Bilateral Ovarian Metastases from ALK Rearranged Non-Small Cell Lung Cancer

Anaplastic lymphoma kinase (ALK) rearrangement, is a kind of driver mutation, accounts for 3%-5% of non-small cell lung cancer (NSCLC). NSCLC patients harboring ALK fusion genes have distinct clinical features and good response to ALK inhibitors. Metastasis from lung cancer to the ovary has rarely been known. We report a case of a 54-year-old woman with bilateral ovarian metastases from ALK rearranged NSCLC. She underwent bilateral salpingo-oophorectomy for ovary masses, which were progressed after cytotoxic chemotherapy although primary lung mass was decreased. Histopathological examination of the ovary tumor showed characteristic adenocarcinoma patterns of the lung and ALK rearrangement.
A Study on Touchless Panel based Interactive Contents Service using IrDA Matrix

Touch panel is mainly applied to pressure type touch panel but it occur a low recognition rate and error during long-term use. So, it is partly applied to capacitive touch panel to compensate for these problems but it also can occur a same problems via pollutions. Touch technology has developed a various method but it is not used because of high costs and difficult installation process. So, in this paper, we proposed an input method of touchless panel using IrDA matrix. This method is conducted using an IrDA Matrix composed of depth sensor. It is possible to offer a various contents for multi user. The proposed technology need a development of a high sensitivity sensing method and high-speed processing method of position information for Seamless operation control. And, it is required high-precision drive technology. Also, we proposed a Seamless user recognition for interactive contents service through a touchless panel using IrDA matrix.

We report on a 64-year-old man with leptomeningeal metastasis (LM) from an epidermal growth factor receptor (EGFR)-mutated adenocarcinoma of the lung. He was treated with paclitaxel, cisplatin. After completion of chemotherapy, he complained of headache, nausea, and vomiting. EGFR-mutated tumor cells were identified from the cerebrospinal fluid (CSF). Second-line therapy with gefitinib, methotrexate was started. After receiving gefitinib for 4 weeks, he had no more headaches or vomiting. Eleven months after initiation of gefitinib, he developed headache and nausea. Chest computed tomography showed aggravation of bone metastasis. Third-line therapy was started with gemcitabine and carboplatin. Two weeks later, he experienced disorientation. After a fourth relapse within the central nervous system, the therapy was switched to erlotinib and significant improvement of LM was achieved. This case shows that LM can be diagnosed by detecting EGFR mutation in CSF and EGFR tyrosine kinase inhibitors are effective for LM from EGFR mutant non-small cell lung cancer.

Introduction: Eosinophilic cholangitis (EC) is a rare benign disorder of the biliary tract. This disorder must be distinguished from Cholangiocarcinoma (CCA) which can be diffi cult since it is characterized by a dense transmural eosinophilic infi ltration of the bile duct. The biliary tree is one of the most frequently affected extrapancreatic organs in autoimmune pancreatitis (AIP), and the term “IgG4-related cholangitis” was recently introduced to refer to the biliary manifestation of IgG4-related systemic disease. In this paper, we discuss an 80-year-old man with EC who was initially diagnosed by hilar CCA or AIP. Case Report: A 80-year-old man presented with epigastric discomfort and nausea. There was no history of foreign travel or signifi cant allergic reactions or medications. Laboratory studies revealed an increased eosinophil count (white blood cell count of 8. 4×103/mm3, with 14. 1% eosinophils). Amylase, lipase and Alkaline phosphatase levels were 208U/L, 21U/L and 114U/L. IgE level was 979. 4 IU/mL. Serum IgG level was elevated to 2260 mg/dL, but serum IgG4 was 369. 3 mg/L. Serology test for parasites were negative. Stool specimens showed no ova or cyst. A computed tomography suggested hilar CCA. A magnetic resonance cholangiopancreatography (MRCP) revealed multifocal irregular stricture in both intrahepatic ducts, common bile duct (CBD) and pancreatic duct (figure 1). Biopsy at papilla and distal CBD revealed no malignant. Biopsies at stomach and duodenum are consistent with Eosinophilic gastroenteritis (figure 2). Bile cytology shows numerous eosinophils and neutrophils. Oral prednisolone and ursodeoxycholic acid were administered. Thereafter, patient`s symptoms were relieved and abnormal laboratory studies were all normalized and MRCP fi ndings showed improvement of irregular stricture. Conclusion: Although the diagnosis of EC is a diffi cult one, biliary stricture is presented with peripheral eosinophilia, EC should be taken into consideration in the differential diagnosis.

<P><B>Abstract</B></P> <P>We investigated a series of uracil analogues by introducing various substituents on the phenyl ring of the <I>N</I>-3 aminoethyl side chain and evaluated their antagonistic activity against human gonadotropin-releasing hormone (GnRH) receptors. Analogues with substituents at the ortho or meta position demonstrated potent in vitro antagonistic activity. Specifically, the introduction of a 2-OMe group enhanced nuclear factor of activated T-cells (NFAT) inhibition up to 6-fold compared to the unsubstituted analogue. We identified compound <B>12c</B> as a highly potent GnRH antagonist with moderate CYP inhibition. Compound <B>12c</B> showed potent and prolonged LH suppression after a single dose was orally administered in castrated monkeys compared to a known antagonist, Elagolix.
