        망막분지정맥폐쇄에서 패턴 광응고술과 기존의 광응고술과의 효과 비교

        Purpose: To compare the clinical effect and complications between patterned laser photocoagulation with short exposure time and conventional laser photocoagulation for neovascularization in branch retinal vein occlusion (BRVO). Methods: A prospective study was conducted on 36 patients (39 eyes) who required laser photocoagulation for neovascularization due to BRVO. The patients were divided into 2 groups, the patterned laser photocoagulation group (laser exposure time 0.02 sec, 3 × 3 array patterned photocoagulation) and conventional laser photocoagulation group (laser exposure time 0.2 sec). Other laser parameters (burn intensity and spot size) were the same. Pain score at the time of treatment was monitored after the laser photocoagulation. In addition, best-corrected visual acuity as well as central macular thickness measurements were performed before the treatment and after 2, 4, 8, and 12 weeks. The regression of neovascularization in fluorescein angiography was monitored during follow-up periods. Results: The patterned laser photocoagulation group had a greater reduction in pain during laser photocoagulation, and a decreased change in central macular thickness after laser photocoagulation than the conventional laser photocoagulation group. In particular, the patterned laser photocoagulation group had a statistically significant difference between the 2 groups at postoperative 8 and 12 weeks. The regression rate of neovascularization between the 2 groups had no statistically significant difference. Conclusions: Patterned laser photocoagulation with a short exposure time decreases pain and postoperative macular edema with no difference in regression of neovascularization. Patterned laser photocoagulation with a short exposure time can be considered as a useful and efficient method for neovascularization in BRVO. J Korean Ophthalmol Soc 2010;51(10):1368-1373

        The purpose of this study is to investigate the effects of perceived quality of m-CRM on employee satisfaction and continuous use intention by dividing the quality dimension of m-CRM perceived by salespeople into information quality, system quality and service quality. The results of the analysis are as follows. First, reliability, situational appropriateness, infrastructure environment, immediate usability, and personalization had a positive effect on satisfaction, and the effect of data security on satisfaction was rejected. Second, Satisfaction has a positive effect on intention of continuous use. Third, the optimality in the effect of the independent variable on the satisfaction was shown to have a moderating effect only in the security. Fourth, the moderating effect of innovativeness among technological readiness, innovativeness has a moderating effect only on the condition that the situation appropriateness and the infrastructure environment satisfy.

        미용치료용 엔디-야그레이저에 의한 황반병증에서 항혈관내피성장인자항체로 치료한 1예

        목적: 미용치료용 고출력 Nd:YAG 레이저에 우연히 노출된 이후 발생한 황반병증 환자에서 조기 유리체강내 항혈관내피성장인자항체 치료를 보고하고자 한다. 증례요약: 36세의 여자 환자가 내원 일주일 전 미용치료용 고출력 Nd:YAG 레이저에 노출된 이후 발생한 우안의 시력저하를 주소로 내원하였다. 환자의 우안 최대교정시력은 0.1이었고 안저검사에서는 우안 중심와 망막하출혈 소견이 관찰되었다. 빛간섭단층촬영에서 우안 중심와 망막하출혈로 인한 장액망막박리 및 망막색소상피-브루크막 복합체 파열 소견을 관찰할 수 있었다. 이에 대해 총 세번에 걸쳐 유리체강내 베바시주맙(Avastin??, 1.25 mg/0.05 mL; Roche Korea, Seoul, Korea) 주사술을 시행하였다. 6개월 후 시력은 초기 내원 시와 변화가 없었으나, 빛간섭단층촬영에서 우안 중심와 망막하출혈 흡수 소견이 확인되었고 이차성 맥락막신생혈관의 발생이나 맥락막출혈의 재발 소견은 보이지 않았다. 결론: 고출력 미용레이저에 의한 황반하출혈 및 맥락막파열 환자에서 조기의 항혈관내피성장인자항체를 이용한 치료를 환자의 형태학적 호전 및 이차성 맥락막신생혈관 방지에 효과적인 치료 방법으로 고려해볼 수 있다. Purpose: To report a follow up of early intravitreal anti-vascular endothelial growth factor (VEGF) antibody treatment in a patient with maculopathy after accidental exposure to a high-powered cosmetic Nd:YAG laser. Case summary: A 36-year-old female patient visited our clinic with decreased visual acuity in the right eye after accidental exposure to a high-powered cosmetic Nd:YAG laser. The best-corrected visual acuity of the right eye was 0.1 and subretinal hemorrhage on the foveal area was observed during fundus examination. Optical coherence tomography revealed serous retinal detachment with hyperreflective disruption of the ellipsoid zone and retinal pigment epithelium-Bruch’s membrane complex. Intravitreal bevacizumab (Avastin??, 1.25 mg/0.05 mL; Roche Korea, Seoul, Korea) injections were performed three times. Visual acuity was unchanged at 6 months after the first visit. However, complete resolution of the subretinal hemorrhage was observed and there were no signs of secondary choroidal neovascularization or recurrence of choroidal hemorrhage. Conclusions: Early anti-VEGF antibody treatment in a patient with subfoveal hemorrhage and choroidal rupture after high-powered cosmetic Nd:YAG laser exposure improved morphology and prevented secondary choroidal neovascularization.

        가토에서 상직근 Z-근연절단술 후 근육 약화 효과

        Purpose: To experimentally investigate the effect for muscle weakness after superior rectus Z-myotomy and histological changes. Methods: Superior rectus muscle fibers of rabbits (16 rabbits, 32 eyes) were cut transversely with scissors across 75% of the muscle in two different positions on opposite sides. In group 1 (16 eyes), myotomies were performed at 2 and 7 mm from the muscle insertion (5 mm gap) and in group 2, performed at 2 and 5 mm (3 mm gap). The change of mark, eyeball position, and muscle tension after myotomy and 4 weeks postoperatively was evaluated, the location of the mark was examined, and muscle tissue biopsy was performed. Results: After Z-myotomy, the marks of the two groups moved significantly posteriorly from insertion within groups (p<0.05), with no significant differences between groups (p=0.469). Eyeball positions of the two groups moved significantly inferiorly (p<0.05); the amount of position change of group 1 was greater than group 2 (p<0.05). When the globe was pulled in opposite directions for muscle action, the degree of change decreased with significant difference within groups (p<0.05), but there were no significant differences between groups (p=0.32). Conclusions: Z-myotomy of the superior rectus muscle affected the recession of eyeball position and weakened the muscle action. Muscle weakening affected by the different gaps between myotomies did not show consistent results. J Korean Ophthalmol Soc 2010;51(8):1121-1128

        The Effect of Perceived Value of Distance Selling on Psychological Distance and Customer Loyalty

        Purpose - This study is to examine whether the perceived value of customers who purchase through a distance selling method such as an Internet shopping mall affects the psychological distance and how this leads to customer loyalty. Also, by analyzing the mediating effect of psychological distance, it was verified whether psychological distance plays a mediating role between the perceived value of distance selling and customer loyalty. Design/Methodology/Approach - Data collection was conducted for foreigners residing in Korea. We asked 250 people in various occupations in Korea to respond voluntarily by sending out a questionnaire made with Google Forms via email and KakaoTalk. Emails sent to respondents and KakaoTalk contain links to online questionnaires. Finally, 203 survey respondents were used for the analysis. Findings - First, the perceived value of distance selling was found to have a significant effect on psychological distance. It was found that functional value and financial value had a significant negative (-) effect on psychological distance, and emotional value had a significant positive (+) effect. However, social value did not affect psychological distance. This means that the better the quality and service, and the more money you save, the less time, space, and social distance you feel. On the other hand, it can be seen that the stronger the desire or emotion to purchase, the more psychological distance is felt. Second, it was found that the psychological distance feeling for long-distance purchase had a negative effect on customer loyalty. This means that as the distance purchasing customers feel a sense of temporal, spatial, and social distance, their repurchase or recommendation intention decreases. Third, as a result of analyzing the mediating effect of psychological distance, it was confirmed that the perceived value of distance selling leads to customer loyalty through psychological distance. This means that distance sellers need marketing efforts to reduce the psychological distance that customers feel in order to increase customer loyalty. Research Implications - It is difficult for customers who make distance purchases through online shopping malls to judge perceived values such as functional values of products. Therefore, reducing the psychological distance of consumers by providing detailed information on the producer, production location, and production process to the distance selling site will increase the repurchase intention. Also, distance sellers need marketing efforts to build customer trust in order to increase perceived value and reduce customers’ psychological distance.

        시간적 초점이 도시재생관광지의 자아일치성과 재방문의도에 미치는 영향에 관한 연구

        본 연구에서는 자아일치성 이론을 바탕으로 시간적 초점의 영향까지 그 동기를 확장하여 도시재생 관광지에 대한 고객들의 선택 행동을 분석하고자 한다. 관광지를 방문하는 관광객의 시간적 초점이 과거/현재/미래 어느 시점에 주목하는지, 그리고 그것이 관광객 행동에 어떠한 영향을 미치는지를 살펴보고자 한다. 특히 시간적 초점, 자아일치성과 실제 관광행위 및 재방문의도 간의 영향관계를 살펴보고, 자아일치성의 매개효과를 검증하고자 한다. 본 연구는 도시재생관광지 방문 경험이 있는 관광객을 대상으로 2020년 6월 17일부터 25일까지 개별면접과 온라인 설문조사를 병행하여 전국적으로 실시하였으며, 총 440부의 설문지가 회수되어 불성실한 설문지 33부를 제외하고 407부를 최종 분석에 이용되었다. 연구결과, 과거 초점과 미래 초점은 각각 실제적 자아일치성과 사회적 자아일치성에 영향을 미쳤으나 현재 초점은 기각이 되었다. 실제적 자아일치성과 사회적 자아일치성 역시 실제 관광 행위와 재방문의도에 유의한 영향을 미쳤으며, 자아일치성의 매개효과는 사회적 자아일치성에 한하여 과거, 미래 초점과 재방문의도 사이에서 매개 역할을 하는 것으로 나타났다. 따라서 본 연구의 시사점은 도시재생 관광지를 활성화하기 위한 방안으로 관광객의 과거와 미래를 떠올릴 수 있는 시간적 초점을 기반으로 자아일치성을 확대할 수 있는 여건을 마련하는 마케팅 전략을 수립한다면 관광객들의 실제 관광행위와 재방문의도를 더욱 증대시킬 수 있을 것이다. In this study, based on the theory of self-consistency, the motive is extended to the influence of temporal focus to analyze the customer's selection behavior for urban regeneration tourist destinations. The purpose of this study is to examine which point of time the tourist's temporal focus of tourists visiting tourist destinations focuses on the past/present/future and how it affects tourist behavior. In particular, this study is to examine the relationship between temporal focus, self-consistency and actual tourism behavior and intention to revisit, and to verify the mediating effect of self-consistency. This study was conducted in parallel with individual interviews and online surveys from June 17 to 25, 2020 targeting tourists with experience visiting urban regeneration tourism sites. A total of 440 questionnaires were collected and 407 copies were used for the final analysis, excluding 33 unscrupulous questionnaires. As a result of the study, the past focus and the future focus had an effect on the actual self-consistency and social self-consistency, respectively, but the present focus was rejected. Actual self-consistency and social self-consistency also had a significant effect on actual tourism behavior and revisit intention, and the mediating effect of social self-consistency was found to play a mediating role between past, future focus and revisit intention. The implications of this study are to revitalize tourist destinations for urban regeneration, and will help to establish a marketing strategy that prepares conditions for expanding self-consistency based on a temporal focus that can recall the past and future of tourists. Through this, it will be possible to further increase tourists' actual tourism activities and revisit intentions.

        가맹본부의 갑질이 상생협력과 관계성과에 미치는 영향

        본 연구의 목적은 프랜차이즈 가맹본부의 갑질에 대한 가맹점 인식이 상생협력정도와 관계성과에 미치는 영향을 파악하는 데 있다. 또한 상생협력이 가맹점주의 갑질 인식과 관계성과 사이에서 매개역할을 하는지 살펴보고자 하였다. 자료는 설문지를 통해 온/오프라인 방식으로 수집하였으며, 주로 프랜차이즈 가맹점주를 대상으로 조사하였다. 수집된 유효설문지 250부를 최종분석에 이용하였다. 본 연구의 분석결과를 요약하면 다음과 같다. 첫째, 가맹본부의 갑질유형중에서 거래상 지위남용과 허위ㆍ과장정보제공은 상생협력에 부(-)의 영향을 미치는 것으로 나타났다. 그러나 프랜차이즈 갑질 중 대인적 갑질, 구속조건부거래, 거래거절은 상생협력과 부(-)의 관계가 나타나지 않았다. 특히 손해배상의무부과행위는 상생협력과 정(+)의 방향으로 나와 연구가설과 정반대의 결과가 도출되었다. 이러한 결과는 한국 프랜차이즈 산업에 갑질 문화가 만연해 있고, 갑질의 행태들이 뿌리 깊은 관행으로 자리 잡혀있어, 가맹본부의 부당한 행위에도 불구하고 가맹점사업자들이 경제적인 손실을 지각하지 못하고 있다는 것을 의미한다. 둘째, 가맹본부와 가맹점간의 상생협력이 좋을수록 관계성과가 좋았다. 계약지속의도와 다점포 운영의도가 높았으며, 계약해지의도는 낮았다. 셋째, 상생협력의 매개효과에 대한 분석결과, 상생협력은 허위ㆍ과장정보제공이 계약 지속의도, 다점포 운영의도에 간접효과만을 보여 완전 매개하였으며, 허위ㆍ과장정보와 계약 해지의도는 직접효과와 간접효과 모두 보여 부분 매개하였다. 이는 가맹본부의 허위ㆍ과장정보제공행위는 상생협력을 통해 관계성과를 악화시킨다는 것을 의미한다. 본 연구를 통해 가맹본부에 대한 가맹점주의 갑질인식은 상생협력에 위협이 되며 관계성과를 악화시켜 지속적인 성장을 저해한다는 것을 확인하였다. The purpose of this study is to investigate how the franchisee’s Gapjil* influcences on the win-win cooperation and their relationship. In addiiton, we would like to examine if the win-win cooperation plays a mediator role between the perception of franchisee owners and the relation performances. The data was collected through a questionnaire method, and the survey was conducted from the franchise owners. 250 copies of the collected valid questionnaires were used for the final analysis. The analysis results of this study are summarized as follows. First, among the sub-factors of franchisor’s gapjil, the abuse of trading position and the provision of false and the exaggerated information were found to have a negative(-) effect on win-win cooperation. But, there was no significant negative(-) relationship with win-win cooperation in interpersonal Gapjil, contingent conditional transaction, and transaction rejection among franchises. In particular, the act of imposing compensation for damages was significant in the direction of the win-win cooperation process and the opposite result of the research hypothesis was drawn. Second, the better the win-win cooperation between the franchisor and the franchisees becomes, the better the relationship performance does. The intentions to continue a contract and operate a multi store were high and,on the other hand, the intention to terminate was low. Third, as a result of analyzing the mediating effect of win-win cooperation, it can be considered that the effect of Gapjil type on relationship performance is mediated by win-win cooperation. Furthemore, win-win cooperation, the provision of false/exaggerated informatioin and the contract termination intention showed only an indirect effffect on the contract continuation intention and multi-store operation intention. And for the case of win-win cooperation, the false/exaggerated information and the contract termination intented to have direct effects. and indirect effects, which were partially mediated. This means that the franchisor’s act of providing falseㆍexaggerated information deteriorates the relationship performance through win-win cooperation. Through this study, it was confirmed that the franchisee’s perception of Gapjil toward the franchisor can be a threat to win-win cooperation and the aggravating the relationship performance disrupts the continuous growth. *Gapjil refers to the arrogant and authoritarian attitude or actions of people in South Korea who have positions of power over others. It is a new word that has appeared on the Internet in Korea since 2013.

        虛位時間對決策情境的虛位誘導效應影響硏究

        隨着國內饥饿營銷、團購、秒殺等一系列虛位營銷方式的興起,通過控制虛位産品的不可得信號呈現的時間來情境性地影響消費者對相關産品的偏好和態度的營銷策略應用更加廣泛。基于産品不可得信號轉換方式界定了虛位時間槪念的兩種表現形式。已有虛位時間的相關硏究主要從企業和産品的視角探討産品從不可得到可得信號轉換的新産品預發布時間,忽略了消費者決策情境視角對産品從可得到不可得信號轉換的虛位時間感知下消費者決策行爲的硏究。基于雙屬性空間虛位誘導效應啓動策略模型,依据心理學時間壓力及認知閉合需要的相關理論,運用著名E-Prime心理學實驗軟件設計虛位時間在決策過程中的6個層次下的消費者決策實驗。實證結果表明:虛位時間同時對虛位誘導效應及其加工流暢性産生影響,而且加工流暢性在虛位誘導效應引發的消費者偏好反轉過程中具有顯著的中介作用,結論不僅彌補和豊富了消費者決策行爲中的虛位誘導效應的理論框架,幷進一步提高虛位營銷策略的效率和效益。 With the rising of phantom marketing in Chinese market, such as hunger marketing, group purchase, sec-killing and so on, marketing strategies that can influence consumers’ preference and attitude about related products by controlling the time of presenting an phantom product become more widely. On account of conversion mode of unavailable signal of products, it defines two types of phantom time. It has been mostly concerned about preannouncements time of new products from unavailable to available signal in light of enterprises and products, but has not explored consumer decision-making study on phantom time of products from available to unavailable signal. Based on Chinese consumer decision-making context cue using the famous psychology experiment software named as E-prime based on priming model of phantom decoy effects in double attribute space and according to relevance psychological theory of time pressure and need for cognitive closure, this paper designs six phantom time experiments of consumer decision-making. The results show: phantom time both exert positive influence on phantom decoy effects and processing fluency, and more importantly, processing fluency plays a significant medium role in the course of consumer preference reversal caused by phantom decoy effects. Its conclusions not only make up and enrich the theory framework of phantom decoy effects on consumer decision-making behavior, but also improve phantom marketing efficiency and profits.

        비당뇨성 유리체출혈에 대한 임상분석

        목적: 비당뇨성 유리체출혈 환자에서 유리체절제술의 수술시기와 시력예후와의 상관관계 및 추정되는 원인질환을 알아보고자 하였다. 대상과 방법: 유리체출혈로 유리체절제술을 시행받은 환자 97안을 대상으로 의무기록을 바탕으로 추정되는 원인질환을 조사하였다. 또한 출혈 후 6주를 기준으로 조기와 후기 수술군으로 나누어 수술 전 후 최대교정시력 및 시력 향상 정도, 유리체출혈 후 유리체절제술까지의 기간과 시력과의 상관관계 등에 대해 비교하였다. 결과: 망막정맥분지폐쇄, 망막열공, 외상이 원인인 경우 조기 수술군과 후기 수술군의 수술 전 평균시력, 수술 1년 후 시력평균, 시력향상치의 평균은 통계적으로 유의한 차이를 보이지 않았으나(p>0.05)망막중심정맥폐쇄의 경우는 조기수술군이 후기수술군에 비해 시력향상치의 평균이 유의하게 높았다(p=0.005). 결론: 망막중심정맥폐쇄 이외의 비당뇨성 유리체출혈이 있는 환자에서 수술의 시기가 통계학적으로 유의한 시력 차이를 가져오지는 않았으나 조기 수술로 시력회복을 도모하고 유리체출혈의 원인 질환을 규명하여 합병증을 줄이는 방향으로 치료 계획을 세우는 것이 중요할 것으로 사료된다. Purpose: To investigate the correlation between the time of vitrectomy and visual prognosis and possible causes of vitreous hemorrhage in non-diabetic patients diagnosed with vitreous hemorrhage. Methods: This study investigated the possible causes of vitreous hemorrhages based on the medical records of 97 eyes that underwent vitrectomy. Subjects were divided into early (within 6 weeks) and late (after 6 weeks) groups based on the time of surgery, and the best corrected visual acuity, visual acuity improvement, and the correlation between the time of surgery and the visual acuity were analyzed. Results: There was no significant difference in average visual acuity before a vitrectomy, average visual acuity 1 year after the surgery, mean visual acuity improvement between the two groups when the underlying cause was branch retinal vein occlusion, retinal tear, or trauma (p>0.05). However, when the underlying cause was central retinal vein occlusion, the mean visual acuity improvement was significantly higher in the early surgery group (p=0.005). Conclusions: The timing of vitrectomy in non-diabetic patients with vitreous hemorrhages caused by diseases other than central retinal vein occlusion did not significantly increase the visual acuity. However, it is important to promote early visual recovery and find out underlying disease of vitreous hemorrhage by early vitrectomy in order to prevent complications.
