口內接近法에 依한 下顎骨 部分切除術 및 Resin Graft로 治驗한 良性 骨芽細胞腫 1例

This is to report an uncommon case of Benign Osteoblastoma occured in left mandibular body of 19 year-old man. The authors treated the patient successfully by intra-oral partial mandibulectomy with resin graft, and examined histopathologically. The results are as follows; 1. Beingn Osteoblastoma is a vascular osteoid and bone-forming tumor which occured uncommonly in mandible, and has progressive swelling followed by pain. 2. In this case, it was impossible to treat the beign osteoblastoma with usual methods such as curettage and conservative resection, because the mass was hard, fused closely to the intact surrounding bone and extended to the inferior border of mandible. 3. We had a good functional and cosmetic result by an intra-oral partial mandibulectomy with resin graft, and could not seen any complications such as infection or rejection phenomena associated with resin allograft.

The author has studied clinically and histopathologically on the 132 cases of tongue tumors in Koreans during the period of Jan., 1964 to Dec., 1976. Those materials were obtained from the Department of Pathology, College of Medicine of Yonsei University. The results of these studies are as follows; 1. Among the 132 cases of tongue tumor, benign tumors were 62 cases(47%) and malignant tumors were 70 cases(53%). 2. In the 62 cases of benign tumor of the tongue, the highest incidence was 22 cases(35.5%) of papilloma, and the lowest was one(1.6%) of Myxochondroma. 3. In the 70 cases of malignant tumor of the tongue, carcinomas were 67 cases(95.7%) and sarcomas were 3 cases(4.3%). Of these, the highest incidence was 59 cases(84.4%) of epidermoid carcinoma, and the lowest was each one(1.4%) of anaplastic carcinoma, angiosarcoma, reticulum cell sarcoma and malignant hemangioendothelioma. In 59 cases of epidermoid carcinoma well-differentiated group was 35 cases(59.3%), moderately well-differentiated group was 13 cases(22.0%) and poorly-differentiated group was 11 cases(18.7%). 4. The sexual incidence was relatively same in benign tumor, but male was predominant in malignant tumor(2.5:1). As to the age distribution of tongue tumor, each decade group showed relatively same distribution in benign tumor, but 40-60 decade group showed the highest incidence(56 cases or 80%) in malignant tumor. Average age for tongue tumor was 37.8 years in benign tumor and 53.6 years in malignant tumor. 5. The most common complaint of benign tumor was visible mass(45 cases or 72.6%), followed by pain and discomfort(9 cases or 14.6%) and foreign body sensation(3 cases or 4.8%), etc.. Malignant tumor showed 29 cases(41.4%) of visible mass, 21 cases (30.0%) of ulceration and 11 cases(15.7%) of pain & discomfort, etc.. 6. The most common site of origin of benign tumor was anterior 23 portion(29 cases or 46.9%), followed by dorsum(19 cases or 30.6%) and posterior 1/3 portion(8 cases or 12.9%) etc.. Malignant tumor showed 41 cases(58.6%) of anterior 2/3 portion, 16 cases(22.9%) of posterior 1/3 portion and 4 cases(5.7%) of dorsum, etc.. 7. The size of lesion was 2cm. or less in most cases(66.1% in benign tumor and 54.3% in malignant tumor). 8. Among the 36 cases of malignant tumor which were able to determine TNM classification, the highest incidence was 12 cases(33.3%) of stage Ⅲ, and the lowest was 3 cases(8.3%) of stage Ⅱ. The more posterior lesion was and the more large lesion was, the more its metastatic rate was increased.

This is a retrospective study on facial bone fractures of Koreans. The study was based on a series of 817 patients with facial bone fractures treated as inpatient at Severance Hospital of Medical and Dental College of Yonsei University during the period of Jan., 1971 through Dec., 1981. The results obtained are as follows: 1. Traffic accident was the most frequent cause of facial bone fractures (46.8%). 2. The ratio of Men to Women was 3.8: 1, and admissions for facial bone fractures have been increased year after year. 3. The age frequency was the highest in second decade (37.6%) and age distribution of patients ranged from 1½years to 83 years. 4. The most common location of facial bone fractures was Mandible (37.6%), and Zygoma complex (23.5%), Nasal bone (19.9%), Maxilla (9.3%) and Others (8.4%)were next in order of frequency. One location was fractured in 75% and multiple locations were involved in 35%. Among the 735 patients except for Others (82), one-location involvements were 610(83.0%), two locations were 96(13.1%), three-locations were 23(3.1%), and entire-locations were 6(0.8%). 5. In 380 patients of Mandible fractures, 310(83.8%) had fractures only in Mandible and 70(16.2%) had another facial bone fractures. The most frequent fx. site of Mandible was Symphyseal area (38.6/) and Simple fracture was the most frequent in type of fracture (63.4%). Teeth or alveolar bone trauma cases were 31% and Open-reduction with IMF was major method of treatment (51.4%) in Mandible fractures. 6. In 91 patients of Maxillary fractures, 21 (20%) had fractures only in Maxilla and 70(78%) had another facial bone fractures. Le Fort II and Unilateral fractures (22%) were two Major type of Maxillary fx., and Le Fort I II & III were 4 cases (4.4%). Teeth or alveolar bone trauma occured in 37.4% and Open-reduction was major method of tretment (37.8%) in Maxillary fractures. 7. In 230 patients of Zygoma complx fractures, 139(60.4%) had fractures only in Zygoma complex and 91(39.6%) had another facial bone fractures. Among these, Zygoma fx. were 105(45.7%), Zygomatic arch fx. were 94(40.9%) and Zygoma & Zygomatic arch fx. were 31(13.4%), and Left Zygoma was the most frequent site. Teeth or alveolar bone trauma occured in 15.2%, and Observation (32.4%) and Open-reduction (24.5%) were major methods of treatment in Zygoma complex fx. 8. In 194 patients of Nasal bone fractures, 141(72.7%) had fractures only in Nasal bone and 53(27.3%) had fractures on another facial bones. Closd-reduction (51.1%) and Observation (31.1%) were major methods of treatment. 9. In 817 patients of facial bone fractures, 21 patients(2.5%) were expired in Hospital and 12 of these were expired in 48 hours (1.5% ). Head (48.1%), Eye (12.2%) and Lower extremeties (12.1%) were injured commonly with facial bone fractures.

This is a case report of mandibular prognathism with anterior open bite by intraoral Obwegeser-Dalpont s Sagittal split osteotomies in the rami with genioplasty. We suspected this facio-dental deformity to be caused by developmental hypertrophy of mandible, proper selection of operative method was determined by depending upon preoperative work-ups such as radiographic survey, study model analysis and model surgery, cephalometric analysis and gross evaluation of facio-dental deformities. After surgical correction, Improvement of esthetic and functional problem has been obtained and maintained satisfactorily without any complications.

This is a case report of the bilateral bony ankylosis of temporomandibular joint associated with micrognathia, which was corrected by arthroplasty with silicone rubber implant in left side, condylectomy and coronoidotomy in right side. We suspected this deformity to be arised from unknown high fever in childhood. Ankylosis occured before mixed dentition period was main etiological factor of facial deformity. So, the earlier correction is the better result. It is to prevent recurrence of ankylosis and improve the prognosis that adequate bone resection, proper selection of interposing material and post-operative continuous forceful mandibular movement.

NBCCS is a hereditary disorder characterized by multiple defects of ectodermal and mesodermal structure, which is chiefly affected on skin, jaws & skeleton, CNS, eye and sexual organ. It is well associated with multiple odontogenic keratocyst on the jaws. Thus, we intended to investigate other anomalies presenting on NBCCS in cases of multiple odontogenic keratocyst which were treated in our hospital from 1980 to 1989. Here we reported 6 cases of NBCCS.
