        골다공증을 동반한 척추체 압박골절에 대한 경피적 척추 성형술 - 예비보고 -

        Objective : Percutaneous vertebroplasty is an effective and minimally invasive procedure consisting of the injection of a PMMA(polymethyl methacrylate) into the vertebral body compression fracture with osteoporosis. Matherials and Methods : Twenty-eight procedures were performed for vertebral body compression fractures with osteoporosis in 25 patients(22 women, 3 men). The mean age was 65.9 years old. The inclusion criteria for percutaneous vertebroplasty were 1) acute vertebral body compression fracture with osteoporosis, 2) expected high operative morbidity in old age, 3) no neurologic deficits, 4) no or minimal canal enchroachment, 5) patient refusal of invasive surgery. All patients underwent MR images before the procedure. Under local anesthesia, after the percutaneous needle puncture of the involved vertebra via a transpedicular approach and venography using the water soluble contrast material, PMMA injection was introduced into the fractured vertebral body. Results : The procedure was technically successful in all patients. All patients experienced excellent pain relief (complete pain relief ; 10, marked pain relief ; 14). One patient experienced marked pain relief, however, the patient died during the follow-up period due to stomach cancer. There were twelve paravertebral tissue leaks, twelve paravertebral venous plexus leaks, four epidural leaks and one intradiskal leak, but no clinically significant complications occurred in all patients. Conclusion : Percutaneous vertebraoplasty is a valuable procedure in the treatment of vertebral body compression fracture with osteoporosis, providing immediate pain relief and early mobilization. MRI is the most reliable diagnostic tool for identifying painful fractured vertebral body.

        본태성 수전증과 파킨슨병 환자에서 미토콘드리아 DNA 비교 분석

        Essential tremor(ET) is the most common movement disorder however there has been little agreement in the neurologic literature regarding diagnostic criteria for ET. Familial ET is an autosomal dominant disorder presenting as an isolated postural tremor. The main feature of ET is postural tremor of the arms with later involvement of the head, voice, or legs. In previous studies, it was reported that ET susceptibility was inherited in an autosomal dominant inheritance. As with previous results, it would suggest that ET might be associated with defect of mitochondrial or nuclear DNA. Recent studies are focusing molecular genetic detection of movement disorders, such as essential tremor and restless legs syndrome. Parkinson's disease(PD) is a neurodegenerative disease involving mainly the loss of dopaminergic neurons in substantia nigra by several factors. The cause of dopaminergic cell death is unknown. Recently, it has been suggested that Parkinson's disease many result from mitochondrial dysfunction. The authors have analysed mitochondrial DNA(mtDNA) from the blood cell of PD and ET patients via long and accurate polymerase chain reaction(LA PCR). Blood samples were collected from 9 PD and 9 ET patients. Total DNA was extracted twice with phenol followed by chloroform : isoamylalcohol. For the analysis of mtDNA, LA PCR was performed by mitochondrial specific primers. With LA PCR, 1/3 16s rRNA~1/3 ATPase 6/8 and COI~3/4 ND5 regions were observed in different patterns. But, in the COI~1/3 ATPase 6/8 region, the data of PCR were observed in same pattern. This study supports the data that ET and PD are genentic disorders with deficiency of mitochondrial DNA multicomplexes.

        공주지역의 강우강도-지속기간-빈도곡선 개발

        본 연구는 공주지역의 한국확률강우량도를 이용하여 확률강우량을 산정한 후 강우강도-지속기간-빈도곡선식을 개발하는데 목적이 있다. 공주지역의 재현기간별 확률강우강도식의 산정결과 강우강도식은 장 단기간으로 구분하는 것이 타당하였고, 강우강도식의 신뢰성을 설명하는 장기간의 결정계수(<TEX>$R^2$</TEX>)는 <TEX>$0.9924{\sim}0.9971$</TEX>로써 매우 높게 나타나고 있기 때문에 본 연구에서 제시한 재현기간별 확률강우강도식이 상당히 의미 있는 것으로 사려된다. 공주지역의 확률강우강도식은 최소자승법을 사용하여 Talbot형, Sherman형, Japanese형, 일반형의 4가지로 분석한 결과 본 연구에서 적용한 Sherman형이 가장 적합한 것으로 나타났다. 따라서 공주지역의 수공구조물 설계시 본 연구에서 산정된 재현기간별 확률강우강도식을 이용함으로써 보다 정도가 높은 설계를 할 수 있으리라 판단된다. This paper is to derive the Intensity-Duration-Frequency Curve at Kong-Ju area after estimating probable rainfall depths using Rainfall Frequency Atlas of Korea. It has been suggested that the probable rainfall intensity formulas should be classified by short and long term basis in this area. The coefficients of determination of the probable rainfall intensity formulas are calculated as high as 0.9924 through 0.9971. Four types of rainfall intensity formulas such as Talbot type, Sherman type, Japanese type, General type are considered to determine the best type for the Kong-Ju area. Sherman type applied in this study can be determined as the representative probable rainfall intensity formula in the area. Therefore the rainfall intensity formulas for the selected return period in this study provide valuable insight into the estimation of the rainfall intensity. The developed Intensity-Duration-Frequency Curve can be used to provide a better hydraulic design at Kong-Ju area.

        척추 결핵의 전방유합술시 Titanium Mesh Cage의 효과

        Objective : The maintenance of the correction of kyphotic deformity is one of the difficult problem in tuberculous spondylitis after anterior debriment and fusion with tricortical bone graft. The goal of this study is to find out the efficacy of titanium mesh cage impacted with autogenous bone chip in tuberculous spondylitis treated with anterior intervertebral fusion. Materials and Method : Twelve patients were treated with anterior intervertebral fusion using titanium mesh cage for tuberculous spondylitis from January 1996 to June 1999. We analized the changes in the correction of kyphotic deformity, changes of ESR and CRP, fusion state and recurrence after anterior intervertebral fusion with titanium mesh cage. Results : Clinical symptoms were improved in all twelve patients without any neurologic complications. The mean kyphotic angle corrected was 7.3 degrees immediately after operation, but the loss of correction of kyphotic angle was 2.2 degrees after 3 months and 2.6 degrees after 6 months. We found that the loss of correction of kyphotic deformity occurred mainly within the first 3 months after surgery. Only one patient, suffered from acute hepatic failure after first operation and had an insufficient anti-tuberculous medication therapy, showed recurrence of tuberculous spondylitis after 6 months. The patient underwent a second operation with posterior fixation procedure with good outcome. The changes of ESR and CRP were not specifically important factor to reveal recurrence of tuberculosis of the spine in our series. Conclusion : The surgical procedure of tuberculous spondylitis using titanium mesh cage with bone chip seems to be an effective procedure to minimize loss of the correction of kyphotic deformity without any aggravating inflammatory change and recurrence with titanium mesh cage, when sufficient debridement and anti-tuberculous chemotherapy are achieved.

        본태성 수전증 환자의 미토콘드리아 DNA 분석

Materials & Methods : Blood samples were collected from PC and 9 ET patients. Total DNA was extracted twice with phenol followed by chloroform : isoamylalcohol. For the analysis of mtDNA, LA PCR was performed by mitochondrial specific primers. Results : With this technique, deletions of large quantities were detected within several regions of mtDNA in ET patients except for D-loop and CO I regions. Conclusion : The authors believe that ET is a genentic disorder with deficiency of mitochondrial DNA multicomplexes and mitochondiral dysfunction could be one of major causative factors of ET. Mitochondrial dysfunction may play an important role in the pathogenesis and possibility of disease progression among familial group with ET patients.

        The intradural lumbar disc herniation which was first described by Dandy in 1942 is unusual. The pathogenesis is obscure and preoperative diagnosis is not easy. The patient usually have more severe neurologic deficits than those found in the much more common extradural disc herniations. We experienced two cases of the intradural lumbar disc herniation who had previous disc operations and the symptoms and neurologic signs were improved after removal of the intradural sequestration of disc fragments.

        Acute intraabdominal aortic obstruction ends in progressive fatal course or severe permanent disability unless it is diagnosed and treated promptly. However, the incidence of such disease is very rare, so there is very little chance for a neurosurgeon to encounter a patient with acute intraabdominal aortic obstruction. The authors present a case of 62-year-old man with severe low back pain and acute paraplegia caused by acute intraabdominal aortic obstruction.

        두부외상 후 동반 발생한 경동맥해면정맥동루와 내경동맥박리증 1례

        A carotid-cavernous fistula (CCF) is a pathologic arteriovenous communication between the carotid artery and the cavernous sinus, and the incidence of CCF as a result of craniofacial trauma is less than 0.2%. Traumatic internal carotid artery dissection (TICAD) is a rare phenomenon, occurring in 0.02 to 0.67% of cervical blunt trauma cases. We report a rare case of CCF associated with ICAD after craniofacial trauma injury, which is very rare. A 23-year-old woman presented with left ptosis on 21 days after a car crash accident. She had suffered from headaches, diplopia, tinnitus, and dizziness. Brain MRI revealed no mass or aneurysmal sac or intracranial hemorrhage, but cerebral angiography showed CCF with ICAD. She was treated with a ICAD percutaneous transarterial stent and endovascular embolization, 1 months after cerebral angiography. clinicians treating patients with craniofacial injuries should have a complete understanding of these patients` entity, because urgent intervention may improve outcome.

        악성수막종의 두개강 외 전이

        Meningioma is the most common benign primary intracranial tumor and rarely metastasizes. Malignant meningioma is a World Health Organization Grade III tumor and constitutes 1 to 3% of all meningioma. The prognosis of the patient of malignant meningioma is very poor because of its frequent recurrence and distant metastasis. There is no guideline concerning the management of extracranial metastasis of meningioma. A 57-old female patient presented with headache and visual disturbance due to huge skull base meningioma. She underwent subtotal mass excision with craniotomy and TSA followed by radiation therapy. However, follow-up brain magnetic resonance imaging (MRI) revealed tumor growing and the patient received additional surgical resection. After 2nd surgery the patient was presented with weakness of both lower extremities. Whole spine MRI demonstrated thoracic spine metastasis compressed spinal cord. Multiple nodules of lung and liver were noticed as well. We report a patient of malignant meningioma with multiple extracranial metastases including review of literature.
