Classical and Quantum Chaos and Control of Heat Flow

We discuss the problem of heat conduction in classical and quantum low-dimensional systems from a microscopic point of view. At the classical level we provide convincing numerical evidence for the validity of the Fourier law of heat conduction in linear mixing systems, i.e., in systems without exponential instability. At the quantum level, where motion is characterized by the lack of exponential dynamical instability, we show that the validity of the Fourier law is in direct relation to the onset of quantum chaos. We then study the phenomenon of thermal recti.cation and brie°y discuss the dierent types of microscopic mechanisms that lead to the recti.cation of heat °ow. The control of heat conduction by nonlinearity opens the possibility to propose new devices, such as a thermal recti.er.

The effects of grain refinement on the microstructure and mechanical properties of a secondary AlSi7-Cu3Mg gravity die cast cylinder head are reported. Metallographic and image analysis techniques havebeen used to quantitatively examine the macro- and microstructural changes occurring with the addition ofgrain-refining agent. The results indicate that the AlTi5B1 addition produces a fine and uniform grainstructure throughout the casting; this effect is more pronounced in the slowly solidified regions. The initialcontents of Ti and B, which are present as impurity elements in the supplied secondary alloy ingots,are not sufficient to produce effective grain refinement. Under the present casting conditions, the combinedaddition of AlTi5B1 and Sr does not produce any reciprocal interaction or effect on primary α-Aland eutectic solidification. Grain refinement improves the mechanical properties of the as-cast AlSi7Cu3Mgalloy and produces higher Weibull moduli, thus increasing the reliability of the casting. For automotivestructural components, this could be considered an increase in safety.
Vacation experiences begin at home: A cruise line example

A cruise ship passenger is asked to complete a customer satisfaction survey a few days after returning home from her vacation. The passenger details a nightmarish ordeal concerning her departure day experiences and comments that while her experience on the ship met her expectations, the moments that stand out most in her mind are those of travelling to the ship and reaching her cabin for the first time. While she acknowledges that the cruise line was not directly responsible for most of her misery, she asks that the managers of the cruise line do what they can to mitigate these problems.
Pain in Patients with Dystrophic Epidermolysis Bullosa: Association with Anxiety and Depression

Objective: We investigate the presence and the quality of pain in patients with dystrophic epidermolysis bullosa (DEB), and its correlation with the level of anxiety and depression. Methods: We collected data from 27 DEB patients and 26 healthy individuals. DEB patients and controls completed 1 scale for the quality of pain, and 1 scale for anxiety and depression. Pain was assessed with the short form of the McGill Pain Questionnaire, whereas anxiety and depression were assessed with the Hamilton rating scale for anxiety and depression. Results: DEB patients and healthy control individuals were homogeneous for age and gender (p>0.05). A statistically significant difference in the two groups was seen for sensory pain rating scale (p<0.001), affective pain rating scale (p=0.029), total pain rating scale (p<0.001), visual analogue scale (p=0.012) and present pain intensity (p=0.001), but not for anxiety (p=0.169) and depression (p=0.530). The characteristics of pain that showed a significant difference between DEB patients and healthy controls were shooting, splitting, tender and throbbing (p<0.05). In DEB patients pain was not correlated with anxiety or depression (p>0.05), whereas a slight correlation between pain and anxiety was found in healthy controls (p<0.05). No difference was found between quality of pain and anxiety-depression in DEB patients (p>0.05), but was between the DEB dominant and the recessive form of DEB (p=0.025). Conclusion: The perception of pain in DEB patients appears greater than in healthy individuals, with splitting and tender characteristics being the most significant ones, but was not associated with anxious and/or depressive symptoms.
Radial forearm free flap in a patient with an unusual radial artery variation: a case report

Head and neck reconstruction poses unique challenges in rehabilitating surgical defects in terms of integrity, function, and form. The radial forearm free flap (RFFF) has been widely used for defect coverage, especially in the head and neck area, but its versatility allows it to be used for soft-tissue reconstruction in various parts of the body. The vascular features of the flap are quite constant and reliable. Nevertheless, abnormalities of the forearm vascular tree have been described over the decades. We report a case of intraoral reconstruction after verrucous carcinoma recurrence in a 74-year-old woman with an unusual forearm flap, which we called the median forearm free flap, based on a median branch of the radial artery that was preoperatively detected using handheld Doppler ultrasonography. The distally located skin paddle was predominantly supplied by the aberrant median vessel with its perforators. The flap was thus safely harvested with this atypical pedicle. Successful reconstruction of the intraoral defect was achieved, with an uneventful postoperative course.
Interdisciplinary rehabilitation of a root-fractured maxillary central incisor

Single-tooth implantation has become a common treatment solution for replacement of a root-fractured maxillary incisor in adults, but the long-term esthetic results can be unfavorable due to progressive marginal bone loss, resulting in gingival recession. In this case report, a maxillary central incisor with a root fracture in its apical one-third was orthodontically extruded and extracted in a 21-year-old female. Implant surgery was performed after a 3-month healing period, and the final crown was placed about 12 months after extraction. After 12 years, favorable osseous and gingival architectures were visible with adequate bone height and thickness at the buccal cortical plate, and no gingival recession was seen around the implant-supported crown. Although modern dentistry has been shifting toward simplified, clinical procedures and shorter treatment times, both general dentists and orthodontists should be aware of the possible long-term esthetic advantages of orthodontic extrusion of hopelessly fractured teeth for highly esthetically demanding areas and should educate and motivate patients regarding the choice of this treatment solution, if necessary.
