        Primary Ureteral Transitional Cell Carcinoma in a Dog

        14살령의 중성화 수컷 개가 혈뇨와 배뇨통으로 내원하였다. 복부초음파 검사에서 편측성 수신증과 요관 벽이 국소적으로 두꺼워진 수뇨관증이 진단되었다. 수술적 탐색으로 좌측 근위 요관에서 요관 유래의 관내 종괴가 확인되었으며, 종괴 절제술이 실시되었다. 요관 종괴는 조직학적 검사를 통해 유두상 이행세포암종으로 진단되었다. 환자는 안정적으로 회복되었으나, 수술 3개월 후 또 다른 종괴가 우측 신장에서 발견되었다. 신장 종괴에 대한 세포학적 검사 결과 간엽세포종양으로 진단되었다. A 14-year-old, castrated male mixed-breed dog weighing 9 kg was presented for hematuria and dysuria. Abdominal ultrasound showed unilateral hydronephrosis and hydroureter with focal thickening of the ureteral wall. Surgical exploration revealed an intraluminal mass arising from the proximal left ureter. Mass resection was performed. Histopathology of the ureteral mass was consistent with a papillary transitional cell carcinoma. The patient recovered well post-operatively, but was diagnosed with another tumor three months later, this time in the right kidney. Fine-needle aspiration cytology of the renal mass revealed an epithelial cell tumor with mesenchymal features.

        Authors experienced a unique case of craniopharyngioma which had evolved rapidly after 4 years of total resection for supratentorial meningioma. A 58-year-old woman presented with headache and visual deterioration. Previously, she had undertaken surgical removal of frontal convexity meningioma 4 years ago and had been well without any postoperative sequelae thereafter. Brain magnetic resonance imaging demonstrated a newly developed suprasellar mass. Pertinent operative procedure was performed and histological verification was made as an adamantinomatous craniopharyngioma. She has been showing unremarkable clinical course up to date. Possible pathogenic mechanisms of de novo development of craniopharyngioma are disscussed with review of case.

        朝鮮後期西洋醫學의 受容과 人體觀의 變化

        Societal changes in the late Chosun Dynasty needed a corresponding endorsement in terms of ideological logic. The social thoughts(ideology) dominated by Chu Hsi(朱熹) Neo-Confucianism had attempted to maintain its underlying social system by adhering to the establishment of landlordism as well as the consolidation of caste system. However, the advent of new ideas and things from the West demanded modification or complementation of the world view held by the social thoughts of Chosun Dynasty based on the Chu Hsi Neo-Confucianism. The understanding of the human body derived from the Wests medieval medicine was introduced through the literature like Chuchih Chuncheng(主制群徵) as things and ideas transported from the West span through varied fields. Accounts of Chuchih Chuncheng articulated in detail the anatomical structure of the human body and attached great importance to liver (肝), heart(心) and brain(腦) associated with blood. It is Lee yik(李瀷) that introduced enthusiastically the perspective of Chuchih Chuncheng on the human body. He could take in the discourse of the transferred Western medicine through the mediation of his own epistemology of nature that scrutiny for particulars leads to the universal. Furthermore, the Western medicine that he absorbed and introduced was thought to be a form of methodology by which he could look into the human body as he valued highly self-apprehension(自得) and experience (經驗), and kept open-minded toward other different thoughts. He took notice of the function of organs governing features of human as a living thing, and blood that mediated between the organs. Leeyiks accounts of the human body were fragmented and incoherent and largely based on the conventional Confucian point of view. Nontheless, he viewed the human body as a dynamic being accommodating to the change of time and made room to allow for intrepreting his accounts of the human body in terms of flowing qi(游氣) and blood(血). It is Jeong yak-yong(丁若鏞) that developed further such way of thinking. he reviewed critically the conventional viewpoint of medicine through the Western medicine and attempted to reframe conventional medical knowledge, though to a certain extent. On the other hand, Shin hu-dam(愼後聃) and Ahn cheong-bok(安鼎福) belonging to the rightist group of Seong-ho(星湖)s school held the framework of reification(形化) that allowed for understanding that the circulation of the qi inside the human body is separated from, and thus independent of circulation of the celestial qi, though they did not get fully involved in adopting medical knowledge coming from the Western world. Unfortunately, following suppression on the Western-Studies(西學) swept away the historical occasion that could have provided an opportunity with which to make sense of the human body as an object of the investigation of universal principles(格物) from the perspective of the imported Western medicine.

      • $^{18}F$-FDG PET/CT 검사에서 신장 방사능의 효과적인 배설 방법에 관한 연구

        FDG PET/CT 검사에서 FDG는 신장 및 요료계를 통하여 배설된다. 이 방사능의 배설을 촉진 시키는 방법을 비교 및 분석하여 각 의료기관의 실정에 맞는 방법을 선택하는데 도움을 주고자 한다. 8개월간 본원에서 PET/CT 검사를 시행한 환자 중 신장에 병적 병변이 없고 기능이 정상인 환자 총 30명을 대상으로 하였다. FDG-PET/CT 기본검사를 마친 환자들을 무작위로 10명씩 다음 세 집단으로 나눠 지연검사를 촬영하였다. (1) 1시간 대기한 집단(Delay group), (2) 수분을 약 700 mL 섭취한 후 1시간 대기한 집단(Hydration group), (3) 이뇨제를 정맥주사하고 30분 대기한 집단(Lasix group). 평가는 육안평가와 정량분석을 하였다. 육안평가는 기본검사와 비교하여 각 집단별 지연검사 영상에서 신장의 방사능 배설을 네 단계로 나누어 비교 및 분석하였다. 정량분석은 기본검사의 표준화섭취계수에서 지연검사의 표준화섭취계수의 감소율을 측정하여 신장 방사능의 배설률로 활용하였다. 육안평가상 Lasix group이 가장 배설이 잘 되었고 그 다음은 Hydration group이었다. Delay group은 효과적인 배설이 이루어지지 않았다. 정량분석 결과 신장방사능 배설률은 Delay group $9.2{\pm}20.7%$, Hydration group $28.1{\pm}22.8%$, Lasix group $29.5{\pm}23.1%$이었다. Delay group과 Hydration group간, Delay group과 Lasix group간의 배설률은 통계적으로 유의한 차이를 보였다. Hydration group과 Lasix group간은 유의한 차이가 없었다. 본 연구에서 이뇨제를 사용하는 것이 신장의 방사능 배설을 촉진시키는데 가장 효과적임을 확인할 수 있었다. 또한 수분을 섭취하는 방법도 비슷한 효과를 보였으며 수분 섭취 없이 지연영상을 얻는 것은 효과가 없음을 확인 하였다. 이상의 방법들을 각 의료기관 실정에 맞게 적용함으로써 영상의 질 향상에 도움을 줄 수 있을 것이다. Purpose: Renal excretion is the main route of FDG clearance in FDG PET/CT scan. Applying optimal method of renal excretion is very important for enhancing image quality and diagnostic accuracy. We evaluated several methods of renal excretion in FDG PET/CT scan. Materials and Methods: Thirty patients with normal renal function were prospectively included. Patients were divided into three group and undergone early and delayed FDG PET/CT scans. (1) Delay group; at 1 hour later of early scan, delayed scan was performed without additional hydration, (2) Hydration group; at 1 hour later of early scan, delayed scan was performed with additional oral hydration (700 mL of water), (3) Lasix group; lasix was administered at the end of early scan and dealyed scan was performed 30 min later. Early and delayed scans were compared to evaluate efficiency of renal excretion. Visual and quantitative analyses were performed by experienced physician and technologist of nuclear medicine. Results: On the visual analysis, renal excretion was the most evident in Lasix group followed by Hydration group. Delay group showed poor renal excretion. On the quantitative analysis, washout rates were $9.2{\pm}20.7%$, $28.1{\pm}22.8%$ and $29.5{\pm}23.1%$ for Delay, Hydration and Lasix groups, respectively. Conclusion: Administration of lasix was the best method for enhancing renal excretion. Delayed scan with hydration was also efficient method, but delayed scan without hydration was not adequate method.

        ECU 및 TCU 신호를 이용한 자동차의 공회전 자동정지 제어 시스템 개발

        본 연구에서는 자동차가 정지하면 지정된 수초이내에 자동차의 공회전 상태를 자동적으로 정지시키고, 운전자가 출발하고자 할 때는 약속된 전기신호로 엔진의 시동을 거는 차량용 공회전 자동정지 제어시스템을 개발하였다. 이 장치는 다수의 선별된 엔진제어유닛과 변속기제어유닛의 전기신호를 입력신호로 하는 마이크로프로세서와 기타 주변 전자회로로 구성되어 있으며 자동차의 운전상태를 인식할 뿐 아니라 냉각수 온도에 따라 그 작동을 조정하고 재시동시 연료분사스킵 기법을 적용하여 자동차의 연료절약 및 유해배출가스를 저감하는 효과를 가지고 있다. 국내의 자동차 연비 및 유해배출가스측정 공인기관에서 ECE15+EUDC모드에서 본 제어시스템 장착차량의 주행시험을 실시하여 그 효과를 확인하였고 실차 적용가능성을 확보하였다. In this study, an automatic idle stop control system was developed, which automatically stops the idle state engine as it detects the stop state of the car for several seconds and restarts the stopped engine with the driver's electrical signal. It is composed of microprocessor and the related electronic circuits and communicates with ECU and TCU. With accurate and confirmed operating performance, the control system was equipped in the test car which was proceeded the ECE15+EUDC mode test. It is confirmed that the control system, ASG has much favourable effects on reducing the fuel consumption and harmful exhaust emissions.

        도재용착용 비귀금속과 열가압성형도재의 전단결합강도 연구

        Purpose: Heat pressed ceramics, used for all ceramic restorations, have the additional advantage of being technically less change through using of the lost-wax technique. Conceptually, combining the ceramic with the clinically proven reinforcing ability of a metal framework would be advantageous; however, cause of mismatching of fusion between ceramics and metal frameworks which from differences of casting temperature and coefficient of thermal expansion, pressed ceramics could not be used with a metal framework. The purpose of this study was to compare shear bond strength of press-to metal ceramic to porcelain fused non precious metal and feldspatic porcelain fused non precious metal. Methods: The 30 metal specimens were casted in a porcelain fused non precious metal nickel-chromium alloy. They were divided into 3 groups by surface treatment and applied ceramic: $125{\mu}m$ aluminium oxide sandblasting and veneered feldspatic porcelain (group FP), $125{\mu}m$ aluminium oxide sandblasting and had press-to-metal ceramic applied (group PC), porcelain bonder (gold bonder) fused on surface of metal specimens and had press-to-metal ceramic applied (group PCG). In each group 10 metal specimens were used. The press-to-metal ceramic applied 20 specimens had ash-free wax pattern applied, the metal-wax complexes invested, and were pressed with heat press ceramic. All specimens were subjected to shear bond strength test at a crosshead speed of 1.0 mm/min. Results: The results of measured in Mean SD and data were analyzed by one-way AVOVA (p= .05) and Tukey HSD test (p= .05).: group FP $16.090{\pm}1.841$ MPa, group PC $12.620{\pm}1.8256$ MPa, group PCG $10.920{\pm}0.9283$, significant differences between all groups (p < .05). Significant differences were found in each between group FP and group PC, group FP and group PCG (p < .05). Conclusion: The shear bond strength of press-to-metal ceramic to porcelain fused non precious metal was described higher in unused gold bonder group than used gold bonder groups.

      • 바텀 애시를 콘크리트 잔골재로 사용하기 위한 활용성에 관한 연구

        본 연구는 석탄을 사용하여 발전한 후 남은 폐기물인 석탄회를 콘크리트의 잔골재로 재활용하기 위한 실험적 연구이다. 석탄회는 크게 플라이 애시와 바텀 애시로 구분될 수 있다. 플라이 애시의 경우 콘크리트의 배합 시 시멘트를 대체하는 재료로 많이 활용되고 있는 반면 바텀 애시의 경우 대부분이 매립되고 있는 등 재활용률이 낮은 실정이다. 본 연구에서는 재활용률이 낮은 바텀 애시의 활용률을 증대시키고자 콘크리트의 잔골재로 일부 치환하였으며 활용성을 확인하였다. 콘크리트 배합 시 물시멘트 비를 고정한 상태에서 바텀 애시의 사용량을 증가 시키게 되면 작업성이 현저히 떨어지게 되므로 일정 수준이상 사용하기 위한 대책이 필요하다. 이러한 문제를 해결하기 위하여 폴리카르본산계 고성능 감수제를 사용하여 콘크리트를 배합하였고 콘크리트 비빔 직후 및 비빔 1시간 후 유동성 및 공기연행성을 비교하여 실제 레미콘이나 배치 플랜트에서 생산된 콘크리트가 현장에 타설되는 시간을 모사하였다. 연구 결과 바텀 애시를 잔골재의 30%정도 치환한 콘크리트 비빔 1시간 후 작업성과 공기연행성을 확보할 수 있었고, 압축강도는 다소 감소하였으나 바텀 애시를 콘크리트 잔골재로 사용할 수 있는 가능성을 확인할 수 있었다. This is an experimental study for recycling coal ash left over from coal use as a potential fine aggregate in concrete. Coal ash is generally divided into either fly ash or bottom ash. Fly ash has been utilized as a substitution material for cement in concrete mixes. On the other hand, bottom ash has the problem of low recycling rates, and thus it has been primarily reclaimed. This study partially substituted fine concrete aggregates with bottom ash to increase its application rate and therefore its recycling rate; its suitability for this purpose was confirmed. The concrete's workability dropped noticeably with increasing bottom ash content when a fixed water-cement ratio of concrete mix was used. Thus, concrete mixes with higher ratio levels are required. To address this problem, concrete was mixed using a polycarboxylate high-range water reducing agent. The fluidity and air entrainment immediately after mixing the concrete and 1 h after mixing were measured, thereby replicating the time concrete is placed in the field when produced either in a ready-mixed concrete or in a batch plant. As a result of this research, the workability and air entrainment were maintained 1 h after mixing for a concrete mixture with approximately 30% of its fine concrete aggregates substituted with the bottom ash. A slight drop in compression strength was seen; however, this confirmed that potential of using bottom ash as a fine aggregate in concrete.
