      • Polar Value Analysis of Corneal Astigmatism in Intrastromal Corneal Ring Segment Implantation

        <P><I>Purpose.</I> To evaluate surgically induced astigmatism (SIA) and the average corneal power change in symmetric intrastromal corneal ring segment (ICRS) implantation.<I> Methods.</I> The study included 34 eyes of 34 keratoconus patients who underwent symmetric Intacs SK ICRS implantation. The corneal pocket incision meridian was the preoperative steep meridian. Corneal power data were obtained before and 3 months after Intacs SK ICRS implantation using scanning-slit topography. Polar value analysis was used to evaluate the SIA. Hotelling's trace test was used to compare intraindividual changes.<I> Results.</I> Three months postoperatively, the combined mean polar value for SIA changed significantly (Hotelling's <I>T</I><SUP>2</SUP> = 0.375; <I>P</I> = 0.006). The SIA was 1.54 D at 99° and the average corneal power decreased significantly by 3.8 D.<I> Conclusion.</I> Intacs SK ICRS placement decreased the average corneal power and corneal astigmatism compared to the preoperative corneal power and astigmatism when the corneal pocket incision was made at the preoperative steep meridian.</P>

        전격성 제1형 당뇨병은 특발성 당뇨병환자에서 임상 경과가 짧고 진단 당시 심각한 대사 합병증을 동반하면서 췌장 효소 수치가 높고 췌도 특이 자가항체가 음성인 새로운 아형으로 알려져 있으며, 일본과 일본 외에서의 유병률에는 차이가 있는 것으로 보인다. 아직 전격성 제1형 당뇨병의 발생 기전은 명확하지 않으며 유전적 감수성이 있는 개체에서 바이러스 감염 등의 환경적인 인자와의 연관성이 가능한 병태생리로 제시되고 있으며 이에 대한 추가적인 연구가 요구된다. 저자들은 케톤산증으로 내원하여 전격성 제1형 당뇨병을 진단받은 증례를 경험하였기에 문헌고찰과 함께 보고 하는 바이다. Some patients with idiopathic type 1 diabetes have a fulminant disorder characterized by the absence of insulitis and of diabetes-related antibodies, a remarkably abrupt onset and high serum pancreatic enzyme concentrations. This is referred to as fulminant type 1 diabetes. A 29-year-old man suffering from abdominal pain, nausea and vomiting consulted to our hospital. Laboratory findings revealed high blood glucose level and the evidence of diabetic ketoacidosis, but the serum HbA1c was normal nevertheless. The low level of plasma C-peptide indicated the loss of endogenous insulin secretion. The patient satisfied the criteria for the diagnosis of fulminant type 1 diabetes. After the patient has been treated with insulin, his symptoms were improved and abnormal laboratory data were normalized. We report this case with a review of the literature

        Cataract Surgery Practice in the Republic of Korea: A Survey of the Korean Society of Cataract and Refractive Surgery 2020

        Purpose: To describe current cataract surgery practice patterns and trends among Korean ophthalmologists. Methods: A survey was conducted among members of the Korean Society of Cataract and Refractive Surgery in October 2020. Of the 998 questionnaires, 262 (26.3%) were received for analysis. Data were analyzed using descriptive statistics and compared with those of previous surveys. Results: The largest percentage of respondents (39%) had <5 years of practical experience, and 40% had >11 years of practical experience. The average, median, and mode monthly volumes of cataract surgeries performed by the Korean Society of Cataract and Refractive Surgery members were 31, 20, and 10 cases, respectively. Topical anesthesia was administered by 85% of the respondents. For intraocular lens (IOL) calculations, 96% of the respondents used optical biometry. The proportion of surgeons providing femtosecond laser-assisted cataract surgery increased significantly from 5% in 2018 to 29% in 2020. This increase was accompanied by an increase in the multifocal IOLs. Those who implant multifocal IOL for >10% of their cases increased from 16% (2018) to 29% (2020). Topical nonsteroidal anti-inflammatory drugs were prescribed postoperatively by 76% of the respondents. Most respondents (70%) prescribed these anti-inflammatory drugs for 4 weeks. Conclusions: This survey provided a comprehensive update on current cataract surgery practice in the Republic of Korea. The results highlighted the increasing use of premium IOLs, femtosecond laser-assisted cataract surgery, optical biometry, and topical anesthesia to better meet the patients’ needs.

      • 이중 박동형 생명 구조장치의 개발(T-Pls)

        심혈관 질환 특히 협심증과 급성 심근 경색증 등 관상동맥 질환이 급증함에 따라 병원 밖이나 응급실 중환자실 심도자 검사실등에서 급성 심장 쇼크에 빠지거나 심장마비가 발생하는 경우가 많이 발생하고 있다. 이러한 심혈관 질환에는 정류성 체외 생명보조장치가 널리 사용되고 있으나 이러한 장치는 그 무게와 크기로 인해 응급상황에 대처할 수 있는 능력이 작고, 막형 산화기의 전단에 인가되는 압력이 크며, 시스템 구성상 혈액이 공기에 노출된다. 그리하여 박동성 혈류를 공급할 수 있는 공기에 노출되지 않는 시스템(Close System)이 필요하다. 박동성 혈류를 공급하기 위하여 단일 구동펌프를 이용한 시도가 있었으나 회로내의 순간 압력 상승등으로 인한 단점이 도출되었다. 이러한 기존의 체외생명보조장치의 단점을 극복하고자, 한국형 인공심장의 원리를 이용한 박동식 이중 구동형 혈액펌프를 개발하였고, T-Pls(Twin-Pls)라 명칭하였다. 이 시스템의 구동원리는 막형산화기를 중심으로 전 ㆍ 후에 혈액펌프를 위치하여 혈액펌프를 Actuator가 한번씩 짜주는 것이다. 이런 방법으로 구동할 경우 막형산화기 후단에 있는 펌프가 음압을 동시에 일으킴으로, 막형산화기에 인가되는 큰 양압을 줄인다는 가정하에 이 시스템을 개발하였고, in vitro 실험을 통하여 각 부분의 압력을 측정하고, 총 12건의 동물실험(대형동물 : 6건, 소형동물 : 6건)을 통하여 그 장점을 검증하였다. Many cases of Acute cardiac shock and Cardiac arrest in Emergency room and ICU have been increasing. In this case, ECMO with Centrifugal pump has been used generally. However, due to the heavy weight and big size, the system is not adequate for Emergency cases. And other defects of this system are that Membrane Oxygeantor's pressure is high and blood are exposed to the air. There was some tries of ECMO using pulsatile pump, but it was found that the weak point of these system is high peak pressure and hemolysis. To overcome these defects, we have developed Twin Pulsatile Life System and we named 'T-Pls'. The mechanism of 'T-Pls' is that Membrane oxygenator Outlet Pump(MOP) make negative pressure when Membrane oxygenator Inlet Pump(MIP) provides high positive pressure, and the negative pressure will decrease positive pressure of Membrane Oxygeantor. Our group analyzed this advantage through In-vitro and 12 Cases In- Vivo test(Big animal : 6 cases, small animal : 6 cases).

        Isolation and Characterization of a New Alkaloid from the Seed of Prunus persica L. and Its Anti-inflammatory Activity

        Persicaside has been isolated as a new alkaloid natural compound from a methanol (EtOA)-soluble extract of Prunus persica seed. It was purified by a combination of chromatographic techniques and recrystallization. The structure of Persicaside was determined by extensive NMR experiments and mass ppectroscopic data. It inhibited nitric oxide (NO) and prostaglandin E2 (PGE2) production via suppression of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2 expression in rat osteoblast sarcoma cells (ROS 17/2.8) in concentration-dependent manner whereas it spares the COX-1 enzyme activity.

        Spatial and seasonal distributions of the phototrophic dinoflagellate Biecheleriopsis adriatica (Suessiaceae) in Korea: quantification using qPCR

        The phototrophic dinoflagellate Biecheleriopsis adriatica is a small suessioid species characterized by a fragile thinwall. Although the morphology of this dinoflagellate is well established, there is currently little information availableon its distribution and the environmental factors that influence this distribution. Thus, to investigate the spatial andseasonal distributions of the vegetative cells of B. adriatica in Korean waters, surface water samples were collected on aseasonal basis from 28 stations in the East, West, and South Sea of Korea and Jeju Island from April 2015 to October 2018,and abundances of the vegetative cells of B. adriatica were quantified using quantitative real-time polymerase chainreactions, for which we developed the species-specific primer and probe set. Simultaneously, major environmentalparameters, including temperature, salinity, nutrient concentrations, and dissolved oxygen concentrations were measured. The vegetative cells of B. adriatica were detected at 20 of the 28 sampling stations: 19 stations in summer and 6 inautumn, although from no stations in either spring or winter. The ranges of water temperature and salinity at sites wherethis species was detected were 17.7-26.4°C and 9.9-34.3, respectively, whereas those of nitrate and phosphate concentrationswere not detectable-96.2 and 0.18-2.66 μM, respectively. Thus, the sites at which this species is found are characterizedby a narrow range of temperature, but wide ranges of salinity and concentrations of nitrate and phosphate. Thehighest abundance of the vegetative cells of B. adriatica was 41.7 cells mL-1, which was recorded in Jinhae Bay in July2018. In Jinhae Bay, the abundance of vegetative cells was significantly positively correlated with the concentration ofnitrate, but was negatively correlated with salinity. On the basis of these findings, it appears that the abundance of B. adriatica vegetative cells shows strong seasonality, and in Jinhae Bay, could be affected by the concentrations of nitrate.

      • Influence of Seasons, Extenders, Slow and Rapid Freezing on Seminal Characters in Korean Native Bucks

        <P>Contents</P><P>The success of AI technology is based on both semen quality and freezing process. In order to establish the semen freezing techniques in Korean native bucks, factors affecting the success were evaluated in the present study. Semen collected by electro-ejaculation from bucks during four distinct seasons was evaluated for semen volume and pH, sperm motility and survivability. The semen volume, concentration and total cell were higher in spring, summer and less in winter. Yet, there were no seasonal differences in the proportional data of live sperm, motility score and pH of semen among seasons. The percentage of live sperm after thawing was found to increase with increased concentration of lactose in Tris-Egg yolk-glycerol (TY-G), being highest in TY-G supplemented with 180 m<SMALL>M</SMALL> lactose (TYL180-G), but did not differ between TY-G and TYL120-G. Sperm motility was enhanced by employing 2.0 h equilibration time with rapid freezing method. In conclusion, semen could be frozen with high success rates for further use of AI in breeding techniques and to preserve the Korean native bucks.</P>
