Aims: To determine the risk factors of endoscopic variceal ligation (EVL) induced ulcer bleeding Methods: A total of 430 patients who had undergone EVL in Chonnam National University Hospital from January 2014 to October 2016 were reviewed. EVL was performed for prophylaxis or acute hemorrhage. The patients were classified into two groups: a bleeding group (n=33) and a non-bleeding group (n=397). The patients who had endoscopically confirmed EVL induced ulcer bleeding were included in the bleeding group. The patients without any bleeding sign after the EVL were included in the non-bleeding group. Data including baseline clinical characteristics, endoscopic findings, treatment methods for EVL induced ulcer bleeding, and clinical outcomes were collected and analyzed. Results: EVL induced ulcer bleeding occurred in 7.7% (n=33) of the patients. There were more patients with hepatitis C virus infection (21.3% vs. 13.1%, P=0.005), Child-Pugh classification C (36.4% vs. 13.9%, P=0.008), and concomitant gastric varix form 3 (GV F3) (30.3% vs. 11.8%, P=0.012) in the bleeding group. Initial prothrombin time (PT index, %), Child-Pugh score, Model for end-stage liver disease (MELD) score were associated with EVL induced ulcer bleeding (P=0.026, P< 0.05, and P=0.022, respectively). More bleeding related death was observed in the bleeding group (27.3% vs. 3.6%, P=0.005). In multivariate analysis, MELD score > 10 [OR: 3.42, 95% CI: 1.10-10.64], concomitant GV F3 [OR: 14.1, 95% CI: 2.84-71.43], and detachment of o-ring bands on follow-up endoscopy [OR: 8.06, 95% CI: 2.55-25.64] were independent predictive factors of EVL induced ulcer bleeding. In regarding to treatment methods of EVL induced ulcer, various endoscopic modalities were tried for hemostasis [EVL in eight cases (24.2%), endoscopic variceal obturation (EVO) with cyanoacrylate in 6 cases (18.2%), argon plasma coagulation (APC) in one case (3%), Sengstaken-Blakemore (SB) tube in 3 cases (9.1%), and proton pump inhibitor (PPI) only in 15 case (45.5%)]. Conclusions: MELD score> 10, concomitant gastric varices F3, and detachment of o-ring bands on follow-up endoscopy are risk factors for EVL induced ulcer bleeding.

Background: Traditional symptom-based therapies of irritable bowel syndrome (IBS) are directed at the relief of individual IBS symptoms. However, prognostic factors for responses to these treatments have not yet been well studied. The purpose of this study was to evaluate predictors for treatment response in patients with IBS. Methods: A total of 183 patients diagnosed with IBS according to Rome III criteria and underwent lactulose breath test (LBT) from October 2010 to June 2014 were reviewed retrospectively. All participants had checked global IBS symptom questionnaire on the day of the LBT. Overall improvement of global IBS symptoms was reassessed after 4 weeks of treatment. Results: Sixty-one subjects (33.3%) were classified as diarrhea-predominant IBS (IBS-D), 38 (20.8%) as constipation-predominant IBS (IBS-C), 17 (9.3%) as mixed-symptoms IBS (IBS-M), and 67 (36.6%) as unspecifi ed type IBS (IBS-U). LBT results were not signifi cantly different in relation to IBS subtypes. One hundred and nineteen patients (65%) had positive LBT, while 64 patients (35%) had negative LBT. Among the 119 patients with positive LBT, overall symptom improvement was observed in 91 patients (76.5%). Subjects with underlying psychological problems were less likely to have symptom improvement (4/28 vs. 3/91 p=0.031). High basal hydrogen level (>20 ppm) was more frequent in the subjects without symptom improvement (6/28 vs. 6/91, p=0.023). However, underlying psychological problems and high basal hydrogen level were not signifi cantly associated with poor treatment response in patients with negative LBT. Conclusions: High basal hydrogen production correlates with poor treatment response in patients with IBS and positive LBT. This result suggests that LBT can be used as a tool for prediction of treatment response in patients with IBS and positive LBT.

Haplodiploid sex determination occurs in a wide range of animals, especially in Hymenoptera, where a fertilized egg develops into a diploid female and unfertilized into a haploid male. However, recent studies on diploid functional males in some wasps suggest that the simple addition of paternal gene by fertilization may not be enough to explain female offspring production in the sex determination system. Recently, activation of sex determination gene (tra) was found to have a pivotal role in determining the sex of Nasonia vitripennis. In N. vitripennis, tra is activated only on the paternal genome (i.e. sperm) not on the maternal counterpart (i.e. egg). Such parent specific activation of a gene is controlled by a epigenetic factor, DNA methylation. However, in Trichogramma kaykai, Wolbachia induces female offspring production without sperm. Therefore all female offspring are clonal to the maternal gene. This violates the role of activated sex determination gene (tra) from sperm in the wasp. We hypothesize that Wolbachia has an ability to activate the gene by demethylation. This hypothesis indicates that the target of sex ratio distorting endosymbionts may be an upstream gene. It will enhance our understanding of evolution of haplodiploid sex determination.
Congenital fetal syphilis presenting with progression of ultrasound findings: A case report

Congenital fetal syphilis can cause by spirochetes cross the placenta from about 14 weeks of gestation. The clinical manifestations of congenital fetal syphilis are the results of active infection and inflammation and they are influenced by gestational age, stage of maternal syphilis, maternal treatment, and immunological response of the fetus. Untreated syphilis in pregnancy leads to adverse outcomes, including still birth, prematurity, low birth weight, congenital disease among newborn babies. We have a case of active fetal syphilis infection and rapid course of disease in untreated maternal primary infection. A 16-year-old woman at 28+2 weeks of gestation was referred with maternal reactive of FTA-ABS Ig M and Ig G, and fetal hepatosplenomegaly, severe ascites, and thickened placenta on ultrasound scan. The woman had a history of painless small vulvar sore about one month ago, and a report of ultrasound scan showed fetal abdomen with scanty ascites at 27+3 weeks of gestation. Thus the woman had started treatment with benzathine penicillin 2.4 million units. However, the fetus resulted in intrauterine fetal death at 28+6 weeks gestation with severe fetal hydropse, ascites, and pleural effusion. Polymerase chain reaction of amniotic fluid for Treponema pallidum was positive and FTA-ABS Ig M in cord blood was reactive. It is important in all pregnant women during the first trimester of pregnancy should be screened serologically for syphilis.

Two scleractinian species are newly recorded in Korea: Leptoseris mycetoseroides Wells, 1954 and Phyllangia hayamaensis (Eguchi, 1968). The two genera Leptoseris Milne Edwards and Haime, 1849 and Phyllangia Milne Edwards and Haime, 1848 are also newly recorded in Korea. The specimens were collected from the subtidal zones of Jeju-do Island, Korea by SCUBA diving from 1991 to 2007. Leptoseris mycetoseroides is characterized by its platelike growth form, intratentacular budding, irregularly developed collines, single styliform columella, and even septa and septocostae. Phyllangia hayamaensis is distinguished by its encrusting and plocoid growth form of corallites basally united with common coenosteum, trabecular columella, and irregular septal arrangements and paliform lobes.
