        커프 압력 조절에 따른 혈류량 변화 평가를 위한 임피던스법의 구현

        N/A In this study, we measured the blood flow on arm by non-invasive method and implemented a system to measure variation of the blood flow by estimating bio-electrical impedance and arterial pressure according to cuff pressure. The implemented system measured impedance variation according to pressure variation applied by artificial cuff pressure on the measuring position. The system consisted of pressure measuring part and impedance measuring part using 4-electrode method. Pressure measuring part was composed of semiconductor pressure sensor and electronic circuit for signal processing of sensor output signal. In addition, impedance measuring part was composed of constant current source circuit and lock-in amplifier for detecting impedance signal. We conducted experiments of impedance measuring part using standard resistance for performance evaluation of the implemented system. In addition we experimented to estimate variation of the blood flow by measuring impedances of the experimental group. We estimated ratio of the blood flow resistance using mean arterial pressure and variation of the blood flow. As a result the ratio of the blood flow resistance and variation of blood flow were in an inverse relationship with each other and the correlation coefficient was -0.96776.

        휴대형 혈액가스분석 시스템의 구현 및 성능평가

        본 연구에서는 동맥혈액 내의 혈액가스(pH, pCO₂, pO₂)를 측정할 수 있는 휴대용 혈액가스 분석 시스템을 구현하였다. 구현된 시스템은 크게 하드웨어부와 소프츠웨어부로 구분하였으며 하드웨어부는 메커니즘과 전자회로부로 구분하여 설계하였다. 소프트웨어부는 각 모듈별로 작성하였으며, 운용프로그램, 세척프로그램, 교정프로그램, 측정프로그램 등으로 구성하였다. 그리고 시스템의 정확한 교정을 위하여 2점 교정방식을 사용하였고, 1점 보정방식을 추가 적용하여 측정의 정확성을 높이고자 하였다. 시스템의 평가를 위하여 각 전극의 감응특성을 조사하였다. 그리고 정확도 평가를 위하여 표준용액을 이용하여 측정실험을 하고 통계적인 분석을 하였다. 그 결과 계수변화율이 1.12이내이었고, 최대오차가 1.298%이내인 정확도를 나타내었으며, 휴대형 ABGA의 개발 가능성을 확인하였다. In this study, we implemented the measurement of pH, pO₂, pCO₂ of the arterial blood on a portable blood gas analysis system. This system is consist of two parts of hardware and software. The hardware part is divided into a fluidic mechanism and an electronic circuit unit. The system program is composed of operating, washing, correcting, and measuring routines. Both of 1-point and 2-point calibration schemes were used to enhance the accuracy of the measurement. In order to evaluate the performance of the developed system, we measured and performed statistical analysis on the characteristics of the sensing electrode response. As a result, coefficient variation was within 1.12, and maximum error was within 1.298%. We confirmed development possibility of portable blood gas analyzer.

        Objectives:With the observation of factors influencing depression, self-reported happiness, self-reported stress and suicidal attempt, this study compared and evaluated the mental health of adolescents from multicultural families and monocultural families in Korea. Methods:This study compared the characteristics of adolescents from multicultural and monocultural families based on the analysis of data of The Tenth Korea Youth Risk Behavior Web-based Survey and analyzed the factors influencing adolescent depression, self-reported happiness, self-reported stress and suicidal attempt. Results:There was no significant difference in the levels of depression, self-reported happiness, and self-reported stress between adolescents from multicultural and monocultural families. However, suicidal ideation and suicidal attempt were significantly higher in adolescents from multicultural families. Conclusion:There was no significant difference between the mental health of adolescents from multicultural and monocultural families. Because depression is the greatest factor for suicidal attempt, and violence has stronger relationship with depression of adolescents from multicultural families, further study is needed to prevent this trend.

        심활성도 압반사 제어 모델을 이용한 심혈관시스템 모델링 및 시뮬레이션

        본 연구에서는 심혈관시스템 내의 압력 변화를 감지하는 압수용체 중 가장 대표적인 대동맥 압수용체의 시뮬레이션을 위한 심활성도 압반사 제어모델을 제안하였다. 그리고 제안된 모델은 압반사 조절, 시간지연을 포함한 전기회로 모델들로 구성하였으며, 대동맥동의 압반사 조절시 시간지연이 심주기와 일회 심박출량에 주는 영향을 관찰할 수 있도록 하였다. 심활성도 압수용체 제어 모델에서 시간지연의 기전은 대동맥동 압수용체에서 감지된 압력 정보가 구심성 신경으로 전달되고, 이 정보는 중추신경을 거쳐 원심성 신경으로 전달되어 제어 기능을 수행한다. 제안된 모델의 시뮬레이션 결과 시간지연에 따라 심혈관시스템 변이성의 세가지 패턴을 관찰할 수 있었다. 먼저 시간지연이 2.5초 이상일 경우에는 대동맥압, 일회심박출량, 심박동수가 비주기적으로 발생하고 불규칙인 것을 관찰할 수 있었고, 시간지연이 0.1초에서 2.5초 사이일 경우에는 주기적인 진동이 발생함을 관찰할 수 있었다. 그리고 시간지연이 0.1초 이하인 경우에는 심박동수와 동맥압-심박동수의 궤적은 안정상태를 유지함을 관찰할 수 있었다. In this paper, we proposed a heart activity control model for simulation of the aortic sinus baroreceptor, which was the most representative baroreceptor sensing the variance of pressure in the cardiovascular system. And then, the heart activity control model composed electric circuit model of the cardiovascular system with baroreflex control and time delay sub-model to observe the effect of time delay in heart period and stroke volume under the regulation of baroreflex in the aortic sinus. The mechanism of time delay in the heart activity baroreflex control model is as follows. A control function is conduct sensing pressure information in the aortic sinus baroreceptor to transmit the efferent nerve through central nervous system. As simulation results of the proposed model, we observed three patterns of the cardiovascular system variability by the time delay. First of all, if the time delay over 2.5 second, aortic pressure and stroke volume and heart rate was observed non-periodically and irregularly. However, if the time delay from 0.1 second to 0.25 second, the regular oscillation was observed. And then, if time delay under 0.1 second, then heart rate and aortic pressure-heart rate trajectory were maintained in stable state.

        한국 성인에서 주관적 인지저하를 동반한 우울증의 특성과 삶의 질 : 제 7기 국민건강영양조사를 중심으로

        Objectives : This study aimed to investigate quality of life, severity of depression, suicidality, subjective health and subjective stress of depression with subjective cognitive decline in Korean adults. Methods : We used the 7th KNHANES data to enroll 415 participants with a score of 10 or higher on Patient Health Questionnaire-9 (PHQ-9), aged 20-64. Depression was divided into two groups based on the presence/absence of subjective cognitive decline. Demographic and psychological characteristics were compared between two groups. Correlation analysis of subjective cognitive decline, quality of life, depression, suicidal idea was carried out. To detect which variables influenced quality of life, a multiple regression analysis was carried out. Results : Among the 415 participants, 98 had depression with subjective cognitive decline. We identified significant differences in age, marital status, education, employment between the two groups. After adjusting for these variables, depression with subjective cognitive decline had lower EuroQol-5D index scores, more severe depressive symptoms without cognition and worse subjective health than depression without cognitive decline. There was a significant correlation between subjective cognitive decline and quality of life (r=-0.236, p<0.001), suicidal idea (r=0.182, p<0.001), depression score without cognition (r=0.108, p=0.028). Through multiple regression analysis, subjective cognitive decline was predictor of reduced quality of life (β=-0.178, p<0.001). Conclusions : Depression with subjective cognitive decline has poor quality of life and severe depression. Cognitive decline should be considered to improve treatment result in depression. 연구목적 국내의 성인을 대상으로 주관적 인지 저하를 동반한 우울증에서 삶의 질, 우울의 심각도, 자살 관련 특성, 주관적 건강 상태 및 스트레스 인지에 대해 조사하고자 하였다. 방 법 국민건강영양조사 제 7기 자료를 이용하여 Patient Health Questionnaire (PHQ-9)에서 10점 이상인 인구 415명을 대상으로 하였다. 주관적 인지 저하 유무에 따라 두 군의 특성 차이를 비교했다. 주관적 인지 저하와 삶의 질, 우울, 자살사고의 상관성을 보기 위해 상관분석 및 회귀 분석을 실시했다. 결 과 주관적 인지 저하를 동반한 우울증 집단이 삶의 질과 주관적 건강이 더 좋지 않았고 우울의 심각도가 심했다. 주관적 인지 저하와 삶의 질은 음의 상관관계, 자살사고와 우울은 양의 상관관계를 보였다. 회귀 분석을 통해 주관적 인지 저하가 삶의 질에 부정적 영향을 미치는 것을 확인했다. 결 론 주관적 인지 저하를 동반한 우울증은 삶의 질을 저해하고 우울의 심각도가 심하며 삶의 질, 자살사고 및 우울을 악화시키는 것으로 확인되었다.

      • 산업현장에 적용 가능한 IMU 센서 기반의 사용자 낙상감지 시스템 구현

        본 논문에서는 산업현장에서 낙상 사고 발생 시 신속한 사고 발견 및 조치를 위하여 산업현장에 적용가능한 IMU 센서 기반의 사용자 낙상감지 시스템을 구현하였다. 구현된 시스템은 사용자의 움직임에 따른 데이터를 기반으로 낙상감지 분류 알고리즘을 통해 낙상 상태를 판단한다. 낙상감지 분류 알고리즘은 계측된 데이터를 상보 필터 등의 전처리 과정을 수행한 후 가속도와 각도 데이터를 검출하여 낙상을 판단한다. 낙상감지 시스템의 성능 평가를 위하여 산업 현장에서 움직임 및 낙상 상황을 가정하여 낙상감지 판단의 정확성 평가를 진행하였다. 실험 결과, 30분의 일상생활 중 낙상 검출 정확도가 93.26%임을 확인하였다. 따라서 본 논문에서 구현된 IMU 센서 기반의 사용자 낙상감지 시스템은 일상생활 중 낙상을 검출할 수 있음을 확인하였다. 향후, 움직임에 따른 오차를 줄여 정확성을 높이고 다양한 상황에 따른 낙상 감지를 적용할 수 있는 연구를 지속적으로 수행하고자 한다. In this paper, an IMU sensor-based user fall detection system that can be applied to industrial sites is implemented for rapid accident detection and action in case of a fall accident in an industrial field. The implemented system determines the state of a fall through a fall detection classification algorithm based on data according to the user’s movement. The fall detection classification algorithm performs preprocessing on the measured data, such as a complementary filter, and then detects acceleration and angle data to determine a fall. In order to evaluate the performance of the fall detection system, the accuracy of the fall detection judgment was evaluated assuming movement and fall conditions at the industrial site. As a result of the experiment, it was confirmed that the accuracy of detection of falls during 30 minutes of daily life was 93.26%. Therefore, it was confirmed that the IMU sensor-based user fall detection system implemented in this paper can detect falls in daily life. In the future, we plan to continuously conduct research that can improve accuracy by reducing errors caused by movement and apply fall detection according to various situations.

        조현병 환자의 인지기능 및 심리사회적 요인과 직업재활과의 연관성

        Objectives:This study was performed to assess the effect of cognitive function on occupational rehabilitation and to review the clinical and psychosocial factors that may affect occupational rehabilitation in patients with schizophrenia. Methods:A total of 58 patients were enrolled and divided into three groups : group of occupational rehabilitation (n=18), day-care(n=19), and out-patient care(n=21). For the cognitive function assessment, the Korean version of the MATRICS Consensus Cognitive Battery(MCCB) and the Korean version of the UCSD Performance-based Skills Assessment (UPSA) were performed. To assess clinical symptoms, the Positive and Negative Syndrome Scale, the Korean version of the Personal and Social Performance Scale(PSP), and the Clinical Global Impression–Schizophrenia scale(CGISCH) were performed. For the psychosocial factors assessment, the Korean Insight Scale for Psychosis and the Korean Version of 4th Revision of Schizophrenia Quality of Life Scale were performed. Results:On the MCCB, the occupational rehabilitation group showed higher scores than the day-care group in the verbal learning and social cognition domains. On the UPSA, the occupational rehabilitation group showed higher scores than the day-care group in the communication domain. No significant difference was found between the groups in the total scores of these cognitive function tests. On the CGI-SCH and PSP, the occupational rehabilitation group revealed better outcomes than the other groups. Conclusion:The schizophrenia patients with occupational rehabilitation had higher cognitive functions on some subscales, which may reflect social cognitive functions, and higher CGI-SCH and PSP scores.

        인제대학교 의과대학 의예과 교육과정 개발과 편성 사례

        One important purpose of premedical education is to help students attain core competencies during the two-year premedical program. Since premedical curriculum is the foundation of medical education, it is necessary to develop the premedical curriculum in accordance with the overarching curriculum of a medical school. Inje University's College of Medicine (IUCM) has attempted to integrate competency-based medical education into the premedical education curriculum. The revised premedical curriculum aims to develop students' intrinsic competencies during premedical years. In an effort to do so, IUCM defined competencies for premedical education, analyzed and designed premedical courses according to the defined competencies, and suggested learner-centered instructional methods. As a result of implementing the revised premedical curriculum, students have reported both positive experiences and difficulties. Based on the results, it will be necessary to continuously improve the premedical curriculum as well as develop appropriate methods for evaluating student achievement of the defined competencies.
