        중증 외상환자에서 mesh를 이용한 일시적 수술창 봉합의 경험

        Background: Abdominal compartment syndrome has multiple etiologies that are not only related to trauma but also any problem condition in the absence of abdominal injury. To determine whether prevention of the abdominal compartment syndrome after celiotomy for trauma victims justifies the use of temporary abdominal coverage with monofilament knitted polypropylene mesh (Malex mesh) in severely injured patients. Method: Medical records at the Ajou University Medical Center were reviewed for a 32-month period from May 1st, 2002 to December 31st, 2004. Twenty-nine consecutive patients requiring celiotomy who were survived until at the end of celiotomy received temporary abdominal coverage and staged abdominal repairs with Malex mesh. One of them was dissecting aortic aneurysm patient and the others were all trauma victims. Malex mesh prosthesis coverage was used in cases of abdominal compartment syndrome due to excessive fascial tension, severe bowel edema and retroperitoneal hemorrhage or edema followed by staged abdominal repairs. Result: Eighteen of twenty-nine patients were survived. Demographic characteristics, injury severity number of abdominal-pelvic bone injuries, mortality rate, complications, number of operations for permanent closure, required time for permanent closure showed no difference between man and women or child and adult. Except one dissecting aortic aneurysm patient, trauma cases showed 3.24±0.98 injury sites. All cases that received temporary abdominal coverage and staged abdominal repairs did not show abdominal compartment syndrome. 10.08±5.85 days and 2.27±0.82 times of operation required making permanent abdominal closure after temporary abdominal coverage followed by staged abdominal repairs. Most of surviving patients have shown antibiotic-resistant organism and fungus infection. Patients who received permanent closure recovered from infectious problem completely. Conclusion: The use of Malex mesh for temporary abdominal coverage in severely injured patients undergoing celiotomy was effective treatment method.

        Vascular replicas are effective platforms that can simulate endovascular interventions in the treatment of intracranial aneurysm (IA), and an IA typically forms in the tortuously structured cerebral artery circulation known as the circle of Willis (CoW). Effective and reliable endovascular procedure simulation before actual clinical treatment is greatly helpful for neurosurgeons, but the simulation performance depends on how identical a CoW replica is to a patient’s CoW. Here, we fabricated a tubular, transparent, and patient-specific CoW replica with realistic dimensions and elasticity using a layer-by-layer coating method. We first constructed a real-scale CoW core substrate using an extrusion-based 3D printing technology, then coated it layer by layer to manipulate vascular wall thickness, and finally dissolved the core substrate to produce a tubular CoW structure. After fabricating the CoW replica, we demonstrated the endovascular procedure simulation using a newly-developed neurointerventional endovascular simulator to check the effectiveness of the CoW replica.

        가족구조의 변화가 성인이행기 발달에 미치는 영향 : 주관적 건강상태, 우울, 교육성취를 중심으로

        본 연구는 한국복지패널 1~2차년도를 활용하여 가족구조의 변화 여부가 성인이행기 신체적 발달, 우울, 교육성취에 미치는 영향을 확인하고 나아가 가족구조의 변화 시기에 따라 성인이행기 발달에 미치는 영향이 상이한지 살펴보았다. 연구 결과는 다음과 같다. 첫째, 0~18세 때 가족구조의 변화 여부는 성인이행기 우울, 교육성취에 부정적 영향을 미치지만 주관적 건강상태에는 유의미한 영향을 미치지 않는다. 둘째, 0~18세 때 가족구조의 변화 여부는 성인이행기 주관적 건강상태에 부정적 영향을 미치지 않지만, 세부 발달단계로 나누어 분석한 결과, 영유아기 때의 가족구조의 변화는 성인이행기 주관적 건강상태에 부정적영향을 미친다. 셋째, 영유가기 때의 가족구조의 변화는 성인이행기 우울에 영향을 미치지만, 아동기와 청소년기 때의 가족구조의 변화는 유의미한 영향을 미치지 않는다. 넷째, 영유 아기, 아동기, 청소년기 때의 가족구조의 변화는 모두 교육성취에 유의미한 부정적 영향을 미치며, 그 영향의 크기는 영유아기, 청소년기, 아동기 순이다. 이를 통해, 아동·청소년기 때의 가족구조의 변화가 성인이행기 발달에도 지속적으로 영향을 미치고 있으며, 가족구조 변화의 시기에 따라 영향을 미치는 발달영역과 강도가 다소 상이함을 알 수 있다. 특히, 주관적 건강상태와 우울과 관련하여 영유아기 때의 가족구조의 변화만 영향을 미쳤지만, 교육성취는 모든 발달단계가 유의미한 영향을 주며 특히, 영유아기, 청소년기, 아동기 순으로 그 영향정도가 큰 것으로 나타났다. 이는 가족구조의 변화가 일어난 시점의 자녀의 발달단계에 따라 영향을 받는 발달영역이 상이하기 때문에, 한부모 가족 자녀를 위한 정책 및 서비스의 종류는 달라야 함을 시사한다. The family environment children are exposed to growing up greatly influences their future potential and achievements. Previous findings show that changes in family structure during childhood, particularly those resulting from divorce or death, cause lasting negative consequence that affect the child physically, psychologically, economically, and socially. Unfortunately, single-parent households are becoming increasingly common in Korea, nearly doubling to more than a million cases in the last two decades. Existing domestic and international studies of this area tend to focus on the short-term effects of growing up in a single-parent household. In addition, these studies group their samples in ways that result in findings that may be too broad or are not necessarily an accurate representation of the subjects. This study attempts to address some of these shortcomings by focusing on the long-term effects of how changes in family structure early in children`s lives affect achievement during their transition to adulthood. In addition, it takes into account the development cycle the child is in at the time of family restructuring, and what kind of long-term effects result from that. In this analysis, we find that there are several cases of statistically significantly differences in domain achievement depending on the developmental stage the child was in when the parental divorce or death occurred. The findings indicate that changes in family structure during the infant/toddler period influence health condition and depression, while changes in family structure during middle-childhood and adolescence do not. Meanwhile, changes in family structure during any point in the developmental stages have negative effects on educational attainment, with the severity of these negative effects depending on when the family changes occur. The negative effect on educational attainment is most prominent when a change in family structure occurs during the infant/toddler period, followed by adolescence, then middle-childhood

        초등학교 저경력 교사의 스포츠 동호회 참여 경험과 교육적 의미

        이 연구는 초등학교 저경력 교사의 스포츠 동호회 활동이 학교 적응 및 개인의 삶과 체육수업에 영향을 준 도움들을 드러냄으로써 교사의 스포츠 동호회 활동 촉진을 유도하는 기초자료를 제시하는 데 목적이 있다. 이를 위해서자문화 기술지의 연구방식을 활용해 연구자가 스포츠 동호회 활동에 참여하며 겪는 과정들을 기술하고 해석하는 과정을 통해 저경력 교사로서 겪게되는 어려움을 어떻게 극복해 나가며 삶에 어떤 영향을 미치는지 그리고 교육적인맥락에서 어떠한 의미가 있는지를 살펴보았다. 연구 결과 저경력 교사의 스포츠 동호회 활동 참여는 교직 생활에서겪는 학교 업무와 학급경영 및 수업에 관한 다양한 어려움을 해소하는 데 도움을 주는 것으로 나타났다. 스포츠 동호회 활동을 통해 신체적 정신적 건강을 유지하였으며 이는 삶의 행복을 느끼게 되는 원천이 되었다. 그리고 스포츠 활동 및 동호회 회원들과의 교류를 통하여 체육교육에 대한 새로운 가치관을 갖도록 해주었고 수업의 질 향상에도 긍정적 역할을 하는 것으로 확인되었다. The purpose of this study is to present basic data that induce teachers to promote sports club activities by revealing the help of elementary school low-career teachers sports club activities influenced school adaptation and individual life and physical education classes. To this end, we looked at how researchers overcome the difficulties they face as low-experience teachers by describing and interpreting the processes they experience while participating in sports club activities, and how they affect their lives and have meaning in an educational context. As a result of the study, it was found that the participation of low-experience teachers in sports club activities helps to solve various difficulties such as school work, class management, and teaching methods in teaching life. Through sports club activities, physical and mental health was maintained, which became a source of happiness in life. In addition, through sports activities and exchanges with club members, it was confirmed that they had new values for physical education and played a positive role in improving the quality of classes.
