two mangrove species, Sonneratia alba and Avicennia alba, were investigated using 96-well culture plates. Plants of these two species naturally grow at the seaward side of a mangrove forest. Cotyledon protoplasts of S. alba showed halophilic nature to NaCl, KCl, and $MgCl_2$ at low concentrations (10-50 mM) when cultured in Murashige and Skoog's (MS) medium containing 0.6 M mannitol. $CaCl_2$ at a concentration higher than 25 mM was inhibitory to cell growth. On the other hand, in protoplast culture of A. alba suspension cells, which were induced from cotyledon tissues, in the modified amino acid (mAA) medium containing 1.2 M sorbitol, tolerance to NaCl, $MgCl_2$ and $MgSO_4$ were observed at a wide range of concentrations up to 400 mM. $CaCl_2$ was always inhibitory for cell divisions in A. alba, but stimulatory for spherical enlargement of cells. However, no difference in cell enlargement was observed among other salts. Similarity and difference in reactivity to salts between protoplasts and suspension cells from our previous studies were discussed in relation to the site of salt tolerance or halophilic adaptation within mangrove cells. For protoplast cultures, the site(s) for response of S. alba and A. alba are located in the cytoplasm and/or the cell membrane.

        INTRODUCTION : Various types of organic reactions in the crystalline state, so-called "topochemical reaction", have been reported in this century." Among the reactions, [2+2] photocycloaddition of diolefin crystals are one of the most intensively investigated examples. In the topochemical reaction in a pure sense, the space group symmetry is perfectly preserved throughout the reaction as were demonstrated by the crystalline-state photopolymerization of 2,5-distyrylpyrazine (2,5-DSP) and photodimerization of propyl $\alpha$-cyano-4-[2-(4-pyridyl) ethenyl]cinnamate. 2,5-DSP was initially prepared by Franke and in 1958 Koelsch and Gumprecht described briefly in their article of diazine-derivatives that it was found that the compound (2,5-DsP) was converted into a colorless insoluble polymer (\ulcorner) dec. 331~331$\circ$ when the solid was exposed for a few hours to ultraviolet light. Wright described in his book as if Koelsch and Gumprecht had investigated both the structure of the photoproduct and the crystal structure 2,5-DSP in detail. However, in the paper, they have not described any study on chemically correct analysis and crystallography of the resulting photoproduct at all. In 1967, one of present authors (M. H.) independently made the same observation as Franke that brilliant yellow crystal of 2,5-DSP was converted into powdery white substance under the sunlight in the course of a preparative study of pyrazine-2,5-dicarboxylic acid from 2,5-DSP. He investigated this phenomenon and concluded for the first time that a linear highmolecular-weight polymer crystal ([$\eta$] > 3.0 in $CF_3COOH$ solution) with recurring cyclobutane units in the main chain, had been produced from 2,5-DSP crystal by the action of sunlight.on of sunlight.
