This is a case report of the orthognathic surgery improved true mandibular prognathism by sagittal split ramus osteotomy with intraoral approach. The sagittal split ramus osteotomy was made on the lingual and buccal aspects and was then connected along the anterior border of the ascending ramus. After the mandible was displaced to the backward direction, direct bone wiring near the anterior border of ascending ramus was applied for stabilization. For preventing the postoperative swelling & hematoma, hemovac was inserted. There were no severe complications except postoperative swelling and edema for 2 days. Intermaxillary fixation was maintained for 8 weeks and postoperative orthodontic treatment was performed for 3 months.

구내 경사 분열 골절단술(Intraoral oblique splitting osteotomy)에 의한 하악 전돌증의 치험례

저자들은 2명의 하악 전돌증 환자에게 수술전 치열교정후 구내 하악지 경사 분열 골절단술을 시행하고 술후 치열교정을 계속 시행중이며 이 술식으로 다음과 같은 결과를 얻었다. 1. 전돌된 하악골의 후방이동으로 환자의 안모 추형과 심리적 열등감을 해소했으며 교합관계는 Angel 1급으로 회복하였으며 전반적인 부정교합을 치열교정과 더불어 해소했다. 2. 술후 간염과 골편 파절, 안면 지각이상, 측두하악 장애는 나타나지 않았으며 48시간 지난후 안면부 종창은 서서히 해소되었다. 3. 수술후 악간 고정은 8주간 시행했으며 악간 고정 제거 1주 후 정상적인 개구가 가능했다. 4. 경조직의 후방이동에 따른 연조직의 하순과 하악 이부에서의 이동량은 본 증례에서는 거의 1 : 1로 나타났다. 5. 악간 고정 기관과 술후 치열고정 기간중의 회귀현상은 약간 관찰되었으나 이에 대하여는 더 오랜 기간의 연구 관찰이 필요하고 관찰중이다. This is a report of 2-cases of mandibular prognathism corrected by Intraoral oblique spliting osteotomy of mandibnlar ramus. The Intraoral oblique splitting osteotomy is a modification of sagittal split osteotomy of ramus and it is documented by Yoshida, on 1985. By this method, authors obtained the following results. 1. The patients esthetic, psychological and functional problems were dissolvd by setback of mandibular prognathism. 2. The postoperative infection, splitted bone segments fracture, paresthesia of the face and T.M.J.dysfunction were not appeard. 3. Postoperative intermaxillary fixation was maintained for 8 weeks. The patients could open their mouths in normal range after a week of intermaxillary fixation removal. 4. The soft tissue changes of lower lip and chin were about 1 : 1 to the hard tissue changes. 5. During intermaxillary fixation period and postoperative orthodontic treatment, slight relapse was observed. Now, the patients are under postoperative orthodontic treatment.

Postoperative infection following orthognatic surgery is rare. When postoperative infections occur, the initial treatment consists of incision and drainage of the affected area, culturing to identify bacterial stains and verify antibiotic sensitivity, and the institution of the appropriate antibiotic regimen. Some plates and screws may eventually require removal, the initial therapy should be attempted to retain the plates and screws until adequete healing has taken place. In orthoganthic surgery, intra-operative complications as a lesion of inferior alveolar nerve, fracture of osteomised segments, incomplete sectioning, malposittion of segments, haemirrhage may occur. The surgeon should be familiar with possible complications to be caused and how to manage them. Prevention of postoperative infection following the orthognathic surgery consists of minimal periosteal reflection, aseptic management of operation field, proper surgical technique, rigid fixation, prophylactic antibiotics.

저자 등은 8년간 dilantin의 복용으로 상하악간에 전반적인 치은비대증을 나타낸 환자를 치은절제술과 치은성형술을 시행하고 계속적인 치석제거와 치은연하소파술을 시행하여 재발됨이 없이 심미적으로나 기능적으로 만족한 결과를 얻었기에 이에 보고하는 바이다. Enlargement of the gingiva caused by phenitoin an anticonvulsant used in the treatment of epilepsy, occurs in some of the patients receiving the drug. Its incidence varies from 3 to 62 percent, with the greater frequencies in younger patients. The hyperplasia is usually generalized throughout the mouth, but is more severe tendency in the maxillary and mandibular anterior regions. 18 year old male patient was admitted to our Department of Dentistry with the complaint of generalized painless gingival swelling. After the consult of the N.M. and laboratory study, the gingivectomy and gingivoplasty was performed. The periodontal pack and tin foil was applied on the attached gingiva to protect a surgical site and bleeding control. We obtained a good result of improved esthetics and function.

영남대학교 의과대학 부속병원 치과 ·구강외과에 상 ·하악 전돌을 가진 환자가 안모 추형으로 인한 심미적 장애와 상악 전치부 치간이개 및 하악전치부 치열부정을 주소로 내원하였다. 안면치조기형에 대한 육안적평가, 방사선학적검사, 두부계측사진분석 및 진단 모형분석 등 다각적인 분석 결과 상 ·하악 치조 전돌증으로 진단을 내렸으며, 내원 당시 본 환자는 전반적으로 치주병학적 문제점을 가지고 있었고 개인적으로 군입대라는 특수상황하에 있었기 때문에 치주조직에 대한 손상을 최소로 하면서 치료기간을 최대한 단축시킬 수 있는 치료계획을 세웠다. 상하악 전치부에 대한 술전교정 치료 후 상악골은 전방부분절 골절단술로 후방으로 이동시키고 하악골은 전방부분 치근단 하방 골절단술로 후하방으로 이동시키므로서 상하악 전돌증에 대한 악교정 외과적 처치를 하여 좋은 결과를 얻었기에 이에 보고하는 바이다. The authors treated a case of bimaxillary dentoalveolar protrusion corrected by anterior segmental osteotomies: As presurgical treatment & process, closing of the upper anterior spacing with fixed appliances was worked out, cephalometric prediction & model surgery was done, and fabrication of intra-arch acrylic resin splints were made. It was not necessary to make an intermaxillary fixation with wire after surgery. Only intramaxillary fixation of anterior retracted mobile segment with resin pkate was required for 8 weeks. After surgical treatment, leveling of the upper and lower arch with rectangular arch wire were accomplished as the intrusion of anterior teeth proceeded. We will continue to improve the class Ⅱ molar relationship by using class Ⅱ elastics and have a good occlusion through the orthodontic treatment.

부정유합된 상악골 골절로 인해 발생한 부정교합의 상악골 수평 골절단술과 후방분절 골절단술에 의한 치험례

저자등은 영남대학교 영남의료원 치과로 교통사고후 부정유합으로 인한 부정교합으로 저작곤란을 호소하며 내원한 환자를 상악골 수평 및 후방 분절 골절단술을 시행해 다음과 같은 결과를 얻었다. 1. 교통 사고후 부정유합된 상악골을 수평 및 후방 분절 골절단술을 이용하여 전방 치열의 반대교합과 구치부의 개교 및 scissor's bite를 해소했다. 2. 술후 감염등 특기할 만한 합병증은 발생하지 않았으며 술후의 종창은 48시간이 지나면서 서서히 감소했다. 3. 수술후 악간고정은 8주간 시행했으며 악간고정 제거 1주후 정상적인 개구가 가능했다. 4. 절단된 상악골 고정시 확고한 고정과 회귀성향을 줄이기 위해 miniplates로 고정했다. This is a case report of correction of malunioned maxilla after traffic accident by Le Fort I osteotomy and posterior segmental osteotomy. By this procedure, authors obtained the following results. 1. The malinioned maxilla after traffic accident which had anterior crossbite, posterior open bite and scissor's bite were corrected by Le Fort I osteotomy and posterior segmental osteotomy. 2. No postoperative infection and specific complication were seen in this case. 3. Postoperative intermaxillary fixation was maintained for 8 weeks. And then, the patient could open his mouth in normal range after a week of intermaxillary fixation removal. 4. For rigid fixation and reducing relapse, the osteotomized maxilla was fixed with miniplates.
