        Obesity may be erythropoietin dose-saving in hemodialysis patients

        Background: In dialysis patients, the obesity-survival paradox still requires an explanation. Anemia and high doses of erythropoiesis-stimulating agents (ESAs) are associated with worse outcomes in the hemodialysis (HD) population. In the present study, we explored the relation between obesity and anemia control in a sample of maintenance HD patients in Egypt. Methods: This multicenter observational study included 733 patients on maintenance HD from 9 hemodialysis centers in Egypt. Clinical and laboratory data as well as average doses of ESAs and parenteral iron were recorded. The erythropoietin resistance index (ERI) was calculated. Results: Obesity, defined as a body mass index (BMI) ≥ 30 kg/m<sup>2</sup>, was present in 22.6% of the studied population. The target hemoglobin level (10.0-11.5 g/dL) was achieved in 27.3% of non-obese and 25.3% of obese patients, with no significant difference. The median serum ferritin and the values of transferrin saturation index did not differ significantly between these two groups. The weekly ESA dose was significantly lower in obese than in non-obese patients (P = 0.0001). A trend toward higher ESA doses and ERI values was observed in patients with lower BMIs (P < 0.0001). Multiple linear regression revealed that the BMI and urea reduction ratio were the strongest predictors of the ERI. Conclusion: Our study adds more evidence to obesity-associated advantages in HD patients. BMI may determine ESA response, with better responses observed in patients with higher BMIs.

        The Internet of Medical Things (IoMTs) are to be considered an investment and an improvement to respond effectively and efficiently to patient needs, as it reduces healthcare costs, provides the timely attendance of medical responses, and increases the quality of medical treatment. However, IoMT devices face exposure from several security threats that defer in function and thus can pose a significant risk to how private and safe a patient's data is. This document works as a comprehensive review of modern approaches to achieving security within the Internet of Things. Most of the papers cited here are used been carefully selected based on how recently it has been published. The paper highlights some common attacks on IoMTs. Also, highlighting the process by which secure authentication mechanisms can be achieved on IoMTs, we present several means to detect different attacks in IoMTs

        Inducing Potential Mutants in Bread Wheat Using Different Doses of Certain Physical and Chemical Mutagens

        Mutation is an effective strategy not only for creating novel variation into crop genome but also for direct releasing adapted and high-yielding genotypes. The current work explores inducing genetic variability in bread wheat using physical and chemical mutagens. Three wheat cultivars were treated by three mutagens; gamma irradiation (five doses; 250, 300, 350, 400 and 450 Gray); laser ray (three treatments; 1, 1.5, and 2 hour exposure) and EMS (three concentrations; 0.2, 0.3 and 0.4%). Besides, a combination of physical (laser) and chemical (EMS) mutagens using middle range of each treatment (1.5 hour laser and 0.3% EMS) was attempted to be applied. The treated seeds were sown in the first season and 4050 M1 plants were harvested. The harvested seeds were sown in the second season, and 78750 M2 plants were obtained. The selection was performed in second season (M2) based on morpho-physiological and yield traits; flag leaf area, flag leaf chlorophyll content, plant height, spike length, grain yield per plant and its components. Based on evaluated traits fourteen mutants were selected to be evaluated in the third generation (M3). The results indicated that the used mutagens had direct impact and significantly improved agronomic traits in derivative mutants compared to their parent cultivars. Moreover, the maximum increment in yield related traits were obtained by 0.4% EMS, 1 and 2 hour-laser, 350-Gy, 1.5 hour × 0.3% EMS and 250-Gy. The obtained results highlighted the importance of these doses of applied mutagens to induce useful genetic variability in bread wheat for improving grain yield and contributing traits.

        In this paper, finite element method is applied to solve boundary control problem governed by elliptic variational inequality with an infinite number of variables. First, we introduce some important features of the finite element method, boundary control problem governed by elliptic variational inequalities with an infinite number of variables in the case of the control and observation are on the boundary is introduced. We prove the existence of the solution by using the augmented Lagrangian multipliers method. A triangular type finite element method is used.

        Improving the Description of the Suspended Particulate Matter Concentrations in the Southern North Sea through Assimilating Remotely Sensed Data

        The integration of remote sensing data of suspended particulate matter (SPM) into numerical models is useful to improve the understanding of the temporal and spatial behaviour of SPM in dynamic shelf seas. In this paper a generic method based on the Ensemble Kalman Filtering (EnKF) for assimilating remote sensing SPM data into a transport model is presented. The EnKF technique is used to assimilate SPM data of the North Sea retrieved from the MERIS sensor, into the computational water quality and sediment transport model, Delft3D-WAQ. The satellite data were processed with the HYDROPT algorithm that provides SPM concentrations and error information per pixel, which enables their use in data assimilation. The uncertainty of the transport model, expressed in the system noise covariance matrix, was quantified by means of a Monte Carlo approach. From a case study covering the first half of 2003, it is demonstrated that the MERIS observations and transport model application are sufficiently robust for a successful generic assimilation. The assimilation results provide a consistent description of the spatial-temporal variability of SPM in the southern North Sea and show a clear decrease of the model bias with respect to independent in-situ observations. This study also identifies some shortcomings in the assimilated results, such as over prediction of surface SPM concentrations in regions experiencing periods of rapid stratification/de-stratification. Overall this feasibility study leads to a range of suggestions for improving and enhancing the model, the observations and the assimilation scheme.

        Antioxidant, anti-inflammatory, and anti-fibrotic properties of olive leaf extract protect against L-arginine induced chronic pancreatitis in the adult male albino rat

        Chronic pancreatitis (CP) is an inflammatory disease affects the pancreas with upcoming fibrosis and notable parenchymal destruction. CP poses a high risk for pancreatic carcinoma. The present study aimed to investigate, for the first time up to our knowledge, the effect of olive leaf extract on L-arginine induced CP with referral to some of its underlying mechanisms. Forty adult male albino rats were divided equally into four groups; control, olive leaf extract treated (200 mg/ kg orally once daily), CP group (300 mg L-arginine/100 g body weight intraperitoneally, once daily for 3 weeks then every 3 days for the subsequent 3 weeks), and CP treated with olive leaf extract group. At the end of the experiment, body weight, serum glucose, serum insulin, homeostatic model assessment of insulin resistance (HOMA-IR), serum amylase and lipase as well as tissue superoxide dismutase (SOD), and malondialdehyde (MDA) levels were assessed. Pancreatic tissues were subjected to histological and immuno-histochemical studies. The CP group revealed significant decrease in body weight and increase in serum glucose, serum insulin, HOMA-IR score, serum amylase, and serum lipase levels. Significant increase in MDA level and decrease in SOD level were detected. Marked degenerative changes and fibrosis were detected. Upregulation of alpha smooth muscle actin (α-SMA), transforming growth factor beta (TGF-β), caspase-3, and interleukin-6 (IL-6) immunoreactions were implicated in CP pathogenesis. Olive leaf extract alleviated all the examined parameters via its-antioxidant, anti-inflammatory, and anti-fibrotic properties. Olive leaf extract can protect against CP and restore pancreatic functions.

        Effect of Patient Empowerment Model on Smoking Cessation: Randomized Controlled Trial

        Background: Smoking is a preventable cause of chronic morbidity. Patient empowerment is a process through which people establish greater control over their health-related decisions and actions. To assess the effect of patient empowerment versus health education on the nicotine dependence score and progress of patients under different stages of smoking cessation. Methods: This was a single-blinded randomized controlled clinical trial that included 76 smokers attending family medicine clinics. Participants were divided into two groups: empowerment and health education groups. Their nicotine-dependence score and smoking cessation stage were identified. All study participants were subjected to five health education sessions with a 3-month follow-up period. Results: The mean nicotine-dependence score decreased significantly in both groups after the intervention. This decrease was slightly higher in the empowerment group; however, the difference was not statistically significant. After the intervention, 16.7% of the health education and 30.0% of the empowerment group transitioned from stage 1 to stages 2–4 of smoking cessation, with the change being statistically significant only in the empowerment group. There was no statistically significant difference in the number of study participants who stopped smoking between the health education and empowerment groups. Conclusion: Both the empowerment model and traditional health education have similar positive effects on decreasing the nicotine-dependence level. There was a significant improvement in the stage of change for patients under the empowerment model, although there was no statistically significant difference between the groups regarding the number of participants who stopped smoking.

        Influences of luting cement shade on the color of various translucent monolithic zirconia and lithium disilicate ceramics for veneer restorations

        Purpose. The purpose of this study was to assess the effect of resin cement shade on the color of different novel ultratranslucent monolithic zirconia and lithium disilicate veneer materials. Materials and methods. For a total of 40 specimens, flat cylindrical discs with a 9-mm diameter and 0.5-mm thickness were created using CAD/CAM technology. The specimens were divided into five groups according to their material (n = 8) (e.max, Prettau, Aidite, Shofu and Dima) using A1 shade. Resin discs with the same diameter and shade as the specimens served as tooth-colored substructures. Three shades (neutral, light and warm) of resin cement try-in pastes (Variolink Esthetic LC) were used as the luting cement material. The color of each material group was measured before and after cementation using the three cement shades, and the CIE L*a* b* coordinates were obtained with a spectrophotometer. Values for the translucency parameter (TP) and color change delta E (E) before (baseline) and after cementation of each specimen were determined. To compare differences among the material groups within each shade of cement and among various shades of cement within each material, the data were analyzed using one-way ANOVA and post hoc testing. Results. Color coordinates L*, a* and b* significantly changed after the application of try-in pastes relative to baseline values, with a noticeable decrease in lightness (L*) (P < .05). A significant color change (∆E) was observed in all tested materials after cementation, with ∆E values exceeding 3.3 (P < .05). Although TP changed after cementation for most materials tested, these changes were not statistically significant (P > .05). Shofu and Dima ceramics showed the lowest TP values, while Aidite and Prettau showed the highest TP values. For e.max, translucency decreased after cementation with neutral and warm shades, and it significantly increased after cementation with a light shade. Conclusion. The shade of cement significantly altered the final color of the ceramic veneer material to a level above the threshold at which the clinical perception of color change occurred (> 3.3). The TP was not influenced by the cement shade. The translucency levels of the novel ultratranslucent multilayer monolithic zirconia ceramics Aidite and Prettau were higher than that of the lithium disilicate e.max material.
