      • 자궁경부종양에서 Cathepsin-D 표현의 의의

        Various clinical and histopathologic characteristics are currently used to obtain prognostic information about cervical carcinoma, but they do not predict the outcome for any individual patients. Thus, there is a need to identify additional tumor characteristics that are able to predict more accurately the outcome for an individual patient with cervical cancer. In this study, we explored the relationship between cathepsin-D expression and progression of the cervical neoplasia, the correlation between response to neoadjuvant chemotherapy and cathepsin-D expression, and we investigated if tumor cell cathepsin-D expression could serve as a prognostic factor in cervical carcinoma. Tumor tissues were obtained from 14 patients with cervical intraepithelial neoplasia and 52 patients with squamous cell carcinoma of the cervix. Cathepsin-S expression was identified by immunohistochemical methods using monoclonal antibody cathepsin-D (BioGene). Positive cathepsin-D immunoreaction in greater than 30% of carcinoma cells was scored as high expression. High cathepsin-D expression was seen in 15 of 52 invasive cancer but was absent in cervical intraepithelial neoplasia. It was shown that cathepsin-D expression was independent of the rumor grade, tumor size, lymph node involvement, depth of invasion, parametrial invasion, and response to chemotherapy. In disease free survival analysis by log-rank test, cathepsin-D expression was not significantly associated with survival. These results show that cathepsin-D expression is not a clinically useful adjunct to assessment of prognosis in invasive squamous cell carcinoma of the cervix.

      • Charge Coupled Device를 이용한 화상 분석 시스템에 대한 연구

        It is very important to use the Charge Coupled Devices(CCDs) in spectrometric Analysis because we can study the molecular structures, molecular energy level, Intermolecular reaction, absorption or emission, photochemistry, weakly bound Molecular Energy, Fluorescence and Photo dynamic Therapy(PDT). CCD is very essential to study the molecular structures and medical engineering combining laser spectroscopy in the modem physical and chemistry. Thus, this study has manufactured and designed the electromagnetic spectrometric with CCD and then analyzed the phenomenon of foot. A video imaging system was studied for direct observation of the separated parts of bores during photons aborted and scattered. Data aquisition in addition with the possibility of further image processing, made it possible to clear and real image of the separated part of medical engineering. In the yesterday, CCD was thought to be used in only broadcasting system. But nowadays. it is thought to be used the industrial demand in observations, instrumentations and robotics as the industry develops.

        Microspore-derived Embryo Formation in Response to Cold Pretreatment, Washing Medium, and Medium Composition of Radish (Raphanus sativus L.)

        Cold pretreatment, washing medium and composition of nutrient media may have marked effects on microspore embryogenesis. When microspores isolated from radish (Raphanus sativus L. cv. Gwanhun) flower buds were washed with Nitsch & Nitsch (NLN) medium liquid medium containing $130g{\cdot}L^{-1}$ sucrose (NLN-13), yields of microspore-derived embryos were greater than when using B5 liquid medium containing $130g{\cdot}L^{-1}$ sucrose. Microspore viability is known to decrease rapidly with storage; however, in this experiment, microspore viability was maintained for 24 h at $4^{\circ}C$ without media. Among the various medium concentrations used ($0.25{\times}$, $0.5{\times}$, $1.0{\times}$, $2.0{\times}$, and $4.0{\times}$ NLN liquid medium), $0.5{\times}$ NLN liquid medium induced the most efficient formation of microspore-derived embryos. In addition, microspore-derived embryos yields were greater when microspores were cultured in $0.5{\times}$ NLN liquid medium supplemented with $0.25{\times}$, $0.5{\times}$, and $1.0{\times}$ NLN microelements, compared to medium not supplemented with microelements. In this study, the highest yield of microspore-derived embryos was observed when the microspores derived from flower buds were washed using NLN-13 liquid medium and then cultured on $0.5{\times}$ NLN liquid medium supplemented with $0.25{\times}$ NLN microelements, followed by incubation at $25^{\circ}C$ for 30 days.

      • Quality Control for Cancer Clinical Trials

        Clinical trial is a scientific research involving human subjects in which its value depends upon the intergrity of design, conduct and results. If a clinical trial is so methodologically flawe that litter or no reliable information will results, it is unethical to put subjects at risk or even to incovenience them through participation in such a study. To ensure the quality of clinical trials, formal policies and procedures for design, conduct and monitoring of clinical trials need to be established. There are three mechanisms commonly used for quality conyrol of clinical trials; institutional review board (committee), data monitoring committee, and audit committee. The institutional review board (IRB) is an administrative body to protect the rights and welfare of human research subjects recruited to participate in research activities conducted under the auspices of the institution with which it is affiliated. The IRB has the authority to approve, require modifictions in, or disapporove all research activities that fall within its jurisdiction as specified by both the national regulations and local institutional pollicy. The data monitoring committe (DMC) is a formal committee established solely for the purpose of monitoring a clinical trial (periodic review of the results of clinical trial as it is), most commonly for a phase Ⅲ trial. The reasons for monitoring include ethical imperativeness (safety and efficacy), economic constrains, and scientific nees. Scientific principles of monitoring clinical trials are confidentiality (impact on trial conduct), independence (conflict of interest, bias, interaction with patients), and planned analysis. The audit committee is a committee formed (not planned) solely for the purpose of evaluating adherence to the protocol procedures. The audit committee can be either internal or external.

      • Overview of Cancer Clinical Trials

        In the U.S(and other developed countries), cancer is the second leading cause of death for both sexes. Despite aggressive surgery and radiation therapy, recurrence/metastasis rates remain disappointingly high probably secondary to genuine ability of malignant tumors to establish a metastatic clone even before the primary tumor reaches a clinically detectable level. Despite advances made in systemic treatment of cancer over the last 4 decades, systemic therapy has demonstrated only marginal benefit at best and the majority of cancer patients still die from recurrent/metastatic diseases.

      • Long-term Follow-up Studies of Chronic Hepatitis in Korea

        For the purpose of elucidating the clinical and laboratory entities, as well as the structural characteristics of chronic hepatitis in Korea, of establishing its evolution during the period of study and finally of speculating the interrela-tionship among acute viral hepatitis, chronic hepatitis, liver cirrhosis and hepatic cell carcinoma, 222 patients with biopsy-proven chronic hepatits and 100 cases with liver cirrhosis were selected and submitted to clinical. laboratory and histologic studies. Of the cases with liver-cirrhosis, 76 were confirmed by autopsy and the remaining 24 were diagnosed by liver biopsy materials taken during exploratory laparotomy. None had ever been exposed to drug intoxication and none was assocciated with clonor-chiasis. The patients with chronic hepatits were selected from among patients who were hospitalized in either military hospitals from 1958 to 1969 or St. Mary's Hospital, Catholic Medical College from 1967 through 1974.

      • Overview of Adjuvant Therapy for Cancer

        The clinical goals of adjuvant therapy are to decrease(or delay) tumor relapses and to improve patient survival for high-risk patients in whom all gross tumor can be surgically resected. optimal adjuvant therapy requires a close cooperation among health care professionals with various specialtics. There are several malignant diseases for which beneficial roles of adjuvant therapy have been clinically demonstrated.
