The current study was designed to evaluate the positive rates of arthropod allergens by Korean inhalant panel of MAST-CLA in patients with allergic rhinitis. One hundred sixty patients, diagnosed to allergic rhinitis in an Out Patient Center of Soonchunhyang University Chunan Hopsital during Aug 1998 to July 2000, were studied. 1. The overall positive rate among 35 specific allergens was 59.4% (95 patients). 2. The arthropod related allergen accounted for 82.1% (78 patients) of the positive rate. 3. Each positive rate of Dermatophagoides farinae (Df), Dermatophagoides pteronyssinus (Dp), house dust (Hd), and cockroach mix (Cm) was 45.0% (72 patients), 43.1% (69 patients), 28.1% (45 patients), and 8.8% (14 patients), respectively. 4.Significant agreement between arthropod allergens was observer (P < 0.05). 5. The obseved agreement between Df and Dp was 95.6%, showing most significant Kappa index (Kappa = 0.911, P < 0.001). There results suggested that although MAST-CLA is a useful measure of identifying specific allergens, the specificity of two mite allergen relatively low, implying because of common allergen between mites in Korean panel of MAST-CLA. Our data also supported that arthropod is the major cause of allergen in patients with allergic rhinitis, however, the MAST-CLA should be modified in order to exclude common allergen of mite and to include more specific mite allergen.

We investigated the association between the oxidative stress and the concentration of heavy metals in blood among welders. The study subjects were male manual CO₂welders(N=127) exposed to several heavy metals. The age distribution was from 20 to 63(GM=33.7) and the geometric mean(GM) of work duration was 4.2 years. We analyze the level of blood lead, cadmium, manganese and chromium which was related with the oxidative stress in the animal studies. Blood selenuim, plasma malondialdehyde(MDA), plasma α-tocopherol and erythrocyte superoxide dismutase(SOD) were analyzed as biological markers of the oxidative stress. The results of univariate analysis showed that the plasma MDA was significantly correlated with α-tocopherol(r=0.327) and blood selenium(r=0.313) concentration. But we could not find out any relationship between the erythrocyte SOD activities and those metal concentrations. Neither smoking nor alcohol consumption was related to the level of heavy metals and the oxidative stress biomarkers. As the results of multiple regression analysis with stepwise selection method, 1) the change of MDA level were dependent with the level of α-tocopherol and blood selenium concentration(R²=19.7), 2) the plasma α-tocopherol level was dependant with MDA and blood chromium concentration(R²=15.6), 3) in case of erythrocyte SOD activity, only the duration of work was significantly selected.

Aims: The Baveno VI guidelines proposed that esophagogastroduodendoscopy(EGD) for screening esophageal varices(EV) can be avoided if liver stiffness(LS) measured by transient elastography(TE) is less than 20 kPa and platelet count is greater than 150,000 cells/μL. However while validation of TE is well proven, 2D-SWE has not been sufficiently validated for EV prediction. The aim of this study is to predict the presence of EV by non-invasive tools combined with 2D-SWE and to compare the diagnostic capabilities with TE. Methods: Between January 2015 and October 2017, 258 patients with compensated advanced chronic liver disease(cACLD) who underwent 2D-SWE and EGD consecutively were enrolled. The AUROC was calculated to evaluate the accuracy of the prediction for the presence of EV using 2D-SWE, model combining 2D-SWE and platelet count(PC), liver stiffness to spleen/platelet score (LSPS) score and platelet-spleen ratio (PSR) score. 177 patients who underwent simultaneous TE examination were on the subgroup analysis was done. Results: The mean age was 56.8±10.7 years and most common etiology was chronic hepatitis B (45.0%). 83.1% of patients were in Child class A. Prevalence of all-size varices was 41.1%. 2D-SWE alone has good ability to discriminate varices (AUROCs : 0.750, 95% CI, 0.690 to 0.809). Model combining 2D-SWE with platelet count and LSPS using 2D-SWE has better discriminative ability for varices and AUROCs were 0.793 (95% CI, 0.738 to 0.848) and 0.813 (95% CI, 0.760 to 0.865) respectively. For 177 patients who performed TE and 2D-SWE simultaneously, there was no difference in predictive abilities when other factors such as albumin, bilirubin, ALT, Platelet count, hemoglobin, spleen diameter were adjusted in multivariate analysis. Conclusions: 2D-SWE combined with platelet count and LSPS seemed to be useful to predict EV. In addition, 2D-SWE has similar diagnostic performance with TE for predicting presence of EV.

Resveratrol, a polyphenolic compound, is a naturally occurring phytochemical and is found in a variety of plants, including food such as grapes, berries and peanuts. It has gained much attention for its potential anticancer activity against various types of human cancer. However, the usefulness of resveratrol as a chemotherapeutic agent is limited by its photosensitivity and metabolic instability. In this study the effects of a synthetic analogue of resveratrol, HS-1793, on the proliferation and apoptotic cell death were investigated using MCF-7 (wild-type p53) and MDA-MB-231 (mutant p53) human breast cancer cells. HS-1793 inhibited cell growth and induced apoptotic cell death in a concentration-dependent manner. The induction of apoptosis was determined by morphological changes, cleavage of poly(ADP-ribose) polymerase, alteration of Bax/Bcl-2 expression ratio and caspase activities. Flow cytometric analysis revealed that HS-1793 induced G2/M arrest in the cell cycle progression in both types of cells. Of note, HS-1793 induced p53/p21(WAF1/CIP1)-dependent apoptosis in MCF-7 cells, whereas it exhibited p53-independent apoptosis in MDA-MB-231 cells. Furthermore, HS-1793 showed more potent anticancer effects in several aspects compared to resveratrol in MCF-7 and MDA-MB-231 cells. Thus, these findings suggest that HS-1793 has potential as a candidate chemotherapeutic agent against human breast cancer.

Background/Aims: Small bowel tumors are uncommon accounting for only 3 to 6% of all gastrointestinal tumors. The diagnosis of small bowel tumors has been usually made at an advanced stage because of the infrequency and absence of symptoms specific for early diagnosis, resulting in poor prognosis. This study was conducted to investigate the clinical features of small bowel tumors in Korea. Methods: We reviewed and analyzed clinical manifestations, laboratory results and survivals in 63 patients with small bowel tumors diagnosed at the Asan Medical Center between June 1989 and January 1996. Results: Among 63 tuors reviewed, 58 were malignant tumors and 5 were benign tumors. The malignant tumors consisted of 25 adenocarcinomas, 15 malignant lymphomas, 12 leiomyosarcomas and 6 metastatic carcinomas. Adenocarcinomas were located mainly in the duodenum and malignant lymphomas in the ileum or duodenum, while leiomyosarcomas were evenly distributed. Clinical symptoms of these patients included abdominal pain (71.4%), palpable mass (12.7%), weight loss (10.0%) and bleeding (8.0%). Anemia and positive stool occult blood test were found in 55.0% and 46.7%, respectively. The median duration of symptoms before the diagnosis was 4 weeks in malignant tumors and 36 weeks in benign tumors. Twenty-seven of 30 patients (90.0%) with duodenal tumors were diagnosed by duodenoscopy, while small bowel series and CT scans detected 9 of 11 (82.0%) and 25 of 35 (71.4%) small bowel tumors, respectively. The incidence of metastasis to regional lymph nodes and distant organs was higher in adenocarcinoma and malignant lymphoma than leiomyosarcoma. The 2-year survival rates for patients with adenocarcinomas, malignant lymphomas, and leiomyosarcomas were 23, 57, and 60%, respectively. Conclusions: These results suggest that small bowel tumors should be considered in patients having unexplained abdominal pain, anemia, and/or positive stool occult blood test.

In Albright's hereditary osteodystrophy (AHO) including the syndromes of pseudohypoparathyroidism (PHP) and pseudopseudohypoparathyroidism (PPHP), multiple areas of intracutaneous ossification are often encountered. The characteristic features are short stature, round face, short neck, obesity; cutaneous ossifications, and various skeletal anomalies including short metacarpal arid metatarsal bones, curve of radius, arid brachydactyly. The patient was a 10-month-old male infant. He presented slightly depressed erythematous hard plaques on the left upper chest and left thigh. We had taken biopsies from both skin lesions, confirming cutaneous ossification or bone formation. He also had the characteristic features of AHO. He had a history of admission due to patent ductus arteriosus and atrial septal deft. The laboratory results showed slightly decreased calcium; increased phosphorus and PTH levels. The patient received no specific corrective measures because his calcium and phosphorus levels were not far from normal values until newly developed similar skin lesions appeared.

The Role of Spleen Volume in Predicting Portal Hyper-tension in Patients with Cirrhosis

Aims: It is already well known that the size of the spleen increases as liver cirrhosis aggravates. However, there is few studies for establishing the correlation between portal hypertension and spleen volume (SV). In this study, we aimed to see the relationship between the splenic volume and hepatic venous wedge pressure (HVPG) and to find what determine the portal pressure. Methods: From May 2007 to February 2018, 337 cirrhotic patients who measured SV and HVPG simultaneously were included. SV was measured based on the ellipsoid volume calculation formula by measuring length, height, and width. Other clinical and biochemical factors which may contribute to increase portal pressure were also collected. Results: Mean HVPG was 14.2 ± 5.3 and Child-pugh class A, B, and C patients were 34%, 40%, and 16%, respectively. A significant positive correlation was noted between splenic volume and HVPG levels (r=0.127, P=0.019). Below 16mmHg of HVPG, the stronger correlation was noted between them (r=0.202, P=0.003) but this relation was not observed above 16mmHg of HVPG. We divided HVPG into various stages of portal hypertension(HVPG<5, 5≤HVPG<10, 10≤HVPG<16, HVPG≥16). The average spleen volume of each group tended to increase with increasing HVPG value. Mean splenic volume for each group was 320.74cm3, 464.78 cm3, 501.06 cm3, 525.94 cm3 respectively. But there was no significant difference in the mean spleen volume between the groups., HVPG was correlated with presence of ascites (P=0.038) as well as SV (P=0.043) after adjusted by MELD, Na, albumin, platelet count. Conclusions: The spleen volume tended to increase with increasing HVPG and this association was stronger when HVPG was below 16 mmHg. More studies with larger numbers are needed to predict the portal pressure non-invasively.
