      • Survival Analysis of Biliary Tract Cancer Cases in Turkey

        Background: Because of the relative rarity of biliary tract cancers (BTCs), defining long term survival results is difficult. In the present study, we aimed to evaluate the survival of a series of cases in Turkey. Materials and Methods: A totally of 47 patients with billiary tract cancer from Mersin Goverment Hospital, Acibadem Kayseri Hospital and Kayseri Training and Research Hospital were analyzed retrospectively using hospital records between 2006-2012. Results: The median overall survival was $19.3{\pm}3.9$ months for all patients. The median disease free and overall survivals were $24.3{\pm}5.3$ and $44.1{\pm}12.9$ months in patients in which radical surgery was performed, but in those with with inoperable disease they were only $5.3{\pm}1.5$ and $10.7{\pm}3.2$ months, respectively. Conclusions: BTCs have a poor prognosis. Surgery with a microscopic negative margin is still the only curative treatment.

        ETS FUZZY DRIVER MODEL FOR SIMULTANEOUS LONGITUDINAL AND LATERAL VEHICLE CONTROL

        In this paper, evolving Takagi-Sugeno (eTS) fuzzy driver model is proposed for simultaneous lateral andlongitudinal control of a vehicle in a test track closed to traffic. The developed eTS fuzzy driver model can capture humanoperator’s driving expertise for generating desired steering angle, throttle angle and brake pedal command values byprocessing only information which can be supplied by the vehicle’s on-board control systems in real time. Apart from otherfuzzy rule based (FRB) models requiring human expert knowledge or off-line clustering, the developed eTS driver model canadapt itself automatically, even ‘from scratch’, by an on-line learning process using eTS algorithm while human driver issupervising the vehicle. Proposed eTS fuzzy driver model’s on-line human driver identification capability and autonomousvehicle driving performance were evaluated on real road profiles created by digitizing two different intercity express ways ofTurkey in IPG© CarMaker® software. The training and validation simulation results demonstrated that eTS fuzzy drivermodel can be used in product development phase to speed up different tests via realistic simulations. Furthermore eTS fuzzydriver model has an application potential in the field of autonomous driving.

        Anthropometric-Somatotype and Strength Profiles and On-Water Performance in Turkish Elite Kayakers

        The aim of this study was to investigate some physical and strength characteristics and relationships of these characteristics with performance of Turkish national flatwater kayak team male athletes. Eleven male Turkish national team paddlers, mean (SD) age 21,5 (2,16) years old, participated in this investigation. Testing was conducted over 5 days and included anthropometric, somatotype measurements and strength, running and flatwater kayak tests (200 m., 500 m., 1000 m.). Anthropometric assessments taken were the measurements of body mass, stature, the sum of four skinfolds and estimated percent body fat. Girth measurements and bone breadths were taken to allow the calculation of somatotype using the Heath-Carter procedure. Pearson product moment correlations were employed to determine the relationship of all parameters with flatwater kayak performance. The significant negative correlations were found between 500 m. and 1000 m. performance times and body weight (respectively; r=0.78, p<0.05 and r=0.71, p<0.05). The significant negative correlations were found between 1 RM bench row and 200 m. and 500 m. performance times (respectively; r=0.68, p<0.05 and r=0.80, p<0.01). Significant relationships with 200 m. time were found for the biceps and flexed biceps girth (respectively, r=-0.70, p<0.05, and r=-0.80, p<0.01). Significant relationships with 500 m. time were found for the biceps, flexed biceps and thigh girth (respectively; r=-0.76, p<0.01, r=-0.86, p<0.01, r=-0.81, p<0.01). Physical characteristics of Turkish national flatwater kayak team athletes were found similar when compared to international class flatwater kayakers from different countries except upper body dimensions. Because of the strong relationships between upper body dimensions and flatwater kayak performance; the training of Turkish flatwater kayakers should be more focused on improving upper body musculature and strength.

      • Is Season a Prognostic Factor in Breast Cancer?

        Background: Some studies have indicated an inverse relationship between cancer risk and sunlight exposure. Others have reported that the prognosis of some cancers such as prostate, colon, ovarian and non melanoma skin cancer, were affected by the season in which the cancer was diagnosed. In our study, we evaluated whether season is prognostic in Turkish patients with breast cancer. Materials and Methods: A total of 517 patients from Kayseri Training and Research Hospital were analysed retrospectively. Patients were divided into 4 groups according to season of cancer diagnosis: winter, spring, summer and autumn. The prognostic factors for disease free survival and overall survival were investigated. Results: No significant differences were found among groups regarding prognostic factors overall. Only estrogen receptor status and lymphovascular invasion were independent prognostic factors (p=0.001 and p=0.001 respectively). We found significantly differences for mean disease free survival among groups (p=0.019). Winter group had better mean DFS while summer group had worse DFS. Mean overall survival was similar in the four groups (p=0.637). Conclusions: The season is not an independent predictive factor. However, due to interaction with other factors, we think that the season of cancer diagnosis is important for cancer prognosis.

      • Evaluating and Managing Hyponatremia

        Although hyponatremia is a common electrolyte disorder, its evaluation and management are not well defined. When diagnosed, hyponatremia should be categorized based on four criteria: volume status, urinary Na+, serum K+, and acid-base balance. This approach helps to determine what the cause of hyponatremia is and how it should be treated. Initially, hypovolemic hyponatremia, including cerebral salt wasting syndrome (CSWS), is treated by volume resuscitation and salt supplementation. Euvolemic hyponatremia, including the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), is treated by fluid restriction and salt supplementation, and hypervolemic hyponatremia is treated by fluid restriction and salt restriction. Hyponatremia can be managed well using these primary treatments and medications.

      • Lack of Sunlight Exposure Influence on Primary Glioblastoma Survival

        Background: The prognosis of primary glioblastoma (GBM) is poor. Approximately 2/3 of primary brain tumor diagnoses are GBM, of which 95% are primary lesions. In this study, we aimed to evaluate whether more sunlight exposure has an effect on survival of patients with primary GBM. Materials and Methods: A total of 111 patients with primary GBM were enrolled from Kayseri in inner Anatolia which has a cold climate (n: 40) and Mersin in Mediterranean region with a warm climate and more sunlight exposure (n: 71). The patients with primary GBM were divided into two groups as Kayseri and Mersin and compared for progression free survival (PFS) and overall survival (OS).Results: The PFS values were 7.0 and 4.7 months for Kayseri and Mersin groups, respectively (p=0.10) and the repsective OS values were 13.3 and 9.4 months (p=0.13). We did not found any significant difference regarding age, sex, comorbidity, smoking, surgery, resurgery, adjuvant chemoradiotherapy and palliative chemotherapy between the groups. Conclusions: We found that more sunlight exposure had no impact on prognosis of patients with primary GBM, adding inconsistency to the literature about the relationship between sunlight and GBM.

        Lysophosphatidylethanolamine (LPE) Improves Fruit Size, Color, Quality and Phytochemical Contents of Sweet Cherry c.v. '0900 Ziraat'

        Lysophosphatidylethanolamine (LPE) affects the quality of flowers, fruits, and other horticultural products. Studies have provided evidence that LPE can accelerate ripening of fruits and prolong shelf-life at the same time. In this study, the influence of LPE on anthocyanin accumulation and phytochemical characteristics of sweet cherry was investigated. LPE ($10mg{\cdot}L^{-1}$) was applied to a commercial sweet cherry c.v. '0900 Ziraat' orchard two and four weeks before harvest for two treatment years (2011 and 2012). Preharvest applications of LPE resulted in significant improvement in both pomological and phytochemical attributes at harvest. LPE treatment led to a 17% increase in fruit weight and a 6% increase in soluble solid content when averaged over two experimental years. Fruit phytochemical content and antioxidant capacity were increased significantly. The average total phenolic content of LPE-treated fruits for the two years was $703{\mu}g$ gallic acid equivalent (GAE)/g fresh weight (g FW) compared to $569{\mu}g$ GAE/g FW in the untreated control. Fruits treated with LPE had a 27% and 16% more anthocyanin than the control fruits in 2011 and 2012. Antioxidant capacity of fruits, as measured by TEAC (Trolox equivalent antioxidant capacity) assay, was 12.5 and $11.4{\mu}mol$ TE/g FW in LPE-treated and untreated control fruits, respectively, when averaged over two experimental years. Our results suggest that preharvest application of LPE may have the potential to increase anthocyanin accumulation, improve fruit quality and enhance phytochemical characteristics of sweet cherries.

        Convolvulus galaticus, Crocus antalyensis, and Lilium candidum Extracts Show Their Antitumor Activity Through Induction of p53-Mediated Apoptosis on Human Breast Cancer Cell Line MCF-7 Cells

        Conventional and newly emerging treatment procedures such as chemotherapy, catalytic therapy, photodynamic therapy, and radiotherapy have not succeeded in reversing the outcome of cancer diseases to any drastic extent, which has led researchers to investigate alternative treatment options. The extensive repertoire of traditional medicinal knowledge systems from various parts of the world are being re-investigated for their healing properties. It has been reported that several members of the Convolvulaceae, Iridaceae, and Liliaceae families have antitumor activity against some tumor cell lines. Here we first report that Convolvulus galaticus, Crocus antalyensis, and Lilium candidum species have cytotoxic activity on human breast cancer cell line MCF-7 cells. Plant samples were collected and identified, and their cytotoxic effects on the MCF-7 cell line were examined at different concentrations of methanol extracts. We found that all three plants have cytotoxic effects on MCF-7 cells but that C. galaticus has the strongest cytotoxic effect even in the lowest extract concentration tested (0.32 μg/mL). Our results indicate that these plant extracts have cytotoxic effects on human breast carcinoma cell line MCF-7 cells and that this cytotoxic effect comes from p53-mediated stimulation of apoptosis.

        Overnight hospital stay and/or extended recovery period may allow long-duration oral and maxillofacial surgeries in the operating room of a dental hospital in an outpatient setting: a single-center experience

        Objectives: The requirement for overnight hospital stay should be considered preoperatively according to patient-related factors, type of surgery, and anesthetic management plan. In this study, we aimed to define the major factors that influence consideration of overnight hospital stay in patients undergoing oral and maxillofacial (OMF) surgery in an operating room (OR) of a dental hospital in an outpatient setting. Materials and Methods: The records of patients who underwent oral procedures under general anesthesia between 2014-2017 were reviewed. Results: A total of 821 patients underwent oral procedures under general anesthesia; 631 of them underwent OMF surgery in the OR of a dental hospital, and 174 of these patients were hospitalized for overnight stay. There was no significant difference in the number of patients with comorbidities between the outpatient and hospitalized patient groups (P=0.389). The duration of surgery was longer in the hospitalized patient group (105.25±57.48 vs 189.62±82.03 minutes; P<0.001). Double-jaw (n=15; 310.00±54.21 minutes) and iliac crest grafting surgeries (n=59; 211.86±61.02 minutes) had the longest durations. Patients who underwent iliac crest grafting had the highest rates of hospitalization (79%). The overall recovery period was longer in outpatients (119.40±41.60 vs 149.83±52.04; P<0.001). Conclusion: Duration of surgery was the main determinant in considering whether a patient required overnight hospital stay. However, patients with an American Society of Anesthesiology physical status score <3 may be scheduled for OMF surgery in the OR of a dental hospital in an outpatient setting regardless of duration of surgery if overnight hospital stay is planned or an extended recovery period is provided until patients meet the discharge criteria.

        Determination of the relationship between the fatigue and social support levels of cancer patients: a cross-sectional study

        Background/Aims: Cancer is seen as one of the most important health problems of our time. In the world and in our country, the most common death cause after cardiovascular diseases in the order of diseases that result in death is cancer. This descriptive, cross-sectional study is done in order to determine the relationship between fatigue and social support levels of cancer patients. Methods: Research was carried out in Cukurova University Hospital in Adana, which is located in the south of Turkey, between December 2014 and December 2015. Data were collected by interview form prepared by the researcher, Piper Fatigue Scale and Social Support Patient Form. Data was analyzed by percentile, mean, independent t test, one-way analysis of variance, correlation and multiple regression. Results: The average score of Social Support Patient Form is 131.1 ± 15.5, and Piper Fatigue Scale total score mean of the participants is 5.8 ± 2.4. A low level of negative correlation was found between the social support to cancer patients scale emotional support subdimension and the Piper Fatigue Scale affective subdimension. Conclusions: As a result of the research, it was seen that the level of fatigue of the participants had moderate and level of perceived social support was positive. The age, marital status, and education status of the participants do not affect the fatigue and social support levels.
