부인암 환자 말초 혈액의 CD4(+)CD25(+)조절 T림프구 분포

목적: 조절 T림프구 (regulatory T lymphocyte)는 암세포에 대한 면역관용 (immune tolerance)을 일으켜 특정 항원에 대한 T림프구의 작용을 감소 혹은 상실시킴으로써 암세포의 빠른 성장, 국소 침입과 혈행성 전이에 관여하는 것으로 알려져 있다. 현재 암환자의 말초혈액 내 조절 T림프구의 분포에 대한 연구는 초기 상태이고, 난소암을 제외한 부인암에서 CD4(+)CD25(+)조절 T림프구 분포 및 Foxp3의 발현정도에 대해서는 그 연구결과가 거의 없는 상태이다. 따라서 본 연구에서는 암환자에서 CD4(+)CD25(+)조절 T 림프구의 분포와 Foxp3의 표현정도를 확인하고자 한다. 연구 방법: 본 연구는 2005년 3월부터 2005년 9월까지 서울아산병원 산부인과에서 조직 병리학적으로 자궁경부 편평세포암종, 장액성 난소암종, 자궁내막선암종으로 처음 진단받은 환자 각각 10명의 말초혈액과 자궁경부상피내암 환자 10명 및 건강한 여성 10명의 말초혈액을 이용하여 이루어졌다. FACS (flow cytometry analysis) 를 통해 CD4(+)CD25(+)조절 T림프구 분포와 PCR과 전기영동을 통해 Foxp3의 표현 정도를 알아보았다. 각 군 간의 비교 및 유의성 검정은 ANOVA를 이용하여 분석하였으며, 두 군 간의 p값이 0.05 미만인 경우에 통계적으로 유의한 것으로 판정하였다. 결과: 첫째, 정상 대조군과 비교하여 부인암 환자의 말초혈액에서 총 CD4+ T림프구 중 CD4(+)CD25(+high)조절 T림프구의 발현이 증가되어 있었으나, CD4(+)CD25(+)조절 T림프구의 비율은 대조군과 유의한 차이가 없었다. 둘째, 자궁경부암의 전암 병소인 자궁경부상피내암에 비해 자궁경부암 환자에서 총 CD4+ T림프구 중 CD4(+)CD25(+high)조절 T림프구의 비율이 유의하게 높았다. 셋째, 각 부인암의 병기에 따른 CD4(+)CD25(+), CD4(+)CD25(+high)조절 T림프구 비율은 차이가 없었다. 넷째, 대조군과 달리 부인암 환자의 말초혈액에서 전사요소인 Foxp3의 발현이 증가되어 있어, 암세포가 존재할 경우 말초혈액에서 암세포에 의해 자연 획득한 T림프구가 증가함을 알 수 있었다. 결론: 본 연구를 통해 부인암 환자의 말초혈액에서 조절 T림프구 중 CD4+CD25+high조절 T림프구 수가 증가되어 있음을 확인하였고, 이에 의해 암세포에 대한 면역무시, 면역억제작용으로 암세포의 발현, 성장, 전이에 관여하는 것으로 보인다. 이를 토대로 현재 시도되고 있는 항암면역치료를 부인암 환자에게도 적용할 수 있는 가능성이 높을 것으로 사료된다. Objective: Recently the existence of a CD4(+)CD25(+) regulatory (Treg) population has been described in rodents and humans. It is unclear how the immune response cells interact to tumor cells effectively, but the malignant tumor cell growth was suppressed by the main effect of T lymphocytes and natural killer cells in experimental studies using various biologic response modifier. This study was performed to investigate the proportion of CD4+CD25(high) Tregs and expression of Foxp3 in Peripheral blood (PBL)s in patients with cervical, ovarian or uterine cancers. Methods: Blood samples were collected from 10 healthy women and a total of 40 patients with gynecologic cancer at department of Obstetrics and Gynecology, Asan Medical Center, Seoul, Korea, from March 2005 to September 2005, were enrolled in study group. Information regarding patient history and tumor stage was recorded. They were diagnosed at same center at first, and never been treated any therapy. The population of CD4(+)CD25(+high) Tregs as a percentage of total CD4+cells was evaluated by flow cytometric analysis. We measured the proportion of Treg cell that co-express CD4 and CD25 in the peripheral blood lymphocytes form patients with either cervical, ovarian uterine cancer or carcinoma in situ of cervix. Expression of Foxp3 in the CD4+subsets defined by electrophoresis. Results: The following tumor entities were included cervical cancer (n=10. 7 in stage Ⅰ, 1 in stage II, 1 in stage III, 1 in stage Ⅳ); ovarian cancer (n=10. 4 in stage Ⅰ, 0 in stage II, 5 in stage III, 1 in stage Ⅳ), ; uterine cancer (n=10. 9 in stage Ⅰ, 0 in stage II, 0 in stage III, 1 in stage Ⅳ). In cervical cancer patient, ovarian cancer patients, uterine cancer patients and healthy women, the proportion of CD4+CD25(high) Tregs was 4.53% (SD 2.30), 6.89% (SD 7.81), 4.37% (SD 2.43) and 0.87% (SD 0.57) of the total CD4+cells respectively. The proportion of CD4+CD25(+high) T cells was significantly higher in cervical cancer patients (p=0.016), ovarian cancer patients (p=0.001) and uterine cancer patients (p=0.038) when compared with healthy women. But there was no significant difference in proportion of CD4+CD25+ Tregs comparing with healthy women. Expression of Foxp3 was significantly thicker in tumor-associated lymphocytes than control T cells by electrophoresis. Conclusion: In conclusion, our data suggested that the increase in frequency of regulatory T cells might play a role in modulation of the immune response against cervical, ovarian, uterine cancer could be important in design of immunotherapeutic approaches.
