        Ultrasound-guided femoral and sciatic nerve blocks combined with sedoanalgesia versus spinal anesthesia in total knee arthroplasty

        Background: Although regional anesthesia is the first choice for patients undergoing total knee arthroplasty (TKA), it may not be effective and the risk of complications is greater in patients who are obese or who have spinal deformities. We compared the success of ultrasound-guided femoral and sciatic nerve blocks with sedoanalgesia versus spinal anesthesia in unilateral TKA patients in whom spinal anesthesia was difficult. Methods: We enrolled 30 patients; 15 for whom spinal anesthesia was expected to be difficult were classified as the block group, and 15 received spinal anesthesia. Regional anesthesia was achieved with bupivacaine 62.5 mg and prilocaine 250 mg to the sciatic nerve, and bupivacaine 37.5 mg and prilocaine 150 mg to the femoral nerve. Bupivacaine 20 mg was administered to induce spinal anesthesia. Hemodynamic parameters, pain and sedation scores, and surgical and patient satisfaction were compared. Results: A sufficient block could not be obtained in three patients in the block group. The arterial pressure was significantly lower in the spinal group (P < 0.001), and the incidence of nausea was higher (P = 0.017) in this group. Saturation and patient satisfaction were lower in the block group (P < 0.028), while the numerical pain score (P < 0.046) and the Ramsay sedation score were higher (P = 0.007). Conclusions: Ultrasound-guided sciatic and femoral nerve blocks combined with sedoanalgesia were an alternative anesthesia method in selected TKA patients.

      • Physico-chemical and mineralogical study of ancient mortars used in Harran area (Turkey)

        Very limited studies have been accomplished concerning the historical structures around Harran area. Collected mortar samples from the historic structures in the area were tested to explore their mechanical, chemical and mineralogical properties. Mortar samples from three different points of each historical structure were taken and specified in accordance with the related standards taking into consideration their mechanical, chemical and mineralogical properties. By means of SEM-EDX the presence of organic fibres and calcite, quartz, plagioclase and muscovite minerals has been examined. Additionally, by means of XRF analysis, oxide ($SiO_2$, $Al_2O_3$, and $Fe_2O_3$) percentages of mortar ingredients have been specified, also. According to the test results obtained, it was confirmed that the mortars had densities ranging between $1.51-2.10g/cm^3$, porosity values ranging between 8.89-35.38% and compressive strengths ranging between 5.02-5.90 MPa. Specimen HU, which has the highest durability and lowest water absorption and porosity, was the mortar taken from the most intact building in the mosque complex. This result is most likely due to the very little fine aggregate content of HU. In contrast, HUC mortars with a small amount of fine particles and brick contents yielded slightly lower compressive strengths. The interesting point of this study is the mineralogical analysis results and especially the presence of ettringite in these historic mortars linked to the use of pozzolanic materials. Survival of these historic structures in Harran Area through centuries of use and, also, having been subjected to many earthquakes can probably be explained by these properties of the mortars.

        The Effects of Potato Puree and Bread Crumbs on Some Quality Characteristics of Low Fat Meatballs

        The purpose of this study was to determine the effects of using different amounts of potato puree (PP) (10 or 20%) and 10% bread crumbs (BC) as an extender and also control samples (C) with no added extender on chemical composition, energy values, cooking analyses, colour measurements, water holding capacity (WHC), penetration values, thiobarbituric acid value (TBA) and sensory analyses of meatballs. Meatball samples were cooked in a pre-heated $180^{\circ}C$ electric oven. Uncooked meatballs formulated with 20% PP had the highest moisture content. No significant differences were recorded for protein contents of uncooked samples. The highest cooking yield was found in samples extended with 10% BC. Increasing PP from 10% to 20% increased cooking yield of meatballs. 20% PP increased moisture and fat retention values and water holding capacity of meatballs. Meatballs with 10% BC had the lowest (the hardness in the texture) and meatballs with the 20% PP had the highest (the softness in the texture) penetration values. Formulating meatballs at a level of 20% resulted lower $L^*$ values. TBA values of control samples were higher than in PP added samples at the end of the storage period. Flavour scores for meatballs formulated with PP were higher than control and meatballs formulated with BC. Meatballs formulated with 10% PP had similar overall acceptability with meatballs added with 10% BC.

        Evaluation of Patients Treated in Intensıve Care Due to COVID-19: A Retrospective Study

        Background The aim of this study is to report the demographic characteristics, clinical features, treatment protocols, comorbidities, imaging findings, prognosis and factors affecting mortality in critically ill patients with coronavirus disease 2019 (COVID-19) in the intensive care unit. Materials and Methods This retrospective cohort study consists of adult (≥18 years old) patients hospitalized in a tertiary hospital intensive care unit of with COVID-19. The independent effects of possible factors identified in previous analyzes on survival were analyzed using univariate and multivariate logistic regression analysis. Results The mean age of all patients was 70.2 ± 13.9 years. Of the 200 patients, 139 (69.5%) had died. White blood cells (19.2 ± 76.1 × 109 per L), neutrophil/lymphocyte ratio (15.4 ± 65.1), d-dimer (2,558.4 ± 4,574.2 ng/mL), ferritin (1,481.2 ± 4,447.4 μg/L) and C-reactive protein (CRP) (12.1 ± 11.9 mg/dL) levels were high at the time of admission. According to the results of univariate regression analysis; presence of additional disease (odds ratio [OR]: 3.837; P = 0.015), older age (OR: 1.027; P = 0.015), reverse transcriptase-polymerase chain reaction (RT-PCR) positivity (OR: 2.58; P = 0.019), higher heart rate (OR = 1.027; P = 0.028), higher APACHE II score (OR: 1.049; P = 0.012), higher sequential organ failure assessement (SOFA) score(OR: 1.479; P = 0.014), high d-dimer levels (OR: 3.180; P <0.001) and high CRP levels (OR: 1.035; P = 0.028) increases the risk of death. When patients with full data for all variables in the multivariate logistic regression model were evaluated; positive RT-PCR (OR=4.105; P = 0.005), older age (OR: 1.033; P = 0.024), higher heart rate (OR: 1.042; P = 0.006), higher (SOFA) score (OR: 1.477; P <0.001), high d-dimer levels at admission (OR: 3.459; P = 0.002) and diabetes mellitus (OR: 3.433; P = 0.035) increase the risk of death. Conclusion Mortality of critically ill patients with COVID-19 pneumonia was high (69.5%). Older patients and acute respiratory distress syndrome were at higher risk of death. High SOFA score, high d-dimer at admission, and presence of diabetes mellitus were associated with high mortality.       • Fragility-based rapid earthquake loss assessment of precast RC buildings in the Marmara region

        Seismic risk assessment studies are one of the most crucial instruments for mitigating casualties and economic losses. This work utilizes fragility curves to evaluate the seismic risk of single-story precast buildings, which are generally favored in Marmara’s organized industrial zones. First, the precast building stock in the region has been categorized into nine sub-classes. Then, seven locations in the Marmara region with a high concentration of industrial activities are considered. Probabilistic seismic hazard assessments were conducted for both the soil-dependent and soil-independent scenarios. Subsequently, damage analysis was performed based on the structural capacity and mean fragility curves. Considering four different consequence models, 630 sub-class-specific loss curves for buildings were obtained. In the current study, it has been determined that the consequence model has a significant impact on the loss curves, hence, average loss curves were computed for each case investigated. In light of the acquired results, it was found that the loss ratio values obtained at different locations within the same region show significant variation. In addition, it was observed that the structural damage states change from serviceable to repairable or repairable to unrepairable. Within the scope of the study, 126 average loss functions were presented that could be easily used by non-experts in earthquake engineering, regardless of structural analysis. These functions, which offer loss ratios for varying hazard levels, are valuable outputs that allow preliminary risk assessment in the region and yield sensible outcomes for insurance activities.

      • Breast, Cervical, and Colorectal Cancer Screening Status of a Group of Turkish Women

        Introduction: The aim of the study was to determine the breast, cervical, and colorectal cancer screening rates and the influencing factors in a group of Turkish females. Methods: This descriptive study was conducted in a School of Nursing. The study sample consisted of 603 females who were the mothers/neighbors or relatives of the nursing students. Data collection forms were developed by the investigators after the relevant literature was screened and were used to collect the data. Results: Of the women aged 30 and over, 32.8% had undergone a pap smear test at least once in their life. Of those aged 50 and over, 48.2% had undergone mammography at least once and FOBT had been performed in 12% of these women in their life. Having heard of the screening tests before, knowing why they are done, and having information on the national cancer screening program were important factors influencing the rates of women having these tests done. Discussion: The results of this study show that the rates of women participating in national cervical, breast, and colorectal cancer screening programs are not at the desired levels. It is suggested that written and visual campaigns to promote the service should be used to educate a larger population, thus increasing the participation rates for cancer screening programs.
