        Very-Late Onset Diffuse Large B-cell Lymphoma with CD3 Coexpression in a Kidney Transplant Patient

        B-lineage non-Hodgkin lymphoma may aberrantly coexpress T-cell markers. In general population, however, cases of diffuse large B-cell lymphomas with CD3 co-expression are rare because the CD3 marker is the most lineage specific T-cell antigen. We report a case of CD3 coexpressed diffuse large B-cell lymphoma in a 47-year-old male patient presented with dyspepsia who had transplanted a kidney 17 years ago. An esophagogastroduodenoscopy displayed an ulcerated mass in the gastric antrum. The pathology of the mass was monomorphic post-transplant lymphoproliferative disorder - specifically, CD20- and CD3- positive diffuse large B-cell lymphoma. Resection of the mass and postop chemotherapy were performed. A follow-up computerized tomography showed disapperance of tumor. No recurrence was observed until 7 month after treatment. Nevertheless, the patient`s renal function gradually aggrevated and progressed to end stage renal disease. As far as we know, this is the first case of diffuse large B-cell lymphoma with CD3 coexpression after kidney transplant.

        In vitro hemorheological effects of parenteral agents used in peripheral arterial disease

        Peripheral arterial disease (PAD) is a frequent manifestation of systemic atherosclerosis. In PAD hemorheological parameters were defined as risk factors in a number of studies and several therapeutic agents were tried in these conditions. Our study aims to investigate and compare the in vitro hemorheological effects of various drugs generally used in the parenteral treatment of intermittent claudication and critical limb ischemia. Blood samples of healthy male volunteers were incubated with iloprost, alprostadil, pentoxifylline, sulodexide or pentosan polysulfate at calculated therapeutic serum concentration. Hematocrit (Hct) was determined by microhematocrit centrifuge. Plasma and apparent whole blood viscosities (WBV) were evaluated by capillary viscometer. Red blood cell aggregation was measured by LORCA (laserassisted optical rotational cell analyzer) aggregometer, and LORCA ektacytometer was used for measuring erythrocyte deformability at $37^{\circ}C$. Iloprost, alprostadil, and pentoxifylline incubation did not have any significant effect on plasma and apparent WBV. Elongation index increased in samples incubated with alprostadil at low shear stresses 0.95 and 0.53 Pa (p < 0.05). Sulodexide significantly improved WBV and Hct/WBV ratio (p < 0.05). Incubation with pentosan polysulfate resulted in higher WBV, lower Hct/WBV ratio and deterioration in the aggregation parameters (p < 0.05). Sulodexide may have beneficial effect on a macrorheological parameter; alprostadil may improve a microrheological parameter. Hemorheological alterations could be important in PAD patients with hampered vasodilator capacity.

      • Graphene nanopatterns with crystallographic orientation control for nanoelectronic applications

        The possibility of parallel processing of several features was investigated experimentally for the two methods allowing the crystallographically controlled nanopatterning of graphene: scanning tunneling lithography (STL) and carbothermal etching (CTE). It was found that with multitip systems both methods are suitable for parallel processing. CTE has the advantages that only in the atomic force microscope (AFM) indentation phase is needed the multitip system and it can reveal the location of grain boundaries, so that the nanodevices can be placed in a way that they do not cross grain boundaries. STL is well suited for purposefully producing twisted graphene multilayers with precisely-know misorientations of the individual layers, as also evidenced by Moire-type patterns observed in the atomic resolution scanning tunneling microscopy (STM) images.

        Tissue Doppler-derived E/e` ratio as a parameter for assessing diastolic heart failure and as a predictor of mortality in patients with chronic kidney disease

        Background/Aims: Diastolic dysfunction occurs frequently in patients with chronic kidney disease (CKD) and is associated with heart failure (HF) or mortality. We investigated whether the ratio of early diastolic mitral inflow velocity to early diastolic mitral annulus velocity (E/e′ ratio), estimated using tissue Doppler imaging, has prognostic value for cardiovascular morbidity and all-cause mortality in patients with CKD. Methods: For 186 patients with CKD of stages III to V, we obtained echocardiograms with tissue Doppler imaging. A 5-year follow-up of 136 patients was performed based on hospital records and telephone interviews. The enrolled patients (79 males and 57 females) were categorized into the following CKD subgroups: stage III (n = 25); stage IV (n = 22); and stage V (n = 89). Results: The average follow-up period was 30.45 months and the mean age of the patients was 61.13 years. The mortality rate after 5 years was 60.0%. The causes of death were: sepsis, 21.9%; HF, 16.2%; and sudden death, 15.2%. Age (p = 0.000), increased C-reactive protein level (p = 0.018), and increased E/e′ ratio (p = 0.048) were found to correlate with mortality. Age (p = 0.000), decreased ejection fraction (p = 0.003), and increased E/e′ ratio (p = 0.045) correlated with cardiovascular event. Conclusions: The E/e′ ratio can predict mortality and cardiovascular events in patients with CKD who have diastolic dysfunction.

        Comparison of Spiritual Needs between Patients with Progressive Terminal Kidney Disease and Their Family Caregivers

        Purpose: The purpose of this study was to compare differences in spiritual needs (SNs) and factors influencing SNs between patients with progressive terminal kidney disease and their family caregivers. Methods: An explorative comparative survey was used to identify the SNs of patients (N=102) with progressive terminal kidney disease undergoing hemodialysis and their family caregivers (N=88) at a general hospital located in Seoul, South Korea. The data were analyzed using descriptive statistics, the chi-square test, the independent t-test, one way analysis of variance, the Scheffe test, and multiple regression with dummy variables. Results: The SNs among family caregivers were higher than in the patient group. SNs were higher among those who were religious in both groups. Loving others was the highest-ranked subdimension in the patient group, followed in descending order by maintaining positive perspective, finding meaning, Reevaluating beliefs and life, asking "why?", receiving love and spiritual support, preparing for death, and relating to God. In the family group, the corresponding order was maintaining positive perspective, loving others, finding meaning, receiving love and spiritual support, preparing for death, relating to God, and asking "why?". The factors that had a negative influence on the level of SNs were not being religious in the patient group and having only a middle school level of education in the family group. Conclusion: The results of this study may serve as evidence that spiritual care for non-cancer patients' family caregivers should be considered as an important part of hospice and palliative care.

        Lung ultrasonography in pregnant women during the COVID-19 pandemic: an interobserver agreement study among obstetricians

        Purpose: This study investigated interobserver agreement in lung ultrasonography (LUS) in pregnant women performed by obstetricians with different levels of expertise, with confirmation by an expert radiologist. Methods: This prospective study was conducted at a tertiary "Coronavirus Pandemic Hospital" in April 2020. Pregnant women suspected to have coronavirus disease 2019 (COVID-19) were included. Two blinded experienced obstetricians performed LUS on pregnant women separately and noted their scores for 14 lung zones. Following a theoretical and hands-on practical course, one experienced obstetrician, two novice obstetric residents, and an experienced radiologist blindly evaluated anonymized and randomized still images and videoclips retrospectively. Weighted Cohen's kappa and Krippendorff’s alpha tests were used to assess the interobserver agreement. Results: Fifty-two pregnant women were included, with confirmed COVID-19 diagnosis rate of 82.7%. In total, 336 eligible still images and 115 videoclips were included in the final analysis. The overall weighted Cohen’s kappa values ranged from 0.706 to 0.912 for the 14 lung zones. There were only seven instances of major disagreement (>1 point) in the evaluation of 14 lung zones of 52 patients (n=728). The overall agreement between the radiologist and obstetricians for the still images (Krippendorff's α=0.856, 95% confidence interval [CI], 0.797 to 0.915) and videoclips (Krippendorff's α=0.785; 95% CI, 0.709 to 0.861) was good. Conclusion: The interobserver agreement between obstetricians with different levels of experience on still images and videoclips of LUS was good. Following a brief theoretical course, obstetricians' performance of LUS in pregnant women and interpretation of pre-acquired LUS images can be considered consistent.

        Synthesis and Characterization of Pb@GaS Core–Shell Fullerene-Like Nanoparticles and Nanotubes

        New types of core–shell nanoparticles are reported: Pb@GaS fullerene-like and nanotubular structures, achieved via the continuously high reactor temperatures and ultra-hot stronggradient annealing environments created by highly concentrated sunlight. Structural and chemical characterizations suggest a formation mechanism where vaporized Pb condenses into nanoparticles that are stabilized as they become covered by molten GaS, the ensuing crystallization of which creates the outer layers. Hollow-core GaS fullerene-like nanoparticles and nanotubes were also observed among the products, demonstrating that a single solar procedure can generate a variety of core–shell and hollow nanostructures. The proposed formation mechanisms can account for their relative abundance and the characterization data.

      • Moire superlattices in strained graphene-gold hybrid nanostructures

        <P>Graphene-metal nanoparticle hybrid materials potentially display not only the unique properties of metal nanoparticles and those of graphene, but also additional novel properties due to the interaction between graphene and nanoparticles. This study shows that gold nanoislands can be used to tailor the local electronic properties of graphene. Graphene on crystalline gold nanoislands exhibits moire superlattices, which generate secondary Dirac points in the local density of states. Conversely, the graphene covered gold regions undergo a polycrystalline -> Au (111) phase transition upon annealing. Moreover, the nanoscale coexistence of moire superlattices with different moire periodicities has also been revealed. Several of these moire periodicities are anomalously large, which cannot be explained by the standard lattice mismatch between the graphene and the topmost Au (111) layers. Density functional theory and molecular dynamics simulations show for the first time that in such cases the graphene and the interfacial metallic layer is strained, leading to distorted lattice constants, and consequently to reduced misfit. Room temperature charge localization induced by a large wavelength moire pattern is also observed by scanning tunneling spectroscopy. These findings can open a route towards the strain engineering of graphene/metal interfaces with various moire superlattices and tailored electronic properties for nanoscale information coding.       • Initial Stage of Graphene Growth on a Cu Substrate

        <P>The growth of graphene on copper foil has attracted attention in the last two years due to its feasibility for a controllable growth process. One of the key issues remaining for practical application of graphene in solid-state devices is growth with a large grain size. Because the C–C bond in graphene is strong enough to prevent the evaporation–condensation process, Smoluchowski ripening is expected to be the dominant process for coalescence. In this article, we present the initial growth process of graphene on a Cu foil via the chemical vapor deposition method by using secondary electron microscopy and Raman microscopy. In contrast to the other transition-metal substrates, such as Ir and Rh, the center of graphene islands binds to the substrate more rigidly than the edge. For the growth with a large grain size, the graphene should be grown on a substrate with a low diffusion barrier for the carbon clusters (or islands) with low flux; this is the controlling parameter for the grain size.
