Real-World Analysis of Therapeutic Outcome in Type 1 Diabetes Mellitus at a Tertiary Care Center

Insulin replacement in type 1 diabetes mellitus (T1DM) needs intensified treatment, which can either be performed by multiple daily injections (MDI) or continuous subcutaneous insulin infusion (CSII). This retrospective analysis of a real-world scenario aimed to evaluate whether glycaemic and cardiovascular risk factors could be controlled with CSII outclass MDI as suggested by recent evidence. Data from patients with either insulin pump (n=68) or injection (n=224) therapy at an Austrian tertiary care centre were analysed between January 2016 and December 2017. There were no significant differences with regard to the latest glycosylated hemoglobin, cardiovascular risk factor control or diabetes-associated late complications. Hypoglycaemia was less frequent (P<0.001), sensor-augmented therapy was more common (P=0.003) and mean body mass index (BMI) was higher (P=0.002) with CSII treatment. This retrospective analysis of real-world data in T1DM did not demonstrate the superiority of insulin pump treatment with regard to glycaemic control or cardiovascular risk factor control.

Globalization and Exchange-Rate Pass-Through in Europe: Is There a Link?

This paper assesses the impact of globalization on Exchange-Rate Pass-Through into import prices in three core eurozone countries characterized by different degrees of openness. We looked at various indicators of globalization and used data since 1983 to find evidence of a generalized link between globalization and Exchange-Rate Pass-Through. In particular, factors related to trade integration, such as an increase in the import penetration rate or lower trade tariffs, reduce the degree of Exchange-Rate Pass-Through. However, the rising prominence of China in European imports does not contribute to the decline in pass-through. Overall, our findings show that while Exchange-Rate Pass-Through is incomplete, it remains significant even when controlling for the effects of trade globalization.

Hepatic carcinoma expressing inhibin: case report of a proposed novel entity and review of the literature

Hepatic carcinoma expressing inhibin is a recently described neoplasm with varied architecture, including trabecular, pseudoglandular, follicular/microcystic, organoid, solid and tubular patterns of growth. We report a case of hepatic carcinoma expressing inhibin that occurred in a 47-year-old woman presenting with epigastric and back pain. The tumor was located in the left hepatic lobe and measured 12 cm in diameter. On immunohistochemical stains, the neoplastic cells were positive for inhibin, as well as cytokeratins 7, 8/18 and 19. There was mild focal expression of synaptophysin, and lack of expression of hepatocytic markers. The histogenesis of hepatic carcinoma expressing inhibin is presently uncertain. From a practical point of view, this neoplasm can potentially cause diagnostic pitfalls by simulating other primary or metastatic tumors, such as hepatocellular carcinoma, cholangiocarcinoma, neuroendocrine tumors, and follicular carcinoma of thyroid gland. Performing inhibin immunostain could assist in the differential diagnosis of liver tumors with unusual histologic features.

First In-Human Medical Imaging with a PASylated 89Zr-Labeled Anti-HER2 Fab-Fragment in a Patient with Metastatic Breast Cancer

Purpose PASylation® offers the ability to systematically tune and optimize the pharmacokinetics of protein tracers for molecular imaging. Here we report the first clinical translation of a PASylated Fab fragment (89Zr∙Df-HER2-Fab-PAS200) for the molecular imaging of tumor-related HER2 expression. Methods A patient with HER2-positive metastatic breast cancer received 37 MBq of 89Zr∙Df-HER2-Fab-PAS200 at a total mass dose of 70 μg. PET/CT was carried out 6, 24, and 45 h after injection, followed by image analysis of biodistribution, normal organ uptake, and lesion targeting. Results Images show a biodistribution typical for protein tracers, characterized by a prominent blood pool 6 h p.i., which decreased over time. Lesions were detectable as early as 24 h p.i. 89Zr∙Df-HER2-Fab-PAS200 was tolerated well. Conclusion This study demonstrates that a PASylated Fab tracer shows appropriate blood clearance to allow sensitive visualization of small tumor lesions in a clinical setting.

Behaviour evaluation of shear connection by means of shear-connection strips

Comparison of behaviour of shear connections by means of shear-connection strips (perfobond and comb-shaped strips) and headed studs under static and repeated loading, possible failure modes of concrete dowels and ways of the quantitative differentiation of some failure modes are described in the paper. The article presents a review of knowledge resulting from the analysis of shear-connection effects based on tests of perfobond and comb-shaped strips carried out in the laboratories of the Faculty of Civil Engineering of the Technical University of Kosice (TU of Kosice) in Slovakia and their comparison with results obtained by other authors.

Thrombocytosis as a prognostic marker in stage III and IV serous ovarian cancer

ObjectiveThrombocytosis is an adverse prognostic factor in many types of cancer. We investigated if pre-treatment increased platelet counts provide prognostic information specifically in patients with stage III and IV serous ovarian cancer which is the most common clinical presentation of ovarian cancer. MethodsPlatelet number on diagnosis of stage III and IV serous ovarian adenocarcinoma was evaluated in 91 patients for whom there were complete follow-up data on progression and survival. Survival and progression free survival of patients with normal platelet counts (150-350 ×109/L) was compared with that of patients with thrombocytosis (>350 ×109/L) by X2 and logrank tests. ResultsThe median age of the patients was 66 years-old. From the 91 patients, 52 (57.1%) had normal platelet counts (median, 273 ×109/L; range, 153-350) at diagnosis of their disease and 39 patients (42.9%) had thrombocytosis (median, 463 ×109/L; range, 354-631). In the group of patients with normal platelet counts, 24 of the 52 patients had died with a median survival of 43 months (range, 3-100). In the group of patients with thrombocytosis, 24 of the 39 patients had died with a median survival of 23 months (range, 4-79). In the entire group of 91 patients there was a statistically significant difference of the overall survival and progression-free survival between the two groups (logrank test P=0.02 and P=0.007, respectively). ConclusionIn this retrospective analysis of stage III and IV ovarian cancer patients, thrombocytosis at the time of diagnosis had prognostic value regarding overall survival and progression-free survival.

Luxury product and brand purchasing behavior: A conceptual perspective

Over the recent years, there has been a steady increase in the number of customers in luxury market. Such a substantial rise has gained much attention from both academics and industry experts seeking to better understand the meanings and factors involved in luxury consumption. Therefore, this special section aims to enrich and update the existing body of knowledge on luxury product and brand purchasing behaviour. We have encouraged the contributors to forward both conceptual and empirical papers reflecting current concerns in the field of luxury shopping, more specifically in luxury product and brand purchasing behaviour. In the end, this special section includes three papers on external store environment, meaning of luxury, and luxury brand fashion. Certainly, additional references are necessary for better understanding the meaning and contents of luxury consumption and main drivers behind it. The special section introduces the reader to some of these important references but also notes that there is still avenue for more studies in the future.

Rigidity and nonexistence of Riemannian immersions in semi-Riemannian warped products via parabolicity

In this paper, we study complete Riemannian immersions into a semi-Riemannian warped product obeying suitable curvature constraints. Under appropriate differential inequalities involving higher order mean curvatures, we establish rigidity and nonexistence results concerning these immersions. Applications to the cases that the ambient space is either an Einstein manifold, a steady state type spacetime or a pseudo-hyperbolic space are given, and a particular investigation of entire graphs constructed over the fiber of the ambient space is also made. Our approach is based on a pa\-ra\-bo\-li\-ci\-ty criterion related to a linearized differential operator which is a divergence-type operator and can be regarded as a natural extension of the standard Laplacian.
