        일부 개업의의 우울 및 Burnout 정도의 관련요인

        In order to investigate the prevalence and the factors related to the depression and burnout among private practice physicians, a SDS(self-rating depression scale) and MBI(Maslach burnout inventory) -based questionnaire study was performed on 344 private practice physicians in Kwangju and Chonnam area. The results were summarized as follows. 1. Mean SDS score was 38.3 in total subjects and the prevalence rate of depression was 48.8%. As for the frequency order of the items of the SDS, decreased libido, diurnal variation and hopelessness were relatively high, and suicidal rumination, constipation and agitation were noted low. 2. Noticeable factors related with depression were smoking, coffee use, sleeping time and satisfaction with income. 3. As a result a factor analysis with the MBI data, five factors named as emotional exhaustion, depersonalization, personal accomplishment, involvement and self-interest were extracted. Statistical analysis of the data demonstrated that 48.8% of the physician sample reported high scores on emotional exhaustion, and 45.3% scored high on depersonalization. Personal accomplishment scores remained high with 45.3% reporting high personal accomplishment. 4. Variables related to the burnout were age, sleeping time, family size religion, medical speciality, duration of practice setting, visiting patient number, closing day per month and job satisfaction. 5. In the relationship with depression, burnout was closely related to depression. Above results showed that the high percentage of private practice physicians experiencing depression and burnout suggests the need for further research to establish trends, to identify causal factors, and to develop avenues to reduce stress.

        QED(양자전기역학)로부터 영감을 얻는 신학적 주제들

        QED is the abbreviation of quantum electrodynamics. It may be an uncharted water of science for the most of the Christian theologians. But I think it is a field which can induce the theological imaginations. It is worth while for the theology to have a dialogue with QED because it can give us a chance to think over some theological themes once more deeply. To avoid the difficulty in mathematics, Richard P. Feynman, who is 1965 Nobel laureate in Physics, once gave a lecture to address the main contents of QED for the ordinary people at UCLA. The lecture had been edited to be a book^(19)), which was translated into Korean in 2001. So, the Korean Theologians can explore QED without significant knowledge in mathematics. In that book, I dealt with the concept of time as the first theme that theologically matters. QED physicists report that it is observed that time flows in reverse direction in an experiment in which micro particles interact. It means that a particle disappears before it emerges. The event of disappearance happens earlier than that of emergence of the very particle which has already disappeared. In QED, they explain that the particle has moved in opposite direction of time. It has moved from the future (disappearance) to the past (emergence). In QED, it is proved that this is an order of nature itself. From this insight through QED, we can say that the biblical reports of “dreams (f. e. Gen 37f.)” and the prophecy can also be explained based on the concept of the reversed time. In dreams, one realizes some future events in advance. The prophets transmit the promise of God here and now which will be realized in future. In both cases, the cognition of an event happens before the real occurrence of the very recognized event. We see here an example of the reverse flow of time which moves from the future back to the past. While happens only in the cognitive realm, the event of the resurrection testifies that the ultimate end of this world is happening here and now. The time of the end has already begun in the resurrection of Jesus Christ and is going on here and now. Here is the reverse of time extremely real. This concept of time was represented by J rgen Moltmann and Wolfhart Pannenberg, for example. Moltmann described that the time of the revelation flows from the future through the present into the past. The science of 20th century had rejected such a theological concept based on the presupposition of the absolute, straight flow of time from past to future. But the dialogue with the QED now makes sure that the presupposition is wrong. So, we are led to evaluate J. Moltmann`s and W. Pannenberg`s theology once more by the concept of the reverse time. The second theme that I find suitable for the dialogue between theology and QED is the mystery of the number “3”. In QED, it has been found that a neutron or a proton is composed of the three strong combined quarks. It marks the limitation of the human sight because the three quarks cannot be divided into each pieces. It is not sure whether they have an attribute of a material particle, but they are sure to compose a core of a nucleus by the unit of “three”. On the boundary between the thing to be seen and the thing not to be seen lies the logic of the Trinity. The Christian tradition has taught that in God`s being one is composed of the three and the three is one. To the science of 20th century, this was sure to be absurd. But the dialogue with QED makes us sure that the mystery of “3” in the doctrine of Trinity is actualized in the micro-dimension of nature. While the theologians think about the Trinity between the immanent being of God and the creation of the world, QED physicists find it at the very boundary between the thing to be seen (proton and neutron) and the thing not to be seen (quarks).         슬관절 골관절염 환자와 정상인의 보행 시 슬관절 가속도

        목적: 보행 시 슬관절에는 내측으로 지면반발력이 작용하게 되어 외측으로 회전력이 발생하게 된다. 퇴행성 슬관절염은 이 회전력에 의해 슬관절 내측으로 가해지는 힘이 커져서 발생할 수 있는 것으로 알려져 있다. 이에 퇴행성 슬관절염 환자와 정상인의 슬관절 가속도를 측정하여 보행 시 슬관절에서 나타나는 가속도의 양상을 비교해보고자 하였다. 대상 및 방법: 정상 여성 22명과 퇴행성 슬관절염으로 진단 받은 성인 여성 22명을 대상으로 하였다. 같은 종류의 양말과 신발을 착용하여 편안한 걸음을 걷는 상태에서 무선가속도 측정시스템을 이용하여 슬관절의 가속도 변화를 측정하였다. 측정범위가±2 g (중력가속도)인 삼축 가속도 센서 CXL03LF3 (Crossbow, USA)를 비골두에 부착하여 가속도를 측정하였고 Mann-Whitney 검정법과 Spearman의 상관분석법을 이용하였다. 결과: 1) 퇴행성 슬관절염 환자에서는 정상인에 비해 부하 반응기 직후에 나타나는 슬관절 가속도의 첫 번째 최고치가 외측 방향과 앞쪽방향으로 크게 나타났다(p<0.01). 2) 퇴행성 슬관절염 환자에서는 정상인에 비해 편안한 걸음을 걷는 상태의 분속수가 유의하게 적었으나(p<0.01) 분속수와 슬관절 가속도의 상관관계는 없었다. 3) 퇴행성 슬관절염 환자와 정상인 모두에서 부하 반응기 직후에 나타나는 슬관절 가속도의 첫 번째 최고치가 외측 방향과 앞쪽 방향간에 유의한 상관관계를 보였으며(p<0.01) 전후측, 내외측, 상하측간에도 상관관계가 있었다(p<0.01). 결론: 퇴행성 슬관절염 환자에서는 보행시 정상인보다 부하 반응기 직후에 나타나는 슬관절 가속도의 최고치가 크게 나타났다. 이러한 결과로 보아 슬관절의 동적 가속도 형태를 관찰하는 것이 슬관절 질환 진단의 한 방법으로 사용될 수 있을 것으로 생각한다. Background: To investigate and describe the acceleration of the knee during walking in patients with osteoarthritis of knee and healthy adults. Methods: 22 patients with osteoarthritis of knee and 22 heathy adults participated in this study. The accelerometer fixed at left fibular head was used to record anterior-posterior, medio-lateral and superior-inferior accelerations of the knee during walking. Results: There was significant increase in lateral and anterior 1st peak of acceleration at loading response in the patients with osteoarthrits of knee compared to healthy adults (P<0.01). There was significant decrease in cadence in the patients with osteoarthritis of knee compared to heathy adults (P<0.01), but there was no significant correlation between cadence and 1st peak of acceleration at loading response. There was correlated with anterior and lateral acceleration in all subjects (P<0.01). Also there was relationship between anterior-posterior, medio-lateral and superior-inferior acceleration in all subjects (P<0.01). Conclusion: The accelerometer can be an easy and useful way to evaluate patients with osteoarthritis of knee.

        대역 확산 신호를 위한 지연 시간 추정 알고리즘

        본 논문에서는 대역 확산 (spread spectrum) 방식의 실시간 위치 추적 시스템 (RTLS: real-time locating system)을 위한 초 분해능 지연 시간 추정 (super-resolution time delay estimation) 알고리즘을 개발하고 다중경로 환경에서 성능을 분석한다. 기존의 코릴레이션 방식은 주변에 장애물이 없는 line-of-sight (LOS) 환경에서는 비교적 정확한 지연 시간 추정이 가능하나 지연 시간의 차이가 한 칩 이내인 근접 다중 경로 환경에서는 성능이 급격히 저하되는 문제점이 있다. 따라서 본 논문에서는 도래각 추정에 이용되는 MUSIC 알고리즘을 이용하여 근접 다중 경로 환경에서도 신호의 지연 시간을 추정할 수 있는 초 분해능 지연 시간 추정 알고리즘을 개발하고 이를 ISO/IEC 24730-2.1 RTLS 시스템에 적용하여 성능을 분석한다. In this paper a super-resolution time delay estimation algorithm is developed for real-time locating systems (RTLSs) that employ a direct-sequence spread spectrum technique, along with its performance analysis in multipath environments. The classical correlation method provides relatively good performance in line-of-sight (LOS) environments but its performance seriously degrades in multipath environments, especially when signals are spaced closer than a PN chip. Therefore we shall develop a super-resolution time delay estimation algorithm that may estimate the time delays of multipath signals even in closely spaced multipath environments using the MUSIC algorithm for direction-of-arrival estimation and analyze its performance by applying the algorithm to the ISO/IEC 24730-2.1 RTLS system.
