        급성 홍역 뇌증 환아들의 뇌 SPECT 소견과 유용성

        목 적 : 홍역은 전염력이 매우 높은 급성 유행성 전염병으로 감염 후 뇌척수염, 홍역 봉입체 뇌염 그리고 아급성 경화성 전뇌염의 3가지 신경계 합병증을 일으킨다. 본 연구에서는 홍역 후 급성 뇌염이 발생한 환아들의 임상 양상, 경과 및 결과와 뇌 SPECT의 양상을 비교 분석하였다. 방 법: 2000년 10월부터 2001년 1월까지 4개월 동안 홍역 후 의식저하, 경련, 마비 등의 특징적인 뇌염증상을 보여서 전북대학교병원 소아과에 입원한 11명의 환아를 대상으로 뇌척수액 검사, 홍역 특이항체 검사, 뇌파검사, 뇌 SPECT와 뇌 자기공명영상을 실시하였다. 결 과: 11명의 대상 환아들의 남녀 비는 남아 4명, 여아 7명으로 여아가 많았으며, 연령은 18개월에서 14세까지로 평균 연령은 10.5세였고, 8세 이상의 연장아가 10명(90.9%)으로 대부분을 차지하였다. 홍역 예방접종은 11명 중 10명은 1차 예방 접종을 실시했으며 1명은 실시하지 않았고, 2차 예방 접종을 실시한 환아는 없었다. 뇌척수액의 백혈구는 $158{\pm}157/{\mu}L$이었고 단백은 $124{\pm}60mg/dL$이었다. 홍역 특이항체는 혈청에서는 IgG/M이 10명에서 양성이었고 1명은 모두 음성이었으며, 뇌척수액에서는 9명에서 IgG가 양성이었고 1명에서는 IgM이 양성이었다. 11명에서 실시한 뇌 SPECT는 모두 비정상 소견을 보였고 뇌 자기공명영상은 7명에서 비정상 소견을, 4명에서는 정상 소견을 보였다. 뇌 SPECT에서 혈류 감소는 전두엽(9명), 측두엽(9명), 두정엽(8명), 시상(8명) 부위가 많았고 이외에 기저핵(5명), 후두엽(4명), 소뇌(2명)에서도 혈류감소가 관찰되었다. 결 론: 홍역과 홍역의 심각한 신경계 합병증을 예방하기 위해서는 지속적으로 2차 예방 접종을 시행하여야 하고 홍역에 대한 감시체계의 확립 및 홍역 유행에 대한 신속한 대처가 요구된다. 뇌 자기공명영상에서 정상인 4명 모두가 SPECT에서는 혈류 감소가 관찰된 점으로 미루어 보아 보이지 않은 뇌의 손상이 있었을 것으로 추측할 수 있으며, 감염후 뇌염이 발생한 환아의 뇌 손상 여부를 평가하는데는 뇌 자기공명영상보다 뇌 SPECT가 보다 예민할 것으로 생각된다. Purpose : Acute measles encephalitis(ME) is characterized by an abrupt onset of fever and obtundation, frequently accompanied by seizures and multifocal neurological signs. The aim of this study was to clarify the clinical manifestation, progression and the brain SPECT patterns in patients with acute ME. Methods : This study included 11 children with acute ME admitted to Chonbuk National University Hospital. Ten patients received a first dose of measles vaccine, one patient did not receive a first dose, and no patients received a second dose. ME was diagnosed based on characteristic clinical pictures, measles antibodies by ELISA and abnormal CSF findings. Brain MRI and brain SPECT were performed in 11 patients with acute ME. Results : There were four males and seven females whose ages at onset ranged between 18 months and 14 years(mean : 10.5 years). The main clinical neurologic pictures were loss of consciousness( 10) and seizure(five). The titer of IgG and IgM antimeasles antibodies in serum were positive in 10 patients. In CSF, nine patients had IgG antibodies and one patient had IgM antibodies. The concentration of protein(mean : $124{\pm}60mg/dL$) and WBC counts(mean : $158{\pm}157/{\mu}L$) in CSF were elevated in all patients. In electroencephalographic examination, nine patients showed increased slow waves. Seven of 11 patients(63.6%) revealed high signal intensity on the brain MRI. In contrast, all patients showed hypoperfusion in brain SPECT examination. According to brain SPECT, the perfusion deficits were frequently observed in the frontal lobe(nine), temporal (nine), parietal(eight) and thalamus(eight). Conclusion : Brain SPECT is more sensitive than MRI for the evaluation of brain damage in early stages of acute ME.

        청심연자탕으로 호전된 뇌경색을 동반한 심방세동 환자 치험 3례

        Atrial fibrillation (AF) is a common arrhythmia that is a potent independent risk factor for stroke. The incidence of AF increases with age and most affected people have underlying cardiac disease. An aging society increases the incidence of AF patients. The case was designed to evaluate the improving effect of Chengsim Yeunja-tang (CYT) for atrial fibrillation (AF) with Cb-infarction patients. This patient was treated with CYT and had significant improvement in symptoms and change of EKG. -Heart rate decreased in patients with higher than normal heart rate -RV5+SV1 voltage decreased (LVH improved) -Arrhythmia remained steady. -Palpitation. chest discomfort, dyspnea, headache, dizziness diminished. Results suggest that CYT is an effective treatment for Taeumin AF patients.

        Liver cirrhosis is a disease of the liver in which normal cells are replaced by scar tissue. This condition results in the failure of the liver to perform many of its usual functions. Liver cirrhosis include acsites, jaundice, portal hypertension, varices etc. This report is about one case of ascites with liver cirrhosis. In this case, we administrated Taeumjowetang-gamibang was administered and acupuncture was done in treatment of ascites and its symptoms. After administration of Taeumjowetang-gamibang medication, clinical symptoms and prominently improved and ascites dwindled to nothing. This reports suggests Taeumjowetang-gamibang has desirable effected on ascites.

        고위험군 유두상 갑상선암 환자에 대한 방사성옥소 최대허용선량 측정법 및 치료법의 검증

        Purpose: Radioiodine (I-131) therapy is an effective modality to reduce both recurrence and mortality rates in differentiated thyroid cancer. Whether higher doses shows higher therapeutic responses was still debatable. The purpose of this study was to validate curve-fitting (CF) method measuring maximum permissible dose (MPD) by a biological dosimetry using metaphase analysis of peripheral blood lymphocytes. Methods: Therapeutic effects of MPD was also evaluated in 58 patients (49 females and 9 males, mean age 50⁑11 years) of papillary thyroid cancer. Among them 43 patients were treated with ≤7.4 GBq, while 15 patients with ≥9.25 GBq. The former was defined as low-dose group, and the latter high-dose group. Therapeutic response was defined as complete response when complete disappearance of lesions on follow-up I-131 scan and undetectable serum thyroglobulin levels were found. Statistical comparison between groups were done using chi-square test. P value less than 0.05 was regarded as statistically significant. Results: MPD measured by CF method using tracer and therapeutic doses were 13.3⁑1.9 and 13.8⁑2.1 GBq, respectively (P=0.20). They showed a significant correlation (r=0.8, P<0.0001). Exposed doses to blood measured by CF and biological methods were 1.54⁑0.03 and 1.78⁑0.03 Gy (P=0.01). They also showed a significant correlation (r=0.86, P=0.01). High-dose group showed a significantly higher rate of complete response (12/15, 80%) as compared to the low-dose group (22/43, 51.2%) (P=0.05). While occurrence of side effects was not different between two groups (40% vs. 30.2%, P=0.46). Conclusions: measurement of MPD using CF method is reliable, and the high-dose I-131 therapy using MPD gains significantly higher therapeutic effects as compared with low-dose therapy.         The recent trains are almost being operated by the mechanical propulsion force to drive the gear and wheel with the traction motor. However Magnetic Levitation Vehicle is differently operated. Magnetic Levitation Vehicle is applied with Linear Induction Motor(LIM) that has many advantage like to high capability of going up to slope, low noise, easy to control of speed. So domestic and many advanced countries are interested in Magnetic Levitation Vehicle and they have been studying about it continuously. Thus this paper is studied the LIM test method of static states and guess the optimum driving point by characteristic of static states for LIM. The test items are measurement of thrust force by changed air gap, measurement of thrust force and normal force by changed slip frequency etc.

        Up until now, power centralized trains have been produced due to the maintenance convenience and comfortableness, but there are some problems, such as limitation of viscosity and maintenance difficulty of railroad due to recently increasing axle mass. In order to improve the problems, power decentralized trains have been developed to improve traction power. In the case of using electrical braking system, it is possible to improve braking friction power. Induction motors have been developed for power decentralized high speed train because of less structural defection, and low maintenance and production cost. However, induction motors have limitations, such as assuring enough power capacity and efficiency with reduced size. PMSM (Permanent magnet synchronous motor) have been newly developed to improve shortcomings of induction motors. The PMSM can be produced small and light weighted. Also if the PMSM and induction motors have the same size and power capacity, the PMSM have better power efficiency. In this pater, the prototype and modified type of PMSM for "High-speed Electric Multiple Unit-400㎞/h eXperimmen" will be introduced

        N/A High silicon cast irons(HSCI) with the high acid resistance have been used for the prevention of acid corrosion occurring in various structures under acid conditions. However, the HSCI is only known as one of materials which have high acid resistance, but few work has dealt with this material in domestic. Therefore, in this study, the acid resistance of various cast irons with alloying elements and melting conditions have been examined, and studied the influences of the matrix structures, mechanical properties and morphologies of graphite. The results obtained in this study are as follows : In case of melting temperature, the mechanical properties of specimen manufactured with high temperature of 1650℃ showed higher value because the inclusion and impurity were removed. In case of pouring temperature, the mechanical properties of specimen fabricated below 1350℃ of pouring temperature showed higher value because the amount of gas absorption from atmosphere decreased during the solidification time. The corrosion rate decreased with increase in Si content. On the other hand, Mn addition appeared an opposite trend with Si.

        Cooling type of traction motor for EMU in domestic is mostly an open type. Its system is a cooling air entered through air inlet cool down a traction motor and an hot air by traction motor get out air outlet. It is easy to cool it down but hard to maintain it. To improve an ability of maintenance a total enclosed type traction motor is already developed and used in abroad not an opne type. So we developed a total enclosed type traction motor which will be used in domestic and abroad EMU. We tried to reduce a weight and a size compared with the abroad one. In contrast with open type traction motors which cool off inside of motors total enclosed motors cool down by cooling exterior frame of motors. In this case cooling fins or air fan blowing to the exterior of motors are applied. The total enclosed type traction motor developed by us have two housing to block the foreign substance into inner of a motor and have two cooling fan to easy to reduce a heat happened at a coil. In this paper design of a cooling structure of the total enclosed traction motor developed twice and performance verification through test will be discussed.
