      • Is Target Oriented Surgery Sufficient with Borderline Ovarian Tumors? - Role of Accompanying Pathologies

        Background: There are limited data in the literature related to concomitant genital or extra-genital organ pathologies in patients with borderline ovarian tumors (BOTs). The aim of this study was to evaluate our experience with 183 patients to draw attention to the accompanying organ pathologies with BOTs. Materials and Methods: One hundred eighty-three patients with BOTs, diagnosed and/or treated in our center between January of 2000 and March of 2013 were evaluated retrospectively. Data related to age, tumor histology, lesion side, disease stage, accompanying incidental ipsilateral and/or contralateral ovarian pathologies, treatment approaches, and follow-up periods were investigated. Incidental gynecologic and non-gynecologic concomitant organ pathologies were also recorded. Results: The mean age at diagnosis was 40.6 years (range: 17-78). Ninety-five patients (51%) were ${\leq}40$ years. A hundred and forty-seven patients (80%) were at stage IA of the disease. The most common type of BOT was serous in histology. Non-invasive tumor implants were diagnosed in 4% and uterine involvement was found 2% among patients who underwent hysterectomies. There were 12 patients with positive peritoneal washings. Only 17 and 84 patients respectively had concomitant ipsilateral and concomitant contralateral incidental ovarian pathologies. The most common type of uterine, appendicular and omental pathologies were chronic cervicitis, lymphoid hyperplasia and chronic inflammatory reaction. Conclusions: According to our findings most of accompanying pathologies for BOT are benign in nature. Nevertheless, there were additional malignant diseases necessitating further therapy. We emphasize the importance of the evaluation of all abdominal organs during surgery.

        Effect of surface treatments on shear bond strength of resin composite bonded to CAD/CAM resin-ceramic hybrid materials

        PURPOSE. The purpose of this study was to assess the effect of surface treatments on shear bond strength of resin composite bonded to thermocycled and non-thermocycled CAD/CAM resin-ceramic hybrid materials. MATERIALS AND METHODS. 120 specimens ($10{\times}10{\times}2mm$) from each material were divided into 12 groups according to different surface treatments in combination with thermal aging procedures. Surface treatment methods were airborne-particle abrasion (abraded with 50 micron alumina particles), dry grinding (grinded with $125{\mu}m$ grain size bur), and hydrofluoric acid (9%) and silane application. According to the thermocycling procedure, the groups were assigned as non-thermocycled, thermocycled after packing composites, and thermocycled before packing composites. The average surface roughness of the non-thermocycled specimens were measured after surface treatments. After packing composites and thermocycling procedures, shear bond strength (SBS) of the specimens were tested. The results of surface roughness were statistically analyzed by 2-way Analysis of Variance (ANOVA), and SBS results were statistically analyzed by 3-way ANOVA. RESULTS. Surface roughness of GC were significantly lower than that of LU and VE (P<.05). The highest surface roughness was observed for dry grinding group, followed by airborne particle abraded group (P<.05). Comparing the materials within the same surface treatment method revealed that untreated surfaces generally showed lower SBS values. The values of untreated LU specimens showed significantly different SBS values compared to those of other surface treatment groups (P<.05). CONCLUSION. SBS was affected by surface treatments. Thermocycling did not have any effect on the SBS of the materials except acid and silane applied GC specimens, which were subjected to thermocycling before packing of the composite resin.

        Effects of contamination by either blood or a hemostatic agent on the shear bond strength of orthodontic buttons

        Objective: To evaluate the effects of contamination by either blood or a hemostatic agent on the shear bond strength (SBS) of orthodontic buttons. Methods: We used 45 freshly extracted, non-carious, impacted third molars that were divided into 3 groups of 15. Each tooth was etched with 37% phosphoric acid gel for 30 s. Human blood or the blood stopper agent was applied to the tooth surface in groups I and II, respectively. Group III teeth were untreated (controls). Orthodontic buttons were bonded to the teeth using light-curing composite resin. After bonding, the SBS of the button was determined using a Universal testing machine. Any adhesive remaining after debonding was assessed and scored according to the modified adhesive remnant index (ARI). ANOVA with post-hoc Tukey's test was used to determine significant differences in SBS and Fisher's exact test, to determine significant differences in ARI scores among groups. Results: ANOVA indicated a significant difference between groups (p < 0.001). The highest SBS values were measured in group III ($10.73{\pm}0.96$ MPa). The SBS values for teeth in groups I and II were significantly lower than that of group III (p < 0.001). The lowest SBS values were observed in group I teeth ($4.17{\pm}1.11$ MPa) (p < 0.001). Conclusions: Contamination of tooth surfaces with either blood or hemostatic agent significantly decreased the SBS of orthodontic buttons. When the contamination risk is high, it is recommended to use the blood stopper agent when bonding orthodontic buttons on impacted teeth.

        Biaxial flexural strength and phase transformation of Ce-TZP/$Al_2O_3$ and Y-TZP core materials after thermocycling and mechanical loading

        PURPOSE. The purpose of the present study was to evaluate the effect of thermocycling and mechanical loading on the biaxial flexural strength and the phase transformation of one Ce-TZP/$Al_2O_3$ and two Y-TZP core materials. MATERIALS AND METHODS. Thirty disc-shaped specimens were obtained from each material. The specimens were randomly divided into three groups (control, thermocycled, and mechanically loaded). Thermocycling was subjected in distilled water for 10000 cycles. Mechanical loading was subjected with 200 N loads at a frequency of 2 Hz for 100000 times. The mean biaxial flexural strength and phase transformation of the specimens were tested. The Weibull modulus, characteristic strength, 10%, 5% and 1% probabilities of failure were calculated using the biaxial flexural strength data. RESULTS. The characteristic strengths of Ce-TZP/$Al_2O_3$ specimens were significantly higher in all groups compared with the other tested materials (P<.001). Statistical results of X-ray diffraction showed that thermocycling and mechanical loading did not affect the monoclinic phase content of the materials. According to Raman spectroscopy results, at the same point and the same material, mechanical loading significantly affected the phase fraction of all materials (P<.05). CONCLUSION. It was concluded that thermocycling and mechanical loading did not show negative effect on the mean biaxial strength of the tested materials.

        Background: RA is a chronic systemic disease manifest with multiple joint infi ammation. Vasculitis, heart, lung disease, extra-articular symptoms can also be present. TNF-a,IL-6 and mediators such as cytokines, have been shown to have an important role in this infi ammatory process. Anti-TNF drugs considered to be effective in preventing; RA,disability and joint destruction. CRP is one of the best indicators of infi ammation though, ESR is an indirect indicator of infi ammation and ESR levels affected by age, sex, status and anemia. ESR and CRP levels in patients with RA, disease activity and has been shown to correlate with radiographic fi ndings In this study, we aim to to show the correlation between ESR, CRP levels with disease activity in patients receiving anti-TNF alpha therapy. Methods: In this Retrospective study between January 2006 to March 2010 patients diagnosed with RA were evaluated in two groups. In the study group patients receiving at least one year of TNF-alpha were included where as in the control group patients only receiving DMARD were include. Only female patients were involved in both groups. There were 46 women in the study and 47 women in the control group. Disease severity and DAS 28 score was used to determine the disease activity. For statistical analysis Number Cruncher Statistical System 2007 & PASS 2008 Statistical Software (Utah,USA) was used. Results: In patients receiving anti-TNF alpha therapy DAS 28 scores showed statisticallysignifi cant correlation with the ESR. A statistically signifi cance between DAS 28 and CRP were not found. In control group with only DMARD treatment, the DAS 28.

        Drug allergies can be manifested in a wide specturum such as a mild skin rash to life-threatening epidermal necrolysis. Therefore,allergic reactions, needs to be always kept in mind and etiology should be investigated througly. Case: 43 year old female patient was admitted to our department due to high fever, palpitations and sweating. In physical examination the patient was agitated, her skin was humid and fever was above 39.50C. Her pulse: 125/min, arterial pressure: 80/50 mmHg, respiration: 26/min.In the right gluteal region there was fronculosis. Because of her breast cancer, she had taken chemotherapy recently. For the treatment of fronculosis, she was taking moxifl oxacin. Patient was then evaluated by Infectious diseases and due to the current fi ndings intravenous targocid and tazobactam was started. The patient`s vital signs returned to normal and agitation dissappeared after admission to hospital within 8 hours. Maculopapular skin rashes, occurred after 12 hours after the beginning of IV antibiotic treatment therefore they were discontinued and oral ciprofloksasin and amoksisilin with kluvanat was started. Patient showed; per-oral hypoesthesia,agitation,palpitations just after 3 hours of ciprofl oxacin treatment. In the physical examination;the patient was agitated, pulse: 110/min and sinus tachycardia was present, other systems were normal.The patient`s condition was similar as her admission to the hospital. The emergence of the similar complaints suggested hypersensitivity reaction. Antibiotics were stopped and oral Antihistaminic treatment was started. Vital signs returned to normal after antihistaminic treatment.Agitation and peroral hipoesthesia were resolved Discussion and Conclusion: In the literature; the fluoroquinolones caused adverse events, such as hypotension, tachycardia, long QT syndrome, Torsades de pointese and even cardiac death. In our case, the high fever, tachycardia,hypotension, and neurologic fi ndings were present. There was no skin fi ndings when the patient was admitted to the hospital. Complaints after Ciprofl oxacin treatment were similar to the patients complaints at the hospital admission. Thus, our clinical diagnosis confi rmed hypersensitivity reaction due to moxifl oxacin treatment. In particular,as in our case,become drug hyper-sensitivity reactions and infectious diseases findings, can very similar. Therefore drug allergies should always be kept in mind.

      • Clinicopathological Features of Patients with Malignant Mesothelioma in a Multicenter, Case-Control Study: No Role for ABO-Rh Blood Groups

        Background: Malignant mesothelioma (MM) is an aggressive tumor of mesothelial surfaces. Previous studies have observed an association between ABO blood groups and risk of certain malignancies, including pancreatic and gastric cancer; however, no information on any association with MM risk is available. The aim of this study was to investigate possible associations amoong MM clinicopathological features and ABO blood groups and Rh factor. Materials and Methods: In 252 patients with MM, the ABO blood group and Rh factor were examined and compared with the control group of 3,022,883 healthy volunteer blood donors of Turkish Red Crescent between 2004 and 2011. The relationship of blood groups with various clinicopathological features were also evaluated in the patient group. Results: The median age was 55 (range: 27-86) and 61.5% of patients were male. While 82.8% of patients had a history of exposure to asbestos, 60.7% of patients had a smoking history. Epithelioid (65.1%) was the most common histology and 18.7% of patients had mixed histology. Overall, the ABO blood group distribution of the 252 patients with MM was comparable with the general population. The median overall survival (OS) was 14 months (95% confidence interval, 11.3-16.6 months). The median OS for A, B, AB, and O were 11, 15, 16, and 15 months respectively (p=0.396). First line chemotherapy was administered to 118 patients. The median OS of patients on pemetrexed or gemcitabine was longer than patient who was not administered chemotherapy [17 months (95%CI, 11.7-22.2) vs. 9 months (95%CI, 6.9-11.0); p<0.001]. Conclusions: The results of this study suggest that patients with MM can benefit from treatment with pemetrexed or gemcitabine in combination with cisplatin. We did not observe a statistically significant association between ABO blood group and risk of MM.

        The notion of rectifying curve in the Euclidean space is introduced by Chen as a curve whose position vector always lies in its rectifying plane spanned by the tangent and the binormal vector field t and $n_2$ of the curve, [1]. In this study, we have obtained some characterizations of semi-real spatial quaternionic rectifying curves in $\mathbb{R}^3_1$. Moreover, by the aid of these characterizations, we have investigated semi real quaternionic rectifying curves in semi-quaternionic space $\mathbb{Q}_v$.

        Hypoglycemic Effect of Lentinus strigosus (Schwein.) Fr. Crude Exopolysaccharide in Streptozotocin-Induced Diabetic Rats

        This study reports the hypoglycemic effects of the crude exopolysaccharide (EPS) produced from submerged mycelial culture of Lentinus strigosus (Schwein.) Fr. (Family Polyporaceae) in streptozotocin (STZ)-induced diabetic rats. In a dose-dependent study, diabetic rats were treated with EPS at doses of 50–150 mg/kg of body weight for 7 days. Serum glucose and plasma insulin levels were measured in normal, STZ-induced diabetic, and EPS-treated diabetic rats. Following oral administration of EPS dosages for 7 days, the serum glucose levels in the STZ-induced diabetic rats were reduced up to 21.1% at the dose of 150 mg/kg of body weight. The results revealed that orally administered L. strigosus EPS, at the dose of 150 mg/kg, exhibited a considerable hypoglycemic effect in STZ-induced diabetic rats. Plasma insulin levels of STZ-induced diabetic rats decreased as compared to control group rats (P < .05). Although insulin levels slightly increased in the EPS-treated groups the increase was not statistically significant. The hypoglycemic potential of the EPS was further supported by histological observations of pancreatic islets of Langerhans.
