        Revisiting the Role of Esophageal Mucosal Dilated Intercellular Spaces in the Diagnosis and Pathophysiology of Heartburn

        Background/AimsDilated intercellular spaces (DISs) facilitate the diffusion of noxious agents into the deep layers of the esophageal epithelium. The role of DIS in heartburn pathogenesis is still controversial. Therefore, we aim to reinvestigate DIS in an extensively evaluated group of patients and healthy controls (HCs). MethodsWe classified 149 subjects into the following groups: 15 HC, 58 mild erosive reflux disease (ERD), 17 severe ERD, 25 nonerosive reflux disease (NERD), 15 reflux hypersensitivity (RH), and 19 functional heartburn (FH). A total of 100 length measurements were performed for each patient’s biopsy. ResultsThe overall intercellular spaces (ISs) value of gastroesophageal reflux disease (GERD) patients was higher than that of HC (P = 0.020). In phenotypes, mild ERD (vs HC [P = 0.036], NERD [P = 0.004], RH [P = 0.014]) and severe ERD (vs HC [P = 0.002], NERD [P < 0.001], RH [P = 0.001], FH [P = 0.004]) showed significantly higher IS. There was no significant difference between the HC, NERD, RH, and FH groups. The 1.12 μm DIS cutoff value had 63.5% sensitivity and 66.7% specificity in the diagnosis of GERD. There was a weak correlation (r = 0.302) between the IS value and acid exposure time, and a weak correlation (r = −0.359) between the IS value and baseline impedance. A strong correlation was shown between acid exposure time and baseline impedance (r = −0.783). ConclusionsSince the IS length measurement had better discrimination power only in erosive groups, it is not feasible to use in daily routine to discriminate other nonerosive phenotypes and FH. The role of DIS in heartburn in nonerosive patients should be reconsidered.

        New media has been an important component of modern life with recent development of immersed media-rich environments (Lim & Nekmat, 2008). Jenkins and his colleagues (2006) refer such environments and their dynamics as participatory culture, in which meaning making and consumption processes are influenced by other new media users. These processes are especially attracting youth, who are open to experimentations with online identities. Such experimentation does not necessarily have to be related with entertainment. Rather it can also be related to learning and education. The primary aim of the current study is to understand the relationship between new media literacy and imaginativeness among undergraduate communication majors. The main assumption of the study is that communication undergraduates heavily use new media products and services. And such practices inevitably impact on their creative, social and practical imaginativeness. Findings aim to shed light on how interactions with the new media technologies impact on imaginative processes and practices of aspiring communication professionals.

        The effect of lemon on the essential element concentrations of herbal and fruit teas

        In this study, the effect of lemon addition to the element content of several herbal and fruit teas is investigated. Lemon addition to tea is a traditional cultural practice in many countries. For this purpose, fennel, mint, and sage are selected as herbal teas, and apple, lemon, and rosehip are selected as fruit teas. The essential elements of calcium, cobalt, copper, iron, potassium, magnesium, sodium, phosphorus, selenium, and zinc are determined in the aforementioned teas with and without lemon addition by using an inductively coupled plasma optical emission spectrometer. From the results of the experiments, the lemon addition caused different changes with respect to the tea type. Potassium (1178 ppm) in apple tea with lemon, sodium (215.1 ppm) in fennel tea with lemon, and calcium (81.88 ppm) and magnesium (53.83 ppm) in mint tea with lemon are found to be the four major essential elements in the teas. In general, the elemental contents are increased with the lemon addition for all of the tea types, except for Na in the sage tea.

        Effective and Reliable Speed Control of Permanent Magnet DC (PMDC) Motor under Variable Loads

        This paper presents the effective and reliable speed control of PMDC motors under variable loads and reference speeds. As is known DC motors are more preferred in industrial practices. This is that, the PMDC motors don’t require brush and commutator care and to increase in torque per motor depending on developments in power electronics. In this study, proportional-integral controller (PI) and fuzzy logic controller (FL) have been designed for speed control of PMDC motor. In the design of these controllers, characteristics such as minimum overrun time, response time to the load, settling time and ideal rise time have been taken into consideration for better stability performance. In this design, the best system response was searched by examining the effect of different defuzzification methods onto the fuzzy logic system response. In conclusion, it has been seen that FL controller has a better performance for variable speed-load control of PMDC motor compared to PI controller.

        Relationship between Class III malocclusion and hyoid bone displacement during swallowing

        Objective: The displacement of the hyoid bone (HB) is a critical biomechanical component of the swallowing function. The aim of this study was to evaluate the swallowing-induced vertical and horizontal displacements of the HB in subjects with 2 different magnitudes of skeletal Class III malocclusion, by means of realtime, balanced turbo-field-echo (B-TFE) cine-magnetic resonance imaging. Methods: The study population comprised 19 patients with mild skeletal Class III malocclusion, 16 with severe skeletal Class III malocclusion, and 20 with a skeletal Class I relationship. Before the commencement of the study, all subjects underwent cephalometric analysis to identify the nature of skeletal malformations. B-TFE images were obtained for the 4 consecutive stages of deglutition as each patient swallowed 10 mL of water, and the vertical and horizontal displacements of the HB were measured at each stage. Results: At all stages of swallowing, the vertical position of the HB in the severe Class III malocclusion group was significantly lower than those in the mild Class III and Class I malocclusion groups. Similarly, the horizontal displacement of the HB was found to be significantly associated with the severity of malocclusion, i.e., the degree of Class III malocclusion, while the amount of anterior displacement of the HB decreased with an increase in the severity of the Class III deformity. Conclusions: Our findings indicate the existence of a relationship between the magnitude of Class III malocclusion and HB displacement during swallowing.
