        스포츠손상의 자기공명영상 진단

        스포츠손상의 진단에는 임상에서 필요한 경우에 MRI를 포함한 여러 가지 영상 검사를 시행하게 된다. 이러한 검사를 추가로 시행하면 진단에 확신이 서고, 손상의 범위와 정도를 정확하게 알 수 있으므로 좀 더 적극적으로 적절한 치료법과 재활요법을 선택할 수 있는 장점이 있다. 이런 노력을 통해 아마추어는 물론이고, 직업적 운동 선수의 운동 능력을 온전히 보전하게 하여 오랫동안 최상의 경기력을 지킬 수 있게 하는 것이 스포츠의학의 최종 목표이다. 그러나 영상진단에서 관찰되는 이상 소견이 모두 임상적 연관성이 있는 것은 아니므로, 영상 결과의 임상 적용에 주의를 요하며, 궁극적으로는 환자의 증세를 치료해야 하는 것이지, 방사선 영상의 이상 소견을 치료하는 것이 아니라는 사실을 기억해야 한다. 따라서 엘리트 운동 선수의 경우, 손상을 받을 경우와의 비교를 목적으로 평소에 기준 영상 검사를 해 둘 가치가 있을 것이다. 우리나라도 프로스포츠의 발달이 계속되고 아마추어 운동을 즐기는 인구가 폭발적으로 증가하고 있으므로, MRI를 포함한 여러 영상법을 스포츠의학에 활발하게 적용하기 위하여 더욱 많은 스포츠관련 의사의 지속적인 관심과 열의가 필요하다. MRI (magnetic resonance imaging) is widely applied to diagnose sport-related injuries including bone and soft-tissue injuries. Because the MR characteristics of hydrogen nuclei depend upon on their local tissue environment, soft tissue structures of similar density may exhibit difference in signal intensity (brightness on image). This enables MRI to get high soft-tissue contrast resolution superior to other imaging modalities such as computed tomography (CT). Advantages of MRI are its capacity to depict occult bone trauma or bone bruise and ligamentous or tendinous injuries that are not visible on conventional radiographs. MRI does not expose the patient to radiation dose, so it can be used safely for pediatric patients. In this review, we will discuss on the osseousand soft-tissue injuries of the extremities which could be helpful from MRI in various clinical situations.

        방풍통성산이 비만인에 미치는 영향에 대한 임상적 연구

        Recently, people are becoming fatter in most parts of the world. Obesity is regarded as an important risk factor to adversely affect the health of humans, associated with hypertension, hyperlipidemia, diabetes, and cardiovascular diseases. In the present study, therefore, we tested the short time efficiency and safety of Bangpoongtongsungsan as a drug to decrease body weight in subjects with obesity. A total 48 volunteers (24 man, 24 woman) with body mass index (BMI : weight in kg divided by square of height in meters) of $23㎏/m^2$ participated in clinical study for 12 weeks, from 2008, March 01 to 2008, Jun 30. The subjects was received a dietary supplement of 12 pieces hard capsules per day (4 pieces in one, 3 times in a day) during the clinical study. At the end of clinical study, body weight of subjects was significantly decreased from $72.21{\pm}11.44\;kg$ to $70.53{\pm}11.67\;kg$ (p<0.05). Waist circumferences of subjects were reduced $91.58{\pm}6.43\;cm$ to $85.25{\pm}7.70\;cm$ (p<0.05). Also, a BMI of subjects was significantly decreased $26.07{\pm}2.35\;kg/m^2$ to $25.63{\pm}2.63\;kg/m^2$ (p<0.05). Particularly, a decrease in body weight, waist circumferences, and BMI was more significant in woman than man. But, other biochemical levels including lipid and glucose in plasma had no changes at the end of clinical study. In conclusion, administration of Bangpoongtongsungsan in subjects with high body weight fat could inhibit obesity associated with a decrease in body weight, waist circumferences, and BMI.

        이중에너지컴퓨터단층촬영을 이용한 통풍의 진단

        Gout is the most common crystal-associated arthropathy. Gout is caused by deposition of monosodium urate crystals within the joints, and it is often associated with hyperuricemia. Acute gout involves the first metatarsophalangeal joint (podagra) in approximately 50% of cases and its peak incidence occurs in middle age. Although the clinical features can help with making the diagnosis of gout, many inflammatory diseases such as cellulitis, pseudogout and septic arthritis can mimic or coexist with it. The definitive diagnosis requires polarized light microscopy of the fluid aspirated from the involved joint and this shows needle-shaped, negative birefringent monosodium urate crystals. However, joint aspiration can be technically difficult, and none of the conventional imaging modalities for gout specifically identifies the chemical composition of uric acid. The advent of Dual-Energy CT (DECT) is a noninvasive method that has the potential to confirm gout and monitor the response to treatment. DECT scan can show monosodium urate deposition by using color coding. The authors performed DECT scans for detecting uric acid deposition and confirming the gout noninvasively.

        Iodine - 131 - Iodomethyluridine을 이용한 종양세포증식의 영상화에 관한 실험적 연구

        N/A Purpose: Noninvasive imaging of tumor cell proliferation could be helpful in the evaluation of tumor growth potential and could provide an early assessment of treatment response. Radiolabeled thymidine, uridine and adenosine have been used to evaluate tumor cell proliferation. These nucleoside analogs are incorporated into DNA during proliferation. Iodine-131-Iodomethyluridine, an analog of Iodine-131-Iododeoxyuridine, is also involved in DNA/RNA synthesis. The purpose of this study was to develop Iodine-131-Iodomethyluridine and image tumor proliferation using Iodine -131-Iodomethy-luridine. Materials and Methods: Radiosynthesis of Iodine-131-5-Iodo-2'-O- methyluridine(Iodine-131-Iodomethyluridine) was prepared from 10 mg of 2'-O-methyluridine (Sigma chemical Co., St. Louis, Missouri)-and 2.1 mCi(SP. 10Ci/mg) of Iodine-131-labeled sodium iodide in 100 μl of water using iodogen reaction. Female Fischer 344 rats were inoculated in the thigh area with breast tumor cells (13765 NF, 105 cells/rat S.C.). After 14 days, the Iodine-131- Iodomethyluridine 10μCi was injected to three groups of rats(3/group). The percent of injected dose per gram of tissue weight was detennined at 0.5-hours, 2-hours, 4-hours, and 24-hours respectively. Tumor bearing rats after receiving Iodine-131-Iodomethyluridine(50μCi/rat, IV) were euthanized at 2 hours after injection. Autoradiography was done using freeze-dried 50 μ m coronal section. After injection of Iodine-131-Iodomethyluridine (100μCi/rat, IV) in three breast tumor-bearing rats, planar scintigraphy was taken at 45 minutes, 90 minutes and 24 hours. Results: Iodine-131-Iodomethyluridine was conveniently synthesized using iodogen reaction. The biodistribution showed fast blood clearance and the tumor-to-tissue uptake ratios showed that optimal imaging time was at 2 hours postinjection. Autoradiogram and planar scintigram indicated that tumor could be well visualized. Conclusion: The findings suggest that Iodine-131- Iodomethyluridine, a new radio-iodinated nucleoside, has potential use for evaluation of active regions of tumor growth.

      • 지역사회(익산시) 골다공증의 유병률

        Purpose: To evaluate the prevalence of osteoporosis as a major public health problem in a community society of Korea (Iksan city. South Korea). Materials and Methods: Face to face interviewing in 140 women who volunteered for the osteoporosis screening program of Wonkwang University Hospital was done. The following were excluded: women with previous trauma, hormone replacement therapy, and suspicious secondary osteoporosis. Both posterior-anterior and lateral dual energy x-ray absorptiometry measurement of the lumbar spine were performed in the remaining 101 women using the Hologic QDR 2000 bone densitometer. Bone mass measurement results were categorized according to the WHO definition. The prevalence of osteoporosis and osteopenia was calculated. In addition, correlation between bone mineral density and demographic characteristics (age, weight, height, and body-mass index (BMI, Kg/㎡) was evaluated with the correlation and multiple regression analyses. Results: The prevalence of osteopenia and osteoporosis was 39.6%, 25.7% respectively: 35.5%, 11.3% under 50 years; 46.1%, 48.7% above 50 years. The correlation, between bone mineral density (BMD) and demographic characteristics was followed: age. negatively related (r=-0.549, P<0.01); weight, positively (r=0.224, P<0.05); height, positively (r=0.204, P<0.05); BMI, positively (r=0.123, P>0.05). The age factor among them was significant. Conclusions: The prevalence of both osteopenia and osteoporosis in a Korean community society is high and BMD is significantly decreased according to age. these results may suggest that osteoporosis is a major public health problem.

        Sclerosing hemangiomas which uncommonly occur in the pulmonary parenchyma rarely have the air-meniscus sign on the radiologic examinations. We report a case of histopathologically proven sclerosing hemangioma with the air-meniscus sign of the lung on both chest radiograph and CT.

      • Multicentric Castleman 병 : 1예 보고 A Case Report

        Castleman disease is a rare disease entity of unknown etiology. Although the mediastinum is its most common location, it also occurs in other areas of the body, usually where lymph nodes are normally found. The multicentric form of the disease has been recognized only recently as a disease with an aggressive, often malignant course. The authors present the radiologic findings of a case of multicentric Castleman disease.

        Wegener's granulomatosis is characterized by necrotizing and granulomatous disease which involves upper, lower respiratory tract and kidney. The basic pathophysiologic mechanism of Wegener's granulomatosis is still unknown. We report a case of Wegener's granulomatosis which involved lung and pleura and was diagnosed by clinical symptoms, radiologic findings and anti-neutrophilic cytoplasmic autoantibodies.

      • 측두골 골절 : 고해상력 전산화 단층촬영 HRCT

        High resolution computed tomographic(HRCT) findings in 23 patients with temporal bone fractures were reviewed. Seventeen patients suffered from longitudinal fractures of the petrous bone, which was with a contralateral atypical fracture in one patient. Transverse and direct mastoid fractures were detected in three patients, respectively. The ossicular changes were noted in three patients of longitudinal fracture and also in one of transverse fracture. In all cases, HRCT allowed rapid and detailed evaluation of temporal bone fractures.
