비대상성 간경변증 환자에서 베타차단제와 알부민 사용 관련 업데이트

간경변증에서 나타나는 대표적인 합병증으로 위식도 정맥류 출혈 및 복수가 있다. 정맥류 발생의 주요기전은 문맥압항진증이며 과역동적 순환의 기전에 의하여 더욱 악화된다. 비선택적 베타차단제 사용이 식도정맥류가 있는 간경변증 환자에서 초출혈 및 재출혈 예방 효과가 있다는 다수의 연구들이 있었으나, 일부 연구에서 복수를 동반한 간경변증 환자에서 비선택적 베타차단제 사용의 부정적 효과를 보고하였다. 그러나 최근 진행된 연구들에서는 비선택적 베타차단제의 정맥류 출혈 감소 및 생존률 개선에 대한 긍적적 효과를 보여주고 있다. 알부민은 간경변증 환자에서 대량의 복수천자로 인한 부작용을 감소, 자발성 세균성 복막염 환자에서 신기능 장애 및 간신증후군 발생을 감소 및 간신증후군에서 terlipressin과 병합요법으로 사용되고 있다. 또한 간경변증 환자에서 장기간의 알부민 투여가 간경변증 합병증 발생 감소 및 생존률 향상에 도움을 준다는 연구들이 보고되고 있다.

With the introduction of the direct acting antiviral agents (DAA) for the treatment of chronic hepatitis C, many changes are taking place. Sustained virological response (SVR) has been reported as 58-86% in peg-interferon and ribavirin combination therapy for chronic hepatitis C patients, but SVR was improved by more than 90% as from the DAA is used. In Korea, several regimens are approved for genotype 1 and 2 chronic hepatitis C. In the near future, once-daily regimen for short period will provide high rates of SVR among both previously treated and untreated patients infected with HCV genotype 1, 2, 3, 4, 5, or 6, including those with compensated cirrhosis. It is need to understand how to use the currently approved DAA therapy which are treatment indication, treatment duration, drug-drug interactions and safety profile in clinical practice.

Pros: Discontinuing NUCS [nucleos(-t)ide] treatment before HBsAg loss

Treatment guidelines for the management of chronic hepatitis B (CHB) recommend to use nucleos(t)ide analogues(NAs) when to begin the antiviral therapy. Although the ideal clinical endpoint is to achieve HBsAg seroclearance, HBsAg seroclearance rates are very low. In HBeAg positive CHB, NA can be stopped when HBeAg seroconversion is achieved and antiviral treatment has been maintained at least for 12 months. However, cessation should be decided carefully since 40-90% of patients developed reactivation. In HBeAg negative CHB, cessation of treatment should be individually decided according to the clinical treatment response and the baseline severity of the liver disease. Treatment with NA should be continued until the loss of HBsAg. The substantial benefit of stopping therapy is reducing costs. However, stopping NAs may be associated with severe HBV flares and hepatic decompensation. Therefore, it is needed to understand who can stop treatment. Also, Monitoring for viral relapse and acute exacerbation of disease is mandatory after discontinuation.
