It Sometimes Happens: Staging Surgery in Coexisting Graves' Disease and Thyroid Cancer

Thyroidectomy is a safe procedure often performed either for benign or malignant thyroid diseases. Complication rate is low and bilateral recurrent laryngeal nerve (RLN) injury associated with thyroidectomy is rarely described. The RLN may be injured bilaterally and damage is usually recognized postoperatively. With an increased use of intraoperative neural monitoring (IONM), an adaptation of the resection strategy appears to be necessary in case of an intraoperative loss of signal of the first operated side with total thyroidectomy planned. We review a case of a 21-year-old female with a history of Graves' disease who underwent a total thyroidectomy in a 2-stage procedure due to a loss of RLN function detected intraoperatively. The patient recovered uneventfully from the 2 surgeries.
Efficacy of Percutaneous Thermal Ablation of Papillary Thyroid Carcinoma

Percutaneous thermoablation (PTA) has become an increasingly used option in the interventional treatment of symptomatic, cytologic and sonographic benign solid thyroid gland disease. Currently, there is no high-level evidence of treatment for differentiated thyroid cancer by means of PTA. Surgery is a standard treatment for primary and also for recurrent thyroid cancer, followed by radioactive iodine and thyroid hormone therapy. PTA, however, can be used in patients at high-risk and in patients who refuse to undergo repeated surgery. The Task Force Committee of the Korean Society of Thyroid Radiology has developed recommendations for the optimal use of PTA for thyroid nodules. However, there are still several questions on safe distance to the laryngeal nerves, no data on duration of PTA application around the nerves, no intra-procedure assessment of laryngeal nerves function and position. Future experimental studies are required.
Modified Research Utilization Questionnaire: Development and Validation Study among Italian Nurses

Purpose: The purpose of this study was to develop the modified research utilization questionnaire (MRUQ) and to establish its content and face validity, construct validity, and reliability. Methods: This study has a multiphase (three phases), methodological, and cross-sectional design. First, research utilization questionnaire (RUQ) was translated into Italian, which is the target language to develop the M-RUQ. Second, the RUQ psychometric proprieties were assessed using exploratory factorial analysis to identify ambiguous or problematic items (e.g., cross-loadings) (cross-sectional sampling A). The RUQ modification (i.e., item deleting, wording modification, and scoring procedure) represented the development of the M-RUQ among Italian nurses. The third phase was aimed to confirm the construct validity of the M-RUQ and to test its stability and internal consistency (crosssectional sampling B). Results: This study's findings show that M-RUQ has a three-dimensional structure and a total of 22 items. The M-RUQ shows evidence of validity and reliability. Precisely, the factorial structure coming from an exploratory factorial analysis on the first sample (n ¼ 504) was confirmed by a final model of confirmatory factorial analysis (CFA) on a second sample (n ¼ 362). The final CFA model showed adequate goodness of fit, where all the factor loadings showed values higher than .40. Cronbach's a was satisfactory for each domain and for the overall scale. Furthermore, the M-RUQ showed good stability described by the testeretest. Conclusion: The M-RUQ should be used to assess research utilization among nurses for educational or research purposes to address the practice. Further research about its validity and reliability is suggested.

Objective: To evaluate the efficacy and safety of systematic lymph node dissection (SyLND) at the time of interval debulking surgery (IDS) for advanced epithelial ovarian cancer (AEOC). Methods: Systematic literature review of studies including AEOC patients undergoing SyLND versus selective lymph node dissection (SeLND) or no lymph node dissection (NoLND) after neoadjuvant chemotherapy (NACT). Primary endpoints included progression-free survival (PFS) and overall survival (OS). Secondary endpoints included severe postoperative complications, lymphocele, lymphedema, blood loss, blood transfusions, operative time, and hospital stay. Results: Nine retrospective studies met the eligibility criteria, involving a total of 1,660 patients: 827 (49.8%) SyLND, 490 (29.5%) SeLND, and 343 (20.7%) NoLND. The pooled estimated hazard ratios (HR) for PFS and OS were, respectively, 0.88 (95% confidence interval [CI]=0.65–1.20; p=0.43) and 0.80 (95% CI=0.50–1.30; p=0.37). The pooled estimated odds ratios (ORs) for severe postoperative complications, lymphocele, lymphedema, and blood transfusions were, respectively, 1.83 (95% CI=1.19–2.82; p=0.006), 3.38 (95% CI=1.71– 6.70; p<0.001), 7.23 (95% CI=3.40–15.36; p<0.0001), and 1.22 (95% CI=0.50–2.96; p=0.67). Conclusion: Despite the heterogeneity in the study designs, SyLND after NACT failed to demonstrate a significant improvement in PFS and OS and resulted in a higher risk of severe postoperative complications.

Some researchers consider soccer matches as the stylization of a war in other battlefields. Such an approach has largely been used to interpret the violent phenomena related to the soccer environment, while less attention has been paid to the potential role of political and economic interactions between countries in determining the aggressive attitude of players on the pitch. In our paper we empirically investigate whether and how political hostility among countries reverberates on a soccer pitch by influencing players’ aggressiveness. The analysis focuses on official matches played by national teams in the final phases of the European and World Cup tournaments since 2000. We estimate a negative binomial regression including both political and sport variables, and we find that (a) commercial hostility, (b) the level of diplomatic relationships,(c) power asymmetry and (d) education gap between countries are positively and significantly associated with aggressiveness of the players on the pitch, approximated by the number of yellow and red cards. That is, briefly stated, international hostility reverberates onto the pitch. Moreover,sport covariates present the expected signs, namely, results show that the closeness of the teams,their ranking and the stage of the game (knockout stages with respect to the group phases) are also crucial in determining the cautions.
