        치성감염에 의한 하행 괴사성 종격동염: 증례보고

        Descending necrotizing mediastinitis (DNM) is a complication of odontogenic or oropharyngeal infections that can spread to the mediastinum. Such infections is serious, leading to sepsis and frequently to death. Even in this era of antibiotics, the mortality rate associated with DNM is approximately 40%. It is difficult to diagnose early because clinical and radiologic findings appear in the late stage of the infection. Delayed diagnosis is the principal reason for the high mortality in DNM. Therefore, descending necrotizing mediastinitis requires an early and aggressive surgical approach to reduce the high morbidity and mortality associated with this disease. We experienced a case of odontogenic infection followed by acute mediastinitis, so present now with the review of literatures.

        반응표면분석법을 이용한 쌀귀리 단백질의 알칼리 추출 공정 최적화

        알칼리 추출법을 이용하여 쌀귀리로부터 단백질을 추출하고 추출수율을 최적화하기 위한 알칼리 농도 조건 및 침전 pH 조건을 반응표면분석을 통해 검토하였다. 추출수율은 추출용매인 알칼리 농도가 증가할수록 그리고 침전 pH 4.9 부근에서 높은 경향을 보였으며, 알칼리 농도가 침전 pH에 비하여 보다 큰 영향을 미치는 것으로 나타났다. 알칼리 추출 공정으로 얻어진 귀리 단백질 농축물의 질소용해지수는 등전점 부근인 pH 5에서 최소값을 나타내었고, pH 3 이하, pH 7 이상에서는 급격히 증가하였다. 알칼리 추출 공정의 최적화를 위한 회귀분석 결과, 알칼리 농도 0.06 N, 침전 pH 4.7에서 최대 수율 85.89%로 예측되었으며 예측모델식과 실제 측정값을 비교하였을 때 유의적인 차이가 크게 나타나지 않아 설계된 실험모델식은 적합한 것으로 나타났다. 따라서 기존의 연구들은 알칼리 추출법을 이용한 귀리 단백질 추출 시 알칼리 농도와 침전 pH가 각기 다름을 보였지만, 본 연구를 통하여 귀리 단백질 농축물 제조를 위한 알칼리추출 공정의 최적 조건은 알칼리 농도 0.06 N, 침전 pH 4.7인 것으로 확인되었다. In this study, an attempt was made to produce oat protein concentrates from defatted oat groat by alkali extraction. Independent variables formulated by D-optimal design were NaOH concentration (X1, 0.005~0.06 N) for extraction and precipitation pH (X2, pH 4.0~6.0), and the dependent variable was extraction yield (Y1, %). Experimental results were analyzed by response surface methodology to determine optimized extraction conditions. Extraction yield increased both with an increase in NaOH concentration of the extraction solution and when approaching a precipitation pH of 4.9, and NaOH concentrations were a major influencing parameter. Solubility of oat protein concentrates showed a minimum value (i.e., 0.1%) at pH 5 and increased substantially at pH values in the range of ≤ pH 3 or ≥ pH 7, reaching a maximum value at pH 11 (i.e., 76%). Regression equation coincided well with the results of the experiment. Optimized extraction conditions to maximize extraction yield were 0.06 N NaOH (X1) for extraction and pH 4.7 (X2) for precipitation.

        초임계 CO2 유체 추출법으로 얻은 탈지 유채박 중 표면활성 물질의 유화 성질 연구

        This study was carried out to investigate the emulsifying properties of surface-active substances from defatted rapeseed cake by supercritical CO2 extraction. Based on the interfacial tension data, a supercritical fluid extract (SFE) with the lowest value of 14.16 mN/m was chosen for evaluation which was obtained from No. 2 extraction condition (150 bar, 65℃, 250 g). For emulsions with SFE, some physicochemical properties (i.e., fat globule size, creaming stability, zeta potential etc) were investigated according to changes in SFE concentration, pH, and NaCl addition in an emulsion. It was found that fat globule size was decreased with increasing SFE concentration in emulsion, with showing a critical value at 0.5 wt%, thereby resulting in less susceptibility to creaming behavior. The SFE emulsion also showed instability at acidic conditions (pH<7.0) as well as by NaCl addition. This was coincided with zeta potential data of emulsion. In addition, SSL (sodium stearoyl lactylate) found to be suitable as a co-surfactant, as it helped considerably in decreasing fat globule size in emulsions and its optimum concentration to be over 0.03 wt%, based on 0.1 wt% SFE in emulsion.

        초임계 CO<sub>2</sub> 유체 추출법을 이용한 탈지 유채박 중 표면활성물질 추출의 최적화

        본 실험에서는 탈지 유채박 중 표면활성물질의 추출을 위해 초임계 $CO_2$ 유체 추출법을 이용하였다. 추출 독립변수는 추출압력(150~350 bar), 온도($33{\sim}65^{\circ}C$), 보조용매량(ethanol, 150~250 g)으로 하였으며, D-optimal design에 의한 반응표면 분석을 통해 추출수율, 중성지질 분획, 인지질 분획, 당지질 분획 함량 등의 종속변수에 대한 최적 추출조건을 검토하였다. 그 결과, 압력, 온도, 그리고 보조용매량이 증가함에 따라 종속변수 즉, 추출수율, 인지질, 당지질 함량 등은 증가하였으나, 중성지질 함량은 감소하는 것으로 나타났으며, 그중 보조용매량이 각 종속변수에 가장 큰 영향을 주는 것으로 나타났다. 본 연구를 통해서 유도된 회귀식 모형은 실험을 통해 얻은 결과와 잘 일치하였으며, 초임계 $CO_2$ 유체 추출에 있어서 종속변수인 추출수율, 인지질, 당지질 함량 등을 최대로 하면서 중성지질 함량을 최소로 하는 최적화된 추출조건은 추출압력 350 bar, 추출온도 $65^{\circ}C$, 보조용매량 228.55 g으로 분석되었으며, 이 조건에서 추출수율은 5.98%, 추출물은 중성지질 3.0%, 인지질 69.43%, 당지질 17.46% 등의 조성을 나타낼 것으로 예측되었다. In this study, an attempt is being made to extract surface-active substances from defatted rapeseed cakes by supercritical carbon dioxide fluid. Independent variables for the extraction process, being formulated by D-optimal design, are pressure (150~350 bar), temperature ($33{\sim}65^{\circ}C$ and co-solvent (ethanol, 50~250 g). The dependent variables of the extraction yield, the content of neutral lipids, phospholipids and glycolipids in the extracts were analyzed upon the results through the response surface methodology. As for the extraction yield, it was found to increase with increasing independent variables, among which the co-solvent proved to be a major influencing parameter. Similar trends were found for the content of surface-active substances (i.e, phospholipids and glycolipids) in the extracts, except for the content of neutral lipids. Regression equations were suggested to coincide well with the results from the experiments. Extraction conditions are being optimized to maximize the extraction yields, the content of phospholipids, and glycolipids were 350 bar (pressure), $65^{\circ}C$ (temperature) and 228.55 g (co-solvent), respectively.

        급성 치성상악동염으로 인한 안구후농양: 증례보고

        Retrobulbar abscess is a rare, but severe complication of paranasal sinusitis. The clinical presentations are eyelid swelling, erythema, proptosis, conjunctival chemosis, restricted ocular movement, and decreased visual acuity. Diagnostic methods available for evaluating retrobulbar abscess include sinus X-ray, ultrasonography, computed tomography (CT), and bacterial culture. For the treatment of retrobulbar abscess, immediate surgical drainage and systemic antibiotic therapy are needed. Proper diagnosis and treatments are necessary for preventing visual loss, cavernous sinus thrombosis, subdural abscess, and other lifethreatening complications. A patient, a 30-year-old man, was admitted to our hospital because of progressive eyelid swelling, erythema, ptosis and decreased visual acuity on the right eye after endodontic treatment. The sinusitis occurred secondary to the infection from an upper molar tooth. The spread of the infection led to the orbit via ethmoidal sinus and posterior orbital wall. Immediate surgical intervention was performed and systemic antibiotics was administrated. The symptoms and signs are improved after treatments, so we present our case with a brief review of the literature.

        20개 조경수종의 형성층전기저항치의 수종간 차이와 계절적 변화

        The objectives of this study were to obtain standardized basic data on cambial electrical resistance (CER) of ornamental trees to be used for estimation of tree vigor and to compare CER between shrubs, deciduous, and coniferous trees in relation to bud opening, flowering, and growth cessation. Eighty healthy trees with four trees each for twenty ornamental tree species growing at an university campus in Suwon were selected and their CER was measured using a Shigometer every week from March to May , once a month from June to October and every two weeks in November, 1996. The CER of all tree species was high in March, started to decrease in April, maintained minimum in May through August, began to increase in September, and returned in November to the similar high level to March. Among the 20 species, Metasequoia glyptostroboides showed the lowest(5.5kΩ) annual average CER, while Cercis chinensis and Ligustrum obtusifolium had the highest(22.7kΩ, 22.9kΩ) annual average CER. The lowest CER(2.4kΩ) in summer(June through August) was obtained from Wisteria florihunda, second lowest(2.5kΩ) from Metasequoia glyptostroboides, the highest CER(46.8kΩ) during dormant season from Euonymus japonica, and second highest(45.0kΩ) obtained from Ligustrum obtusifolium. The CER of most species, particularly shrub species, started to decrease with bud opening, and many species, particularly shrubs and deciduous trees with large amount of flowers, showed sharp decrease with flowering. When CER was compared between shrubs and trees, shrubs showed higher average CER than trees, and seasonal difference in CER of evergreen species was smaller than that of deciduous species. It is interesting to note that the deciduous Metasequoia glyptostroboides had the lowest annual average CER.

        코로나바이러스감염증-19 유행 대비 요양병원 감염관리 교육 요구도 우선순위 분석: Borich 요구도와 The Locus for Focus Model 활용

        기저질환을 가지고 있고 면역 기능이 상대적으로 약한 노인이 많은 요양병원은 코로나19 감염자 발생 시 걷잡을 수 없는 상황에 놓일 수 있다. 집단감염 취약시설은 각별한 주의가 필요하며, 적극적인 감염관리 활동과 감염관리 프로그램의 중재가 필요하다. 이에 본 연구는 경기도 요양병원을 대상으로 교육 요구도를 파악하고, 좀 더 발전된 형태로 요구도에 따른 우선순위를 제시하여 코로나19 상황에서 요양병원 감염관리 전담자 대상 교육·훈련 프로그램 개발에 필요한 시사점을 마련해보고자 하였다. 연구 결과, 첫째, ‘감염관리 전담자가 없는 병원’이 28.4%로 나타났으며, 감염관리 전담자가 없는 요양병원은 있는 병원에 비하여 교육 경험률과 교육 내용 이해 수준 모두 낮게 나타났다. 둘째, 총 8개 영역의 31개 내용 중 ‘코로나19 검사 진행 시 개인보호구 사용’, ‘검사 공간을 타 공간과 분리하는 방법’, ‘검사 공간의 환기 기준 및 방법’, ‘검사 공간의 소독 방법’, ‘타 의료기관으로의 환자 이송 방법’, ‘의료기관 내 코로나19 확진자 발생시 역학조사 프로세스’, ‘개인보호구 폐기’ 7개가 교육 최우선 내용으로 조사되었다. 이와 관련한 요양병원 대상 교육 프로그램 개발과 시행이 우선적으로 이루어져야 할 필요가 있으며, 요양병원의 감염관리 활동이 체계적으로 진행될 수 있도록 현실적인 감염관리 전담자 지정 방안과 명확한 수가 기준안 마련 등 추가적인 노력이 필요하다. Infection-vulnerable facilities, such as long-term care hospitals, need special care and active infection control activities and programs. This study attempted to identify the educational needs and priorities of long-term care hospitals in Gyeonggi province, and to seek implications for the development of infection control education programs as it relates to the COVID-19 situation. According to the study, first, 28.4% of long-term care hospitals were found to have no dedicated personnel for infection control. Second, 7 out of 31 educational contents in 8 areas were surveyed as the top priority: Use of personal protective equipment (PPE) when conducting a COVID-19 test, how to separate the test space from other spaces, criteria and methods for ventilation of the test space, how to disinfect the test space, how to transfer patients to other medical institutions, epidemiological investigation process in case of COVID-19 confirmed in medical institutions and disposal of PPEs. Based on these results, the development and implementation of long-term care hospital education programs should be carried out first. In addition, further efforts must be made, such as appointing an infection manager to reflect the reality and establish clear compensation standards, so that infection control activities at long-term medical hospitals can be carried out systematically.

        상악동에 발생한 국균증에 관한 증례보고

        This infection includes an allergic bronchial reaction, an aspergilloma, and an invasive aspergillosis, which is more common in immunosuppressed patients and presents as multiorgan disease. The maxillary sinus is a relatively common site for formation of an aspergilloma where it forms around a nexus of foreign body. It has been indicated that the foreign body containing zinc extruded into the maxillary sinus has the potential to become infected and form an aspergilloma. The treatment of this disease is radical surgery and adjunctive systemic antifungal therapy. This is a case report of solitary aspergillosis of the right maxillary sinus. We experienced a case of right maxillary sinus aspergillosis in a 37 years old male who has dull pain on right temporal and zygoma area. We reported a case with review of literature.
