면역억제제를 복용 중인 환자에서 안구표면 편평상피세포암의 안구 내 침범 1예

목적: 면역억제제를 복용 중인 환자에서 발생한 안구표면 편평상피세포암의 안구 내 침범에 대한 임상적, 조직학적 소견과 치료 경과에 대해 보고하고자 한다. 증례요약: 신장이식 후 17년간 면역억제제를 복용 중인 59세 남자 환자가 재발하는 우안 결막의 하얀 병변을 주소로 내원하였다. 병변의 절제적 생검 후 절제부위에 mitomycin C 점적 및 양막이식술을 시행하였다. 조직학적 검사상 편평상피세포암으로 확인되었다. 당시 시행한 안와 전산화 단층촬영검사상 특이소견은 관찰되지 않았다. 술 후 6개월간 Interferon alpha 2b 안약을 점안하였다. 술후 1년째 우안 결막 병변이 재발하여 절제적 생검과 mitomycin C 점적, 반복적 냉응고요법, 양막이식술을 시행하였다. 같이 시행한 안와 전산화 단층촬영검사에서 우안 내측에 1.9 cm의 고밀도 종괴가 관찰되어 편평상피세포암의 안구 내 침범으로 진단하고 우안안와내용물제거술을 시행하였고, 조직학적 검사상 편평상피세포암으로 확인되었다. 결론: 면역억제제 복용 환자에서 발생한 안구표면 편평상피세포암은 빠르게 다른 부위로 침범할 수 있음을 고려할 때 수술적 완전절제 후에도 적극적인 경과 관찰이 필요하다. Purpose: To report a case of ocular surface squamous cell carcinoma with intraorbital extension in a patient with renal transplantation and long-term immunosuppressive therapy. Case summary: A 59-year-old Korean male presented with a whitish mass in the medial limbus and conjunctiva of the right eye. The patient had undergone renal transplantation 17 years prior due to lupus nephritis and was on systemic immunosuppression with daily prednisolone (10 mg), tacrolimus (5 mg), and mycophenolate sodium (720 mg). The complete excision of the mass was performed and mitomycin C application and amniotic membrane transplantation on the excised area were combined. Histopathological examination revealed the mass was squamous cell carcinoma. There were no abnormal findings on the orbit computed tomography (CT). The patient was additionally treated with topical interferon alpha 2b 6 months postoperatively. One year later, a mass recurred at the same site in the right eye. The complete excision of the mass, mitomycin C application, cryotherapy, and amniotic membrane transplantation were performed. Orbit CT showed a 1.9 cm-sized intraorbital mass involving the medial rectus of the right eye. The orbital exenteration was performed and the intraorbital mass was histologically proven to be squamous cell carcinoma. Conclusions: Ocular surface squamous neoplasia in patients with renal transplantation and long-term immunosuppressive therapy should be monitored closely for the possibility of orbital invasion.

An endophytic bacterial strain, Arthrobacter humicola YC6002, was isolated from a surface sterilized root of Korean turf grass (Zoysia japonica) collected from Jinju, Korea. This strain showed inhibitory effect on germination and shoot growth of radish. The inhibition of germination and shoot growth of radish seeds varied depending on the age of culture and the temperature at which it was incubated. The culture filtrate of 1/10-strength Tryptic Soy Broth medium, incubated for 48 hours at $30^{\circ}C$, showed the highest inhibitory effect on radish seed germination and shoot growth (92% inhibition as compared to control). The active compound with seed germination and shoot growth inhibition was purified and identified as 3-phenylpropionic acid. The purified compound had 53% and 93% inhibitory effect on seed germination and shoot growth of radish for 500 and 1000 ppm solutions, respectively.

Syringocystadenoma papilliferum (SCAP) and tubular apocrine adenoma (TAA) are rare benign sweat gland tumors. SCAP and TAA may have a histopathologic overlap, but few cases of a SCAP combined with a TAA have been reported. Herein we describe an unusual case of a SCAP co-existing with a TAA located on the back of a 12-year-old girl. (Ann Dermatol 23(S2) S151~S154, 2011)

Background Self-sampling procedures to detect severe acute respiratory syndrome coronavirus 2 is important for patients who have difficulty visiting the hospital and may decrease the burden for health care workers (HCWs). The objective of this study was to evaluate the diagnostic performance, stability and usability of self-collected nasal and oral combo swabs and saliva specimens. Materials and Methods We conducted a case-control study with 50 patients with coronavirus disease 2019 (COVID-19) and 50 healthy volunteers from March, 2021 to June, 2021. We performed real-time reverse-transcription polymerase chain reaction to compare the diagnostic performance of self-collected specimens using positive percent agreements (PPAs). Results The PPAs between self-collected and HCW-collected specimens were 77.3 - 81.0% and 80.5 -86.7% for the combo swabs and saliva specimens, respectively. The PPAs increased to 88.9 - 89.2% and 81.2 - 82.1% with a cycle threshold value ≤30. Conclusion The diagnostic performance of self sampling was comparable to that of HCW sampling in patients with high viral loads and may thus assist in the early diagnosis of COVID-19. Clinical Characteristics of Human Parvovirus B19 Infection in Children

Purpose: Human parvovirus B19 infection is widespread and has a heterogeneous clinical spectrum, ranging from asymptomatic infection to potentially life-threatening complications. We investigated the various clinical features of human parvovirus B19 infection during an outbreak of the virus in our community. Methods: A retrospective chart review study was conducted at the Pusan National University Children's Hospital from December 2017 to April 2019. We investigated the clinical features of children with parvovirus B19 immunoglobulin M or parvovirus B19 DNA detected using polymerase chain reaction. Results: A total of 24 children were diagnosed with parvovirus B19 infection. Twelve (50%) had lace form rashes, and four (16.7%) had petechial rashes. Two (8.3%) were diagnosed with fever without a focus. Six (25%) developed aplastic crisis as a complication of infection, of whom three were previously diagnosed with hereditary spherocytosis and three with acute lymphoblastic leukemia. Conclusions: In addition to erythema infectiosum, the parvovirus B19 infection can present clinically with various types of rashes and fever without a focus. Furthermore, hematologic manifestations such as neutropenia and aplastic crisis can occur during infection.

This study compared multicultural Korean children's narratives with those of monocultural children. Thirty five children with mothers from Japan or the Philippines and thirty five peers participated in narrative generation tasks. For preschool children, two stories of five-cut pictures (Pae & Lee, 1996) were provided and a frog story (Mayer, 1969) was used for school-aged children. In the results, both preschool- and school-aged children from multicultural backgrounds (MUC) performed less efficiently than children with monocultural background (MOC) in narrative composition. As for preschool children, MOC children produced more story grammars than MUC children, especially in the category of result. For school-aged children, there was no difference between the total number of story grammars of MUC children and that of MOC children, although NMC children produced storygrammar of background more frequently than that of NMC children. MOC children performed higher than MUC children in the percent of appropriate cohesive device usages. Interestingly, MUC preschool children used appropriate cohesive devices less frequently than the MOC group, meanwhile MUC school-aged children usedinappropriate cohesive devices more frequently than MOC children.. In conclusion, MUC children develop story grammars commensurate to their peers although their story composition and cohesive devices need to be more finely tuned

The objectives of this study was to evaluate current visible light curing units regarding microhardness and microleakage. Fourty samples of composite resin(Z-250, 3M) were cured by different light curing units (Flipo, LOKKI; Credi II, 3M; XL 3000, 3M: Optilux 500, Demetron) in acrylic blocks. Microhardness was measured using a calibrated Vickers indenter on both top and bottom surfaces after 24 hours of storage in air at room temperature. Class V cavities were prepared on buccal and lingual surfaces of fourty extracted human molars. Each margin was on enamel and dentin/cementum. Composite resin(Z-250, 3M) was filled in cavities and cured by four different light curing units (Flipo, LOKKl; Credi II, 3M; XL 3000, 3M: Optilux 500, Demetron). The results of this syudy were as follows: Microhardness 1. Flipo showed low microhardness compared to Optilux 500, Credi II significantly in upper surface. Flipo didn't show a significant difference compared to XL 3000. 2. The microhardness resulting from curing with Flipo was lower than that of others on lower surfaces. Microleakage 1. Dentin margin showed significantly high dye penetration rate than enamel margin in all groups(p<0.05). 2. No significant differences were found on both enamel and dentin margin regarding curing units.
