증례보고 : 지주막낭종으로 인한 압박성 시신경 병증 1예

목적: 6개월 전부터 시작된 우안의 시력저하를 주소로 내원한 50세 남자환자에서 지주막낭종 1예를 경험하였기에 이를 보고하고자한다. 증례요약: 50세 남자 환자가 6개월 전부터 발생하여 진행하는 우안의 시력저하를 주소로 내원하였다. 최대교정시력 우안 0.2, 좌안1.0이었고, 안압은 우안 13, 좌안 18 mmHg였다. 안저검사에서 양안의 창백한 시신경과 망막신경섬유층결손이 관찰되었고, 시야검사에서 우안의 주시점에서 10도 이내로 남은 중심시야와 좌안의 하측 시야 결손이 관찰되었다. 뇌자기공명영상 검사에서 시신경과 시신경교차부위를 압박하는 지주막낭종이 발견되었다.결론: 정상안압녹내장으로 진단받고 치료 중이던 환자에서 시신경 위축과 시야결손을 동반한 지주막낭종을 경험하였다. 전형적인 녹내장과는 다른 비특이적 증상이 관찰되는 경우 뇌 병변을 고려하여 뇌자기공명영상 검사를 시행하는 것이 진단 및 치료에 도움이 될것으로 생각한다. Purpose: To report a case of suprasellar arachnoid cyst with compressive optic neuropathy. Case summary: A 50-year-old man presented with decreased visual acuity in the right eye of 6 months duration. Best corrected visual acuity was 0.2 in the right eye, 1.0 in the left eye, and the intraocular pressure was 13 mm Hg in the right eye, and 18 mm Hg in the left eye. Fundus examination showed pale optic disc in the right eye and retinal nerve fiber layer defects in both eyes. Visual field examination revealed a central visual field of 10° in the right eye and an inferior visual field defect in the left eye. The brain magnetic resonance image (MRI) showed a suprasellar arachnoid cyst that compressing the optic nerve and chiasm. Conclusions: The author experienced arachnoid cyst accompanied by optic disc atrophy and visual field defect in a patient diagnosed and treated for glaucoma. In cases of non-specific clinical features that differ from typical glaucomatous presentations, the utilization of brain MRI appears to be helpful in diagnosis and treatment.
