        치조제 증강을 위한 $HTR^{TM}$ 중합체 이식후 치유과정;증례보고

        The purpose of this study is to evaluate the effect of $HTR^{TM}$ (hard tissue replacement, Bioplant Inc, U.S.A) polymer on short-term healing as a grafting material for alveolar ridge augmentation. A 48-year-old female presented insufficient bone height and width for the placement of implants. $HTR^{TM}$ polymer was used for ridge augmentation. Bone biopsy was harvested 8 months after the ridge augmentation procedure. $HTR^{TM}$ polymer displayed rapid bone regeneration and mature lamellar and trabecular bone redevelopment. Clinical and histologic observation from the treatment of the patient presented suggest that $HTR^{TM}$ polymer seems to be a appropriate material for alveolar ridge augmentation.

        외상후 치아를 보전하는 방법

        외상성 손상은 치아 손상 자체 뿐만 아니라 악안면부의 손상을 동반하는 흔히 발생하는 현상으로 사고발생후 외모의 손상을 일으키기 때문에 추후 환자의 사회생활에도 영향을 미칠 수 있다. 따라서 미연에 손상이 일어나는 것을 방지하는 것이 가장 중요하겠으나 이미 발생해버린 손상에 대해서 신속하고 정확한 대처를 통해 피해를 최소화할 수 있다고 하겠다. 치료의 궁극적인 목표는 사고 이전의 교합관계의 회복과 같은 기능적인 측면부터 정상적인 안모의 회복을 통한 삶의 질 향상과 같은 정신적인 측면까지 다양하다. 치과 영역의 응급상황에 특화된 분야가 아직 미흡한 상황이지만 앞에서 다룬 기본적인 초기 처치 원칙에 입각하여 진료가 이루어진다면 추후 환자에게 발생할 수 있는 추가 비용이나 합병증 등은 많이 줄어들 것으로 생각된다. Maxillofacial trauma with dental trauma tends to be increasing as development of civilization. Proper dental first aid is easy method to prevent further posiible complication. So we will describe on a simple dental treatment for traumatic teeth in this paper.

        Bardach씨 술식을 통한 이차성 구순열비변형의 교정

        The author presents Bardach' s technique for the residual unilateral cleft lip nasal deformity, The key to a successful and stable correction of the nasal deformity is to lengthen the columella on the cleft side and to mobilize alar cartilage from its surrounding tissue, creating a symmetric shape and length, The major advantages of the technique are lengthening of the cleft columella and creation of a symmetric and well-projected nasal tip.

        치과임플란트 대상환자의 치료결정에 영향을 미치는 요인

        본 연구는 노년인구의 증가에 따른 치아상실로 매년 증가하고 있는 임플란트 치료에 있어 대상 환자가 치료를 결정하는데 영향을 미치는 요인을 분석하여 긍정적인 인식 전환과 임상적 기준 마련에 관한 기초자료를 제공하는데 있다. 2010년 7월부터 약 3개월간 서울시와 경기도 소재의 13개 치과 의료기관의 환자 217명을 대상으로 임플란트 치료군과 비치료군으로 구분하여 설문조사를 시행하였다. 임플란트 치료군은 50대(30.3%)가, 비치료군은 20대(35.7%)가 가장 높았고, 치료군 환자의 구강관리가 양호하였으며, 긍정적인 태도를 보였다. 다중로지스틱 회귀분석으로 분석한 결과, 임플란트 치료결정에 영향을 미치는 요인은 연령, 성별, 교육기간, 월 소득, 치주조직상태, 정기검진여부, 상담태도, 병원신뢰도 등으로 나타났다. 따라서 예방적 차원의 정기적인 구강검진과 관리의 필요성에 대한 교육 및 홍보가 필요하며, 임플란트 치료결정에 긍정적인 인식을 가질 수 있도록 의료진의 상담 기술을 키우고 병원의 신뢰도 향상을 위한 다각도의 노력이 필요하다. 아울러 임플란트 치료 결정에 환자의 막연한 주관적 판단보다는 결정을 용이하게 할 수 있는 명확한 임상적 기준 마련이 필요하며, 비용 부담없이 임플란트 치료를 받을 수 있도록 정부 차원의 재정적 지원이나 정책적인 대안이 요구된다. This study subject is to provide appropriately basic information about annually increased implant treatment by their lost teeth base on increasing old people and also analyze factors influencing implant treatment decisions for patient. For about 3 month from July on the 2010, This survey was carried out at 13 dental clinic as target of 217 patients on categorized into implant treatment-group and non-treatment-group in Seoul and Gyeonggi-do. Implant treatment-group showed highest portion at 50(30.3%), non- Implant treatment-group showed highest portion at 20’s(35.7%), And The patients of implant treatment-group are good oral condition, and they showed a positive attitude. Result of analysis by multiple logistic analysis(regression), Factors of influencing implant treatment decision are age, gender, period of education, monthly income, condition of periodontal tissue, regular checkup, counseling attitude, trust in hospital. Therefore, proactive management about the necessity of regular dental check-up is needed the education and promotion. And medical team should have efforts to Improving technique of medical consultations and Improving the trust of the hospital’s for implant treatment decision. Also, The patient’s subjective determine rather than it is important to preparing to facilitate decision of specific clinical basis. And The governments financial support and policy alternatives are certainly required in order to be implant treatment without cost burden.

        사람 골육종 세포 Saos2에서 아미노산 수송계 L의 발현 및 기능적 특성

        Amino acids are required for protein synthesis and energy sources in all living cells. The amino acid transport system L is a major nutrient transport system that is responsible for Na+-independent transport of neutral amino acids including several essential amino acids. In malignant tumors, the Ltype amino acid transporter 1 (LAT1), the first isoform of system L, is highly expressed to support tumor cell growth. In the present study, the expression and functional characterization of amino acid transport system L were, therefore, investigated in Saos2 human osteogenic sarcoma cells. RT-PCR and western blot analyses have revealed that the Saos2 cells expressed the LAT1 and the L-type amino acid transporter 2 (LAT2), the second isoform of system L, together with their associating protein heavy chain of 4F2 antigen (4F2hc) in the plasma membrane, but the expression of LAT2 was very weak. The uptakes of [14C]L-leucine by Saos2 cells were Na+-independent and were completely inhibited by the system L selective inhibitor, 2-aminobicyclo- (2,2,1)-heptane-2-carboxylic acid (BCH). The affinity of [14C]L-leucine uptake and the inhibition profiles of [14C]L-leucine uptake by various amino acids in the Saos2 cells were comparable with those for the LAT1 expressed in Xenopus oocytes. The majority of [14C]Lleucine uptake is, therefore, mediated by LAT1 in the Saos2 cells. These results suggest that the transports of neutral amino acids including several essential amino acids into Saos2 human osteogenic sarcoma cells are for the most part mediated by LAT1. Therefore, the Saos2 human osteogenic sarcoma cells are excellent tools for examine the properties of LAT1. Moreover, the specific inhibition of LAT1 in tumor cells might be a new rationale for anti-tumor therapy.

        A new field in dental implantology is developing with the goal of finding new ways to improve the osteoconductivity of bone substitutes and to study new molecules able to dictate cellular differentiation and improve bone regeneration. The real future in bone regeneration seems to be in connection with the rhBMP 2s, currently obtained by synthesis using recombinant DNA. Since the first rhBMP 2 studies in humans by Boyne, There are many studies for bone regeneration at oral and maxillofacial area. The rhBMP 2 is widely used at sinus augmentation, alveolar bone defect, and socket preservation.

        재생된 골에 식립한 넓은 직경의 나사형 임플란트에 대한 유한요소법적 분석

        The purpose of this study was to investigate the distribution of stress within the regenerated bone surrounding the implant using three dimensional finite element stress analysis method. Using ANSYS software revision 6.0 (IronCAD LLC, USA), a program was written to generate a model simulating a cylindrical block section of the mandible 20 mm in height and 10 mm in diameter. The 5.0 × 11.5-mm screw implant (3i, USA) was used for this study, and was assumed to be 100% osseointegrated. And it was restored with gold crown with resin filling at the central fossa area. The implant was surrounded by the regenerated type IV bone, with 4 mm in width and 7 mm apical to the platform of implant in length. And the regenerated bone was surrounded by type I, type II, and type III bone, respectively. The present study used a fine grid model incorporating elements between 250,820 and 352,494 and nodal points between 47,978 and 67,471. A load of 200N was applied at the 3 points on occlusal surfaces of the restoration, the central fossa, outside point of the central fossa with resin filling into screw hole, and the functional cusp, at a 0 degree angle to the vertical axis of the implant, respectively. The results were as follows: 1. The stress distribution in the regenerated bone-implant interface was highly dependent on both the density of the native bone surrounding the regenerated bone and the loading point. 2. A load of 200N at the buccal cusp produced 5-fold increase in the stress concentration at the neck of the implant and apex of regenerated bone irrespective of surrounding bone density compared to a load of 200N at the central fossa. 3. It was found that stress was more homogeneously distributed along the side of implant when the implant was surrounded by both regenerated bone and native type III bone. In summary, these data indicate that concentration of stress on the implant - regenerated bone interface depends on both the native bone quality surrounding the regenerated bone adjacent to implant and the load direction applied on the prosthesis.

        경골이식에 관한 임상 연구

        The purpose of this retrospective study was to evaluate the treatment outcome of proximal tibial graft. Thirty-nine patients were grafted with proximal tibia for the repair of maxillofacial defect. Complications can be considered incidental and neglectable clinical significance. It is concluded that the proximal tibial metaphysis offers a promising alternative sources of cancellous bone for the repair of maxillofacial defect.

        백서에서 Medpor<sup>®</sup> 매식후 조직반응에 관한 실험적 연구

        $Medpor^{(R)}$(porous polyethylene) Surgical Implants are used for the augmentation or restoration of bony contour in craniofacial defects. The purpose of this study is to evaluate the ingrowth of soft tissue and bone after application in calvaria of rats. The experiment was carried out in 60 rats. The reflected periosteum was resutured after implantation of $Medpor^{(R)}$ as a experimental site, while in the calvarial bone the reflected periosteum resutured without implantation as a control site. The histologic examination was performed after 1-, 2-, 4-, 8-, 12-, 24-weeks implantation in calvaria of rats. I concluded that there was abundant ingrowth of soft tissue and bone without any adverse tissue response and that it shows good stability.
