        Acute Dystonia after Using Single Dose Duloxetine: Case Report

        Duloxetine is a balanced and potent serotonin and noradrenaline reuptake inhibitor which is known to be effective in depression and anxiety disorders. The common adverse effects include dry mouth, nausea, insomnia, somnolence, dizziness and constipation. Reported adverse effects of the extra pyramidal symptoms (EPS) are rare. In this case, a patient who suffered from acute dystonia, after only one dose of 30 mg duloxetine is presented.

        Comparison of the Pullout Strength of Different Pedicle Screw Designs and Augmentation Techniques in an Osteoporotic Bone Model

        Study Design: Mechanical study. Purpose: To compare the pullout strength of different screw designs and augmentation techniques in an osteoporotic bone model. Overview of Literature: Adequate bone screw pullout strength is a common problem among osteoporotic patients. Various screw designs and augmentation techniques have been developed to improve the biomechanical characteristics of the bone–screw interface. Methods: Polyurethane blocks were used to mimic human osteoporotic cancellous bone, and six different screw designs were tested. Five standard and expandable screws without augmentation, eight expandable screws with polymethylmethacrylate (PMMA) or calcium phosphate augmentation, and distal cannulated screws with PMMA and calcium phosphate augmentation were tested. Mechanical tests were performed on 10 unused new screws of each group. Screws with or without augmentation were inserted in a block that was held in a fixture frame, and a longitudinal extraction force was applied to the screw head at a loading rate of 5 mm/ min. Maximum load was recorded in a load displacement curve. Results: The peak pullout force of all tested screws with or without augmentation was significantly greater than that of the standard pedicle screw. The greatest pullout force was observed with 40-mm expandable pedicle screws with four fins and PMMA augmentation. Augmented distal cannulated screws did not have a greater peak pullout force than nonaugmented expandable screws. PMMA augmentation provided a greater peak pullout force than calcium phosphate augmentation. Conclusions: Expandable pedicle screws had greater peak pullout forces than standard pedicle screws and had the advantage of augmentation with either PMMA or calcium phosphate cement. Although calcium phosphate cement is biodegradable, osteoconductive, and nonexothermic, PMMA provided a significantly greater peak pullout force. PMMA-augmented expandable 40-mm four-fin pedicle screws had the greatest peak pullout force.

        Production of Metal Oxide Containing Antibacterial Coated Textile Material and Investigation of the Mechanism of Action

        The main aim of this study was to produce PVC coated textile based antibacterial textile material and to investigate the antibacterial mechanism with detailed analyzes. Metal oxide (calcium oxide, zinc oxide, magnesium oxide) powders were used to provide antibacterial functionality to coated materials. Metal oxide concentrations were varied between 5-35 %. Antibacterial tests were performed according to ISO 22196-2011 standard. Antibacterial efficiency of the samples was tested for each metal oxide type and concentration with L. innocua species. The antibacterial mechanism was investigated with ESR technique, fluorescent microscobe and microplate reader using DCFH-DA probe, UV-vis spectrometer using fluorescein probe. The results indicated that the antibacterial effect of used metal oxides was strongly arisen from radical oxygen species. The morphology of coatings was investigated with SEM and the distribution of metal oxide particles on the surface was examined with EDX analysis and EDX mapping. The changes on the molecular basis of the coating due to the metal oxide addition was analyzed with FT-IR spectroscopy. High antibacterial efficiencies (up to 100 %) were detected. It is suggested that the non-toxic metal oxides can be used as an effective and economically feasible alternative to conventional antibacterial additives for industrial applications such as conveyor belts.

        Biodiversity and technological properties of yeasts from Turkish sourdough

        In this study, yeasts were isolated and characterized from twelve traditional sourdough samples which belongs to Black Sea and Aegean regions of Turkey. Twenty six yeast species were isolated and identified by both 26S rDNA sequencing and FTIR spectroscopy. Saccharomyces cerevisiae (50%), Torulaspora delbrueckii (40%) and Kluyveromyces marxianus (10%) were found in 12 Turkish traditional sourdough samples. S. cerevisiae was found to be the most dominant species in Aegean region while T. delbrueckii was the most frequently isolated species in Black Sea region. Some technological properties of isolated yeast species such as acidity development, resistance to NaCI and potassium sorbate, and yeast effect on bread quality were investigated. Breads were prepared by S. cerevisiae TGM38 strain demonstrated the highest bread volume compared the other yeasts used in the study. This study showed the yeast diversity and technological properties of traditional Turkish sourdough breads fermented by chosen yeast species.

        Monte Carlo Computation of the Energy Deposited by Protons in Water, Bone and Adipose

        Protons are most suitable for treating deeply-seated tumors due to their unique depth dose distribution. The maximum dose of protons is a pronounced peak, called the Bragg peak, with zero dose behind the peak. The objective of radiation therapy with protons is to deliver the dose to the target volume by using this type of distribution. This is achieved with a finite number of Bragg peaks at the depth of the target volume. The location of the peak in terms of depth depends on the energy of the protons. Simulations are used to determine the depth dose distribution of proton beams passing through tissue, so it is important that experimental data agree with the simulation data. In this study, we used the FLUKA computer code to determine the correct position of the Bragg peak for proton beams passing through water, bone and adipose, and the results were compared with experimental data.

        Dandy Walker Variant and Bipolar I Disorder with Graphomania

        Cerebellum is known to play an important role in coordination and motor functions. In some resent studies it is also considered to be involved in modulation of mood, cognition and psychiatric disorders. Dandy Walker Malformation is a congenital malformation that is characterized by hypoplasia or aplasia of the cerebellar vermis, cystic dilatation of the fourth ventricle and enlargement of the posterior fossa. When the volume of posterior fossa is normal, the malformation is called Dandy Walker Variant. Case is a 32 year old male with a 12 year history of Bipolar I Disorder presented with manic and depresive symptoms, including dysphoric and depressive affect, anhedonia, suicidal thoughts and behaviours, thoughts of fear about future, overtalkativeness and graphomania, increased energy, irregular sleep, loss of appetite, increased immersion in projects, irritability, agressive behavior, impulsivity. Cranial Magnetic Resonance Imaging was compatible to the morphological features of Dandy Walker Variant.

        Flow behavior, structures and flow regime were determined in a circulating fluidized bed riser (0.203 m i.d.×5.9 m high) of FCC particles (dp = 70 μm, ρs = 1700 kg/m3). A momentum probe was used to measure radial momentum flux profiles and to distinguish between local net upward and downward flow regions.Time-mean dynamic pressure decreases towards the wall and, the fast fluidization flow regime was observed to coexist with dense suspension upflow (DSU) in the range Ug = 5-8 m/s, Gs = 10-340 kg/m2s. The annular downflow layer disappears locally with increasing solids mass flux (Gs) at a constant Ug, with achievement of the DSU regime. New correlations are developed to predict the thickness of solids down-flowing layer based on solids mass flux and momentum flux. They account for the effect of height on the thickness, and cover high Gs ranges near the onset of the DSU regime. Also, a new flow regime map is proposed distinguishing the fast fluidization, DSU and dilute pneumatic transport flow regimes.

      • Impact of Adjuvant Chemoradiotherapy for Rectal Cancer on the Long-Term Quality of Life and Late Side Effects: A Multicentric Clinical Evaluation by the Turkish Oncology Group

        Aim: Although preoperative chemoradiatherapy (CRT) has proven its benefits in terms of decreased toxicity, there is still a considerable amount of cases that do not receive postoperative CRT. Oncologists at different geographic locations still need to know the long-term effects of this treatment in order to manage patients successfully. The current paper reports on long-term quality of life (QOL) and late side effects after adjuvant CRT in rectal cancer patients from 5 centers in Anatolia. Methods: Rectal cancer patients treated with postoperative CRT with minimum 1-year follow-up and were in complete remission, were evaluated according to RTOG and LENT-SOMA scales. They were also asked to complete Turkish version of EORTC QLQ-C30 questionnaire and the CR-38 module. Each center participated with the required clinical data. Results: Two hundred and thirty patients with median age of 55 years participated and completed the study. Median follow-up time was 5 years. All patients received RT concomitant with chemotherapy. Common parameters that both increased functional health scales and yielded better symptom scores were long term interval after treatment and sphincter-saving surgery. In addition, surgery type and follow-up time were determined to be predictors of QOL scores and late toxicity grade. Conclusion: Postoperative CRT was found to have a great impact on the long term QOL and side effects in rectal cancer survivors. The factors that adversely affect these are abdominoperineal resection and shorter interval. The findings may encourage life-long follow-up and cooperation with patients, which should be mentioned during the initial counseling.
