      • 생쥐 소장의 interstitial cells of Cajal에서 ATP-민감성 K^(+)통로를 통한 향도잡이 전류의 조절

        Objectives: This study was performed to investigate whether ATP-sensitive K^(+) channels exist in interstitial cells of Cajal (ICC) of mouse small intestine and the physiological role of ATP-sensitive K^(+) channels in gastrointestinal motility. Methods: ICC were isolated from mouse small intestine and cultured for 2 days. Electrical activities of ICC were recorded using the whole-cell patch-clamp technique at 30°C. Results: ICC displayed rhythmic slow waves and spontaneous inward currents (pacemaker currents). Pinacidil, an ATP-sensitive K^(+) channel opener, induced membrane hyperpolarization and decreased amplitude of slow waves in current clamping mode. Also, pinacidil inhibited amplitude and frequency of pacemaker currents and activated outward currents at a holding potential of -70 mV. Pinacidil effects on slow waves and pacemaker currents were blocked by glibenclamide, a specific blocker of ATP-sensitive K^(+) channel. Forskolin, an activator of adenylate cyclase, induced membrane hyperpolarization and inhibited pacemaker currents like as pinacidil and blocked by glibenclamide. Conclusions: These results suggest that ATP-sensitive K^(+) channels exist in ICC and may be play an important role in regulating gastrointestinal motility through the modulation of pacemaker activities of ICC.

      • 고정화 백색부후균을 이용한 제지폐수처리

        The discharge of papermill wastewater has been continuously increased from 512,000 ㎥/day in 1987 to 735,000 ㎥/day in 1994. The method used for treating this type of industrial wastewater has been an activated sludge process, which may be the most effective and widely used form of biological wastewater treatment, but this method causes some operational problems when it is applied for treating a papermill wastewater: i. e.) 1) activated sludge processes are efficient for reducing BOD removal but are not effective in reducing COD level due to a higher fraction of NBDCOD caused mainly by celluloses and lignins, 2) the extra sludge production is too much. It is known that lignins can be soundly degraded by Phanerochaete chrysosporium called "a white rot fungi". Treating papermill wastewater using an immobilized Phanerochaete chrysosporium would give some relative benefits: 1) it can reduce a significant fraction of NBDCOD of the papermill wastewater, 2) the sludge production can be reduced. The results of this study can be used to provide not only new and innovative design parameters for prototype treatment process but also to renovate the existing treatment facilities. The results of this study are follows; 1) Nutrient(P) injection had no significant effect on the removal efficiency. 2) The operational condition that could keep the national discharge limit of effluent("Na" zone, less than COD_Mn 90㎎/ℓ and less than BOD_5 80㎎/ℓ) was about 0.33㎏-BOD_5/㎥·day or less, and was required for the same limit more than 8 hours of hydraulic time. 3) The sludge produced from treating one cubic meter of papermill wastewater was about 0.001 ∼ 0.06 ㎏/㎥, which is much less than the amount of activated sludge produced.

        Sulfasalazine and 5-aminosalicylic acid (mesalamine) analogues arc most commonly used to treat mild or moderately active ulcerative colitis and Crohn's disease and to maintain remission. Sulfasalazine, which consists of a sulfonamide antibiotic (sulfapyridine) linked by an azoUbond with an antiinflammatory salicvlate (mesalamine). Sulfasalazine is poorly absorbed in the upper digestive tract and is split in the colon by bacteria into sulfapyridine and mesalamine. Sulfapyndinc is absorbed from the colon, undergoes acctylation in the liver, and is excreted in urine. Mesalamine is poorly absorbed from the colon and is excreted in the fcces after acetylation by colonic bacteria or within the colonic epithelium. The primary fecal metabolite, N-acetyl-aminosalicylic acid, is inactive because of limited epithelial uptake. Formulations of mesalamine have been developed to maximize its release at sites of inflammation while limiting its absorption. For oral administration, it has been formulated within acid stable enteric delivery systems or conjugated with alternative, carriers. Olsalazine, an aminosalicylic acid dimer, requires bacterial azo-reduction, like sulfasalazine, but releases two molecules of aminosalicylic acid into the colon. A primary advantage of the newer mesalamine derivatives over sulfasalazine is the improved tolerance. This benefit of mesalamine must be weighed, however, against the considerably lower cost of sulfasalazine. Rare hypersensitivity reactions, including pneumonitis, pancreatitis, hepatitis, nephritis, and worsening of colitis, have been reported with mesalamine. Aminosalicylates have been used in women during pregnancy and breastfceding and in children without untoward effects. Topical formulations of mesalamine are effective as first-line therapy for mild or moderately active disease and as maintenance therapy for distal ulcerative colitis. Mesalamine enemas are also effective in treating distal colitis that is unresponsive to oral aminosalicylates or corticosteroids. Oral aminosalicylates are effective in treating mild or moderately active ulcerative colitis and Crohn's disease and in maintaining remission in both diseases. A dose response relation is apparent with both active and maintenance therapy, although with maintenance treatment.

      • 백색부후균 Phanerochaete chrysosporium의 고정화에 관한 연구

        One of the important things for biological wastewater treatment is to maintain microorganism to high concentration purely. For this, a white rot fungus, Phanerochaete chrysosporium is immobilized because it is known that lignin, nondegradable substances of the papermill wastewater, can be degraded soundly. The purpose of this study is to find an appropriate mixing ratio of immobilized Phanerochaete chrysosporium and filling ratio when a reactor is operated. The results of this study are follows; 1) The ratios of mixture between 2:1, 1:1, 1:2 white rot fungi Phanerochaete chrysosporium with PVA had a little effect on respiration rates. The amount of suspended solid produced from 2:1 ratio was negligible and the stability of the mixture was also good at 2:1. 2) The maximum removal efficiency was obtained when the filling rate was 25%.

        This study was conducted in order to investigate removal efficiency of turbidity, organic matter, nitrogen and phosphorus to reduce environmental pollution of treatment livestock wastewater with mixed proportion of loess and lime. The result of study, the removal of turbidity, COD_Cr and T-P increased along the dosage, and was to be fixed above the appropriate dosage. In the removal turbidity, the appropriate mixed proportion of loess and lime was Loess:Lime = 3:7, the appropriate dosage was 30g. In the removal COD_Cr, T-P and TKN was Loess:Lime = 3:7, 30g, Loess:Lime = 5:5, 20g, Loess:Lime = 5:5, 40g, respectively.

      • 난치성 복수 및 신부전증을 동반한 간경변증 환자에서 복수천자 및 혈액투석을 통한 치험2례

        The prevalence of hepatitis B in Korea is so high that 4% to 8% of whole population are hepatitis B patients or hepatitis B carriers and also the number of admissions from cirrhotic ascites far exceeds that of European countries or North America. Traditionally we treat ascites patient with diuretics or paracentesis method. As the liver cirrhosis worsens, the diuretics become less effective and paracenteses are needed more frequently in some of these patients. Until now. we use traditional paracentesis method in which we throw away the aspirated fluid. But frequent paracenteses lead to the loss of appetite and general weakness and sometimes bring renal insufficiency in these patients due to paracentesis induced intravascular volume depletion. Other methods, like ascites reinfusion or peritoneovenous shunting, have been studied in other countries, but rarely being practiced in our country due to complicaions of these procedures. We would like to present a new paracentesis method using hemodialysis machine. By simply connecting the drainage tube of paracentesis to the hemodialysis circuit, we were able to drain out the ascites fluid from peritoneum and remove pure water from the blood by ultrafiltration with hemodialysis. This simple method could prevent intravascular volume contration from paracentesis and also delay the reaccumulation of ascites fluid in peritoneum, so, we could treat ascites without causing thirsty or hypotension.

        우리나라 경기변동의 영구성과 일시성

        본 고에서는 단일변수 비관측요인 모형과 동태적 2요소 체제전환모형을 이용하여 우리나라 경기변동의 영구성과 일시성에 대하여 고찰한다. 우리나라 산출량의 추세적 변동을 추정함에 있어서 가장 뚜렷한 특징은 외환위기 이후의 자료를 포함할 때 산출량의 추세적 변동이 심화된다는 점이다. 이는 외환위기 충격이 전 기간에 걸쳐 산출량 변동에 영향을 미쳤음을 의미하므로 체제전환모형을 사용하였다. 체제전환의 가능성을 고려할 때 외환위기 이전과 이후에 걸쳐서 추세성장률의 변동폭 및 경기변동의 영속성이 커진다. We investigate the permanent and temporary components of business fluctuations in Korea, using univariate unobserved component model and dynamic two-factor regime-switching model. Korea's financial crisis in 1997 changed the dynamic properties of business fluctuations in the direction of increasing the permanent component. We show that the regime changed to reduce the output in the first and second oil shock period and financial crisis period permanently.

      • 수술 전 진단된 충수돌기 점액류종 1례

        Primary unruptured appendiceal mucocele is a rare disease. It is very difficult to diagnose preoperatively because of the rarity of classical symptoms and low incidence. Usually it is discovered incidentally during abdominal ultrasonography(US), computed tomography(CT), or appendectomy for acute appendicitis. If untreated, one type of mucoceles may rupture producing a potentially fatal entity known as pseudomyxoma peritonei, therefore preoperative diagnosis is very important. With the advent of colonoscopy, abdominal US and CT, it has been possible to preoperatively diagnose mucocele of appendix. We experienced a case of appendiceal mucocele in a 60 year-old woman who had symptoms of lower abdominal discomfort and right lower quadrant abdominal pain, and it was diagnosed preoperatively by colonoscopy, abdominal US and CT. So we present this case with review of literature.

        Hepatocellular carcinoma frequently metastasizes through direct extension, lymphatic, and hematogenous routes. The most common sites of metastasis are the lungs and the lymph nodes, glands, bones, spleen, kidneys, colon, and pleura are the sites of metastasis in decreasing order of frequency. But metastasis to the ovary is very rare. It is thought that metastasize to the ovary occurs through infiltration or dissemination. However, we observed ovarian metastasis of primary hepatic tumor in a 34-year old female who suffered from lower abdominal pain. At first, it was diagnosed as a primary ovarian tumor. But histologically, it presented sinusoidal pattern surrounded by the tumor cells and contained rounded by the tumor cells and contained round hyaline globules in minority of the cytoplasm. Then, the histologic features of percutaneous, sono-guided liver biopsy speecimen of the liver mass are similar to those of resected ovarian tissues. Therefore, we concluded that ovarian tumor was originated from hepatocellular carcinoma. In summary, this report examines a case of hepatocellular carcinoma with ovarian metastasis in a female and reviews the literature.
