한방병원에 입원한 뇌경색 환자의 기능회복과 관련된 특성에 대한 연구

Objectives : Stroke is the most common disabling neurological disease of adult life. The treatment and rehabilitation of stroke patients is an important and challenging area in the field of medicine. In Korea, a lot of stroke patients are treated using oriental medicine and there have been reports on their functional recovery. The aim of this study was to prove the therapeutic effect of oriental medicine on ischemic stroke. We also tried to identify characteristics of patients whose functional recovery was prominent and studied whether or not the use of fluids, anticoagulants, antiplatelets and neuroprotective agents increased the functional recovery of patients. Methods : We studied 794 patients within 2 weeks after first attack of ischemic stroke who were admitted to Kyung-Hee Oriental Medical Center from 1999 to 2000. Their maximum admission period was 60 days. Results : Motor power grade, NIHSS and MBI scores of patients improved during hospitalization significantly. Factors affecting functional recovery were admission period, motor power grade, NIHSS and MBI at admission. Age, waist-hip ratio and HDL cholesterol also affected functional recovery of patients. Whether or not fluids, anticoagulants, antiplatelets and neuroprotective agents were administered had nothing to do with functional recovery. Conclusion : Oriental medicine is effective for treatment and rehabilitation of ischemic stroke patients. When we manage stroke, we have to consider some important factors.

기능적 전기자극이 뇌성마비 환자의 족저굴곡근 경직에 미치는 영향

Spasticity is the most troublesome problems in the management in cerebral palsy. The purpose of this study was to observe the effect of FES to spasticity. 8 cerebral palsy children were selected for this study. Assessment was carried out before treatment for obtain baseline measurement of spasticity and reassessment were carried out at after 10th., 20th. treatment sessions and 24hours after treatment. The results of this study were as following that MAS scores were significantly reduced after 10th and 20th after treatment compared with pre-treatment. MAS scores were significantly reduced after 20th compared with pre-treatment. These results indicated that FES appears to reduce significantly MAS scores and maintained 24hours after treatment compared with pre-treatment.

부신피질기능저하증에 대한 한의학적 치험 1례

Adrenal insufficiency is caused by adrenal failure or impairment of the hypothalamic-pituitary-adrenal axis. The main symptoms of adrenal insufficiency are chronic fatigue, nausea, vomiting, anorexia, and weight loss. We report a case of adrenal insufficiency in a 38-year-old female. The patient complained of headache, dizziness, anorexia, and general weakness. We treated her with Iggisaengjin-tang and acupuncture. After treatment, the patient's symptoms were improved and serum cortisol levels rose to a normal range without the aid of steroid supplementation therapy. This case suggests that Korean medicine can be effective in the treatment of adrenal insufficiency, but more clinical reports are needed.

사례기반추론 기법을 이용한 개인화된 추천시스템 설계 및 구현

본 논문에서는 인터넷 컨텐츠 사이트에서 묵시적인 평가정보를 이용한 새로운 사례기반 추천시스템을 설계하고 구현하였다. 본 시스템은 크게 사용자 프로파일 생성 모듈, 유사도 계산 및 추천 모듈, 개인화된 메일링 모듈로 구성된다. 사용자 프로파일 생성 모듈에서는 사용자가 컨텐츠를 이용하면서 남긴 로그 기록을 이용하여 컨텐츠에 대한 개인별 선호도를 추출할 수 있는 속성내, 속성간 가중치를 제시하였다. 유사도 계산 및 추천 모듈에서는 사용자 프로파일과 새로운 컨텐츠간의 유사도를 측정하기 위한 유사도 계산식을 제시하였다. 개인화된 메일링 모듈에서는 개인별 선호도에 의해 구성된 추천 컨텐츠를 플렛폼-독립적인 XML 문서 형식으로 변환하여 발송한다. 제안된 모델에 대한 추천 효율을 검증하기 위해 평균절대오차(MAE)와 반응자작용특성(ROC) 값을 이용하여 제안한 추천 모델과 협동적 필터링 기법과 비교 실험하였다. 실험결과, 본 논문에서 제안한 모델의 추천 효율이 기존의 협동적 필터링 기법보다 우수함을 보였다. We design and implement a new case-based recommender system using implicit rating information for a digital content site. Our system consists of the User Profile Generation module, the Similarity Evaluation and Recommendation module, and the Personalized Mailing module. In the User Profile Generation Module, we define intra-attribute and inter-attribute weight deriver from own's past interests of a user stored in the access logs to extract individual preferences for a content. A new similarity function is presented in the Similarity Evaluation and Recommendation Module to estimate similarities between new items set and the user profile. The Personalized Mailing Module sends individual recommended mails that are transformed into platform-independent XML document format to users. To verify the efficiency of our system, we have performed experimental comparisons between the proposed model and the collaborative filtering technique by mean absolute error (MAE) and receiver operating characteristic (ROC) values. The results show that the proposed model is more efficient than the traditional collaborative filtering technique.

약진(藥疹)으로 심한 소양감을 보인 환자에 대한 온청음(溫淸飮) 투여 1례

Drug eruption is a relatively commin dermatological disorder and may cause significant problems. The treatment is to stop using a drug that may cuase a drug eruption. In some cases, steroids and antihistamines can be used supportively. According to some reports, Onchungeum(溫淸飮) has steroid-like effects and antihistaminic effects. We administered Onchungeum(溫淸飮) a patient with severe itching because of a drug eruption. After administration of Onchungeum(溫淸飮), itching decreased to much degrees. It can be suggested that Onchungeum(溫淸飮) can be used to treat symptoms of allergic skin disease like a drug eruption.

세정제 음독후 발생한 역류성 식도질환 및 연하곤란 환자 치험 1례

The recent increased interest in hygiene has led to the use of a variety of detergents everyday life. Accordingly, accidents occur frequently, but proper precautions or treatments have not yet been established for these occasions. The patient in this case had accidentally ingested basic detergent, after mistaking it for a drink. This had caused acid reflux swallowing distress. Treatment in a western medical hospital had resulted in improvement in inflammation, as observed with an esophageal endoscopy test, but the patient still found it almost impossible to drink or eat. Following treatment with herbal medicine and acupuncture, the patient’s food intake increased and the pain and dysphagia were diminished. We cannot generalize based only on this one case study, so further studies are needed.

상지 반복 작업 후 발생한 손목 및 손 부위의 기타 신경손상 치험 1례

Background and Objective: The Korean agricultural population is now rapidly aging. Older people in rural areas are weak due to work-related musculoskeletal disorders. The aim of this case is to report treatment of injuries to the nerves at the wrist and hand level by Korean medicine. Methods: The patient in this case was a 74-year-old male. He had injured nerves at the right wrist and hand level due to agriculture work. We treated him with acupuncture, herbal medicine, etc. Results: The patient showed the first sign of recovery of his wrist after 13 days treatment. After 37 days of treatment, his wrist movement was restored to a nearly normal range. He showed the first sign of recovery in a finger after 19 days of treatment and his grasping power increased from 0% to 60%. The cold sensation in his hand and arm also disappeared. Conclusions: We cannot generalize based only on this one case study. Nevertheless, this report suggests that Korean medicine may be an effective treatment for injury of nerves at the wrist and hand level.

매튜 아놀드와 김광섭은 각각 영국의 산업화와 한국의 근대화를 지나오며 개발우선주의의 비판의식 없는 팽창 과정을 지켜본다. 자연과의 조화를 파괴하며 이루어진 기형적 발전은 인간으로 하여금 전통을 저버리게 하였으며 절대적 존재로부터 얻을 수 있는 심신의 안정을 상실도록 한다. 아놀드와 김광석은 「도버 해안」과 「성북동 비둘기」를 통해 모든 유기적 존재들의 지배자로 군림하려 한 인간이 종국엔 성난 자연에 의해 쫓기는 신세가 되어가는 모습을 지적한다. 그러나 두 작가의 작의는 자연에의 굴복이나 전통 문화로의 회귀에 있지 않다. 그들은 인간이 자신 또한 자연의 일부이면서도 문명의 발전을 위해 자연을 이용하려 하는 이원적 욕망의 존재라는 사실을 견지하고 있기에 자연을 대하는 인간의 태도는 필요에 따라 이중적일 수밖에 없음을 덧붙인다. 아놀드와 김광섭은 외경의 대상인 자연을 통해 인간과 자연이 상호의존적 관계에 놓여 있다는 진리를 깨닫는다면 인간은 소외된 유기체로서 정신적 불구의 삶을 연명하는 것으로부터 벗어날 수 있음을 강조한다. 사회 지식층으로서 ‘올바른 진화’를 주창한 그들은 자연과 유기적 관계에 놓인 인간으로 하여금 자기반성을 촉구하도록 하는 의식의 첨병으로서의 역할을 수행한다. Matthew Arnold and Gwang-seob Kim observe the process of expansion prioritizing development without criticism, passing through industrial society in the U.K. and the era of modernization in South Korea, respectively. The freakish development made destroying the harmony with nature made human beings abandon the traditions and made them lose the stability of the mind and the body, which they could get from the absolute being. Matthew Arnold and Gwang-seob Kim point out the fall of human beings who would reign as the rulers of all organic beings finally into the beings chased by angry nature through Dover Beach and Seongbuk-dong Pigeon. However, the two writers’ intention to write is not for submission to nature or return to traditional culture. They add the point that human attitude towards nature cannot but differ depending on the necessity since they stick to the fact that they are beings of a dualistic desire that they would not be part of nature though they are objects in nature. Thus, Matthew Arnold and Gwang-seob Kim argue that human beings can get out of subsisting the life of mental disability as alienated organisms if they realize the truth that human and nature are placed in an interdependent relationship through nature, a subject of reverence. They, who insisted on “the right evolution” as members of the intellectual class in society, serve as an advance guard for consciousness, who urges human beings in an organic relationship with nature to reflect on themselves.
