Purpose: Various methods are used in clinical crown lengthening procedure, Esthetic is more important in anterior region than in posterior region. Therefore when performing clinical crown lengthening procedure in esthetic region, clinicians should choose method which doesn't impair esthetic. Surgical extrusion could be a good method to achieve esthetic results. Material and Methods: Two patients were scheduled to clinical crown lengthening procedure in esthetic region. Teeth were extruded with periotome carefully to the intended level. Extruded teeth were anchored with sutures. Sutures are removed after 7day. Restorations were seated after extruded teeth were stabilized. Result: Five to six months later, both cases showed favorable esthetic outcome that were harmonious with adjacent teeth. Conclusion: When clinicians are to do clinical crown lengthening procedure in esthetic region, predictable esthetic outcome could be achieved with surgical extrusion.

Purpose: Various kinds of biodegradable membranes are currently used in dental clinics. And the frequency and the necessity of their usage are increasing due to their numerous advantages. Therefore it is important to understand the difference of various membranes and histological reaction against implanted membranes. Materials and Methods: Biodegradable membranes of $Biogide^{(R)}$, $Resolute^{(R)}$, and $Tutodent^{(R)}$ were cut into small pieces by $1.0{\times}0.5cm$. The membranes were implanted 1.5cm apart from each other under the epithelium on the skull of 18 Sprague Dawley rats. The animals were sacrificed at 3, 7, and 14 days after surgical procedure. The specimens were examined by histological analysis. Results: 1. Early period after implantation of the membranes showed connective tissues surrounding membranes and there were a few inflammatory cells present. 2. In $Biogide^{(R)}$ and $Tutodent^{(R)}$ specimens, inflammatory cells and surrounding tissues were shown to infiltrate from outside with slight density difference inside. In $Resolute^{(R)}$ specimens, membranes were fragmented. Inflammatory cells and connective tissues were also observed inside. 3. In $Resolute^{(R)}$ specimen, giant cells were present which implicates that foreign body reaction has occurred. 4. $Biogide^{(R)}$ had lower integrity than other membranes and is not enough to be used alone in defect area. However, $Resolute^{(R)}$ had superior firmness than others. $Tutodent^{(R)}$ had middle level of integrity. Conclusion: This experimental model enabled to observe early inflammatory reactions and morphological changes of materials and can be used to develop and evaluate the efficacy of biodegradable membranes. Duplication of standardized human oral environment will be required in future experiments.
성견의 발치와 협측골 외측에 적용한 교원질 차단막과 재조합 골형성단백질의 치조제 보존 효과

Objective: The aim of this study was to determine the effect of multi-layer of a collagen membrane alone or loaded rhBMP-2 on the buccal plate for ridge preservation after tooth extraction. Material and methods: Following bilateral extraction of the maxillary 1st and 3rd premolars in five dogs, rhBMP-2 loaded collagen membrane was applied to the buccal plates at the 1st premolar and collagen membrane only was applied to the buccal plates at the 3rd premolar unilaterally. The collagen membranes applied in the experimental groups were piled into four layers. The corresponding sites of the contralateral side healed naturally. After 3months of healing, the animals were sacrificed. Radiographic and histologic analyses were performed. Results: There was no significant difference in the healing of extraction socket at both 1st and 3rd premolars. In microcomputed tomography, the widths of the residual ridge of the experimental groups were similar with the controls. Histologically, the experimental groups did not exhibit different pattern compared to the controls regardless of the addition of rhBMP-2. Conclusion: Layering of the collagen membrane with or without rhBMP-2 on the buccal plate failed to show the effectiveness in dimensional preservation of the extraction socket.

Purpose: Recombinant human bone morphogenetic protein-2(rhBMP-2) has been evaluated as potential candidates for periodontal and bone regenerative therapy. In spite of good prospects in BMP applications, there is economically unavailable for clinical use in dental area. The purpose of this study was to evaluate the osteogenic effect of rhBMP-2 produced by E.coli expression system. Materials and methods: Eight-mm critical-size calvarial defects were created in 48 male Sprague-Dawley rats. The animals were divided into 6 groups of 8 animals each. Each group received one of the following: Negative control(sham-surgery control), positive control(absorbable collagen sponge(ACS) alone) and experimental(ACS loaded with rhBMP-2). Defects were evaluated by histologic and histometric parameters following 2- and 8-week healing intervals. Results: The experimental group showed significant defect closure at 2 and 8weeks than the sham surgery and positive control groups. Moreover, the experimental group showed significantly greater new bone and augmented area than the other groups at both 2 and 8weeks. Conclusion: rhBMP-2 produced by E.coli expression system may be effective for bone regeneration.

Various periodontal barrier membranes used in many clinical and experimental fields, and many recent studies of membranes have reported good results. To improve clinical results, selection of barrier membranes is an important factor. So, we need not only to evaluate various barrier mem-branes, but also to understand the property of barrier membranes appropriate to defect characteristics. For this purpose, this study reviewed available literature, evaluated comparable experimental models, and compared various barrier membranes. From above mentioned methods, the following conclusions are deduced. 1. In i-wall periodontal defect models, new bone formation showed a consistent result, almost 30% of the defect size. New cementum formations measured mostly 40% of the defect size, but showed more variations than new bone formations. This seems to be resulted form difference in experimental methods, so standardization in experimental methods is needed for future studies. 2. Application PLGA barrier membrane to periodontal defect demonstrated improved healing in new bone and new cementum. 3. There was a minimal periodontal regeneration with calcium sulfate barrier membrane only. But, there was better healing pattern in combination of calcium sulfate membrane with bone graft material, such as DFDBA, 4. There was no significant difference between the experimental group that used chitosan mem-brane only and the control group. But, in combination with bone graft material for space maintanence, periodontal regeneration was improved. Overall, Space maintenance is a critical factor for Guided tissue regeneration using barrier membranes. Also, a barrier membrane itself that has difficulty in maintaining space, achieved better result when used with graft material.
토끼 두개골 결손부에서 블록형과 분말형 hydroxyapatite의 골전도 효과

Purpose: The purpose of this study was to evaluate the bone regeneration of particulated hydroxyapatite(HA) and block type of hydroxyapatite graft in rabbit calvarial defects. Methods: An 8 mm calvarial circular defects were created in sixteen young adult New Zealand white male rabbits (weight $3.0{\sim}3.5kg$). Each defects were filled with Bio-Oss, particulated HA and block type HA. Sham surgery control defects were filled with blood clots. The specimens were harvested at 4 weeks and 8 weeks for histologic and histomorphometric evaluation. Results: Histomorphometric analysis demonstrated statistical differences in defect closure, new bone formation, and bone density of the four groups. Block type of HA group showed increased bone formation and bone density at 4 weeks and 8 weeks compared with Bio-Oss group or sham surgery control group(p<0.05). Conclusions: Block type of HA is an effective material for osteoconduction in rabbit calvarial defects, which may acts as a guide in use of these products in human application.

Bony exostosis, one of the osseous deformities, could act as a contributing factor of periodontal disease since it makes proper tooth brushing difficult and causes traumatic ulcer. The purpose of this case report is to show improvement of periodontal health by removal of bony exostoses and creation of physiologic osseous form. A 58 year old female patient with recurrent ulcer and pain on bony exostoses located on left maxillary palatal area and both mandibular buccal areas was treated. Exostoses were removed and physiologically positive osseous form was created following vertical grooving, radicular blending, flattening interproximal bone and gradualizing marginal bone. The patient showed no further recurring traumatic ulcer and bone exposure. Moreover, periodontal pocket was eliminated and food impaction was decreased. In conclusion, periodontal health could be achieved through removal of multiple bony exostoses and development of ideal osseous form.
화재시 커튼월 프레임 최적설계조건 도출을 위한 실험적 연구

본 연구에서는 국외 커튼월 내화 Code인 EN 13830, EN 13501-1, EN 1364-3을 사용하여 알루미늄과 스틸프레임에 대해 화재시 최적설계조건 도출을 위한 실험을 실시하였다. 현재 국내 커튼월의 내화 법규는 2008년 '내화 충전구조 세부 운영지침'에서 조인트 부분에 대해서만 내화규정이 정립되어 있다. 따라서 본 연구에서는 EN 13830과 그 하위법을 이용하여 알루미늄과 스틸 커튼월의 최적설계조건 도출을 위한 실험을 실시하였다. 실험 결과 차염성은 알루미늄 커튼월과 스틸 커튼월의 성능시간이 각각 10분 26분으로 약 16분의 차이가 발생하였고, 차열성은 알루미늄 커튼월과 스틸 커튼월이 최고온도에서 6분과 9분으로 약 3분정도의 차이를 보였고 평균온도에서는 9분, 24분으로 약 15분의 차이를 보였다. 그리고 복사열은 성능기준 <TEX>$15Kw/m^2$</TEX>를 통과하는데 알루미늄 커튼월은 16분 스틸 커튼월은 24분을 기록하였다.
지연된 의도적 재식술을 통한 치주 조직 재생 방법의 고찰

Purpose: Delayed intentional replantation was introduced as a new alternative to treat the teeth with severe periodontal involvement. The purpose of this study was to elucidate the possibility of delayed intentional replantation and establish theoretical backgrounds. Materials and Methods: Studies were performed into the following two subjects; ① Clinical evaluation of patients who underwent delayed intentional replantation using clinical and radiographic data. Severe periodontitis involved teeth were carefully extracted and proper time for delayed replantation was evaluated by analyzing inflammation markers (IL 6, TNF α). ② Theoretical studies for efficacy of delayed intentional replantation using ( ) Epigallocatechin 3 gallate (EGCG) for preservation of periodontal ligament cells on root surface by minimizing inflammation and treatment of inflammatory extraction sockets. Results: Meaningful success ratio and survival rate were found in delayed intentional replantation showing reduced bone loss and maintained bone level. Additionally, viability of EGCG applied periodontal ligament cells was much higher than control group. Also, EGCG promoted healing of inflammatory extraction sockets by inhibiting inflammatory cell proliferation. Conclusion: Within the limitations of this study, 1 2 weeks after extraction is an appropriate time to do delayed intentional replantation. Also, EGCG provides helpful effects on viability of periodontal ligament cells and periodontium.
