        성인 코눈물관 부분폐쇄에서 실리콘관 삽입술 후 마이토마이신 점안의 효과

        Purpose: To compare the result of mitomycin C (MMC) instillation after silicone intubation in partial nasolacrimal duct obstruction in adults according to duration of instillation. Methods: An instillation of 0.04% MMC eyedrops was performed on 61 eyes of 38 patients diagnosed with partial nasolacrimal duct obstruction. The clinical result of patient groups, classified according to duration of MMC eyedrops instillation, was analyzed prospectively. The patients groups was as follows: no MMC eyedrop instillation (group A), 1 week of MMC instillation (group B), 2 weeks of MMC instillation (group C), and 3 weeks of MMC instillation (group D). Results: Among all patients, 50 eyes (81.9%) showed improved symptoms. Fifteen eyes (71.4%) in group A, 8 eyes (72.7%) in group B, 13 eyes (86.7%) in group C and 14 eyes (100.0%) in group D had a successful outcome. The success rate was observed in groups D, C, B, and A, in descending order, which was statistically significant (p = 0.025). Conclusions: Silicone intubation is an effective treatment option for partial nasolacrimal duct obstruction in adults and MMC eyedrop instillation may improve silicone intubation results.         메티실린 감수성 표피포도알균(MSSE) 각막염과 메티실린 내성 표피포도알균(MRSE) 각막염의 비교 연구

        Purpose: To investigate the clinical features and treatment outcomes between methicillin-sensitive Staphylococcus epidermidis (MSSE) and methicillin-resistant Staphylococcus epidermidis (MRSE) keratitis groups. Methods: A retrospective analysis of case series was conducted of all patients with keratitis caused only by Staphylococcus epidermidis from January 1997 through December 2008. Sex, age, history of trauma, systemic disease, previous ocular history, antibiotic sensitivity test results, and treatment outcomes were evaluated. Patients were categorized into two groups as MSSE and MRSE according to methicillin-sensitivity result, and a comparative analysis was performed. Results: There were no significant differences in clinical features, such as risk factors or size or location of keratitis between the two groups. All MSSE and MRSE isolates were sensitive to vancomycin, moxifloxacin, and levofloxacin. All MSSE and 17%, 50%, 52%, and 57% of MRSE isolates were sensitive to cephalothin, norfloxacin, ciprofloxacin, and erythromycin, respectively (p < 0.05). There was no significant difference in visual acuity between the two groups. Conclusions: All MSSE and MRSE isolates were sensitive to vancomycin and to third- or fourth-generation fluoroquinolones In addition, approximately 50% of MRSE isolates were sensitive to norfloxacin and ciprofloxacin. There were no significant differences in clinical features of keratitis caused by MSSE versus those of MRSE isolates. Both keratitis groups had relatively good visual prognoses.         코눈물관 부분 폐쇄에서 실리콘관 삽입술 전후의 눈물주머니조영술 비교

        Purpose: To compare the dacryocystographic results before and after silicone tube intubation in partial nasolacrimal duct obstruction. Methods: Dacryocystography was performed on 33 eyes of 17 patients diagnosed with partial nasolacrimal duct obstruction. The anteroposterior (AP) diameters and the mediolateral diameters of the nasolacrimal ducts intubated at the operation were measured by dacryocystography, before the operation and after silicone tube removal. Results: The mean AP, mediolateral diameter and length of nasolacrimal duct in the group who demonstrated improvement after the operation was 2.32 mm, 1.39 mm, and 17.14 mm before the operation, and 2.40 mm, 1.77 mm, and 17.38 mm after the operation, respectively. The mean AP, mediolateral diameter and length of nasolacrimal duct in the group who demonstrated no symptomatic improvement was 2.06 mm, 1.28 mm, and 17.42 mm before the operation, and 2.75 mm, 1.99 mm, and 18.03 mm after the operation, respectively. The alteration of the nasolacrimal duct size in the group with successful postoperative results compared with unsuccessful postoperative results showed no significant difference. Conclusions: The nasolacrimal duct showed expansion in size based on dacryocystographic results after silicone tube intubation in partial nasolacrimal duct obstruction. However, the operation results and the alteration of the nasolacrimal duct size based on dacryocystographic results demonstrated no accordance. J Korean Ophthalmol Soc 2011;52(1):1-6

        난치성 신생혈관 녹내장에서 유리체강내 베바시주맙과 구후 트리암시놀론 동시 주사 효과

        Purpose: To identify the regression of neovascularization and relief of other symptoms after intravitreal bevacizumab and retrobulbar triamcinolone injection in neovascular glaucoma patients with no possibility of visual acuity recovery. Methods: A total of 15 eyes in 15 patients with neovascular glaucoma, who had no possibility of visual acuity recovery and could not be treated with surgical intervention despite pain from October 2008 to May 2009, were reviewed retrospectively. Changes in degree of pain, conjunctival injection, revascularization, and visual acuity were evaluated after injection. Results: Ten of the 15 (67%) neovascular glaucoma patients were male, with a mean age of 62.50±12.79 years. The most common prediposing ocular disease was diabetic retinopathy (9; 60%), and the others were central retinal vein occlusion (4: 26%), central retinal artery occlusion (1; 7%), and uveitis (1; 7%). Change in intraocular pressure was significant (p<0.001), as were decreases in the degrees of pain and conjunctival injection (p<0.001, <0.001) after the injections. Regression of neovascularization continued in 13 patients (87%) after two months. Conclusions: Intravitreal bevacizumab and retrobulbar triamcinolone injection is a safe intervention that can substantially relieve pain experienced by neovascular glaucoma patients.         만성 중심장액맥락망막병증에서 경구 스피로놀락톤의 효과

        목적: 만성 중심장액맥락망막병증 중 유리체강내 베바시주맙주입술 시행에도 호전이 없는 환자에서 경구 스피로놀락톤 치료의 효과를 알아본다. 대상과 방법: 2017년 9월부터 2018년 12월까지, 만성 중심장액맥락망막병증으로 유리체강내 베바시주맙주입술 시행에도 호전이 없는 환자에서 6개월동안 경구 스피로놀락톤 치료를 시행한 17명, 17안을 대상으로 중심황반두께, 망막하액의 높이, 최대교정시력의 변화를 후향적으로 분석하였다. 결과: 17명의 환자가 6개월동안 경구 스피로놀락톤을 복용하였다. 중심황반두께는 치료 전 309.94 ± 105.20 μm에서 3개월째 259.76 ± 81.83 μm, 6개월째 243.11 ± 61.98 μm로 통계학적으로 유의하게 감소하였다(Wilcoxon signed-rank test p=0.016, p=0.001). 망막하액의 높이는 치료 전 138.05 ± 95.69 μm에서 3개월째 70.88 ± 83.13 μm, 6개월째 54.00 ± 56.25 μm로 유의하게 감소하였다(Wilcoxon signed-rank test p=0.002, p=0.000). 최대교정시력(logMAR)은 치료 전 0.30 ± 0.38에서 1개월째 0.35 ± 0.43, 3개월째 0.29 ± 0.43, 6개월째 0.26 ± 0.40이었고 6개월째 치료 전에 비해 유의하게 감소하였다(Wilcoxon signed-rank test p=0.033). 경과 관찰 중 약물 부작용을 보인 환자는 없었다. 결론: 만성 중심장액맥락망막병증환자에서 6개월간 경구 스피로놀락톤 치료는 중심황반두께, 망막하액의 높이 감소, 시력개선에 효과가 있었으며 부작용은 관찰되지 않았다. Purpose: To evaluate the effect of oral spironolactone for non-resolving chronic central serous chorioretinopathy after intravitreal bevacizumab injection. Methods: Seventeen eyes of 17 patients with non-resolving chronic central serous chorioretinopathy after intravitreal bevacizumab injection from September 2017 to December 2018 were treated with oral spironolactone for 6 months, and changes in central macular thickness, subretinal fluid height, and best-corrected visual acuity (BCVA) were analyzed retrospectively. Results: The central macular thickness decreased from 309.94 ± 105.20 μm at baseline to 259.76 ± 81.83 μm at 3 months, and 243.11 ± 61.98 μm at 6 months, which were both statistically significant (Wilcoxon signed-rank test, p = 0.016 and p = 0.001, respectively). The subretinal fluid height decreased from 138.05 ± 95.69 μm at baseline to 70.88 ± 83.13 μm at 3 months, and 54.00 ± 56.25 μm at 6 months, which were both statistically significant (Wilcoxon signed-rank test, p = 0.002 and p = 0.000, respectively). The BCVA (LogMAR) changed from 0.30 ± 0.38 at baseline to 0.35 ± 0.43 at 1 month, 0.29 ± 0.43 at 3 months, and 0.26 ± 0.40 at 6 months. The results at 6 months were statistically significant (Wilcoxon signed-rank test, p = 0.033). There were no side effects in patients treated with oral spironolactone. Conclusions: In chronic central serous chorioretinopathy, treatment with oral spironolactone significantly reduced the central macular thickness, subretinal fluid height, and the BCVA, without side effects.

      • 하이테크 중소기업의 신제품 상용화 모델과 수출결정요인에 관한 연구

        본 연구는 하이테크 중소기업의 신제품 상용화 모델과 수출결정요인에 관한 연구를 살펴보고자 한다. 수출의 결정요인에 대한 주요 이론으로 자리매김해오고 있는 산업조직론, 자원기반 관점, 경쟁전략이론 관점뿐만 아니라 경영자 특성관점과 경영관리 기능 관점까지 포함하여, 하이테크 산업의 수출 및 수출성과의결정요인을 중심으로 분석하고자 하는데 목적이 있다. 본 연구 결과에 따라 다음과 같은 시사점을 나타났다. 첫째, 수출을 중심으로 하는 하이테크 산업의 정부지원과 육성정책이 시장수용에 따라 개정해야 한다. 둘째, 하이테크 산업이 초기시장과 산업 연구영역을 개척하여야 한다. 셋째, 기술상용화 모형에서 주류시장을 고려하면서 초기시장에서의 성공을 바라보아야 한다는 관점을 택하면서 이론적으로 공헌하고 있다. 넷째, 중소기업이 창출하는 하이테크 제품 수용과정에서 소비자 수용의도를 파악해야 한다. This study intends to examine the research on the commercialization model and export decision factors of new products of high-tech SMEs. Determinants of export and export performance of high-tech industries, including not only the industrial organization theory, resource-based perspective, and competitive strategy theory perspective, which have been established as the main theories for export determinants, but also the characteristics of managers and management management functions. It aims to analyze mainly. According to the results of this study, the following implications were shown. First, the government support and fostering policy of export-oriented high-tech industries must be revised in accordance with market acceptance. Second, the high-tech industry should pioneer the initial market and industrial research area. Third, considering the mainstream market in the technology commercialization model, it is contributing theoretically, taking the view that it should look at the success in the initial market. Fourth, it is necessary to grasp the consumer acceptance in the high-tech product acceptance process created by SMEs.

        유리체절제술 후 발생한 눈물막 및 안구표면장애에 대한 0.15% 히알루론산나트륨 점안액의 효과

        Purpose: To confirm the effect of 0.15% sodium hyaluronate on tear film and ocular surface disorders after vitrectomy surgery. Methods: The control group 19 patients and the test group 20 patients who underwent vitrectomy were enrolled. The control group took only antibiotics and steroids eye drops after surgery for 1 month, and the test group used 0.15% sodium hyaluronate eye drops for 2 months additionally. Ocular surface disease index (OSDI), tear film break-up time, Schirmer test, corneoconjunctival staining and meibomian gland seretion test were evaluated before surgery (baseline), 1 day, 1 week, 1 month, and 2 months after surgery. Results: There was significant difference between the control and trial group at 1 week in Schirmer test (p = 0.038). In OSDI, the score significantly increased until 2 months after vitrectomy in the control group (p = 0.021). However, the test group recovered at 2 month after surgery. In control group, TBUT showed significant decrease at 1 month (p = 0.022) and Schirmer test showed significant decrease at 2 months (p = 0.048) compared to the baseline, but there was no difference in the test group. Corneoconjunctival staining showed early increase and then progressive decreasing tendency at postoperative period in both groups, but there was a significant decrease of corneal staining at 2 months (p = 0.007) compared to the baseline only in the test group. Conclusions: Tear film and ocular surface disorders that occurred after vitrectomy worsen after surgery and then mostly recover. Postoperative 0.15% sodium hyaluronate instillation can alleviate the dry eye symptoms and is thought to contribute to early improvement of clinical findings.

        특발망막앞막에서 수술 전 정면빛간섭단층촬영 영상과 수술 후 시력 호전과의 관계

        Purpose: To investigate the correlation between en face optical coherence tomography and improvements in the postoperative prognoses of idiopathic epiretinal membranes. Methods: The medical records of 59 epiretinal membrane patients who had epiretinal membrane peeling between January 2005 and January 2016, and were followed up for > 12 months, were retrospectively reviewed. The preoperative en face images were divided into four sections involving three circular areas (6,000 µm diameter circle, 3,000 μm diameter circle, and 1,000 μm diam-eter circle) and one square (6,000 × 6,000 μm). The surface area where no epiretinal adhesion was present was quantified by measuring the number of black pixels using image-editing software (Adobe Photoshop CS6, Adobe Systems, San Jose, CA, USA). Then the correlations among the value of black pixels, preoperative and postoperative visual acuities, and central retinal thickness were analyzed. Results: The best-corrected visual acuity (BCVA) was significantly increased after epiretinal membrane peeling (p < 0.001), and the central retinal thickness was significantly decreased (p < 0.001). As the number of black pixels in the circles and the square in the en-face images increased, the postoperative BCVA significantly increased (r = 0.645, p < 0.001; r = 0.590, p < 0.001, respectively). Conclusions: As the nonadhesive surfaces of the epiretinal membrane and the retina in preoperative en face images became wider, the increments of the BCVA after surgery were greater. Therefore, en face optical coherence tomography can be used to predict prognosis after epiretinal membrane peeling. 목적: 특발망막앞막 환자에서 수술 전 정면빛간섭단층촬영(En Face optical coherence tomography) 영상과 술 후 예후와의 관련성에 대하여 알아보고자 한다. 대상과 방법: 2005년 1월부터 2016년 1월까지 특발망막앞막으로 진단된 후 망막앞막 제거술을 받은 환자 중 12개월 이상 경과 관찰한 환자 59명 59안의 의무기록을 후향적으로 분석하였다. 술 전 En Face 영상을 early treatment diabetic retinopathy study (ETDRS) subfields에 준하여, 3개의 원(6,000 μm diameter circle, 3,000 μm diameter circle, 1,000 μm diameter circle)과 1개의 사각형 (6,000 × 6,000 μm square), 총 4가지로 나누었고, 그래픽 편집 소프트웨어(Adobe Photoshop CS6, Adobe Systems, San Jose, CA, USA)를 통해 영상의 black pixel 값을 측정하여, 망막앞막의 유착이 없는 부위를 수치화하였다. 이후 Black pixel 값, 수술 전후 시력 및 수술 전후 중심망막두께와의 상관관계에 대하여 분석하였다. 결과: 망막앞막 제거술 후 최대교정시력은 유의하게 증가하였고( p <0.001), 중심망막두께는 유의하게 감소하였다( p <0.001). 6,000 × 6,000 μm square 및 6,000 μm diameter circle의 En Face 영상 black pixel 값이 증가할수록 수술 전후 최대교정시력 호전 정도가 증가하였으며, 이는 통계학적으로 유의하였다( r =0.645, p <0.001) ( r =0.590, p <0.001). 결론: 망막앞막 환자의 수술 전 En Face 영상에서 망막앞막과 망막의 유착이 없는 부위의 면적이 클수록 수술 후 최대교정시력의 증가량이 많았다. 따라서 망막앞막의 수술 후 시력 예후 예측 인자로서 정면빛간섭단층촬영 영상이 유용할 것으로 생각된다.
