      • 초음파 가이드를 통한 맥락막상강출혈의 치료

        Purpose : Intraoperative suprachoroidal hemorrhage is one of the most serious complications. We report a case of suprachoroidal hemorrhage during phacoemulsification surgery, was managed properly. Methods : Suprachoroidal hemorrhage occurred during phacoemulsificaiton in other hospital. The patient was referred to our department immediately after closing the surgical wound. On initial examination, iris and vitreous were prolapsed through surgical wound. Ciliary body was closely place to anterior chamber, and retina was not visible. Results : Prolapased ocular tissue were reduced by emergency operation. We controlled the patient’s intraocular pressure and inflammation. Ultrasonography was repeatedly performed to check the degree of liquefaction of suprachoroidal hemorrhage. After 9 days of suprachoroidal hemorrhage onset, vitrectomy and drainage through sclera was performed. Postoperative state was stable with successful removal of suprachoroidal hemorrhage. After 2 weeks of secondary operation, retinal detachment has occurred on nasal retinal area, so we performed scleral buckling and endolaser photocoagulation. After 6 months, retina was attached well and intraocular pressure was in normal range. Conclusion : In suprachoroidal hemorrhage, drainage of suprachoroidal hemorrhage after liquefaction of thrombi is effective. After successful drainage, postoperative complications may accompany such as retinal detachment or elevation of intraocular pressure. 목적 : 술중 발생하는 맥락막상 출혈은 매우 심각한 합병증이다. 저자는 수정체 유화술 중에 발생한 맥락막상 출혈을 적절히 대처한 증례를 보고하는 바이다. 대상과 방법 : 수정체 유화술 중 발생한 맥락막상강 출혈환자가 수술창을 봉합 후 즉시 본원으로 전원되었다. 초진시 홍채와 유리체가 수술창을 통해 탈출되어 있었으며, 전방을 통해 모양체가 확인되었으나, 망막은 보이지 않았다. 탈출된 안구내 조직을 응급수술을 통해 복원 후 환자의 안압과 염증을 조절하였다. 초음파를 반복적으로 실시하여 맥락막상 출혈의 액화 정도를 확인하였으며, 출혈 9일 후 유리체 절제술과 공막을 통한 배액술을 시행하였다. 결과 : 맥락막상 출혈이 성공적으로 제거되었으며 술 후 상태는 안정적이었다. 수술 2주 후 비측 망막 박리가 발생하여 공막돌륭술과 안구내 삽관 레이저술을 시행하였다. 6개월 후 망막은 잘 붙어있었으며, 안압은 정상 범위를 유지하였다. 결론 : 맥락막상 출혈의 경우 출혈의 배액 시기를 결정하는 것이 어렵지만, 규칙적인 초음파를 통해 배액술의 시기를 결정할 수 있다. 또한 맥락막상강 출혈의 성공적인 배액 이후에도 망막박리 및 안압상승과 같은 합병증의 동반에 대해 유념해야한다.

        Ginsenosides saponins are phytochemically extracted from red ginseng, and have been regarded as the principal components manifesting the pharmacologic activities. Saponins are very soluble in water but poorly absorbed when administrated orally. Moreover, they have some disadvantages including the decomposition in acid medium. Therefore various designs and techniques have been studied for development of optimal formulation and modification of release profile. This review focused on pH-response polymer and bioadhesive polymer considering the low stability in acid medium and the low permeability in mucosal membrane of saponins. The application of these polymers are considered as potential strategy for improvement of bioavailability in oral delivery of saponins.

      • 평판 및 쉘 구조물의 유한 요소해석을 위한 프로그램 개발

        The original Mindlin-type degenerated plate and shell elements perform reasonably well for moderately thick shell structure. However, for thin shells when full integration is used to evaluate the stiffness matrix, overstiff solutions are often produced due to shear and membrane locking. To overcome the locking phenmena of the degenerated shell element, the assumed strain method is introduced. Using this approach, the finite element analysis program has been developed for the plate and shell under elastic conditions. The program are accommodated in a simple and effective manner by using a layered approach, where in two Gauss point integration scheme is adopted for each layer. Numerical tests for linear elastic problems show an improved performance of this program.

        생명윤리 입법화 과정에서의 정책네트워크 분석에 관한 연구

        생명윤리 입법화 과정은 대규모 자본이 소요되고 고부가가치를 생산하는 미래 선도산업이라는 특성으로 인하여 그것의 연구개발을 촉구하는 입장과 오·남용으로 인한 부작용이 치명적이라는 입장이 매우 첨예하게 대립하는 특징을 갖는다. 우리나라의 경우 인간복제의 시도, 유전자변형동물 등과 같은 예측하지 못하였던 새로운 생명과학기술의 등장으로, 이에 대한 윤리적·종교적 논쟁이 시작되었고 사회적 공론화를 거쳐 합의점에 도달한 부분이 있음에도 불구하고 아직 법제화를 이루지 못하고 있다(이인영, 2002: 120). 즉, 생명윤리관련정책의 기본적인 골격은 생명공학기술의 급격한 발달과 산업화에 따라 인류복지와 건강증진이라는 순기능은 극대화하면서, 인간의 존엄성을 보장하고 인체의 안정성을 침해하지 않도록 하는 것이라고 하겠다. 본 연구에서는 정책네트워크이론을 통해서 생명입법화과정에 참여하는 정책참여자의 상호작용과 구조의 특성을 고찰하였다. 신기술의 발달과 사회윤리라는 양면적인 특성으로 인해서 정책과정에 참여하고 있는 행위자들간의 관계가 매우 첨예한 이해대립에 있다는 것을 알 수가 있었으며, 입법과정에 참여하고 있는 행위자간의 상호작용은 갈등적 성격으로 드러났고, 구조에 있어서는 분산된 형태를 띠는 것으로 나타났다. The purpose of the study is to evaluate the operation, performance, and development issues of the Korean executive agency system newly introduced in 2000. Data on the reported evaluations of the system of 870 bureaucrats in all the 23 Korean executive agencies are obtained using questionnaires. The results reveal that ① on the operational side of the executive agency system, the agency bureaucrats evaluate a little bit positively the personnel management, perfomlance management, and finance management, ② on the adoption of the system, they show generally low levels of satisfaction, and ③ they comparatively positively evaluate the agency chief executive's leadership and job performance capability, and the increase of the service quality after the system adoption. For the development of the system, they recognize the necessity of the expansion of the agency chief power and authority. formal institutionalization for the independency of the agency, and review of the agency selection criteria. However, they show different perceptions on the operation and performance of the agency after the system adoption according to the types of agency. This article based upon the view of policy network focuses on the analysis of the process of the legislation on bioethics and the interest of stakeholders. New technology and its socio-ethic conflict are the most important factor in the process of the legislation on bioethics. The main actors of its legislation process are biotechnology group, NGO and government. They try to play their active roles to acquire the legislation initiative. According to the analysis, policy network has many agencies(actors) in its own network and shows a prominent characteristics of issue network. Interaction of agencies reveals conflicting rather than cooperating among agencies. The structure of policy network is the decentralized one focusing their own interests. The meanings of this study are to review the appearance of new technology and its socio-ethics conflict and to inspect the possibility of theoretical application of policy network. The Korean Local Autonomy system has been rooted through the several ballots of it since 1991. Now, one of the tasks on local autonomy is concentrating the interests and endeavors to increase the self-governing capacity of local government. By accomplishing this task, Korean Local Autonomy system will be reborn as efficient and practical mechanism for providing benefits to residents. This study had a trial of presenting alternatives for development through the capacity building and strategic visions of local government by using strategic planning approach on the basis of problematic perception on the task of local autonomy. Young Weol Gun (District) was selected as a subject local government of this study, which has difficulties in resolving many problems. The five strategic issues were extracted from the result of SWOT analysis, and the strategic alternatives were established with relation to the role of constituents such as ① exercising district chief's active and creative leadership, ② performing the proactive role of local council for consolidating residents, ③increasing the initiative awareness and perception of residents on the development of local government. And the concrete goal was settled as "Consolidating Residents and Revitalizing Local Economic" in case of successful execution of strategic issues through the proactive role performance of each

        In this paper the hardware and algorithm of APRS(Automatic Power Reconfiguration System) are represented. These days power systems are often operated in the radial operation manner. This is due to the increase of fault current capacities as power flow go higher and loop figures of power systems become more complex. Fundamentally radial operation of power systems result in loss of reliability. So, the need to develop automatic restoration system to recover supply reliability. We develop APRS for practical application.

        This paper presents the analysis of queueing model to be applied to the development of real-time system on the base of the computer To analyze the perfarmance of queue, we selected the queueing model, M/M/1 (N) which is employed in general and got the analysis result through simulation. This result becomes the important element when developing a real-time system with message queues.

Therefore, this study focuses on the study of analyzing the influence factor to nonassaulted and assaulted drivers by using Logistics Regression Analysis.

        This paper deals with the magnetic shielding effects in the superconductiong a. c. generator using finite element method. Models of three different shields are studied to compare the magnetic shielding effects. Applying 3 phase ac source to the armature, the magnetic flux densities are calculated for directions and frequencies. Because the stability of the superconducting field winding depends on the screening of the magnetic fields, computed results should be used to design the electromagnetic shield of the superconduction generator.

        Deep level transient spectroscopy(DLTS) is a techique to chacterize traps in semicondoctor. This technieque has shown good sensitivity and accuracy for obtaining the four main physical parameters of deep traps:activation energy, trap concentration, majority or minority trap and capture cross section. DLTS signal is the difference between the transient capacitance at two sampling times(rate window) selected. During the thermal scan, there is a maximum or minimum peak corresponding to trap level. Trap level is found to 0.49 or 0.32eV below the conduotion band at 256K and 175K.
