        Comparative study of two CAD software programs on consistency between custom abutment design and the output

        목적: 두 가지 CAD software에서 각 software의 맞춤형 지대주 디자인과 출력물의 일치도를 비교 평가한다. 연구 재료 및 방법: 3Shape Dental System과Delta9 CAD 소프트웨어를 이용하여 임플란트 식립 석고모델에 맞춤형 지대주를 디자인하였다(CRM STL file).디자인한 지대주를 밀링 한 후, 접촉식 방식으로 스캔하고(Test STL file), Inspection 소프트웨어에서 각 지대주의 Test STL file과 CRM STL file을 중첩하여 오차값을 측정하였다. 결과: 시편의 전체 스캔 오차 비교와 축면 경사각에 따른 축면부위 오차비교에서 Delta9이 더 나은 밀링 재현성을 보였다(P < .05). 마진설정 시, 반경 0.9 mm에서 Delta9의 디자인과 출력물의 일치도가 더 우수했다(P < .05). 반면, Anti-rotation 형태 부여에 따른 유의할 만한 차이는 없었다. 부위별 오차 값 누적 비교에서는 Delta9이 대부분의 시편에서 더 작은 오차 값을 보였다(P < .05). 결론: Delta9이 대부분의 디자인 설정 환경에서 3Shape보다 더 작은 오차 값을 보였다. 이는 Delta9을 사용했을 때, 계획된 디자인과 출력물의 일치도가 3Shape과 유사하거나 더 좋은 출력물을 얻을 수 있음을 의미한다. Purpose: This study was aimed to compare the consistency between the custom abutment design and the output in two CAD software programs. Materials and Methods: Customized abutments were designed by using 3Shape Dental System CAD software and Delta9 CAD software on a plaster model with implants (CRM STL file). After milling of the designed abutments, the abutments were scanned with a contact method scanner (Test STL file). We overlaid the Test STL file with each CRM STL file by using inspection software, and then compared the milling reproducibility by measuring the output error of the specimens from each CAD software program. Results: The Delta9 showed better milling reproducibility than 3Shape when comparing the milling errors obtained with a full scan of all specimens (P < .05) and also when comparing the axial wall region specifically according to the axial angle. With 0.9 mm marginal radius, the Delta9 showed better consistency between the design and the output than 3Shape (P < .05). While, anti-rotation form had no significant difference in error between the two systems. When cumulative errors were compared, the Delta9 showed better milling reproducibility in almost cases (P < .05). Conclusion: Delta9 showed a significantly smaller error for most of the abutment design options. This means that it is possible to facilitate generation of printouts with reliable reproducibility and high precision with respect to the planned design.

        감염관리 분야에서 감시배양검사의 표준화 연구

        의료 관련 감염을 예방하기 위한 필수적 방법은 일관성이 있는 감염감시 시스템을 구축하고 효율적인 감시 통제를 수행하기 위해 신뢰할 수 있는 상황에 대해 진단을 향상시켜나가는 것이다. 또한 의료 종사자의 손과 의복 및 장비는 환자 관리 및 환경과 접촉하여 병원체가 오염되는 요인이다. 병원체를 가진 시설 표면(예: 침대 레일, 침대 옆 탁자), 음용수, 냉각탑용수, 내시경기구, 급식위생, 공기매개, 멸균검사, 내독소검사, 및 의료 장비를 포함한 의료 환경의 오염은 일반적으로 발생한다. 또한 이러한 감염원을 활동 감시를 통해 MLST, PFGE의 기법으로 역학 분석을 수행한다. 따라서 HAI 예방을 위한 환경 감시배양 검사는 환자의 안전과 감염원의 차단을 향상시킬 뿐만 아니라 국가의 감염관리 시스템을 통제하여 최적의 효율적인 감염관리 예방을 가능케 하고 국가의 감염관리 시스템의 안전을 향상시킨다. 결론적으로 감시배양 검사의 표준화를 통해 실효성 있는 감염관리체계에 이바지하고 감염관리 전문인력으로서의 전문성을 확보하고자 한다. 이를 통해 표준화 마련의 일차적인 목표는 의료관련 감염을 줄이고 국가적 의료관리 체계를 향상시키는데 있다. An essential measure to prevent healthcare-associated infections (HAI) is to develop a consistent system of surveillance, thereby promoting a reliable situation diagnosis to perform efficient control for the problem. Patient-to-patient transmission of pathogens within the hospital plays a substantial role in the epidemiology of HAIs. Contamination of healthcare environments commonly occurs, including facilities surfaces (e.g., bed rails, bedside tables), drinking water, cooling tower water, endoscopic instruments, food, airborne, endotoxin test, sterile test and medical equipment, with pathogenic organisms. In addition, epidemiological analysis is performed by multi locus sequence tying, pulsed-field gel electrophoresis for active surveillance. Therefore, an environmental surveillance culture test for prevention improves patient safety and blocks infection agents. Effective infection control and increased safety are possible by controlling the national infection control system. In conclusion, this study contributes to an effective infection control system through the standardization of active surveillance culture laboratory and secure expertise as infection control specialist. The primary objective of the standardization is to improve the safety of the nation’s healthcare system by reducing the rates of HAIs.

      • 혈장성분헌혈 대학생이 느끼는 염려정도에 관한 연구

        Objectives: This study examines the degree of concern experienced during plasma-donation among university students who visited the blood-donation house located in two universities in Gwang-ju City to donate blood-plasma by grasping the variables affecting concern in order to provide basic data establish needed nursing intervention for donator of blood plasma. Method: The subjects of this study were 184 university students and the data were collected by self-reported Questionnaire given 10 minutes after the donation of blood plasma. Period of data collection was from Jul. 1st to Aug. 30th 2003. Results: Total concern mean was 2.29 and for the Question 'When nurse seemed lacking blood collection skill' showed the highest point and the mean as 3.11 and 'Would there be problem in health condition after blood-donation because the amount of donated blood is too much' showed the lowest mean as 1.86. Female students showed higher concern score as 38.74±8.47 than male students as 31.94±9.14 and there was significant difference(t=-5.08, p=.00). The degree of concern showed significant difference in frequency of blood donation(f=5.31, p=.00) and as Scheffes post-test, there were significant differences in '1-5times donated group'(37.32±9.49). For the Question of 'what the most inconvenient matter in plasma-donation', the group answered 'no' for 'it took long time comparing with previous whole blood-donation' was 35.58±9.51, and it was higher than 'yes' group(mean 32.68±9.20) and there was significant difference(t=-2.07, p=.04). Among concern characteristics according to the symptoms blood-plasma-donation, 'sleepy, tired' answered group's mean was 36.61±9.32 and it was higher than that of group answered 'no' for the question 33.36±9.40 and there was significant differencdt=2.19, p=.03) Answered 'got bruise' was 38.36±8.69 and it was also higher than that of group answered 'no' as 33.45±9.43 and there was significant difference(t=2.96, p=.01) The group answered 'no abnormality after blood-plasma-donation' was 36.03±9.33 and it was higher than that of group answered 'yes' for the same question as 32.34±9.30 and there was significant difference(t=-2.67, p=.01). The level of concern according to the nurses' kindness, after Scheffes post-test, there were significant differences between 'kind' group(36.24±8.l4) and 'very kind' group(29.73±10.35), and between 'moderate' group(38.28±9.29) and 'very kind' group(29.73±10.35) (f=7.90, p=.00). Conclusion: The concern level during plasma-donation in university students was showed the most high 'when nurse seemed lacking blood collection skill'. So in order to decrease the level of plasma donator's concern, the nurses must try to increase collecting skill of blood and try to show kindness to the plasma donators also.
