        Infected Aortic Aneurysm with Infective Spondylitis in a Patient with Pre-Aortic Confluence of the Iliac Veins: An Unusual Triple Combination

        It is uncommon for infective spondylitis to coexist with an infected aneurysm of the adjacent abdominal aorta. Also, pre-aortic confluence of the iliac veins or marsupial vena cava is a rare anatomical variant, which increases the potential for venous injury and hemorrhage during an emergency operation. Herein, we report the case of a 57-year-old male with these three pathologies, who was successfully treated with emergency open surgery. During surgery, we noted this anomaly and extensive destruction of the lumbar vertebral bodies. We discuss options to treat these rare pathologies with literature review.

        Spontaneous Transection of the Superficial Femoral Artery with Devastating Outcomes: A Case Report

        Non-traumatic, simultaneous pseudoaneurysms of the bilateral superficial femoral arteries (SFAs) are extremely rare. Spontaneous transection of the SFA is another unique pathology. Here, we present a patient with end stage kidney disease who was diagnosed with bilateral, simultaneous SFA pseudoaneurysms. He had a recent history of methicillin-sensitive Staphylococcus aureus septicemia; therefore, infection was suspected to be the main cause. Complete transection of the SFA was noted during the operative exploration of the symptomatic left side. Wide debridement and autologous vein bypass were performed via a clean route. Unfortunately, two months later, recurrent surgical site infection caused distal anastomotic rupture and fatal sepsis.

        Impact of CO2 Laser Pretreatment on the Thermal Endurance of Bragg Gratings

        The thermal endurance of fiber Bragg gratings (FBGs), written with the aid of 193-nm ArF excimerlaser irradiation on H2-loaded Ge/B codoped silica fiber, and pretreated with a CO2 laser and a subsequentslow cooling process, is investigated. These treated gratings show relatively less degradation of gratingstrength during the thermal annealing procedure. The thermal decay characteristics of treated and untreatedfiber, recorded over a time period of 9 hours, have been compared. The effect on the Bragg transmissiondepth (BTD) and the center-wavelength shift, as well as the growth of refractive-index change during thegrating inscription process for both treated and untreated fiber, are analyzed.

        Development of an Instrument to Measure Patient Perception of the Quality of Nursing Care and Related Hospital Services at the National Hospital of Sri Lanka

        Purpose This study aimed to develop and validate an instrument to measure patient perception of quality of nursing care and related hospital services in a tertiary care setting. Methods We compiled an instrument with 72 items that patients may perceive as quality of nursing care and related hospital services, following an extensive literature search, discussions with patients and care providers and a brainstorming session with an expert panel. A cross-sectional study was conducted at the National Hospital of Sri Lanka. A sample (n = 120) of patients stayed in general surgical or medical units responded to the interviewer administered instrument upon discharge. Item analysis and principal component factor analysis were performed to assess validity, and internal consistency was calculated to measure reliability. Results Of the 72 items, 18 had greater than 20% of responses as ‘not relevant’. A further 11 items were eliminated since item-total correlations were less than .2. Factor analysis was performed on remaining 43 items which resulted in 36 items classifying into eight factors accounting for 71% of the variation. Factor loadings in the final solution after Varimax rotation were interpersonal aspects (.68–.85), efficiency (.62–.79), competency (.66–.68),comfort (.60–.84), physical environment (.65–.82), cleanliness (.81–.85), personalized information (.76–.83),and general instructions (.61–.78). The instrument had high Internal consistency (Cronbach’s alpha = .91). Conclusion We developed a comprehensive, reliable and valid, 36-item instrument that may be used to measure patient perception of quality of nursing care in tertiary care settings.

        Validation of Sinhala Version of Cardiff Wound Impact Schedule in Patients with Diabetic Leg and Foot Ulcers

        Purpose: To validate the Cardiff Wound Impact Schedule (CWIS) to assess the health-related quality of life (HRQoL) of Sri Lankan patients with diabetic leg and foot ulcers. Methods: English version of CWIS was examined for cultural compatibility, translated into Sinhala and pretested. The Sinhala versionwas administered in parallel with the validated Sinhala version of SF-36 by an interviewer to all patients (n ¼ 140) at baseline to determine the construct validity. Reliability of CWIS was measured by internal consistency and test-retest stability. The instrument was readministered in 2 weeks on 33 patients with nonhealing ulcers to determine the test-retest stability and in 3 months on 50 patients with healed ulcers to determine the ability of CWIS to discriminate HRQoL between patients with healed versus nonhealed ulcers. Acceptability of CWIS was assessed by the response rate, completion rate and the average time taken to complete a single interview. Results: The construct validity demonstrated moderately significant correlations between related subscales of CWIS and SF-36 (Spearman’s r = .32 .51, p = .021 to p < .001) for the whole study sample. Internal consistencies (Cronbach a = .68 .86) and test-retest stability (.56 .70) were acceptable. The tool was sensitive in discriminating the impact of the wound on HRQoL in healed versus nonhealed status (p ≤ .001). The tool showed good acceptability. Conclusions: The Sinhala version of CWIS is valid, reliable and acceptable for assessing the impact of wound on HRQoL. This instrument is sensitive in detecting the differences of the impact of healed and nonhealed ulcers on QoL in patients with diabetic leg and foot ulcer.

        Red Color Light at Different Intensities Affects the Performance, Behavioral Activities and Welfare of Broilers

        Red light (RL) marked higher weight gain (WG) and preference of broilers compared to other light colors. This study aimed to investigate how different intensities of RL affect the performance, behavior and welfare of broilers. RL treatments were T1 = high intensity (320 lux), T2 = medium intensity (20 lux); T3 = dim intensity (5 lux), T4 = control/white light at (20 lux) provided on 20L:4D schedule and T5 = negative control; 12 hours dark: 12 hours day light. Cobb strain broilers were used in a Complete Randomize Design with 6 replicates. WG, water/feed intake, feed conversion ratio (FCR), mortality, behavior and welfare were assessed. At 35 d, significantly (p<0.05) highest body weight ($2,147.06g{\pm}99$) was recorded by T3. Lowest body weight ($1,640.55g{\pm}56$) and FCR (1.34) were recorded by T5. Skin weight was the only carcass parameter showed a significant (p<0.05) influence giving the highest (56.2 g) and the lowest (12.6 g) values for T5 and T1 respectively. Reduced welfare status indicated by significantly (p<0.05) higher foot pad lesions, hock burns and breast blisters was found under T3, due to reduced expression of behavior. Highest walking ($2.08%{\pm}1%$) was performed under T1 in the evening during 29 to 35 days. Highest dust bathing ($3.01%{\pm}2%$) was performed in the morning during 22 to 28 days and highest bird interaction (BI) ($4.87%{\pm}4%$) was observed in the evening by T5 during 14 to 21 days. Light $intensity{\times}day$ $session{\times}age$ interaction was significantly (p<0.05) affected walking, dust bathing and BI. Light intensity significantly (p<0.05) affected certain behaviors such as lying, eating, drinking, standing, walking, preening while lying, wing/leg stretching, sleeping, dozing, BI, vocalization, idling. In conclusion, birds essentially required provision of light in the night for better performance. Exposed to 5 lux contributed to higher WG, potentially indicating compromised welfare status. Further researches are suggested to investigate RL intensity based lighting regimen that favors for both production and welfare of Broilers.

        Push-out resistance of concrete-filled spiral-welded mild-steel and stainless-steel tubes

        Spiral welded tubes (SWTs) are fabricated by helically bending a steel plate and welding the resulting abutting edges. The cost-effectiveness of concrete-filled steel tube (CFST) columns can be enhanced by utilising such SWTs rather than the more conventional longitudinal seam welded tubes. Even though the steel-concrete interface bond strength of such concrete-filled spiralwelded steel tubes (CF-SWSTs) is an important consideration in relation to ensuring composite behaviour of such elements, especially at connections, it has not been investigated in detail to date. CF-SWSTs warrant separate consideration of their bond behaviour to CFSTs of other tube types due to the distinct weld seam geometry and fabrication induced surface imperfection patterns of SWTs. To address this research gap, axial push-out tests on forty CF-SWSTs were carried out where the effects of tube material, outside diameter (D), outside diameter to wall thickness (D/t), length of the steel-concrete interface (L) and concrete strength grade (f'c) were investigated. D, D/t and L/D values in the range 102-305 mm, 51-152.5 and 1.8-5.9 were considered while two nominal concrete grades, 20 MPa and 50 MPa, were used for the tests. The test results showed that the push-out bond strengths of CF-SWSTs of both mild-steel and stainless-steel were either similar to or greater than those of comparable CFSTs of other tube types. The bond strengths obtained experimentally for the tested CF-SWSTs, irrespective of the tube material type, were found to be well predicted by the guidelines contained in AISC-360.

        Effects of four dim vs high intensity red color light regimens on growth performance and welfare of broilers

        Objective: Broilers show clear preference towards red color light (RL). However setting of an optimum light intensity is difficult since dim intensities that favor growth reduce welfare. This experiment was conducted to test the most effective RL intensity regimen (Dim [5 lux; DI] vs high [320 lux; HI]) in combination applied at different growth stages that favors for both performance and welfare. Methods: Complete randomize design was adopted with 6 replicates. Treatments were; T1 = early DI (8-21 d)+latter HI (22-35 d); T2 = early DI (8-28 d)+latter HI (29-35 d), T3 = early HI (8-21 d)+latter DI (22-35 d), T4 = early HI (8-28 d)+latter DI (29-35 d) and T5 = control (white light; WT) (8-35 d) at medium intensity (20 lux). Body weight (BW), weight gain (WG), water/feed intake and ratio, feed conversion ratios (FCR) were assessed. Common behaviours (15) were recorded by scan sampling method. Lameness, foot pad dermatitis, breast blisters, hock burning damage were assessed as welfare parameters. Fear reactions were tested using Tonic Immobility Test. Ocular and carcass evaluations were done. Meat and tibiae were analyzed for fat and bone ash respectively. Results: On 35 d, the highest BW (2,155.72±176 g), WG (1,967.78±174 g) were recorded by T2 compared to WT (BWWT = 1,878.22±155, WGWT = 1,691.83±160). But, application of RL, either DI, or HI during early/latter stage had no significant effect on FCR. Under HI, birds showed much higher active behaviours. DI encourages eating. Though LI changed from DI to HI, same trend could be seen even under HI. The highest leg strength (218.5±120 s) was recorded by T2. The lowest leg strength (64.58±33 s) and the highest ocular weight (2.48±1 g) were recorded by T1. Significantly (p<0.05) the highest skin weight (162.17±6 g) but the lowest fat% in meat (13.03%±5%) was recorded by T2. Conclusion: Early exposure to DI-RL up to 28 days followed by exposure to HI-RL is the most favorable lighting regimen for optimizing production, better welfare of broilers and improving health benefits of meat.
